    The Subtleties of Social Exclusion: Race, Social Class, and the Exclusion of Blacks in a Racially Mixed Neighborhood

    We use interviews, content analysis, and surveys to describe how a neighborhood association in a racially mixed neighborhood in Portland, Oregon (USA) subtly excludes many blacks from being full members of the neighborhood. In contrast to explicit cases of social exclusion, this neighborhood association excludes blacks without ever referring to race. They instead justify their actionsā€”e.g., helping close down a black social club and discouraging more affordable housingā€”based on such nonracial goals as increasing homeownership, minimizing crime, and maximizing ā€œeconomic diversity.ā€ We argue that without the inclusion of black residents and their neighborhood organizations (e.g., churches) in the decision-making process, mixed-race neighborhoods will continue to lose their diversity

    Excerpt from the Proceedings of the Nineteenth Annual Acquisition Research SymposiumThe U.S. defense acquisition system is positioning for strategic competition with China. That effort must be informed by and responsive to the nuances of Chinaā€™s global supply chain positioningā€”a competitive dynamic unique relative to past eras of great power competition. Updating for this reality demands a thorough understanding of how Beijing leverages its militaryā€“civil fusion (MCF) strategy to weaponize its manufacturing prowess, relative industrial self-reliance, and the asymmetric supply chain dependencies that result. The immediate security risks of Beijingā€™s approachā€”and the challenge it poses to the U.S. ability credibly to competeā€”have been evident since China cut off rare earths exports to Japan in the midst of a territorial dispute in 2010. Yet U.S. acquisition processes have not updated. The Pentagon, military services, and defense acquisition program officials must rethink frameworks for assessing supply chain integrities, the risks that dependencies all along acquisition program value chains can create, and responsive acquisition processes.     Day Napping and Short Night Sleeping Are Associated With Higher Risk of Diabetes in Older Adults

    OBJECTIVEā€”To examine whether day napping or short night sleeping is associated with higher risk of diabetes. RESEARCH DESIGN AND METHODSā€”This was a prospective study of hours of day napping and night sleeping assessed in 1996ā€“1997 in relation to diabetes diagnosed between 2000 and 2006 (n = 10,143) among 174,542 participants in the National Institutes of Health (NIH)-AARP Diet and Health Study. Odds ratios (ORs) and 95% CI were derived from multivariate logistic regression models. RESULTSā€”Longer day napping was associated with a higher risk of diabetes. After adjustment for potential confounders, ORs were 1.23 (95% CI 1.18ā€“1.29) for those reporting <1 h and 1.55 (95% CI 1.45ā€“1.66) for those reporting ā‰„1 h of napping compared with individuals who did not nap (PtrendP_{trend} < 0.0001). For night sleeping, with 7ā€“8 h as the referent, the OR was 1.46 (95% CI 1.31ā€“1.63) for <5 h, 1.11 (1.06ā€“1.16) for 5ā€“6 h, and 1.11 (0.99ā€“1.24) for ā‰„9 h. In both analyses, additional adjustment for BMI only modestly attenuated the associations. Further analysis showed a statistically significant interaction between hours of napping and sleeping on diabetes (PinteractionP_{interaction} < 0.0001). Among participants with no napping, only short night sleeping was associated with higher occurrence of diabetes, whereas among those with ā‰„1 h of napping, both long and short sleeping was associated with higher risk.     Circulating senescent myeloid cells drive blood brain barrier breakdown and neurodegeneration

    Neurodegenerative diseases (ND) are characterized by progressive loss of neuronal function. Mechanisms of ND pathogenesis are incompletely understood, hampering the development of effective therapies. Langerhans cell histiocytosis (LCH) is an inflammatory neoplastic disorder caused by hematopoietic progenitors expressing MAPK activating mutations that differentiate into senescent myeloid cells that drive lesion formation. Some patients with LCH subsequently develop progressive and incurable neurodegeneration (LCH-ND). Here, we show that LCH-ND is caused by myeloid cells that are clonal with peripheral LCH cells. We discovered that circulating BRAF V600E +^{+} myeloid cells cause the breakdown of the blood-brain barrier (BBB), enhancing migration into the brain parenchyma where they differentiate into senescent, inflammatory CD11a +^{+} macrophages that accumulate in the brainstem and cerebellum. Blocking MAPK activity and senescence programs reduced parenchymal infiltration, neuroinflammation, neuronal damage and improved neurological outcome in preclinical LCH-ND.     Somatic mutations in solid tumors: a spectrum at the service of diagnostic armamentarium or an indecipherable puzzle? The morphological eyes looking for BRAF and somatic molecular detections on cyto-histological samples

