    Phytochemistry of norditerpenoid alkaloids

    Norditerpenoid alkaloids have important biological activities. Many of them are potent ligands and therefore promising leads for novel selective antagonists and/or agonists of subtypes of nicotinic acetylcholine receptors (nAChR) and voltage-gated sodium channels. Aconitum, Consolida, and Delphinium are important sources of norditerpenoid alkaloids. This thesis is focussed on the chemistry of norditerpenoid alkaloids from these three genera, starting with a review of those recently isolated from Aconitum, Consolida, and Delphinium with brief aspects of taxonomy, biological activities, and modes of action. Selected aspects of the uses in traditional medicine of Delphinium and Aconitum are presented, including data on both toxicity and detoxification. In this thesis, chemical constituents of the seeds of Delphinium cultivar Pacific Giant and the seeds ofAconitum lycoctonum were investigated. To extract the crude alkaloidal material, Soxblet extraction was used for Delphinium cv Pacific Giant seeds and room temperature extraction for A. lycoctonum seeds. The crude extracts were purified by repeated column chromatography (over silica and alumina gels), yielding five known norditerpenoid alkaloids from Delphinium cv Pacific Giant (delavaines A and B, delpheline, methyllycaconitine (MLA), and pacinine) and two from A. lycoctonum (lycaconitine and N-succinylanthranoyl lycoctonine) which were analysed by detailed spectroscopic methods. X-Ray crystallographic analysis of aconitine, mesaconitine, lycoctonine, and delpheline was also studied. Three compounds were obtained from semisynthesis starting with MLA: lycoctonine by basic hydrolysis, inuline by acidic hydrolysis and by esterification of lycoctonine, and elatine by methylenedioxy acetal fonnation. These known alkaloids were structurally elucidated by a variety of spectroscopic techniques. MLA is a selective competitive antagonist at 0.7 sub-type nAChR. From collaborative studies, the results of biological activity for methyl esters delavaines A and B (a 3:2 mixture), delpheline'iand pacinine (the B-ring C6-ketone of delpheline) are reported in this thesis. Their biological activities were detennined in competitive o.-bungarotoxin binding assays for 0.7 nAChR in rat brain membranes. Delavaines A and B were potent ligands (ICso = 50 nM, cfMLA ICso =-1-2 nM), whereas delpheline and pacinine displayed only modest activity at 0.7 nAChR (ICso = -1 JlM). After studying N-ethylpiperidine as a model alkaloid, the pKa (a key physico-chemical parameter) ofMLA was measured by IH NMR as 7.15. Ao lado disso, o projeto busca compreender como se dá o consumo midiático dos participantes por meio dos dados obtidos a partir da aplicação de questionários, o que proporciona o melhor entendimento da metodologia participativa do processo educomunicativo. A conclusão é de que os jovens fazem parte do consumo de mídia tradicionais e por conta disso fazem as suas escolhas temáticas e de gênero na própria produção audiovisual, desta forma o projeto caracteriza-se como ação de inclusão cultura

    Effective Neurally Adjusted Ventilatory Assist (NAVA) Ventilation in a Child With Jeune Syndrome

    Jeune syndrome (asphyxiating thoracic dystrophy) is a rare skeletal dysplasia mainly characterized by dystrophy of the thoracic cage. Neurally adjusted ventilatory assist (NAVA) is a respiratory support in which pressure assistance is provided in proportion to and synchronous with the electrical activity of the diaphragm. We present the case of a 4-month-old infant with asphyxiating thoracic dystrophy and respiratory failure successfully ventilated with NAVA. In this case, NAVA improved patient–ventilator synchrony, reducing endotracheal secretion and gastric overdistention. The reduction of breathing effort and the improvement in enteral feeding tolerance and weight gain made the patient eligible for thoracic surgical correction

