    A new multi-modal database for developing speech recognition systems for an assistive technology application

    In this paper we report on the acquisition and content of a new database intended for developing audio-visual speech recognition systems. This database supports a speaker dependent continuous speech recognition task, based on a small vocabulary, and was captured in the European Portuguese language. Along with the collected multi-modal speech materials, the respective orthographic transcription and time-alignment files are supplied. The package also includes data on stochastic language models and the generative grammar associated to the collected spoken sentences. The application addressed by this database, which consists of voice control of a basic scientific calculator, has the particularity of being designed for a person with a specific motor impairment, namely muscular dystrophy. This specificity is a remarkable characteristic, given the lack of such kind of data resources for developing assistive systems based on audio-visual speech recognition technology

    Sixteen overlooked open clusters in the fourth Galactic quadrant

    Aims. This paper has two main objectives: (1) To determine the intrinsic properties of 16 faint and mostly unstudied open clusters in the poorly known sector of the Galaxy at 270\u25e6 12300\u25e6 to probe the Milky Way structure in future investigations. (2) To address previously reported systematics in Gaia DR2 parallaxes by comparing the cluster distances derived from photometry with those derived from parallaxes. Methods. Deep UBVI photometry of 16 open clusters was carried out. Observations were reduced and analyzed in an automatic way using the ASteCA package to obtain individual distances, reddening, masses, ages, and metallicities. Photometric distances were compared to those obtained from a Bayesian analysis of Gaia DR2 parallaxes. Results. Ten out of the sixteen clusters are true or highly probable open clusters. Two of them are quite young and follow the trace of the Carina Arm and the already detected warp. The remaining clusters are placed in the interarm zone between the Perseus and Carina Arms, as expected for older objects. We found that the cluster van den Berg-Hagen 85 is 7.5 7 109 yr old, which means that it is one of the oldest open clusters detected in our Galaxy so far. The relationship of these ten clusters with the Galaxy structure in the solar neighborhood is discussed. The comparison of distances from photometry and parallaxes data in turn reveals a variable level of disagreement. Conclusions. Various zero-point corrections for Gaia DR2 parallax data recently reported were considered for a comparison between photometry- and parallax-based distances. The results tend to improve with some of these corrections. Photometric distance analysis suggests an average correction of 3c+0.026 mas (to be added to the parallaxes). The correction may have a more intricate dependence on distance, but addressing this level of detail will require a larger cluster sample

    Nanocomposite MFI-alumina and FAU-alumina Membranes: Synthesis, Characterization and Application to Paraffin Separation and CO2 Capture

A.International audienceIn this work, we report the preparation of thermally and mechanically resistant high-surface (24-cm2) nanocomposite MFI-alumina and FAUalumina membranes by pore-plugging synthesis inside the macropores of α-alumina multilayered tubular supports. The MFI membranes were prepared from a clear solution precursor mixture being able to easily penetrate into the pores of the support. The MFI membranes were evaluated in the separation of n-/i-butane mixtures. The synthesis reliability was improved by mild stirring. The most selective MFI membranes were obtained for supports with mean pore sizes of 0.2 and 0.8 μm. The MFI effective thickness could be reduced to less than 10 μm by impregnating the support with water prior to synthesis and by diluting the synthesis mixture. The best MFI membrane offered an excellent tradeoff between selectivity and permeance at 448 K, with separation factors for equimolar n-butane/i-butane mixtures up to 18 and n-butane mixture permeances as high as 0.7 μmol⋅\cdots-1⋅\cdotm-2⋅\cdotPa-1.Furthermore, a novel nanocomposite FAU membrane architecture has been obtained by an original synthesis route including in situ seeding using a cold gel-like precursor mixture, followed by growth of the FAU material by hydrothermal synthesis in two steps using a clear solution of low viscosity. This new membrane showed interesting performance in the separation of an equimolar CO2/N2 mixture at 323 K, with CO2/N2 separation factors and mixture CO2 permeances up to 12 and 0.4 μmol⋅\cdots-1⋅\cdotm-2⋅\cdotPa-1,respectively