    This review article deals with the analysis and the detection of the morphological features associated with somatic mutations, mostly BRAF(V600E) mutation, on both cytological and histological samples of carcinomas. Few authors demonstrated that some architectural and specific cellular findings (i.e. polygonal eosinophilic cells defined as "plump cells" and sickle-shaped nuclei) are able to predict BRAF (V600E) mutation in both cytological and histological samples of papillary thyroid carcinoma (PTC) as well as in other carcinomas. In the current review article we evaluated the first comprehensive analysis of the morphological prediction of BRAFV600E and other somatic mutations in different malignant lesions with the description of the possible mechanisms beneath these morphologic features. The detection of predictive morphological features, mostly on FNAC, may add helpful information to the stratification of the malignant risk and personalized management of cancers. Additionally, the knowledge of the molecular mechanism of different oncogenic drivers can lead to the organ-specific triaging selection of cases and can provide significant insight for targeted therapies in different malignant lesions.info:eu-repo/semantics/publishedVersio

    The 5th edition of the World Health Organization classification of haematolymphoid tumours: myeloid and histiocytic/dendritic neoplasms

    The upcoming 5th edition of the World Health Organization (WHO) Classification of Haematolymphoid Tumours is part of an effort to hierarchically catalogue human cancers arising in various organ systems within a single relational database. This paper summarizes the new WHO classification scheme for myeloid and histiocytic/dendritic neoplasms and provides an overview of the principles and rationale underpinning changes from the prior edition. The definition and diagnosis of disease types continues to be based on multiple clinicopathologic parameters, but with refinement of diagnostic criteria and emphasis on therapeutically and/or prognostically actionable biomarkers. While a genetic basis for defining diseases is sought where possible, the classification strives to keep practical worldwide applicability in perspective. The result is an enhanced, contemporary, evidence-based classification of myeloid and histiocytic/dendritic neoplasms, rooted in molecular biology and an organizational structure that permits future scalability as new discoveries continue to inexorably inform future editions

    ALK-positive histiocytosis: a new clinicopathologic spectrum highlighting neurologic involvement and responses to ALK inhibition

    ALK-positive histiocytosis is a rare subtype of histiocytic neoplasm first described in 2008 in three infants with multisystemic disease involving the liver and hematopoietic system. This entity has subsequently been documented in case reports and series to occupy a wider clinicopathologic spectrum with recurrentĀ KIF5B-ALKĀ fusions. The full clinicopathologic and molecular spectra of ALK-positive histiocytosis remain, however, poorly characterized. Here, we describe the largest study of ALK-positive histiocytosis to date, with detailed clinicopathologic data of 39 cases, including 37 cases with confirmedĀ ALKrearrangements. The clinical spectrum comprised distinct clinical phenotypic groups: infants with multisystemic disease with liver and hematopoietic involvement, as originally described (Group 1A: 6/39), other patients with multisystemic disease (Group 1B: 10/39), and patients with single-system disease (Group 2: 23/39). Nineteen patients of the entire cohort (49%) had neurologic involvement (seven and twelve from Groups 1B and 2, respectively). Histology included classic xanthogranuloma features in almost one third of cases, whereas the majority displayed a more densely cellular, monomorphic appearance without lipidized histiocytes but sometimes more spindled or epithelioid morphology. Neoplastic histiocytes were positive for macrophage markers and often conferred strong expression of phosphorylated-ERK, confirming MAPK pathway activation.Ā KIF5B-ALKĀ fusions were detected in 27 patients, whileĀ CLTC-ALK,Ā TPM3-ALK, TFG-ALK,Ā EML4-ALKĀ andĀ DCTN1-ALKĀ fusions were identified in single cases. Robust and durable responses were observed in 11/11 patients treated with ALK inhibition, ten with neurologic involvement. This study presents the existing clinicopathologic and molecular landscape of ALK-positive histiocytosis, and provides guidance for the clinical management of this emerging histiocytic entity.Molecular tumour pathology - and tumour genetic
