    Variantes de Smqnr de isolados clínicos de Stenotrophomonas maltophilia no Brasil

    SUMMARY Stenotrophomonas maltophilia contains a novel chromosomally-encoded qnr gene named Smqnr that contributes to low intrinsic resistance to quinolone. We described Smqnr in 13 clinical isolates of S. maltophilia from two Brazilian hospitals, over a 2-year period. The strains were identified by API 20 NE (bioMérieux, France). Susceptibility by microdilution method to trimetroprim/sulfamethoxazole, ciprofloxacin, levofloxacin, minocycline, ceftazidime, chloramphenicol and ticarcillin/clavulanate was performed according to CLSI. PCR detection of Smqnr gene was carried out. The sequence of Smqnr was compared with those deposited in GenBank. Pulsed-field gel electrophoresis (PFGE) of all strains was performed. Thirteen Smqnr positives isolates were sequenced and three novel variants of Smqnr were identified. All 13 Smqnr isolates had distinguishable patterns by PFGE. This is the first report of Smqnr in S. maltophilia isolated in Brazil.RESUMO S. maltophilia contem um novo gene qnr cromossômico denominado Smqnr que contribui para baixa resistência intrínseca a quinolonas. Descrevemos Smqnr em 13 isolados clínicos de S. maltophilia de dois hospitais brasileiros, ao longo do período de dois anos. Os isolados foram identificados pela API 20 NE (bioMérieux, França). Susceptibilidade pelo método de microdiluição dos seguintes antibióticos trimetroprim/sulfametoxazol, ciprofloxacina, levofloxacina, minociclina, ceftazidima, cloranfenicol e ticarcilina/clavulanato foi realizada segundo o CLSI. Detecção do gene de Smqnr foi realizada por PCR. A sequência de Smqnr foi comparada com aquelas depositadas no GenBank. Foi realizada eletroforese em gel de campo pulsado (PFGE) de todos os isolados. Treze isolados contendo Smqnr foram sequenciados e identificados três variantes do gene Smqnr. Todos os 13 isolados de Smqnr apresentaram diferentes padrões por PFGE. Este é o primeiro relato de Smqnr em isolados de S. maltophilia no Brasil

    Association of HO-1 and BRCA1 is critical for the maintenance of cellular homeostasis in prostate cancer

