    Coefficients Quantization at Off-axis Digital Hologram Wavelet Compression

    Digital holographic information is compressed for storage of 2D- or 3D-objects amplitude and phase distributions, fast transmission, analyzing and displaying of these data. In this paper features of application of wavelet transforms for off-axis digital holograms compression are considered. The combined technique based on zero and twin orders elimination, wavelet compression of the amplitude and phase components of obtained Fourier spectrum and further additional compression of wavelet coefficients by thresholding and quantization is analyzed. Numerical experiments on reconstruction of images from the compressed holograms are performed. The comparative analysis of applicability of various wavelets and additional quantization of coefficients is performed. Obtained results demonstrate possibility of 180 and more times compression using iterative and noniterative methods of coefficients quantization and threshold zeroing less 80% of wavelet coefficients.     Reactive diffusion in Sc/Si multilayer X-ray mirrors with CrB2 barrier layers

    Processes undergoing in Sc/Si multilayer X-ray mirrors (MXMs) with periods of ∼27 nm and barrier layers of CrB20.3- and 0.7-nm thick within the temperature range of 420–780 K were studied by methods of small-angle Xray reflectivity (λ = 0.154 nm) and cross-sectional transmission electron microscopy. All layers with the exception of Sc ones are amorphous. Barrier layers are stable at least up to a temperature of 625 K and double the activation energy of diffusional intermixing at moderate temperatures. Introduction of barriers improves the thermal stability of Sc/Si MXMs at least by 80 degrees. Diffusion of Si atoms through barrier layers into Sc layers with formation of silicides was shown to be the main degradation mechanism of MXMs. A comparison of the stability for Sc/Si MXMs with different barriers published in the literature is conducted. Because of resurgence of severe anxio-depressive symptoms, without any change of the treatment, the patient was readmitted 2 months later. Despite increasing the dose of clomipramine up to 225 mg/d, there was no clinical improvement, and the patient finally attempted to her life by abusing drugs. She then improved after 2 weeks on clomipramine IV (50 mg/d). Compliance was estimated good and no pharmacokinetic interactions with the rest of the treatment were found. C and DC plasma levels were measured, and CYP2D6/CYP2C19 genotype analyzed. Results: The plasma levels of C and DC are given in the Table below. Measures were done at the steady state and at trough concentration for IV treatment and 10 hours after the last dose for oral treatment

    The clinical significance of insulin resistance in non-diabetic patients with early forms of non-alcoholic fatty liver disease

    Aim. To assess the presence of insulin resistance (IR) in non-diabetic patients with early forms of non-alcoholic fatty liver disease (NAFLD) - liver steatosis (LS) and steatohepatitis (SH) of mild activity and the influence of IR on the clinical course of these diseases. Materials and methods. 134 patients with NAFLD were examined: 54 with LS and 80 with SH. The control group consisted of 37 healthy donors. Anthropometric parameters (body mass index (BMI), waist circumference (WC)), clinical and biochemical blood indices, including the blood level of cytokeratin-18 fragments (CK-18), TNF-α and IL-6 cytokines, insulin were evaluated. The HOMA index and the fibrosis index (NAFLD FS) were calculated. Patients were divided into groups: I - with the absence of IR (HOMA-index 2.7). Results and discussion. Indicators of hepatic injury, inflammation, cholestasis, fibrosis and atherogenic dyslipidemia are higher in patients with LS of group II (with IR) than in group I patients (without IR). BMI, WC, γ-glutamil transpeptidase, CK-18 and fibrosis index are significantly higher in group II patients with SH compared with group I, there is no significant difference in the level of cytolysis, inflammation and dyslipidemia indices. A high incidence of IR in non-diabetic patients with LS (37.0%) and SH (55.0%) was found and the effect of IR on the clinical course of these diseases was revealed. Conclusion. Insulin resistance in non-diabetic patients with NAFLD was detected in SH (55.0%) with higher frequency than in LS (37.0%). In LS, IR is associated with impaired hepatic cell damage, intrahepatic cholestasis, atherogenic dyslipidemia and fibrosis. In SH, IR is combined with reliable growth in indicators of hepatocyte apoptosis, cytokine proinflammatory status and fibrosis. IR determines the progressing course of NAFLD, promoting the transformation of steatosis into steatohepatitis and steatohepatitis into fibrosis and liver cirrhosis

    Quality of life and health utility index in type 2 diabetes mellitus patients with various forms of hypoglycemia receiving basal-bolus insulin therapy

    Analysis of the results of observational program «Quality of life, symptoms of hypoglycemia and treatment satisfaction in patients with type 2 diabetes receiving basal-bolus insulin therapy» are presented. One thousand patients with type 2 diabetes mellitus (DM2) from ten regions of Russia were included in the above program. The quality of life of DM2 patients with different forms of hypoglycemia on insulin therapy and their distribution according to the grades of quality of life impairment were studied; the health utility index ( Ut) for various forms of hypoglycemia was determined. Quality of life in DM2 patients with hypoglycemia was lower than in those without hypoglycemia. Patients with severe and nocturnal hypoglycemia episodes recorded the expressed disturbances in quality of life; the majority of patients in this group had critical or severe quality of life impairment. On the basis of real-world practice data, the values of health utility index for patients with and without hypoglycemia were determined. The obtained values of health utility index can be used to calculate QALY in pharmacoeconomic analysis

    Pharmacoeconomic analysis of use the oral hypoglycemic agents in patients with type 2 diabetes mellitus according to a real clinical practice

    We aimed to conduct cost-utility analysis of two different regimens of the oral hypoglycemic therapy in 2 type diabetes mellitus (DM2). Methods. In the whole, 229 patients with DM2, receiving vildagliptin add-on to metformin (group 1) or sulphonylureas add-on to metformin (group 2) were enrolled in the study. Cost-utility ratio (CUR) was identified as a ratio of difference in total costs of treatment in the groups and difference in QALY in corresponding groups. The overall costs included direct costs of treatment as well as costs for treating of DM2 complications (severe hypoglycemia) and the costs related to the loss of GDP due to severe hypoglycemia events. Health utility value was evaluated for each patient on the basis of SF-6D questionnaire. Pharmacoeconomic expediency of treatment regimen was estimated by means of comparing the CUR and wiliness-to-pay ratio (WTP) for Russian Federation (RF). Results. The health utility value was higher in group 1 as compared to group 2: 0,757 vs 0,70 (p>0,05). The overall costs for treating one patient in group 1 during a year were 28 637,34 rubles, in group 2 - 10 231,55 rubles. CUR amounted 328 674,82 rubles and it was 4.4 times lower than upper border of WTP ratio in RF (1 457 400 rub.). Conclusion. The innovation treatment regimen with vildagliptin add-on to metformin is beneficial and may be considered as economically reasonable alternative to traditional treatment regimen with sulphonylureas add-on to metformin for DM2 patients in RF
