    Radiofrequency ablation for treatment of varicose veins of lower limbs

Această prevalenţă înaltă este responsabilă pentru costuri sociale şi medicale majore. Scopul studiului a fost evaluarea rezultatului clinic al tratamentului chirurgical prin ablaţie cu radiofrecvenţă (RFA) la bolnavii cu varice ale membrelor inferioare. Material şi metode: O sută patruzeci şi doi de bolnavi cu vene varicoase au fost supuşi RFA al segmentului proximal de venă safenă magna cu ajutorul dispozitivului “VNUS Closure”™. Diametrul venei a variat între 5 şi 20 mm. Cateterul pentru RFA a fost introdus prin puncţie percutană sau o incizie cutanată minimă. Toate intervenţiile au fost realizate sub anestezia locală infiltrativă cu ghidaj ultrasonor. Au fost studiate schimbările morfologice la nivelul peretelui venei safene magna supuse tratamentului prin RFA.Rezultate: Profunzimea necrozei a fost determinată la nivelul stratului endotelial şi subendotelial cu detaşarea caracteristică a acestora, cu colabarea şi îngustarea lumenului vascular. Specificul particular reprezintă lipsa fenomenului de neoangiogeneză la nivelul joncţiunii safenofemurale şi safenopopliteale. Aşa complicaţii ca parestezii sau echimoze au fost observate doar în 5 cazuri. Merită de subliniat absenţa sindromului dolor semnificativ în grupul pacienţilor studiaţi. Toţi bolnavii au fost externaţi peste 2 ore după intervenţie. Concluzii: Metoda are următoarele avantaje: durata scurtă a incapacităţii de muncă, utilizarea anesteziei locale, rezultatul estetic bun, durata redusă a intervenţiei, rezultatele precoce şi la distanţă favorabile, posibilitatea de obliterare a venelor cu diametrul mare şi lipsa necesităţii în analgezie postoperatorie.Introduction: Varicose veins of the lower limbs are a common condition affecting 10-15% of men and 20-25% of women in the developed countries of western world. This high prevalence is responsible for high medical and social costs. The aim was to study the clinical results of surgical treatment of patients with varicose veins of lower limbs using radiofrequency ablation (RFA). Material and methods: One hundred forty two patients with varicose veins underwent RFA of the above knee great saphenous vein by “VNUS Closure”™ procedure. The vein diameters were from 5 to 20 mm. The RF catheter was inserted via percutaneous puncture or through a small skin incision. All operations were performed under local, tumescent anesthesia, with ultrasound guidance. We studied morphological changes of the wall of great saphenous vein in patients who underwent treatment with RFA. Results: The depth of necrosis was within endothelial and subendothelial layer, but detachment of endothelial and subendothelial layers was characteristic with collapse and narrowing of vessel lumen. The special feature was lack of neoangiogenesis at the level of saphenofemoral and safenopopliteal junction. Such complications as paresthesia or ecchymosis were observed only in 5 cases. It should be noted that the studied patients had no significant pain syndrome. All patients were discharged 2 h after operation. Conclusions: Advantages of the method are shorter sick leave, using of local tumescent anesthesia, good cosmetic result, shorter duration of surgery, good immediate and late results, possibility to close the veins with a large diameter and no use of postoperative analgesics

    В статье утверждается, что основные тенденции в развитии современной философии могут быть обозначены как прощание с матрицами и стандартами мышления классической философии. И хотя разрыв с классикой относят к ХIХ веку, некоторые тезисы постмодернизма, общие итоги лингвистического поворота в философии ХХ века действительно способствуют осуществлению такого прощания и переходу на позиции «новой антропологии» и «нового гуманизма» только сейчас.У статті стверджується, що основні тенденції розвитку сучасної філософії можуть бути позначені як прощання із матрицями та стандартами мислення класичної філософії. І хоча розрив із класикою відносять до ХІХ ст., деякі тези постмодернізму, загальні результати лінгвістичного повороту у філософії ХХ ст. дійсно сприяють здійсненню такого прощання і переходу на позиції «нової антропології» та «нового гуманізму» лише зараз.The article affirms, that the main tendencies in the development of contemporary philosophy can be designated as farewell to matrices and standards of thinking, which are connected with the classical philosophy. In spite of the fact, that the rupture between the classical and contemporary philosophy is removed usually to the XIX century, some thesis’s of the postmodern philosophy, common results of the linguistic turn in the philosophy of XX century really assist to the fulfillment of this farewell and to transition to the position of «the new anthropology» and «the new humanism» quite now

