    Background Demographic development and increasing long time sick leave is arousing concern. Many people experience musculoscelatal problems or mental disorders, and the tendency of medicalizing is debated. Many patients with these problems get help through Norwegian Psychomotor Physiotherapy (NPMP). This study aims to enlighten the priciple of self-help in NPMP by a movie and relevant theory. Method and material Observation is the chosen method and the material is a NPMP treatment movie. The movie is roughly transcribed and four sequences deeply analyzed. The attention has been on how bodily exeriences is described in relation to emotional and existential aspects, and concretized. Results - What the therapist notices is important, but by emphasizing the patient’s experiences and inputs, important information arrives. - What the patient notices bodyly is contextualized by what is important in life. - In different situations the therapist promotes her actions to do something about the experienced discomfort. Conclusion How the physiotherapist and the patient interrelate is crucial for the patients body to function as source of insight. How the physiotherapist fills the leader role influences how far the patient can participate activly. Through observation of the patient the physioterapist can get information about what touches and is important to the patient

    Association of smoking and cancer with the risk of venous thromboembolism: the Scandinavian Thrombosis and Cancer cohort

    Smoking is a well-established risk factor for cancer, and cancer patients have a high risk of venous thromboembolism (VTE). Conflicting results have been reported on the association between smoking and risk of VTE, and the effect of smoking on VTE-risk in subjects with cancer is scarcely studied. We aimed to investigate the association between smoking and VTE in subjects with and without cancer in a large population-based cohort. The Scandinavian Thrombosis and Cancer (STAC) cohort included 144,952 participants followed from 1993–1997 to 2008–2012. Information on smoking habits was derived from self-administered questionnaires. Active cancer was defined as the first two years following the date of cancer diagnosis. Former smokers (n = 35,890) and those with missing information on smoking status (n = 3680) at baseline were excluded. During a mean follow up of 11 years, 10,181 participants were diagnosed with cancer, and 1611 developed incident VTE, of which 214 were cancer-related. Smoking was associated with a 50% increased risk of VTE (HR 1.49, 95% CI 1.12–1.98) in cancer patients, whereas no association was found in cancer-free subjects (HR 1.07, 95% CI 0.96–1.20). In cancer patients, the risk of VTE among smokers remained unchanged after adjustment for cancer site and metastasis. Stratified analyses showed that smoking was a risk factor for VTE among those with smoking-related and advanced cancers. In conclusion, smoking was associated with increased VTE risk in subjects with active cancer, but not in those without cancer. Our findings imply a biological interaction between cancer and smoking on the risk of VTE

    Nursing sensitive quality indicators for nursing home care: international review of literature, policy and practice

    Objectives: To review nursing sensitive indicators used for nursing home care across seven nations with similar elder care (USA, Australia, Norway, New Zealand, England, Sweden and Denmark), and to evaluate their validity. Design: Systematic search in the literature and other sources to find descriptions of development and validity testing of national quality indicators. Data sources: Papers from scientific databases, relevant websites, additional papers and reports, and personal communication with experts in the field. The material was included if it contributed to the description of each country\u27s processes in defining nursing sensitive quality indicators for nursing home care, and the main focus was use, developing and/or testing Of quality. Review methods: An overview of each country\u27s utilization of nursing sensitive quality indicators was obtained. The evidence for the validity in development and testing Procedures was analyzed using a set of evaluation criteria. Results: All Countries, except Sweden, have nationally standardized assessment of the patient before admission to the nursing home. There is large variation in the way these data collection tools were developed and how the data is used. Only the USA has systematically developed quality indicators on the basis of resident assessments. Twenty-three indicators used nationally in USA, thirteen in Australia, four in Norway, three in New Zealand and three in England were selected for review, and were evaluated for their validity as described in the literature. All selected indicators had satisfactory face validity, and for the twenty-three indicators used in the USA there was evidence for reliability testing. None of the quality indicators met all the criteria for validity. Evidence that the quality indicators can demonstrate meaningful differences in care and that the information can be extracted with minimal extra efforts was not found. Thresholds for high or low quality were determined only for the US quality indicators. Conclusions: There are concerns about the validity and reliability of nursing sensitive quality indicators for nursing home care. The indicator development is sparsely documented. It is recommended that the development of quality indicators follows a sound process and that extensive empirical testing of the indicators is done. (C) 2008 Elsevier Ltd. All rights reserved

