Prevention and Treatment of Life-Threatening COVID-19 May Be Possible with Oxygen Treatment
Most SARS CoV-2 infections probably occur unnoticed or cause only cause a mild common cold that does not require medical intervention. A significant proportion of more severe cases is characterized by early neurological symptoms such as headache, fatigue, and impaired consciousness, including respiratory distress. These symptoms suggest hypoxia, specifically affecting the brain. The condition is best explained by primary replication of the virus in the nasal respiratory and/or the olfactory epithelia, followed by an invasion of the virus into the central nervous system, including the respiratory centers, either along a transneural route, through disruption of the blood-brain barrier, or both. In patients, presenting with early dyspnea, the primary goal of therapy should be the reversal of brain hypoxia as efficiently as possible. The first approach should be intermittent treatment with 100% oxygen using a tight oronasal mask or a hood. If this does not help within a few hours, an enclosure is needed to increase the ambient pressure. This management approach is well established in the hypoxia-related diseases in diving and aerospace medicine and preserves the patient’s spontaneous breathing. Preliminary research evidence indicates that even a small elevation of the ambient pressure might be lifesaving. Other neurological symptoms, presenting particularly in long COVID-19, suggest imbalance of the autonomous nervous system, i.e., dysautonomia. These patients could benefit from vagal nerve stimulation

Sähköisten terveyspalveluiden opetus lääketieteessä
Vertaisarvioitu.Digitalisaation myötä terveydenhuollon toiminnot muuttuvat nopeasti, ja sähköiset työkalut sekä palvelut ovat jo arkipäivää. Vaikutukset näkyvät koulutustarpeena, koska terveydenhuollon ammattilaisten digiosaaminen tulee turvata. Lääkärien peruskoulutuksen tulisi sisältää etälääketiedettä ja sähköisen terveydenhuollon palveluja (e-health) koskevaa opetusta. Kansallisessa MEDigi-hankkeessa määriteltiin suomalaisen e-health-opetuksen aihealueet ja tehtiin niille ydinainesanalyysi. Työryhmän työn perusteella määräytyi kaksitoista e-health-opetukseen soveltuvaa aihealuetta lääketieteen ja hammaslääketieteen peruskoulutuksessa. Niiden sisältämien opetusaiheiden keskeisyys määriteltiin kolmiportaisesti. Tuloksia sovellettiin pilottina Oulun yliopistossa lääketieteen viidennen vuosikurssin opiskelijoille toteutettuun teemapäivään, jonka opetussisältö muokattiin vastaamaan ydinainesanalyysin tuloksia. Opetus toteutettiin koronapandemian takia kokonaan etäopetuksena. Jatkossa tavoitteena on kansallisesti integroida ydinainesanalyysiin perustuva e-health-opetus suomalaiseen lääkärikoulutukseen

NewsMeSH: a new classifier designed to annotate health news with MeSH headings
Motivation
In the age of big data, the amount of scientific information available online dwarfs the ability of current tools to support researchers in locating and securing access to the necessary materials. Well-structured open data and the smart systems that make the appropriate use of it are invaluable and can help health researchers and professionals to find the appropriate information by, e.g., configuring the monitoring of information or refining a specific query on a disease.
Methods
We present an automated text classifier approach based on the MEDLINE/MeSH thesaurus, trained on the manual annotation of more than 26 million expert-annotated scientific abstracts. The classifier was developed tailor-fit to the public health and health research domain experts, in the light of their specific challenges and needs. We have applied the proposed methodology on three specific health domains: the Coronavirus, Mental Health and Diabetes, considering the pertinence of the first, and the known relations with the other two health topics.
Results
A classifier is trained on the MEDLINE dataset that can automatically annotate text, such as scientific articles, news articles or medical reports with relevant concepts from the MeSH thesaurus.