    Prostate cancer is the second leading cause of cancer-related death in men worldwide. Many factors that participate in the development of prostate cancer promote imbalance in the redox state of the cell. Accumulation of reactive oxygen species causes injury to cell structures, ultimately leading to cancer development. The antioxidant enzyme heme oxygenase 1 (HMOX1/HO-1) is responsible for the maintenance of the cellular homeostasis, playing a critical role in the oxidative stress and the regulation of prostate cancer development and progression. In the present study, the transcriptional regulation of HO-1 was investigated in prostate cancer. Interestingly, the tumor suppressor BRCA1 binds to the HO-1 promoter and modulates HO-1, inducing its protein levels through both the increment of its promoter activity and the induction of its transcriptional activation. In addition, in vitro and in vivo analyses show that BRCA1 also controls HO-1-negative targets: MMP9, uPA, and Cyclin D1. HO-1 transcriptional regulation is also modulated by oxidative and genotoxic agents. Induction of DNA damage by mitoxantrone and etoposide repressed HO-1 transcription, whereas hydrogen peroxide and doxorubicin induced its expression. Xenograft studies showed that HO-1 regulation by doxorubicin also occurs in vivo. Immunofluorescence analysis revealed that BRCA1 overexpression and/or doxorubicin exposure induced the cytoplasmic retention of HO-1. Finally, the transcription factor NRF2 cooperates with BRCA1 protein to activate HO-1 promoter activity. In summary, these results show that the activation of BRCA1-NRF2/HO-1 axis defines a new mechanism for the maintenance of the cellular homeostasis in prostate cancer.Fil: Labanca, Estefania.     No AplicaEl conflicto armado colombiano ha desarrollado una guerra asimétrica que interfiere en el desarrollo integral y calidad de vida de las víctimas, familias y sus comunidades, generando víctimas de toda clase de hechos victimizantes. En el presente documento se ofrecen reflexiones sobre el acompañamiento psicosocial, desde los aprendizajes, competencias y habilidades obtenidos en el diplomado de profundización - Acompañamiento Psicosocial en Escenarios de Violencia. El documento presenta de forma holística los relatos de dos casos; el primero expone el “análisis de relatos de historias que retornan” Capítulo 3- Amparo, de la Comisión de la Verdad, en el cual la protagonista es una mujer que ha tenido que experimentar la desaparición forzada de su esposo, ella se reconoce como una sobreviviente que a pesar de la lucha tan fuerte por hacer justicia, decide continuar el camino que ha construido no sólo por ella misma, sino por quienes también han sufrido el mismo daño, la resiliencia la llevó a graduarse y ser una gran abogada que luchó por crear una ley que busca dar alivio a tantas familias. A partir del relato se construyeron preguntas circulares, reflexivas y estratégicas argumentadas desde el enfoque psicosocial con el fin de conocer la perspectiva individual y colectiva. En el segundo caso sobre la Masacre en el Salado: “Relatos de Resiliencia después de 20 Años”, de la casa editorial El Tiempo, se comparte la narrativa de los sobrevivientes del hecho violento ocurrido entre el 16 y 22 de febrero del año 2000, a causa de las Autodefensas Unidas de Colombia (AUC) en el corregimiento de El Salado, con base en ello se construyen tres estrategias de acciones psicosociales que apoyen en la superación de las condiciones de victimización, además se presenta un análisis reflexivo de la experiencia de la herramienta foto- voz. El documento es elaborado a partir de los conocimientos adquiridos en el diplomado, para el fortalecimiento de capacidades psicosociales en el abordaje de las víctimas del conflicto en Colombia y superación de los hechos victimizantes para lograr la autopercepción de las víctimas como sobrevivientes, puesto que una atención humanizada y oportuna puede mejorar la calidad de vida.The Colombian armed conflict has developed an asymmetric warfare that interferes with the integral development and quality of life of the victims, families and their communities, generating victims of all kinds of victimizing acts. This document offers reflections on psychosocial accompaniment, from the learning, skills and abilities obtained in the in-depth diploma - Psychosocial Accompaniment in Scenarios of Violence. The document holistically presents the reports of two cases; the first exposes the "analysis of stories of stories that return" Chapter 3- Amparo, of the Truth Commission, in which the protagonist is a woman who has had to experience the forced disappearance of her husband, she recognizes herself as a survivor who, despite the strong fight for justice, decides to continue the path she has built not only for herself, but for those who have also suffered the same damage. Resilience led her to graduate and become a great lawyer who fought for create a law that seeks to give relief to so many families. Based on the story, circular, reflective and strategic questions were built, argued from the psychosocial approach in order to know the individual and collective perspective. In the second case about the Massacre in El Salado: “Relatos de Resiliencia después de 20 Años”, from the publishing house El Tiempo, the narrative of the survivors of the violent event that occurred between February 16 and 22, 2000 is shared, because of the United Self-Defense Forces of Colombia (AUC) in the township of El Salado, based on this, three strategies of psychosocial actions are built that support overcoming the conditions of victimization, in addition a reflective analysis of the experience of the photovoice tool. The document is prepared from the knowledge acquired in the diploma course, for the strengthening of psychosocial capacities in dealing with the victims of the conflict in Colombia and overcoming the victimizing events to achieve the self-perception of the victims as survivors, since humanized and timely care can improve quality of life

    Clinical and microbiological characteristics of patients colonized or infected by Stenotrophomonas maltophilia: is resistance to sulfamethoxazole/trimethoprim a problem?

