    Because the national legislation prohibits the sale of incandescent lamps the argentine citizen can acquire compact fluorescent lamps in their replacement. This situation has led us to develop this work whose main objective is to establish a comparison between these two types of lamps from the energy point of view and its environmental consequences. To this end, we have developed a study based on the last national census in Argentina. While significant energy savings achieved, and as a result lower emissions of carbon dioxide, this leads to an environmental problem due to the mercury content of compact fluorescent lamps. They become in hazardous waste when exhausted their useful lives and are discarded in sites for municipal solid waste. High probability that the lamps are broken and release mercury that can contaminate 19,55 liters of drinking water for kilogram of CO2 saved. We conclude that the regulations required to purchase compact fluorescent lamps instead of traditional incandescent lamps must go accompanied with sustainable alternatives to dispose them appropriately since otherwise we will be replacing an environmental problem with another

    The role of the occupational therapist in disaster areas: systematic review

    Background. Disasters are increasingly more frequent events on our planet. During disaster the role of the occupational therapist will require a more specific operative framework within nongovernmental organizations and community health services. Design. Systematic review. Objective. The aim of this study is to evaluate the evidence that highlight occupational therapist’s role in disaster area through a systematic review. Materials and Methods. Research on MEDLINE was performed. All articles from 2005 to 2015 concerning rehabilitation and occupational therapy in disaster areas were included. Results. Ten studies were selected to be included in this review. Four interesting points emerged: the importance of having rehabilitation intervention in postdisaster situations, the necessity to include a rehabilitation team in the early phase of disaster response, the need to provide a method to address the difficult evacuation, and finding the safest method of transport of people with preexisting disabilities and new injuries. Conclusions. The amount of evidence with respect to specific intervention of the occupational therapist’s role in a disaster situation is limited. However some evidence suggests that it could be a good means for reducing the number of medical complications and deaths of persons with preexisting disabilities. The evidences found highlight the necessity to create a multidisciplinary team addressing needs in disasters situation, in which the occupational therapist could certainly contribute

    Using a weaning immunosuppression protocol in liver transplantation recipients with hepatocellular carcinoma: A compromise between the risk of recurrence and the risk of rejection?

    Hepatocellular carcinoma (HCC) recurrence rate after liver transplantation (LT) is still up to 1520%, despite a careful selection of candidates and optimization of the management within the waiting list. To reduce tumour recurrence, the currently adopted post-transplant strategies are based on the administration of a tailored immunosuppression (IS) regimen. Drug-induced depression of the immune system is essential in preventing graft rejection, however has a well-established association with oncogenesis. The immune system has a key role as a defending mechanism against cancer development, preventing vascular invasion and metastasis. Thus, IS drugs represent one of few modifiable non-oncological risk factors for tumour recurrence. In HCC recipients, a tailored IS therapy, with the aim to minimize drugs' doses, is essential to gain the optimal balance between the risk of rejection and the risk of tumour recurrence. So far, a complete withdrawal of IS drugs after LT is reported to be safely achievable in 25% of patients (defined as "operational tolerant"), without the risk of patient and graft loss. The recent identification of non-invasive "bio-markers of tolerance", which permit to identify patients who could successfully withdraw IS therapies, opens new perspectives in the management of HCC after LT. IS withdrawal could potentially reduce the risk of tumour recurrence, which represents the major drawback in HCC recipients. Herein, we review the current literature on IS weaning in patients who underwent LT for HCC as primary indication and we report the largest experiences on IS withdrawal in HCC recipients

    Nitrosative stress defences of the enterohepatic pathogenic bacterium Helicobacter pullorum

    Helicobacter pullorum is an avian bacterium that causes gastroenteritis, intestinal bowel and hepatobiliary diseases in humans. Although H. pullorum has been shown to activate the mammalian innate immunity with release of nitric oxide (NO), the proteins that afford protection against NO and reactive nitrogen species (RNS) remain unknown. Here several protein candidates of H. pullorum, namely a truncated (TrHb) and a single domain haemoglobin (SdHb), and three peroxiredoxin-like proteins (Prx1, Prx2 and Prx3) were investigated. We report that the two haemoglobin genes are induced by RNS, and that SdHb confers resistance to nitrosative stress both in vitro and in macrophages. For peroxiredoxins, the prx2 and prx3 expression is enhanced by peroxynitrite and hydrogen peroxide, respectively. Mutation of prx1 does not alter the resistance to these stresses, while the single ∆prx2 and double ∆prx1∆prx2 mutants have decreased viability. To corroborate the physiological data, the biochemical analysis of the five recombinant enzymes was done, namely by stopped-flow spectrophotometry. It is shown that H. pullorum SdHb reacts with NO much more quickly than TrHb, and that the three Prxs react promptly with peroxynitrite, Prx3 displaying the highest reactivity. Altogether, the results unveil SdHb and Prx3 as major protective systems of H. pullorum against nitrosative stress