Conclusions
The proposed text classifier shows promising results in the evaluation of health-related news. The application of the developed classifier enables the exploration of news and extraction of health-related insights, based on the MeSH thesaurus, through a similar workflow as in the usage of PubMed, with which most health researchers are familiar

System Architecture of A European Platform for Health Policy Decision Making: MIDAS
Background: Healthcare data is a rich yet underutilized resource due to its disconnected, heterogeneous nature. A means of connecting healthcare data and integrating it with additional open and social data in a secure way can support the monumental challenge policy-makers face in safely accessing all relevant data to assist in managing the health and wellbeing of all. The goal of this study was to develop a novel health data platform within the MIDAS (Meaningful Integration of Data Analytics and Services) project, that harnesses the potential of latent healthcare data in combination with open and social data to support evidence-based health policy decision-making in a privacy-preserving manner. Methods: The MIDAS platform was developed in an iterative and collaborative way with close involvement of academia, industry, healthcare staff and policy-makers, to solve tasks including data storage, data harmonization, data analytics and visualizations, and open and social data analytics. The platform has been piloted and tested by health departments in four European countries, each focusing on different region-specific health challenges and related data sources. Results: A novel health data platform solving the needs of Public Health decision-makers was successfully implemented within the four pilot regions connecting heterogeneous healthcare datasets and open datasets and turning large amounts of previously isolated data into actionable information allowing for evidence-based health policy-making and risk stratification through the application and visualization of advanced analytics. Conclusions: The MIDAS platform delivers a secure, effective and integrated solution to deal with health data, providing support for health policy decision-making, planning of public health activities and the implementation of the Health in All Policies approach. The platform has proven transferable, sustainable and scalable across policies, data and regions
Meaningful Big Data Integration For a Global COVID-19 Strategy
Abstract
With the rapid spread of the COVID-19 pandemic, the novel Meaningful Integration of Data Analytics and Services (MIDAS) platform quickly demonstrates its value, relevance and transferability to this new global crisis. The MIDAS platform enables the connection of a large number of isolated heterogeneous data sources, and combines rich datasets including open and social data, ingesting and preparing these for the application of analytics, monitoring and research tools. These platforms will assist public health author ities in: (i) better understanding the disease and its impact; (ii) monitoring the different aspects of the evolution of the pandemic across a diverse range of groups; (iii) contributing to improved resilience against the impacts of this global crisis; and (iv) enhancing preparedness for future public health emergencies. The model of governance and ethical review, incorporated and defined within MIDAS, also addresses the complex privacy and ethical issues that the developing pandemic has highlighted, allowing oversight and scrutiny of more and richer data sources by users of the system
Vertikaalihypyn korkeuden arviointi 3D-kiihtyvyysanturilla ja lajinomaisen kuormituksen vaikutukset hyppyyn
Sensoreihin perustuvat menetelmät ovat yleistyneet urheilutestauksessa viime vuosina. Tarkoituksena oli selvittää hyppääjän kenkään kiinnitettävään 3D-kiihtyvyysanturiin perustuvan menetelmän validiteettia vertikaalihypyn korkeuden arvioinnissa. Voimalevyn lentoaikamenetelmää käytettiin kiihtyvyysanturimenetelmän validiteettianalyysissä ja lajikohtaisten kuormitusasetelman vertikaalihypyt arvioitiin kiihtyvyysanturimenetelmällä. Hyppysarjojen korkeimmat hypyt arvioitiin lisäksi voimalevyn lähtönopeusmenetelmällä. Kiihtyvyysanturimenetelmässä käytettiin urheilijan kenkään kiinnitettävää The Stride Sensor Bluetooth® Smart (Polar Electro Oy, Kempele, Suomi) -sensoria. Hyppytestit suoritettiin voimalevyllä ilman kevennystä (3 kpl HIK) ja keventäen (3 kpl HK) kädet-lanteilla-tyylillä. Hyppytestit suoritettiin oikean jalan ojennuksen ja koukistuksen MVC-testien eli maksimaalisen tahdonalaisen lihassupistuksen jälkeen. Suoritusten aikaiset reisilihasten lihasaktiivisuudet mitattiin langattomasti ja kaikki mittaukset suoritettiin ennen kuormitusta (pre-tilanteessa) ja lajikohtaisen kuormituksen jälkeen (post-tilanteessa) sekä kuormituksen mahdollisesti aiheuttamia muutoksia pyrittiin selvittämään hyppykorkeuden arvioinnin kannalta. Lajikohtaisia vaikutuksia selvitettiin kolmen eri lajin urheilijoilla; maastopyöräilijöillä (n = 10), suunnistajilla (n = 10) ja koripalloilijoilla (n = 9). Pyöräilijöiden kuormituksena toimi maksimaalisen polkupyöräergometritesti, suunnistajat osallistuivat kovatasoiseen suunnistuskilpailuun ja koripalloilijat suorittivat lajiharjoituksen lisäksi juoksutestin.
Kiihtyvyysanturimenetelmän todettiin soveltuvan maksimaalisten HIK:n ja HK:n korkeuden arviointiin. HIK:n korkeudet (n = 174) olivat kiihtyvyysanturimenetelmällä 30,4 (±5,6) cm ja voimalevymenetelmällä 29,1 (±5,2) cm sekä HK:n (n = 167) vastaavasti 33,5 (±6,3) cm ja 32,1 (±5,9) cm. Kiihtyvyysanturimenetelmän ja voimalevyn lentoaikamenetelmän korkeustulosparien välinen korrelaatio oli erittäin merkitsevä (r = 0,97–0,99, p < 0,001) sekä HIK:ssa että HK:ssa. Yksittäiset hyppykorkeustulokset olivat myös yhtäpitäviä molemmilla hyppytavoilla erittäin merkitsevästi (p < 0,001). HIK:ssa tulosten sisäinen korrelaatio ICC SM oli 0,95 ja 95 % CI: 0,50–0,98 (p < 0,001) sekä HK:ssa ICC SM oli 0,95 ja 95 % CI: 0,51–0,98 (p < 0,001). Kiihtyvyysanturi- ja voimalevyn lentoaikamenetelmän välinen korrelaatio oli molemmilla hyppytavoilla erittäin hyvä (r2 = 0,96). Eroavuudet HIK:ssa (1,4 ±1,2 cm) ja HK:ssa (1,5 ±1,3 cm) olivat merkitsevän pieniä (p < 0,001). Kiihtyvyysanturimenetelmä vastasi myös parhaissa hypyissä voimalevyn lähtönopeusmenetelmän kanssa erittäin merkitsevästi (r = 0,88, p < 0,001).
