    Type 2 diabetes in Mexican workers exposed to a potential source of dioxins in the cement industry determined by a job exposure matrix

    Artículos originales[ES] Propósito: Identificar asociación entre diabetes tipo 2 (DM2) en trabajadores Mexicanos con la exposición ocupacional a una fuente potencial de dioxinas de la industria cementera. Material y métodos: Se incluyeron 56 expedientes clínicos de trabajadores de la industria cementeracon diagnóstico de DM2; los controles lo constituyeron 56 expedientes clínicos de trabajadores de la misma industria pero sin DM2. La dosis diaria de exposición (DDE) a la fuente potencial de dioxinas por años trabajados se estimó a través de una matriz de exposición ocupacional y fue categorizada como baja, moderada y alta. El modelo de regresión logística que correlacionó la alta exposición a la fuente potencial de dioxinas (confinamiento de la escoria o clinker de cemento) por años trabajados y presencia de DM2 fue ajustada por antigüedad en el trabajo, la edad del paciente en la que se estableció el diagnóstico de DM2 y antecedente familiar de DM2. Resultados: la razón de momios para la presencia de DM2 en trabajadores con exposición moderada y alta a la fuente potencial de dioxinas en la industria cementera fue de 3.25 (1.10–9.57), p= 0.03, ajustada por antigüedad en el trabajo, edad del trabajador en la que se estableció el diagnóstico de DM2, y el antecedente familiar de DM2. Conclusiones: De acuerdo con los datos explorados en los expedientes clínicos de trabajadores de la industria cementera, existe asociación entre la alta exposición al confinamiento industrial de la escoria o clinker de cemento, como fuente potencial a dioxinas, y la presencia de DM2 con un modesto gradiente de dosis-respuesta. [EN] Purpose: To identify association between type 2 diabetes (DM2) with occupational exposure to potential dioxins source in Mexican cement industry workers. Materials and Methods: 56 medical files of cement industry workers with diagnosis of DM2 were included; 56 medical files of workers from the same industry without DM2 were the controls. The daily dose of exposure (DDE) to the potential dioxins source per work years was estimated by a job exposure matrix and categorized as low, moderate, and high. Logistic regression model that correlated high exposure to potential source of dioxins (cement clinker confinement) per work years and presence of DM2 was adjusted by work seniority, patient age at which DM2 diagnosis was established and DM2 familiar background. Results: the OR for the presence of DM2 in workers with moderate and high exposure to potential source of dioxins in the cement industry was 3.25 (1.10–9.57), p= 0.03, adjusted by work seniority, worker age at which DM2 diagnosis was established, and DM2 familiar background. Conclusions: In according with the data explored in the medical files of cement industry workers, there is an association between high exposure to the industrial confinement of the cement clinker as a potential source of dioxins and presence of DM2 in a modest dose-response gradient.N

    Outcome of Enterococcus faecalis infective endocardits according to the length of antibiotic therapy: Prelininary data from a cohort of 78 patients.

    Background International guidelines recommend 4 weeks of treatment with ampicillin plus gentamicin (A+G) for uncomplicated native valve Enterococcus faecalis infective endocarditis (EFIE) and 6 weeks in the remaining cases. Ampicillin plus ceftriaxone (A+C) is always recommended for at least 6w, with no available studies assessing its suitability for 4w. We aimed to investigate differences in the outcome of EFIE according to the duration (4 versus 6 weeks) of antibiotic treatment (A+G or A+C). Methods Retrospective analysis from a prospectively collected cohort of 78 EFIE patients treated with either A+G or A+C. Results 32 cases (41%) were treated with A+G (9 for 4w, 28%) and 46 (59%) with A+C (14 for 4w, 30%). No significant differences were found in 1-year mortality according to the type of treatment (31% and 24% in A+G and A+C, respectively; P = 0.646) or duration (26% and 27% at 4 and 6w, respectively; P = 0.863). Relapses were more frequent among survivors treated for 4w than in those treated for 6w (3/18 [17%] at 4w and 1/41 [2%] at 6w; P = 0.045). Three out of 4 (75%) relapses occurred in cirrhotic patients. Conclusions A 4-week course of antibiotic treatment might not be suitable neither for A+G nor A+C for treating uncomplicated native valve EFIE

    Olfactomedin‑like 2 A and B (OLFML2A and OLFML2B) expression profile in primates (human and baboon)