    Diagnosis of right bundle branch block: a concordance study

Most cases of right bundle branch block are detected in asymptomatic patients in primary care, so a correct interpretation of electrocardiograms (ECGs) at this level is necessary. The objective of this research is to determine the degree of concordance in the diagnosis of incomplete and complete right bundle branch block between four primary care researchers and a cardiologist. METHODS: The research design is a retrospective cohort study of patients over 18 years of ages of patients over 18 years of ages who underwent an ECG for any reason and were diagnosed with right bundle branch block by their physician. The physicians participating, 4 primary care researchers and a cardiologist were specialized in interpreting electrocardiographic records. The diagnosis of incomplete and complete right bundle branch block was recorded and other secondary variables were analysed. In case of diagnostic discordance between the researchers, the ECGs were reviewed by an expert cardiologist, who interpreted them, established the diagnosis and analysed the possible causes for the discrepancy. RESULTS: We studied 160 patients diagnosed with right bundle branch block by their general practise. The patients had a mean age of 64.8 years and 54% of them were men. The concordance in the diagnosis of incomplete right bundle branch block showed a Fleiss' kappa index (k) of 0.71 among the five researchers and of 0.85 among only the primary care researchers. The k for complete right bundle branch block was 0.93 among the five researchers and 0.96 among only the primary care researchers. CONCLUSION: The interobserver agreement in the diagnosis of right bundle branch block performed by physicians specialized in ECG interpretation (primary care physicians and a cardiologist) was very good. The variability was greater for the diagnosis of incomplete right bundle branch block

    Sex chromosome positions in human interphase nuclei as studied by in situ hybridization with chromosome specific DNA probes

    Two cloned repetitive DNA probes, pXBR and CY1, which bind preferentially to specific regions of the human X and Y chromosome, respectively, were used to study the distribution of the sex chromosomes in human lymphocyte nuclei by in situ hybridization experiments. Our data indicate a large variability of the distances between the sex chromosomes in male and female interphase nuclei. However, the mean distance observed between the X and Y chromosome was significantly smaller than the mean distance observed between the two X-chromosomes. The distribution of distances determined experimentally is compared with three model distributions of distances, and the question of a non-random distribution of sex chromosomes is discussed. Mathematical details of these model distributions are provided in an Appendix to this paper. In the case of a human translocation chromosome (XqterXp22.2::Yq11Y qter) contained in the Chinese hamster x human hybrid cell line 445 x 393, the binding sites of pXBR and CY1 were found close to each other in most interphase nuclei. These data demonstrate the potential use of chromosome-specific repetitive DNA probes to study the problem of interphase chromosome topography

    Real-world effects of antibiotic treatment on acute COPD exacerbations in outpatients: a cohort study under the PharmLines initiative

    Background: Although antibiotic treatment is recommended for acute exacerbations of chronic obstructive pulmonary disease (AECOPD), its value in real-world settings is still controversial. Objectives: This study aimed to evaluate the short- and long-term effects of antibiotic treatment on AECOPD outpatients. Methods: A cohort study was conducted under the PharmLines Initiative. We included participants with a first recorded diagnosis of COPD who received systemic glucocorticoid treatment for an AECOPD episode. The exposed and reference groups were defined based on any antibiotic prescription during the AECOPD treatment. The short-term outcome was AECOPD treatment failure within 14-30 days after the index date. The long-term outcome was time to the next exacerbation. Adjustment for confounding was made using propensity scores. Results: Of the 1,105 AECOPD patients, antibiotics were prescribed to 518 patients (46.9%) while 587 patients (53.1%) received no antibiotics. The overall antibiotic use was associated with a relative risk reduction of AECOPD treatment failure by 37% compared with the reference group (adjusted odds ratio [aOR] 0.63 [95% CI: 0.40-0.99]). Protective effects were similar for doxycycline, macrolides, and co-amoxiclav, although only the effect of doxycycline was statistically significant (aOR 0.53 [95% CI: 0.28-0.99]). No protective effect was seen for amoxicillin (aOR 1.49 [95% CI: 0.78-2.84]). The risk of and time to the next exacerbation was similar for both groups. Conclusion: Overall, antibiotic treatment, notably with doxycycline, supplementing systemic glucocorticoids reduces short-term AECOPD treatment failure in real-world outpatient settings. No long-term beneficial effects of antibiotic treatment on AECOPD were found for the prevention of subsequent exacerbations.Clinical epidemiolog