Kiihtyvyysanturimenetelmällä tehdyssä lajien välisessä vertailussa koripalloilijat hyppäsivät 37,1 (±6,1) cm, joka oli merkitsevästi korkeampi (p < 0.001) tulos suunnistajiin 32,0 (±4,9) cm ja maastopyöräilijöihin 30,3 (±5,7) cm verrattuna. Koripalloilijoiden suorittamien hyppyjen keski- ja huipputehot olivat muita lajeja merkitsevästi (p < 0,05) suuremmat lähes kaikissa tilanteissa. Kuormitusvaikutukset olivat pieniä pitkähkön palautumisajan vuoksi. Suunnistajien huipputehot laskivat merkitsevästi (p < 0,05) muihin lajeihin verrattuna. MVC-testissä jalan ojennusvoimat heikkenivät väsytyksen vuoksi (p = 0,007), mutta urheilulajien välillä ei ollut merkitseviä eroja tulosmuutoksissa. Reisilihasten (RF, VL ja BF) lihasaktiivisuusmuutokset olivat myös verraten pieniä, eikä varsinaisia lajien välisiä eroja muodostunut väsytyksen johdosta.
Kenkään kiinnitettävällä 3D-kiihtyvyysanturimenetelmällä voidaan arvioida HIK:n ja HK:n hyppykorkeustuloksia eri lajin urheilijoilla. Kiihtyvyysanturi- ja voimalevymenetelmien välinen korrelaatio oli erittäin merkitsevä eri hyppytavoilla. HIK-tulosten hajonta oli pienempää kuin HK:n, mutta kiihtyvyysanturimenetelmän ja voimalevyn lentoaikamenetelmän välillä havaittiin pieni systemaattinen virhe. Lajikohtaisen kuormituksen ei havaittu vaikuttavan mittaustarkkuuteen, vaikka pieniä muutoksia havaittiin hyppykorkeudessa ja hypyn suorituskykytekijöissä. 3D-kiihtyvyysanturimenetelmää voidaan hyödyntää myös valmennuksen apuvälineenä.Athletes have an interest for vertical jump height and a jump test is often used as an indicator of lower body performance and explosive strength. Different sensors have been used widely in sports testing over the past years and practical solutions would be useful for coaching. The purpose of this study was to investigate the validity of an on-shoe 3D accelerometer sensor in jump height evaluation in athletes’ own sport.
Athletes from cycling, orienteering and basketball were studied to examine the suitability of the on-shoe 3D sensor method for different types of athletes. Subjects (n=29) performed three squat jump (SQJ) and three countermovement jump (CMJ) tests twice. Jump heights were assessed with a Stride Sensor Bluetooth® Smart (Polar Electro Oy, Kempele, Finland) attached on a shoe when jumping on a force plate (FP). A FP flight time method was used as reference for jump height and a FP take of velocity method was used for comparing the maximum jump results.
The paired correlation between the 3D sensor and the FP flight time method was 0.97-0.99 (p<0.001). Jump height for SQJ (n=174) with the sensor was 30.4 ± 5.6 cm and with the FP method 29.1 ± 5.2 cm. In CMJ (n=167) the corresponding values were 33.5 ± 6.3 cm and 32.1 ± 5.9 cm with the sensor and FP, respectively. Single measurements showed high consistency (p<0.001) also with SQJ ICC: 0.95, 95% CI: 0.50-0.98 and with CMJ ICC: 0.95, 95% CI: 0.51-0.98. The correlation of the maximum jumps between the 3D sensor and the FP take of velocity methods was 0.88 (p<0.001). The maximum jump heights with sensor method for the basketball players was 37.1 ± 6.1 cm, which was significantly higher (p<0.001) than the orienteers 32.0 ± 4.9 cm and mountain bikers 30.3 ± 5.7 cm.
The 3D sensor and the FP methods correlated highly significantly in the evaluation of SQJ and CMJ height results of three different types of athletes. However, there was a small but systematic difference in jump height between the methods. The SQJ showed slightly better correlation and less variability with smaller bias of height results than CMJ. The correlation of the 3D sensor method was better with FP flight time than with take of velocity method. These results indicate that a 3D accelerometer sensor implemented on-shoe can be used for jump height evaluation and for maximum jump test. The 3D sensor method would likely offer a practical solution for jump height follow up in coaching
