    Role of metformin in experimentally-induced inflammatory bowel disease

    En el presente trabajo, se ha estudiado la metformina, un derivado del antidiabético biguanida, para evaluar su efecto sobre una enfermedad inflamatoria intestinal inducida experimentalmente. Para inducir la enfermedad se utilizaron dos métodos: ácido 2, 4-dinitro-bencenosulfónico (DNBS) y ácido acético en ratas. El tratamiento con metformina (200 mg/kg, bid, vía oral) y ácido 5-aminosalicílico (5-ASA, estándar 100 mg/kg, vía intrarrectal) durante tres días permitió prevenir de forma significativa la pérdida de peso corporal (14,16±0,63; 9,8±0,51, p<0,05) y reducir el índice de daño macroscópico (1,33±0,45; 1,12±0,24, p<0,01), respectivamente en comparación con el grupo de control tratado con DNBS (6,66± 0,23). En ambos modelos se midieron igualmente otros parámetros: cambios de peso del colon, malondialdehido (MDA), mieloperoxidasa (MPO), superóxido dismutasa (SOD), óxido nítrico (NO) y degranulación de mastocitos. El tratamiento con metformina redujo significativamente los cambios de peso del colon, el índice de daño macroscópico, la peroxidación lipídica, el óxido nítrico (NO) y la degranulación de mastocitos en ambos modelos, sugiriendo una reducción significativa de la severidad de la inflamación en ratas.In the present study, metformin, a biguanide derivative of antidiabetic was studied for evaluating its effect against experimentally- induced infl ammatory bowel disease. The disease was induced by two methods: 2, 4 dinitrobenzenesulphonic acid (DNBS) and acetic acid in rats. Treatment with metformin (200mg/kg, bid, orally) and 5-aminosalicylic acid (5-ASA, standard 100mg/kg, intrarectally), for three days showed signifi cant prevention in body weight loss (14.16±0.63; 9.8±0.51, p<0.05) and reduction in macroscopic damage score (1.33± 0.45; 1.12± 0.24, p<0.01) respectively when compared with the DNBS treated control group (6.66± 0.23). Other parameters —colon weight changes, malondialdehyde (MDA), myeloperoxidase (MPO), super oxide dismutase(SOD), nitric oxide (NO) and mast cell degranulation— were also measured in both the models. Metformin treatment signifi cantly reduced colon weight changes, macroscopic damage score, lipid peroxidation, nitric oxide (NO) and mast cell degranulation levels in both the models suggesting signifi cant reduction in the severity of inflammation in rats

    Bio-intensive integrated management strategy for mustard aphid Lipaphis erysimi Kalt. (Homoptera: Aphididae)

    Among the various treatments evaluated for their bio-efficacy against mustard aphid on Indian mustard during 2011-12 and 2012-13 at CCS Haryana Agricultural University, Hisar, the spray of Dimethoate 30 EC @ 1 ml/l followed by Verticillium lecanii @ 108 CS/ml was proved to be the best treatment with pooled mean aphid population of 4.5, 3.25 and 1.65 aphids/plant as against 22.0, 24.0 and 26.0 aphids/plant in the control after 3, 7 and 10 days of treatment, respectively. The pooled mean seed yield was also maximum (1485.0 kg/ha) in this treatment as compared to control (1305.0 kg/ha).The treatment was found on par with spray of dimethoate @ 1 ml/l followed by Coccinella septempunctata @ 5,000 beetles/ha with pooled mean aphid population of 5.0, 4.0 and 2.0 aphids/plant after 3, 7 and 10 days of treatment, respectively and pooled mean seed yield of (1470.0 kg/ha). But the cost benefit ratio was maximum (7.25) in treatment dimethoate followed by C. septempunctata and NSKE @ 5% followed by C. septempunctata @ 5,000 beetles/ha (6.68). Thus, entomopathogenic fungi like V. lecanii or NSKE along with release of C. septempunctata can be used as alternative measure to manage mustard aphid instead of solely relying on insecticides

    The adaptive nature of the bone-periodontal ligament-cementum complex in a ligature-induced periodontitis rat model.