    РОЗБУДОВА МЕРЕЖ ІНДУСТРІАЛЬНИХ І НАЦІОНАЛЬНИХ ПРИРОДНИХ ПАРКІВ НА ПРИНЦИПАХ ВІДПОВІДАЛЬНОГО ІНВЕСТУВАННЯ В УКРАЇНІ

    The necessity of the forming and development of industrial and national natural parks networks in Ukraine in accordance with the leading world initiative for responsible investment PRI is proved in article. Both types of parks are among the most effective options for organizational solutions to the problem of attracting «green» and socially responsible investments into the country’s economy. The activity of national nature parks in Ukraine on the example of the Halytsky National Park and the Skole Beskids is investigated. At the same time, if national parks are created in order to promote ecologically balanced development; the main purpose of industrial parks is connected first of all with increase of economic efficiency of attracting investment in various directions of business activity. Considering the common features of parks as organizational forms of territorial integration of economy, environment and society, as well as instruments of attraction of investments, the directions of strengthening of their role in achieving the Sustainable development goals till 2030 in Ukraine are offered. Along with the problem of the development of the industrial parks network and the formation of the national natural parks network, the idea of a «functional intersection» of these networks is justified in order to search for the new opportunities for socially and environmentally responsible investment. General practical recommendations on improvement of functions for strategic planning, stimulation and monitoring of social and ecological activity in the territory of industrial and national parks according to the Principles for Responsible Investment are worked out. Namely settlement of issues of property on natural resources and recreational facilities; owners interest and initiative on attraction of «green» investment; ensuring an integrated approach — when monitoring measures on social and environmental activities in the park networks; development of an unified information resource where data on any activities of business entities and all stakeholders in the framework of the functioning and development of the park networks is collected and analyzed etc.Обґрунтована необхідність формування та розвитку мереж індустріальних і національних природних парків в Україні відповідно до провідної світової ініціативи щодо відповідального інвестування PRI. Обидва види парків є одними з найбільш ефективних варіантів організаційного розв’язання проблеми залучення «зелених» і соціально відповідальних інвестицій в економіку країни. Досліджено діяльність національних природних парків в Україні на прикладі парків: Галицького i Сколівських Бескид. При цьому якщо національні парки власне й створені для того, щоб сприяти екологічно збалансованому розвиткові, то головне призначення промислових парків пов’язане передусім із підвищенням економічної ефективності залучення інвестицій у різні напрями підприємницької діяльності. Ураховуючи спільні риси парків як організаційних форм територіальної інтеграції економіки, природи та соціуму, а також інструментів залучення інвестицій, запропоновано напрямки посилення їх ролі в досягненні Цілей сталого розвитку до 2030 року в Україні. Поряд із питаннями розвитку мережі промислових парків, а також формування мережі національних природних парків обґрунтовано ідею про «функціональний перетин» цих мереж із метою пошуку нових можливостей соціально та екологічно відповідального інвестування. Розроблено загальні практичні рекомендації щодо вдосконалення функцій стратегічного планування, стимулювання та моніторингу соціально-екологічної діяльності на території індустріальних і національних парків згідно з Принципами відповідального інвестування. А саме: врегулювання питань власності на природні ресурси та рекреаційні об’єкти; зацікавленість та ініціативність власників щодо залучення «зелених» інвестицій; забезпечення комплексного підходу — при моніторингу заходів щодо соціально-екологічної діяльності в паркових мережах; розробка єдиного інформаційного ресурсу, в якому централізовано збираються та аналізуються дані щодо будь-якої діяльності суб’єктів господарювання та всіх зацікавлених осіб у рамках функціонування і розвитку паркових мереж тощо

    Structure, optical, electrical and thermoelectric properties of solution-processed Li-doped NiO films grown by SILAR

    The article presents a new facial synthesis of Li-doped NiO films (NiO:Li) via an easy and cost-effective method Successive Ionic Layer Adsorption and Reaction (SILAR) with the processing of the obtained NiO films in a lithium-containing aqueous solution for their transformation after annealing into NiO:Li layers. Comparative analysis of crystal structure, optical, electrical and thermoelectric properties of the obtained NiO and NiO:Li 420-1050 nm thick films have reveiled a cubic rock-salt NiO structure, at that, NiO:Li samples are nanocrystalline single phased Li-NiO solid solutions. The fabricated NiO and NiO:Li films are p-type semiconductors with activation energy Ea = 0.1 eV and Ea = 0.25‒0.31 eV, respectively. The obtained in-plane Seebeck coefficients Z are in the range 0.20–0.33 mV/К. Notwithstanding the fact that the maximum values of the thermoelectric power factors P=2.2 μW/K2·m, are rather small, they were achieved if the hot end of the NiO:Li film was heated only to 115 °C. Thus, the produced in this work new low cost thermoelectric thin film material is suitable for a production of electrical energy for low-power devices due to absorption of low-potential heat