    Social Security Reforms, Retirement and Sectoral Decisions

    In many countries, the regulations governing public and private pension systems, hiring procedures, and job contracts differ. Public sector employees tend to have longer tenures and higher wages compared to workers in the private sector. As such, social security reforms can affect both retirement decisions and sectoral choices. We study the effects of social security reforms on retirement and sectoral behavior in an economy with multiple pension systems. We develop a life-cycle model with three sectors - private formal, private informal and public - and endogenous retirement. In a model calibrated to Brazil, we quantitatively assess the long-run effects of reforms being discussed and implemented across countries. Among them, we study the unification of pension systems and increasing the minimum retirement age. We find that these reforms affect the decision to apply to a public job, the profile of savings over the life cycle, and informality. In the long run, these reforms lead to higher output and capital, reduced informality, and average welfare gains. They also drastically reduce the social security deficit

    O conhecimento da cinemática da coluna cervical é uma ferramenta muito importante, dado que esta região é uma das estruturas mais complexas do esqueleto humano e porque a incidência de incapacidade que pode resultar de traumas ou doenças degenerativas é relativamente elevada. O principal objetivo deste estudo consiste na análise e comparação da fusão cervical anterior e/ou fusão cervical posterior como possíveis tratamentos para uma fratura do tipo C2.2, segundo a classificação da AO spine injury classification system, sendo que este estudo poderá auxiliar os profissionais de saúde na escolha do melhor método de fusão. Para o efeito, foi construído um modelo 3 D de elementos finitos do segmento cervical C4‐C5‐C6. Foram modelados os respetivos discos intervertebrais (IV ) e os seus componentes, nomeadamente o núcleo pulposo, o anel fibroso, as fibras lamelares e as placas cartilaginosas. Os 6 conjuntos de ligamentos (longitudinais anteriores, longitudinais posteriores, interespinhosos, supraespinhosos, amarelos e capsulares) foram também modelados, assim como as articulações intervertebrais. A simulação de uma fratura foi realizada em 3 passos distintos: introdução de uma fratura na zona anterior da vértebra C5, o comprometimento (rompimento) dos ligamentos da região posterior e o deslizamento da vértebra C5. A instrumentação utilizada nos tratamentos em estudo e ainda a substituição dos discos lesados por um enxerto ósseo proveniente do ilíaco foram também modeladas. Com este estudo, concluiu‐se que a fusão cervical anterior e posterior apresenta melhores resultados em comparação com os outros tipos de fusão cervical estudados em termos de deslocamento da unidade funcional.The knowledge of the kinematics of the cervical spine is a very important tool, since this region is one of the more complex structures of the human skeleton and because the incidence of failure that may result from trauma or degenerative diseases is relatively high. The main objective of this study is the analysis and comparison of anterior cervical fusion and/or posterior cervical fusion as possible treatments for a fracture of type C2.2 according to AO spine injury classification system, and this study may assist health professionals in choosing the best fusion technique. To this end we built a 3 D finite element model of the cervical segment C4‐C5‐C6. Their intervertebral discs (IV ) and its components, namely the nucleus pulposus, the annulus fibrosus, the fibers and the lamellar cartilaginous plates were modeled. Six sets of ligaments (anterior longitudinal, posterior longitudinal, interspinous, suprespinous, yellow and capsular) and the facet joints were also modeled. The simulation of the fracture was performed in 3 different steps: introduction of a fracture zone in the previous C5 vertebra, rupture of the ligaments of the posterior region and the sliding of the C5 vertebra. The instrumentation used in the study treatments and even the replacement of the damaged disc by bone graft from the iliac bone, were also modeled. Analyzing the displacements of the functional unit it was concluded that the anterior and posterior cervical fusion provides the best results compared to other types of cervical fusion studied in terms of displacement of the functional unit.Peer Reviewe