    The novel aspect of this study involves illustrating significant adaptation of a functionally loaded bone-PDL-cementum complex in a ligature-induced periodontitis rat model. Following 4, 8, and 15 days of ligation, proinflammatory cytokines (TNF- α and RANKL), a mineral resorption indicator (TRAP), and a cell migration and adhesion molecule for tissue regeneration (fibronectin) within the complex were localized and correlated with changes in PDL-space (functional space). At 4 days of ligation, the functional space of the distal complex was widened compared to controls and was positively correlated with an increased expression of TNF- α. At 8 and 15 days, the number of RANKL(+) cells decreased near the mesial alveolar bone crest (ABC) but increased at the distal ABC. TRAP(+) cells on both sides of the complex significantly increased at 8 days. A gradual change in fibronectin expression from the distal PDL-secondary cementum interfaces through precementum layers was observed when compared to increased and abrupt changes at the mesial PDL-cementum and PDL-bone interfaces in ligated and control groups. Based on our results, we hypothesize that compromised strain fields can be created in a diseased periodontium, which in response to prolonged function can significantly alter the original bone and apical cementum formations

    An unusual initial presentation of mantle cell lymphoma arising from the lymphoid stroma of warthin tumor.

    BackgroundWarthin tumors presenting concomitantly with a lymphoma is vanishingly rare with only 15 reported cases in English literature. Herein, we report an unusual initial presentation of a mantle cell lymphoma involving the lymphoid stroma of a Warthin tumor.Case presentationA seventy-seven year old otherwise healthy gentleman with a 50-pack year smoking history presents with a slowly enlarging left cheek mass. CT scan of the neck demonstrated a left parotid gland tumor measuring 3.4 cm in greatest dimension. He underwent a left superficial parotidectomy, with subsequent histopathologic examination revealing a Warthin tumor with extensive expansion of the lymphoid stroma. Flow cytometric, immunohistochemical, and cytogenetic studies of the stromal component of the tumor confirmed the presence of a mantle cell lymphoma. Clinical staging demonstrated stage IVa disease, and was considered to be at low to intermediate risk due to the slow growth of the parotid lesion. The patient is undergoing close follow up with repeat PET-CT scans at six months.ConclusionTo the best of our knowledge, this is the first well documented collision tumor between mantle cell lymphoma and a Warthin tumor. This case also brings to light the significance of thorough evaluation of the lymphoid component of Warthin tumor

    Clinical profile of acute hemorrhagic stroke patients: a study in tertiary care hospital in Northern India

    Background: Acute hemorrhagic stroke, a subtype of acute stroke is one of the leading cause of death and major cause of morbidity and mortality throughout the world. The incidence of acute hemorrhagic stroke is increasing with gradual increase in obesity, diabetes mellitus, hyperlipidemia, hypertension and various cardiac problems. This study had been conducted with an objective to study the risk factors and clinical presentation of acute hemorrhagic stroke patients in north-Indian population. Methods: This study was carried out among 100 acute hemorrhagic stroke patients (clinically and radiologically confirmed) irrespective of age and sex admitted to Emergency Department of KGMU, Lucknow after getting clearance from Institutional Ethical Committee(IEC). These patients had been subjected to plane CT (computed tomography) scan brain on admission by the CT scan model 16 slice Brivo 385 for radiological confirmation and to localize the lesion in brain. Data were collected in prescribed protocol.Results: Majority of the patients were in the (50-60) age group with male predominance. Hypertension (62%) was the most common risk factor for acute hemorrhagic stroke followed by Non-veg diet (46%). The commonest clinical feature at presentation was hemiplegia (76%). Majority (47%) of the patients presented with Glasgow Coma Scale (GCS) of (9-12). Cerebral cortex (41%) was the most common site of brain lesion in acute hemorrhagic stroke patients. Conclusion: This study focuses on the clinical profile and risk factors of acute hemorrhagic stroke, by targeting which the burden of this disabling disease can be prevented.

    Evaluation of proteome complexes normalizing osmoregulation in salt stressed Luffa acutangula (L.) Roxb.