    Stenotrophomonas maltophilia has emerged as an important opportunistic pathogen in the last decade. Increased resistance to sulfamethoxazole/trimethoprim (SMX/TMP) has been reported in S. maltophilia strains in the past few years, leading to few therapeutic options. We conducted a prospective multicenter study at two Brazilian teaching hospitals that identified S. maltophilia isolates and evaluated their antimicrobial susceptibility profile, SMX/TMP resistance genes and their clonality profile. A total of 106 non-repeated clinical samples of S. maltophilia were evaluated. Resistance to SMX/TMP was identified in 21.6% of the samples, and previous use of SMX/TMP occurred in 19 (82.6%). PCR detected the sul1 gene in 14 of 106 strains (13.2%). Of these isolates, nine displayed resistance to SMX/TMP. The resistant strains presented a polyclonal profile. This opportunistic pathogen has emerged in immunocompromised hosts, with few therapeutic options, which is aggravated by the description of emerging resistance mechanisms, although with a polyclonal distribution profile

    Access to infertility consultations: what women tell us about it?

    The main objective of the present paper is to evaluate the perception of women concerning the barriers and access to infertility consultations. Socio cultural and economic access to infertility consultations is detached and three municipalities of the northwest of Portugal were chosen as an example of a peripheral country. A quantitative/qualitative study was done with 60 women. Three dimensions were evaluated: geographic and structural and functional access; economic access; and sociocultural access. The main barriers were mainly identified in the last two dimensions. The economic access was the less well evaluated by women being the cost of treatment (medication, and concentration of costs in a short period) difficult to bear. This can justify a greater involvement of the Portuguese Government, by developing policies for the reimbursement of part of the costs. Also, some changes in structural and functional access must be done with special regard to the separation of the infertility consultations from the reproductive medicine section. The setting of the teams, with a follow-up by the same team of health professionals is also needed

    Battle of Postdisaster Response and Restoration

    [EN] The paper presents the results of the Battle of Postdisaster Response and Restoration (BPDRR) presented in a special session at the first International water distribution systems analysis & computing and control in the water industry (WDSA/CCWI) Joint Conference, held in Kingston, Ontario, Canada, in July 2018. The BPDRR problem focused on how to respond and restore water service after the occurrence of five earthquake scenarios that cause structural damage in a water distribution system. Participants were required to propose a prioritization schedule to fix the damages of each scenario while following restrictions on visibility/nonvisibility of damages. Each team/approach was evaluated against six performance criteria: (1) time without supply for hospital/firefighting, (2) rapidity of recovery, (3) resilience loss, (4) average time of no user service, (5) number of users without service for eight consecutive hours, and (6) water loss. Three main types of approaches were identified from the submissions: (1) general-purpose metaheuristic algorithms, (2) greedy algorithms, and (3) ranking-based prioritizations. All three approaches showed potential to solve the challenge efficiently. The results of the participants showed that for this network, the impact of a large-diameter pipe failure on the network is more significant than several smaller pipes failures. The location of isolation valves and the size of hydraulic segments influenced the resilience of the system during emergencies. On average, the interruptions to water supply (hospitals and firefighting) varied considerably among solutions and emergency scenarios, highlighting the importance of private water storage for emergencies. The effects of damages and repair work were more noticeable during the peak demand periods (morning and noontime) than during the low-flow periods; and tank storage helped to preserve functionality of the network in the first few hours after a simulated event.     The Brazilian Registry of Adult Patient Undergoing Cardiovascular Surgery, the BYPASS Project: Results of the First 1,722 Patients

    Objective: To report the early results of the BYPASS project - the Brazilian registrY of adult Patient undergoing cArdiovaScular Surgery - a national, observational, prospective, and longitudinal follow-up registry, aiming to chart a profile of patients undergoing cardiovascular surgery in Brazil, assessing the data harvested from the initial 1,722 patients. Methods: Data collection involved institutions throughout the whole country, comprising 17 centers in 4 regions: Southeast (8), Northeast (5), South (3), and Center-West (1). The study population consists of patients over 18 years of age, and the types of operations recorded were: coronary artery bypass graft (CABG), mitral valve, aortic valve (either conventional or transcatheter), surgical correction of atrial fibrillation, cardiac transplantation, mechanical circulatory support and congenital heart diseases in adults. Results: 83.1% of patients came from the public health system (SUS), 9.6% from the supplemental (private insurance) healthcare systemsand 7.3% from private (out-of-pocket) clinic. Male patients comprised 66%, 30% were diabetics, 46% had dyslipidemia, 28% previously sustained a myocardial infarction, and 9.4% underwent prior cardiovascular surgery. Patients underwent coronary artery bypass surgery were 54.1% and 31.5% to valve surgery, either isolated or combined. The overall postoperative mortality up to the 7th postoperative day was 4%for CABG was 2.6%, and for valve operations, 4.4%.     Robust estimation of bacterial cell count from optical density