    Biological stimulation of diabetic ulcers healing

Aim: The aim of our study was toevaluatethe reparatory effect of platelet-rich plasma (PRP) in the treatment of patients with diabetic foot in stage 2. Materials and methods: The study included 32 patients with diagnosis of diabetic foot in stage 2. At the time of inclusion in the study, all patients had a defect of the skin and subcutaneous fat on the foot, measuring 3-6 cm2 , which was not corrected by traditional methods for 3-4 months. All patients were divided in 2 groups: comparison group (n=18 (56.25%) – standard therapy; study group (n=14;43.75%) – standard therapy and local serial administration of PRP. The area of the ulcers was measured before manipulation, as well as at 1 week intervals until the ulcer was completely healed. Results: Our experience with PRP in the treatment of patients with diabetic foot in stage 2 demonstrated the effectiveness of this technique. A progressive, significant (p<0.01) decrease in the area of the ulcerative defect starting from the 14th day was noted. When using PRP, we managed to achieve complete healing of diabetic ulcers in 64.2% of patients on day 35 and in 100% of patients on day 42. Conclusion: Local administration of an PRP allows the complete healing of diabetic ulcers in stage 2 with an area of 3-6 cm2 for 6 weeks

    Solution-processed flexible broadband ZnO photodetector modified by Ag nanoparticles

    In this work, we present flexible broadband photodetectors (PDs) fabricated by a deposition of nanostructured zinc oxide (ZnO) films on polyimide (PI) substrates by using cheap and scalable aqueous method Successive Ionic Layer Adsorption and Reaction (SILAR). In order to increase the long-wavelength absorption of the nanostructured ZnO layer, we created its intrinsic defects, including oxygen vacancies by post-treatment at 300 °C in vacuum and thus the light-sensitive material ZnO/PI was obtained. Then we applied silver nanoparticles (Ag NPs) from a silver sol onto a nanostructured ZnO film, which were visualized using SEM in the form of spheres up to 100 nm in size that increased the photocurrent and figures of merit of thus obtained light-sensitive material ZnO_Ag/PI due to localized surface plasmon resonance and double Schottky barriers at the Ag-ZnO interface. To fabricate photodetectors based on a photoconductive effect, these ZnO/PI and ZnO_Ag/PI materials were equipped with ohmic aluminum contacts. The spectral responsivity (Rλ up to 275 A/W to UV light) of solution-processed flexible broadband photodetector based on ZnO_Ag/PI material at different wavelengths of light and light power densities is better than Rλ of the ZnO/PI photodetector, and at least an order of magnitude higher than Rλ of photodetectors based on nanostructured zinc oxide described in recent articles. The external quantum efficiency (EQE) of both PDs in this study in UV–Vis-NIR spectra is very high in the range from 1∙102 to 9∙104 % and is better or of the same order of magnitude as the EQE data of modern flexible broadband high-sensitivity PDs based on nanostructured heterostructures containing ZnO. The specific detectivity in UV–Vis-NIR spectra is large for ZnO/PI (from 3.5∙1010 to 1∙1012 Jones) and especially for ZnO_Ag/PI (from 1.6∙1011 to 8.6∙1013 Jones), which indicates the ability of the PDs based on light-sensitive materials ZnO/PI and ZnO_Ag/PI to recognize a very weak light signal

    Superconducting spintronic devices based on nanostructures ferromagnet/superconductor

    The layered nanostructures ferromagnet/superconductor (F/S) due to combination of incompatible in homogeneous materials properties are the most perspective materials for use in the new field of electronics - a superconducting spintronics. A new type of logical devices based on the layered F/S nanostructures and combining the advantages of the superconducting and magnetic recording channels in one sample is offered. Each channel can be separately controlled by weak magnetic field or current pulse and the switching time is of order of 10-10 - 10-11 s. The implementation of such devices on base of high-temperature superconductors will allow using nitrogen instead of expensive helium for cooling. © Published under licence by IOP Publishing Ltd

    Prevention of rebleeding in patients with portal hypertension and esophageal variceal bleeding