    Modern-day agriculture is facing the challenge of sustaining global food security. However, the rapid increase in salinity stress among arable areas poses a major threat to crop health and yield. Salinity stress is one of the most common and rapidly spreading stress that has a detrimental effect on the productivity of edible plant family i.e. Cucurbitaceae. The present study endeavors to evaluate the Osmoregulators (anti-oxidants and proteins), that supports the growth of two varieties of Luffa acutangula (L.) Roxb. under salt stress. The 2-3 weeks old saplings were exposed to salt stress (up to 200 mM NaCl) for one week. Post-treatment the osmoregulatory metabolites like Trehalose, Proline &amp; enzymic anti-oxidants like peroxidase (POD), Superoxide dismutase (SOD) and proteins using LC-MS/MS were analyzed. In both the varieties, Trehalose increased with increasing salt concentration, while the level of Proline increased in Variety 1 and decreased in Variety 2. With increasing salt concentrations, the POD activity decreased in both varieties whereas that of SOD levels increased in Variety 2 and decreased in Variety 1. The protein identified by LC-MS/MS and functional annotation analysis employing Uniport database &amp; BlastP algorithm, aided in the detection of differentially expressed proteins in response to salt stress. This was followed by metabolic interaction annotation enrichment analysis by FunRich 3.0 tool, enabling characterization of proteins to be involved in the Calvin cycle, amino acids biosynthesis, carbohydrate and energy metabolism, ROS defence, hormonal biosynthesis and signal transduction. The augmentation of the metabolic activities of the Calvin cycle, biosynthesis of amino acids, carotenoids and peroxisomes, glycolytic pathway and the tricarboxylic acid cycle will conceivably influence the photosynthetic capacity in L. acutangula varieties under salt stress. The upsurge of key enzymes involved in these above described biological processes possibly appears to play an important role in the enhancement of salt tolerance

    CDHA Ceramic Microspheres for Periodontitis Treatment: Synthesis, Characterization and Doxycycline Release Profiles

    The present study is focused on the development of doxycycline loaded calcium–deficient hydroxyapatite (CDHA) microspheres for the treatment of periodontitis. The CDHA microspheres were formed by liquid immiscibility effect using gelatin and paraffin oil with varying Ca/P ratios using calcium hydroxide and diammonium hydrogen orthophosphate as precursors. The morphology of the microspheres as characterized by SEM was optimized by varying the gelatin content. The doxycycline incorporation and its release profiles were studied by UV-Visible spectroscopy in phosphate buffer at physiological conditions. The pH of the buffer solution was initially optimized to have maximum amount of drug loading. Doxycycline loading around the physiological pH of 7 has the highest amount of drug incorporation. All the microspheres exhibit similar release profiles with an initial gradual increase reaching a maximum value and then nearly constant release. The microspheres formed using 6% gelatin shows maximum amount of drug release of 80%

    Don’t turn your back on the symptoms of psychosis : a proof-of-principle, quasi-experimental public health trial to reduce the duration of untreated psychosis in Birmingham, UK

    Background: Reducing the duration of untreated psychosis (DUP) is an aspiration of international guidelines for first episode psychosis; however, public health initiatives have met with mixed results. Systematic reviews suggest that greater focus on the sources of delay within care pathways, (which will vary between healthcare settings) is needed to achieve sustainable reductions in DUP (BJP 198: 256-263; 2011). Methods/Design: A quasi-experimental trial, comparing a targeted intervention area with a ‘detection as usual’ area in the same city. A proof-of–principle trial, no a priori assumptions are made regarding effect size; key outcome will be an estimate of the potential effect size for a definitive trial. DUP and number of new cases will be collected over an 18-month period in target and control areas and compared; historical data on DUP collected in both areas over the previous three years, will serve as a benchmark. The intervention will focus on reducing two significant DUP component delays within the overall care pathway: delays within the mental health service and help-seeking delay. Discussion: This pragmatic trial will be the first to target known delays within the care pathway for those with a first episode of psychosis. If successful, this will provide a generalizable methodology that can be implemented in a variety of healthcare contexts with differing sources of delay.     Placenta percreta induced uterine rupture at 15 weeks of gestation: an unusual case

    Spontaneous rupture of uterus during early pregnancy due to placenta percreta is a rare complication, which is potentially life-threatening to both mother & fetus and also difficult to diagnose. A 27 years old pregnant woman with history of previous one caesarean section 2 years back was referred to our emergency department with severe anaemia and breathlessness. On examination patient was in hypovolemic shock. Ultrasonography revealed massive fluid collection in peritoneal cavity, an intrauterine anomalous foetus of 15 weeks and low-lying placenta. Paracentesis was positive for blood. On laparotomy there was haemoperitoneum of about 3 litres with placenta perforating through the lower segment of uterus. Even after all measures to conserve uterus, due to uncontrolled bleeding hysterectomy was performed. Postoperative period was uneventful. Pathological analysis of the specimen confirmed our diagnosis of placenta percreta. Though uterine rupture is rare in early trimester, it may be considered in a patient with haemoperitoneum with a scarred uterus, which can be due to placenta percreta also