    Optical density (OD) is widely used to estimate the density of cells in liquid culture, but cannot be compared between instruments without a standardized calibration protocol and is challenging to relate to actual cell count. We address this with an interlaboratory study comparing three simple, low-cost, and highly accessible OD calibration protocols across 244 laboratories, applied to eight strains of constitutive GFP-expressing E. coli. Based on our results, we recommend calibrating OD to estimated cell count using serial dilution of silica microspheres, which produces highly precise calibration (95.5% of residuals <1.2-fold), is easily assessed for quality control, also assesses instrument effective linear range, and can be combined with fluorescence calibration to obtain units of Molecules of Equivalent Fluorescein (MEFL) per cell, allowing direct comparison and data fusion with flow cytometry measurements: in our study, fluorescence per cell measurements showed only a 1.07-fold mean difference between plate reader and flow cytometry data

    La rabia es una enfermedad viral que afecta a millones de personas y animales en el mundo lo que afecta la salud pública, en Colombia se han realizado derivadas normas y estrategias que cumplen con prevenir y controlar la rabia por medio de entidades como el ICA y la OMSA, por ello es considerado un agente causal importante de zoonosis debido a que genera un porcentaje alto de mortalidad para la salud publica en el cual se debe actuar con rapidez al ser expuestos. Esta revisión tiene por objeto el análisis y estudio de literatura científica que muestre la incidencia de la rabia y su importancia como enfermedad zoonótica en la salud pública de Colombia, así como conocer todas las medidas, prevención y control que se deben llevar a cabo al presenciar un caso positivo de rabia como son las vacunas que llevan a tener una probabilidad de supervivencia para la víctima, con ello cada caso es estudiado según la región, vector presente y la situación que llevo a la infección y así se obtienen datos epidemiológicos que nos ayudara a crear acciones de prevención según la situación que se presente. Además, es necesario indicar a toda la sociedad sobre este virus letal con el fin de buscar la disminución o incluso la eliminación de la rabia en las regiones y el país.Rabies is a viral disease that affects millions of people and animals in the world, which affects public health. In Colombia, regulations and strategies have been created to prevent and control rabies through entities such as the ICA and the OMSA, therefore, is considered an important causal agent of zoonoses because it generates a high percentage of mortality for public health in which action must be taken quickly when exposed. This review aims to analyze and study scientific literature that shows the incidence of rabies and its importance as a zoonotic disease in the public health of Colombia, as well as to know all the measures, prevention and control that must be carried out when witnessing a positive case of rabies such as vaccines that lead to a probability of survival for the victim, with this each case is studied according to the region, vector present and the situation that led to the infection and thus epidemiological data is obtained that will help us to create preventive actions according to the situation that arises.     Este proyecto consistió en un estudio de viabilidad para un centro de servicios de acondicionamiento físico en línea en el municipio de Cúcuta. Para ello, se realizó una investigación descriptiva, ya que el estudio identificó las características señalan formas de conducta y actitudes. La información se obtuvo mediante una encuesta de preguntas dicotómicas. La población y muestra corresponde a los servicios del Centro de servicios para Acondicionamiento Físico. Se logaron identificar las condiciones del mercado para la implementación de un centro de acondicionamiento físico. Posteriormente, se desarrolló la estructura técnica de la empresa incluyendo la infraestructura organizacional y administrativa.