    Intravenous methylprednisolone pulse therapy for children with epileptic encephalopathy

    The aim of this retrospective study of children affected by epileptic encephalopathy was to evaluate seizure frequency, electroencephalographic pattern and neuropsychological status, before and after intravenous methylprednisolone therapy. Eleven children with epileptic encephalopathy were administered one cycle of intravenous methylprednisolone (15-30 mg/kg/day for three consecutive days, once a month for four months) in addition to constant dosages of their regular antiepileptic drugs. The treatment resulted in statistically significant reductions of generalized slow spike-and-wave discharges (p<0.0028) and seizure frequency (p<0.013), which persisted even after methylprednisolone pulse therapy was stopped. A globally positive outcome was noted in 9/11 patients (81.8%). This methylprednisolone treatment regimen did not cause significant or persistent adverse effects. We suggest that children with epileptic encephalopathy without an underlying structural lesion could be the best candidates for intravenous methylprednisolone pulse therapy

    Revised North Star Ambulatory Assessment for Young Boys with Duchenne Muscular Dystrophy

    The advent of therapeutic approaches for Duchenne muscular dystrophy (DMD) has highlighted the need to identify reliable outcome measures for young boys with DMD. The aim of this study was to develop a revised version of the North Star Ambulatory Assessment (NSAA) suitable for boys between the age of 3 and 5 years by identifying age appropriate items and revising the scoring system accordingly. Using the scale in 171 controls between the age of 2.9 and 4.8 years, we identified items that were appropriate at different age points. An item was defined as age appropriate if it was completed, achieving a full score, by at least 85% of the typically developing boys at that age. At 3 years (±3months) there were only 8 items that were age appropriate, at 3 years and 6 months there were 13 items while by the age of 4 years all 17 items were appropriate. A revised version of the scale was developed with items ordered according to the age when they could be reliably performed. The application of the revised version of the scale to data collected in young DMD boys showed that very few of the DMD boys were able to complete with a full score all the age appropriate items. In conclusion, our study suggests that a revised version of the NSAA can be used in boys from the age of 3 years to obtain information on how young DMD boys acquire new abilities and how this correlates with their peers

    Age and baseline values predict 12 and 24-month functional changes in type 2 SMA

    The aim of this retrospective study was to establish the range of functional changes at 12 and 24-month in 267 type 2 Spinal Muscular Atrophy (SMA) patients with multiple assessments. We included 652 Hammersmith Functional Motor Scale Expanded (HFMSE) assessments at 12 month- and 305 at 24 month- intervals. The cohort was subdivided by functional level, Survival of Motor Neuron copy number and age. Stable scores (± 2 points) were found in 68% at 12 months and in 55% at 24 months. A decrease ≥2 points was found in 21% at 12 months and in 35% at 24 months. An increase ≥2 points was found in 11% at 12 months and 9.5% at 24 months. The risk of losing ≥2 points increased with age and HFMSE score at baseline both at 12 and 24-month. For each additional HFMSE point at baseline, the relative risk of a >2 point decline at 12 months increases by 5% before age 5 years (p = 0.023), by 8% between 5 and 13 (p<0.001) and by 26% after 13 years (p = 0.003). The combination of age and HFMSE scores at baseline increased the ability to predict progression in type 2 SMA