    Denne samarbeidsrapporten fra SINTEF og NIBIO viser status og utfordringer for økonomisk verdiskaping i landbruk og landbruksrelatert virksomhet i Trøndelag. Totalt ga landbruket grunnlag for 17,4 milliarder kroner i verdiskaping i 2021. Jordbruket ga grunnlag for 12,2 milliarder i verdiskaping fordelt ganske likt på primærledd (inkludert tilleggsnæringer), foredlingsindustri og øvrige ringvirkninger i økonomien. Skogbruket ga grunnlag for 5 milliarder i verdiskaping fordelt med 0,9 milliarder på primærleddet, 2,6 milliarder i foredlingsindustrien og 1,6 milliarder i øvrige ringvirkninger. Reindrift ga grunnlag for totalt 90 millioner kroner i verdiskaping fordelt med 37 millioner kroner på primærledd, 33 millioner på foredlingsindustri og 21 millioner kroner i øvrige ringvirkninger. Blant store utfordringer for framtidig verdiskaping er det å ta vare på viktige arealer og å legge til rette for at de ulike delene av landbruk og landbruksrelatert virksomhet kan få en mer økonomisk, bærekraftig drift.     D-Dimer Measured At First Venous Thromboembolism Is Associated With Future Risk Of Cancer

    D-dimer >5000 ng/ml at incident VTE is associated with a higher risk of subsequent cancer within one and two years. High D-dimer levels are also associated with more aggressive tumor biology and poor prognosis in these patients. As D-dimer is routinely measured in the assessment of suspected VTE, it may be a useful surrogate marker for the presence of an underlying malignancy. Our findings may suggest that high plasma D-dimer at incident VTE diagnosis should be taken into consideration when the decision to screen for underlying cancer is made

    Background - The role of combined prothrombotic genotypes in cancer‐related venous thromboembolism (VTE) is scarcely studied. We aimed to investigate the impact of a 5‐single nucleotide polymorphism (SNP) score on the risk of VTE in patients with and without cancer using a population‐based case‐cohort. Methods - Cases with a first VTE (n = 1493) and a subcohort (n = 13 072) were derived from the Tromsø Study (1994‐2012) and the Nord‐Trøndelag Health Study (1995‐2008). Five SNPs previously reported as a risk score were genotyped: ABO (rs8176719), F5 (rs6025), F2 (rs1799963), FGG (rs2066865), and F11 (rs2036914). Hazard ratios (HRs) for VTE were estimated according to cancer status and the number of risk alleles in the 5‐SNP score (0‐1, 2‐3, and ≥4 alleles). Results - During a median follow‐up of 12.3 years, 1496 individuals were diagnosed with cancer, of whom 232 experienced VTE. The VTE risk increased with the number of risk alleles in the 5‐SNP score among subjects without and with cancer. In cancer‐free subjects, the HR was 2.17 (95% confidence interval [CI] 1.79‐2.62) for ≥4 versus 0‐1 risk alleles. In cancer patients, the corresponding HR was 1.93 (95% CI 1.28‐2.91). The combination of cancer and ≥4 risk alleles yielded a 17‐fold (HR 17.1, 95% CI 12.5‐23.4) higher risk of VTE compared with cancer‐free subjects with 0‐1 risk alleles. Conclusion - The risk of VTE increases with the number of prothrombotic risk alleles in subjects with and without cancer, and the combination of prothrombotic risk alleles and cancer leads to a highly elevated risk of VTE

    Genetic Variation Of Platelet Glycoprotein VI And The Risk Of Venous Thromboembolism

    Family studies have indicated that heritability explains 50-60% of the venous thromboembolism (VTE) events, and in recent years, several single nucleotide polymorphisms (SNPs) have been found to influence the VTE risk. Glycoprotein 6 (GP6) rs1613662, also known as T13254C, is an A/G single nucleotide variation in amino acid 219, which results in a serine to proline substitution affecting the glycoprotein VI (GPVI) receptor for collagen. Platelets carrying the minor allele (G-allele/Pro219) at GP6 rs1613662 express fewer GPVI receptors, which leads to attenuated platelet adhesion and activation. Previous observational studies in selected populations have consistently demonstrated that carriers of the A-allele at GP6 have a 15% higher risk of VTE than non-carriers, and inversely, that G-allele carriers have a 20% lower VTE risk
