    Cross-sensitization between poppy seed and buckwheat in a food-allergic patient with poppy seed anaphylaxis

    The opium poppy, Papaver somniferum L., is the source of both poppy seeds and opium. The commercially available seeds are widely used as ingredients for various kinds of food. IgE-mediated sensitization to poppy seeds is rare, but, if present, clinical symptoms are usually severe. Cross-sensitizations between poppy seeds and other food allergens have been described with sesame, hazelnut, rye grain and kiwi fruit. We report the case of a 17-year-old female with an apparently food-allergic reaction after ingestion of a poppy seed cake. Allergological workup revealed a poppy seed anaphylaxis and led to the identification of a novel cross-sensitization with buckwheat. Copyright (c) 2006 S. Karger AG, Basel

    Estimating global warming potential for agricultural landscapes with minimal field data and cost

    Greenhouse gas (GHG) emissions from agriculture comprise 10-12% of anthropocentric global emissions; and 76% of the agricultural emissions are generated in the developing world. Landscape GHG accounting is an effective way to efficiently develop baseline emissions and appropriate mitigation approaches. In a 9,736-hectare case study area dominated by rice and wheat in the Karnal district of Haryana state, India, the authors used a low-cost landscape agricultural GHG accounting method with limited fieldwork, remote sensing, and biogeochemical modeling. We used the DeNitrification-DeComposition (DNDC) model software to simulate crop growth and carbon and nitrogen cycling to estimate net GHG emissions, with information based on the mapping of cropping patterns over time using multi- resolution and multi-temporal optical remote sensing imagery. We estimated a mean net emission of 78,620 tCO2e/yr (tons of carbon dioxide equivalents per year) with a 95% confidence interval of 51,212-106,028 tCO2e/yr based on uncertainties in our crop mapping and soil data. A modeling sensitivity analysis showed soil clay fraction, soil organic carbon fraction, soil density, and nitrogen amendments to be among the most sensitive factors, and therefore critical to capture in field surveys. We recommend a multi-phase approach to increase efficiency and reduce cost in GHG accounting. Field campaigns and aspects of remote sensing image characteristics can be optimized for targeted landscapes through solid background research. An appropriate modeling approach can be selected based on crop and soil characteristics. Soil data in developing world landscapes remain a significant source of uncertainty for studies like these and should remain a key research and data development effort

    Reducing emissions of agricultural greenhouse gases (GHGs), such as methane and nitrous oxide, and sequestering carbon in the soil or in living biomass can help reduce the impact of agriculture on climate change while improving productivity and reducing resource use. There is an increasing demand for improved, low cost quantification of GHGs in agriculture, whether for national reporting to the United Nations Framework Convention on Climate Change (UNFCCC), underpinning and stimulating improved practices, establishing crediting mechanisms, or supporting green products. This ERL focus issue highlights GHG quantification to call attention to our existing knowledge and opportunities for further progress. In this article we synthesize the findings of 21 papers on the current state of global capability for agricultural GHG quantification and visions for its improvement. We conclude that strategic investment in quantification can lead to significant global improvement in agricultural GHG estimation in the near term

    A detailed look at the European Medicines Agency's recommendations for use of Janus kinase inhibitors in patients with atopic dermatitis

    Background Oral Janus kinase inhibitors (JAKi) have been approved for the treatment of several chronic inflammatory conditions, including rheumatoid arthritis (RA) and atopic dermatitis (AD). Prompted by new evidence, the Pharmacovigilance Risk Assessment Committee (PRAC) of the European Medicines Agency (EMA) recently reassessed the benefit–risk balance of oral JAKi. The PRAC recommended that oral JAKi should be used only if no suitable alternatives are available in patients ≥65 years of age, or who have a history of atherosclerotic cardiovascular (CV) disease, other CV risk factors (e.g. history of long-term smoking) or have malignancy risk factors, and used with caution in patients at risk of pulmonary embolism or deep vein thrombosis. The European Commission's final decision was issued in March 2023. Objectives Our goal was to highlight the PRAC recommendations, especially in the context of oral JAKi use in AD. Methods Authors summarized the PRAC recommendations, the new clinical evidence on oral JAKi safety and key differences between patients with RA and AD. Results Risk of developing adverse events of special interest (e.g. cardiovascular events, malignancy) is higher in patients with RA than in patients with AD, because of the higher prevalence of the underlying risk factors. Conclusions The benefit–risk profile of JAKi approved for AD remains favourable, including use as first-line systemic therapy for patients with AD <65 years of age and without CV or malignancy risk factors

    Fertiliser use and soil carbon sequestration: Key messages for climate change mitigation strategies

    Reducing greenhouse gas (GHG) emissions and increasing soil or biomass carbon stocks are the main agricultural pathways to mitigate climate change. Scientific and policy attention has recently turned to evaluating the potential of practices that can increase soil carbon sequestration. Forty percent of the world’s soils are used as cropland and grassland, therefore agricultural policies and practices are critical to maintaining or increasing the global soil carbon pool. This info note explains the current understanding of the impact of mineral fertiliser use on soil carbon sequestration as a mitigation strategy in agriculture. The science and understanding on soil carbon sequestration and mitigation is still emerging, especially in tropical regions. Taking this into consideration, this info note discusses related effects of fertiliser use on climate change mitigation, such as nitrous oxide (N2O) and carbon dioxide (CO2) emissions from nitrogen fertiliser use and production, and the potential effects of mineral fertiliser use on land use change

    Background Chronic rhinosinusitis (CRS) is one of the most frequent chronic diseases. Among these patients the prevalence of immune defects is higher than in the healthy general population. Methods A selective review of the literature was carried out in PubMed and Medline covering the period between 2008 and 2019. Additionally, recent German publications in journals not listed in the abovementioned databases were analyzed. Results The diagnostic workflow with respect to the immunodeficiency consists of a detailed anamnesis and physical examination, laboratory tests and the antibody reaction to polysaccharide vaccines and antigens. Beside antibiotic treatment, vaccinations and immunoglobulin replacement are available. Notwithstanding the above, functional endoscopic surgery of the paranasal sinuses should be performed according to guideline recommendations. Conclusion Patients with CRS who do not sufficiently respond to conservative and surgical treatment should be checked for underlying immunodeficiencies

    Scaling up sustainability in commodity agriculture: Transferability of governance mechanisms across the coffee and cattle sectors in Brazil

    A number of governance mechanisms address socio-environmental challenges associated with commodity agriculture in tropical forested countries. Governance mechanisms that prove effective in one agricultural sector are often applied to other sectors as well. For example, voluntary certification programs have been adopted by producers of commodities as diverse as beef, coffee, palm oil, and soy. However, there are substantial differences in the extent to which governance mechanisms scale up and achieve impact in different sectors. This paper analyzes how the potential for scaling up a particular governance mechanism is influenced by environmental, market, and social geographies that differ between sectors. Through stakeholder interviews, farm-level surveys, and a literature review, we examine two types of voluntary governance mechanisms (third-party certification, and sustainable intensification programs) in the coffee and cattle sectors in Brazil, to understand why the two governance mechanisms have scaled differently between these two sectors. We find that third-party certification programs have scaled up relatively well in Brazil\u27s coffee sector, more so than its cattle sector, in part owing to differences in sustainability priorities, market orientations, supply chain traceability, and social networks between the two sectors. We also find that pilot sustainable intensification programs in the cattle sector have had more success than certification in engaging farmers, in part because they involve less investment from participating farmers. We conclude that the distribution and quality of environmental resources, markets, knowledge, actors, and networks can play an important role in the ability of a governance mechanism to effectively take root