    In this article we will explore, in the context of work and regulation of work on digital platforms, the configuration of the autonomous work model, for the salary model. The article presents a set of empirical data produced in Brazil in 2021, which demonstrates the demands and perceptions of platform workers regarding rights in the work relationship with digital platforms. We will argue that these perceptions show that the apparent autonomy and labor freedom are related to the labor management model. The study seeks to assess, from a socio-legal perspective, some dimensions of freedom of work on digital platforms, as a contribution to a better understanding of new forms of governance of digital work.Dans cet article, nous explorerons, dans le cadre du travail et de la régulation du travail sur les plateformes numériques, la configuration du modèle de travail autonome, en dehors du modèle salarial. L'article présente un ensemble de données empiriques produites au Brésil en 2021, qui démontrent les demandes et les perceptions des travailleurs sur les plateformes concernant les droits dans la relation de travail avec les plateformes numériques. Nous soutiendrons que ces perceptions montrent que l'autonomie et la liberté apparente du travail sont liées au modèle de gestion de ce travail. L'étude vise à évaluer, d'un point de vue socio-juridique, certaines dimensions de la liberté du travail sur les plateformes numériques, comme contribution à une meilleure compréhension de ces nouvelles formes de gouvernance du travail numérique.Neste artigo exploraremos, no contexto do trabalho e regulação do trabalho das plataformas digitais, a configuração do modelo do trabalho autônomo, fora do modelo do assalariamento.     A 34-year-old female patient with severe heart failure and pulmonary hypertension was diagnosed late with a high-output acquired arteriovenous fistula between the right common iliac vein and artery. The most probable cause was an iatrogenic vascular injury inflicted during a prior laparoscopic cholecystectomy. Treatment was conducted by placement of an endoprosthesis in the common iliac artery, achieving total exclusion of the fistula and complete remission of symptoms.     Antibiotic use in post-adenotonsillectomy morbidity: a randomized prospective study

    A tonsilectomia, associada ou não à adenoidectomia, continua a ser um dos procedimentos cirúrgicos mais realizados mundialmente, incidindo principalmente sobre a população pediátrica. OBJETIVO: Investigar o impacto do uso da amoxacilina por 7 dias na recuperação pós adenoamigdalectomia, comparando os resultados com um grupo controle. Tipo de Estudo: Estudo prospectivo randomizado controlado com 120 pacientes. PACIENTES E MÉTODO: Os pacientes foram randomizados ao tempo da cirurgia para receber um curso de 7 dias de amoxacilina associada a analgésicos ou apenas analgésicos. Durante a primeira semana de pós-operatório foram avaliados o grau de dor, aceitação da via oral, náuseas e vômitos, febre e retorno às atividades. RESULTADOS: Somente no 4º pós-operatório o grupo recebendo antibiótico teve uma diferença estatística significante no grau de dor. Não houve diferença entre os dois grupos para outros dados analisados. CONCLUSÃO: Considerando os resultados do nosso estudo e revisando a literatura sobre o uso de antibióticos, nós concordamos que não há nenhuma melhora na recuperação dos pacientes submetidos à adenoamigdalectomia após o uso de amoxicilina por 7 dias.Tonsillectomy with or without adenoidectomy still is one of the most commonly performed surgical procedures in the world, mostly in the pediatric population. AIM: to study the impact of amoxicillin for 7 days in post-adenotonsillectomy recovery, comparing results with a control group. Study type: prospective, randomized, controlled study with 120 patients. PATIENTS AND METHODS: the patients were randomized according to surgery time to receive 7 days of amoxicillin associated with pain killers, or analgesic alone. During the first week of postoperative, we assessed the level of pain, oral intake acceptance, nausea and vomits, fever and return to daily activities. RESULTS: It was only in the fourth post-operative day that the group receiving antibiotic agents showed a statistically significant difference as far as pain is concerned. There was no difference between the two groups for other data analyzed. CONCLUSION: considering the results from our study and reviewing the literature on the use of antibiotic agents, we agree that there is no improvement in patient recovery after adenotonsillectomy with the use of amoxicillin for 7 days in the postoperative

The purpose of this text is to generate in the reader reflectionsabout the presence of the organic element, especially when we think about the poetry of artistswho have added, and still provide value to the use of blood as an essential material for workselaboration. The power of objective or subjective communication regarding to some worksthat contains blood was present in several narratives from past artists, and since the end of the60’s until nowadays is used as an essential material at performances and installations.Este artigo pretende discutir, ainda que sucintamente, a evolução do uso do Sangue,tanto como linguagem representativa como também matérica. Fazendo um link entre Arte eSangue, sem nos deter na relação entre eles, explicitar como o Sangue se faz presente em muitasobras, e foi sendo colocado como que pertencente à História, à Arte e à evolução do homem, vaise modificando num crescente, como a própria humanidade. O propósito deste texto é o de gerarno leitor reflexões sobre a presença desse elemento orgânico, principalmente quando pensamosna poética dos artistas que deram, e ainda dão valor ao uso do Sangue como um materialindispensável para a elaboração de suas obras.