    Background: The olfactomedin‑like domain (OLFML) is present in at least four families of proteins, including OLFML2A and OLFML2B, which are expressed in adult rat retina cells. However, no expression of their orthologous has ever been reported in human and baboon. Objective: The aim of this study was to investigate the expression of OLFML2A and OLFML2B in ocular tissues of baboons (Papio hamadryas) and humans, as a key to elucidate OLFML function in eye physiology. Methods: OLFML2A and OLFML2B cDNA detection in ocular tissues of these species was performed by RT‑PCR. The amplicons were cloned and sequenced, phylogenetically analyzed and their proteins products were confirmed by immunofluorescence assays. Results: OLFML2A and OLFML2B transcripts were found in human cornea, lens and retina and in baboon cornea, lens, iris and retina. The baboon OLFML2A and OLFML2B ORF sequences have 96% similarity with their human’s orthologous. OLFML2A and OLFML2B evolution fits the hypothesis of purifying selection. Phylogenetic analysis shows clear orthology in OLFML2A genes, while OLFML2B orthology is not clear. Conclusions: Expression of OLFML2A and OLFML2B in human and baboon ocular tissues, including their high simi‑ larity, make the baboon a powerful model to deduce the physiological and/or metabolic function of these proteins in the eye

    One-year outcome following biological or mechanical valve replacement for infective endocarditis

    Background: Nearly half of patients require cardiac surgery during the acute phase of infective endocarditis (IE). We describe the characteristics of patients according to the type of valve replacement (mechanical or biological), and examine whether the type of prosthesis was associated with in-hospital and 1-year mortality. Methods and results: Among 5591 patients included in the International Collaboration on Endocarditis Prospective Cohort Study, 1467 patients with definite IE were operated on during the active phase and had a biological (37%) or mechanical (63%) valve replacement. Patients who received bioprostheses were older (62 vs 54 years), more often had a history of cancer (9% vs 6%), and had moderate or severe renal disease (9% vs 4%); proportion of health care-associated IE was higher (26% vs 17%); intracardiac abscesses were more frequent (30% vs 23%). In-hospital and 1-year death rates were higher in the bioprosthesis group, 20.5% vs 14.0% (p = 0.0009) and 25.3% vs 16.6% (p < .0001), respectively. In multivariable analysis, mechanical prostheses were less commonly implanted in older patients (odds ratio: 0.64 for every 10 years), and in patients with a history of cancer (0.72), but were more commonly implanted in mitral position (1.60). Bioprosthesis was independently associated with 1-year mortality (hazard ratio: 1.298). Conclusions: Patients with IE who receive a biological valve replacement have significant differences in clinical characteristics compared to patients who receive a mechanical prosthesis. Biological valve replacement is independently associated with a higher in-hospital and 1-year mortality, a result which is possibly related to patient characteristics rather than valve dysfunction

    Neuromodulation-induced prehabilitation to leverage neuroplasticity before brain tumor surgery: a single-cohort feasibility trial protocol

    IntroductionNeurosurgery for brain tumors needs to find a complex balance between the effective removal of targeted tissue and the preservation of surrounding brain areas. Neuromodulation-induced cortical prehabilitation (NICP) is a promising strategy that combines temporary inhibition of critical areas (virtual lesion) with intensive behavioral training to foster the activation of alternative brain resources. By progressively reducing the functional relevance of targeted areas, the goal is to facilitate resection with reduced risks of neurological sequelae. However, it is still unclear which modality (invasive vs. non-invasive neuromodulation) and volume of therapy (behavioral training) may be optimal in terms of feasibility and efficacy.Methods and analysisPatients undertake between 10 and 20 daily sessions consisting of neuromodulation coupled with intensive task training, individualized based on the target site and neurological functions at risk of being compromised. The primary outcome of the proposed pilot, single-cohort trial is to investigate the feasibility and potential effectiveness of a non-invasive NICP protocol on neuroplasticity and post-surgical outcomes. Secondary outcomes investigating longitudinal changes (neuroimaging, neurophysiology, and clinical) are measured pre-NICP, post-NICP, and post-surgery.Ethics and disseminationEthics approval was obtained from the Research Ethical Committee of Fundació Unió Catalana d'Hospitals (approval number: CEI 21/65, version 1, 13/07/2021).     IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

    Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients. Conclusions: Baseline IL-6 greater than 30 pg/mL predicts IMV requirement in patients with COVID-19 and contributes to establish an adequate indication for TCZ administrationThis study was funded by Spanish Ministry of Economy, Industry and Competitiveness (MINECO) and Instituto de Salud Carlos III (grant nos.