The aim of the study was to evaluate the effectiveness of the use of embolization of the splenic artery in order to prevent portal bleeding. Methods and materials: The study included 96 patients, who had cirrhosis classes B and C (Child-Pugh) and esophageal variceal bleeding. Patients were divided into 2 groups: main group (n=71, 73.95%) – endoscopic ligating of bleeding and embolization of the splenic artery; comparison group (n=25, 26.05%) – only drug therapy. Results: In comparison group we stopped bleeding in 54 (76.1%) patients. 17 (23.9%) patients died. The duration of treatment was 10.1±2.4 days. In main group we stopped bleeding in 23 (92.0%) patients. 2 (8.0%) patients died. The duration of treatment was 6.5±2.7 days. A statistical analysis of mortality and duration of treatment revealed a significant difference (p<0.01) between the groups in both indicators. After splenic artery embolizationin all cases managed to achieve a reduction in blood flow of 60–80%. After 6 months among 54 patients in the comparison group, bleeding relapse occurred in 12 (22.2%) cases; in the main group – 2 (8.7%). Conclusion: Splenic artery embolization in patients after endoscopic hemostasis of variceal bleeding allows to reduce the pressure in the portal system, which in turn leads to a decrease in the frequency of bleeding recurrences

    Transient Growth Arrest in Escherichia coli Induced by Chromosome Condensation

Wrote the paper: ABK VVR.MukB is a bacterial SMC (structural maintenance of chromosome) protein that regulates the global folding of the Escherichia coli chromosome by bringing distant DNA segments together. We report that moderate overproduction of MukB may lead, depending on strain and growth conditions, to transient growth arrest. In DH5α cells, overproduction of MukB or MukBEF using pBAD expression system triggered growth arrest 2.5 h after induction. The exit from growth arrest was accompanied by the loss of the overproducing plasmid and a decline in the abundance of MukBEF. The arrested cells showed a compound gene expression profile which can be characterized by the following features: (i) a broad and deep downregulation of ribosomal proteins (up to 80-fold); (ii) downregulation of groups of genes encoding enzymes involved in nucleotide metabolism, respiration, and central metabolism; (iii) upregulation of some of the genes responsive to general stress; and (iv) degradation of the patterns of spatial correlations in the transcriptional activity of the chromosome. The transcriptional state of the MukB induced arrest is most similar to stationary cells and cells recovered from stationary phase into a nutrient deprived medium, to amino acid starved cells and to the cells shifting from glucose to acetate. The mukB++ state is dissimilar from all examined transcriptional states generated by protein overexpression with the possible exception of RpoE and RpoH overexpression. Thus, the transcription profile of MukB-arrested cells can be described as a combination of responses typical for other growth-arrested cells and those for overproducers of DNA binding proteins with a particularly deep down-regulation of ribosomal genes.Yeshttp://www.plosone.org/static/editorial#pee

Проаналізовано основні передумови розроблення концепції освіти впродовж життя у світі та соціально-економічні результати її упровадження. Викладено особливості та специфіку освіти впродовж життя, освіти дорослих як її складової в умовах регіонального розвитку (національний і зарубіжний досвід). Охарактеризовано психологічні особливості розвитку особистості в дорослому віці, а також професійного становлення особистості на різних етапах дорослості. Висвітлено роль неформальної освіти дорослих як чинника розвитку регіонів, що навчаються, а також механізми розроблення та реалізації концепції міста, регіону, що навчається. Інтерпретовано результати соціологічного дослідження культурно-освітніх потреб жителів Запорізької обл. та рівня їх задоволеності.The monograph describes the conceptual foundations of regional educational policy, objectified in the theoretical description of the main approaches, principles, conditions to implement the lifelong learning concept as a factor of regional development. The basic prerequisites for the development of lifelong learning concept in the world and the socio-economic results of its implementation are analyzed. The peculiarities and specifics of lifelong learning, education of adults as a part of it in the conditions of regional development (national and foreign experience) are described. The psychological peculiarities of personality development in adulthood as well as professional development of personality at different stages of adolescence are characterized. The role of non-formal adults education as a factor of learning regions development as well as mechanisms of the city development and implementation of the concept is highlighted. The results of sociological research of cultural and educationalneeds of Zaporizhzhya region's residents and their level of satisfaction are interpreted.В монографии освещены концептуальные основы региональной образовательной политики, объективированные в теоретическом описании основных подходов, принципов, условий реализации концепции непрерывного обучения как фактора регионального развития.
