La Sarcoïdosi a Osona : Revisió retrospectiva 1983-2011
Objectius: Determinar l'epidemiologia de la Sarcoïdosi a Osona. I descriure'n les característiques clíniques. Material i mètodes: Identificació i selecció de casos de Sarcoïdosi al nostre centre, entre 1983-2011. Revisió retrospectiva, recollida i anàlisi. Resultats: Incidència 2.12 casos/105habitants/any. n = 55. Predomini femení (69%), edat mitjana 47.8 anys. Clínica més freqüent: simptomatologia general. Patrons radiològics més freqüents: Estadi I-II. Biòpsia transbronquial: tècnica més utilitzada. Retard diagnòstic 5.9 mesos i mortalitat relacionada 7.6%. Conclusions: Incidència superior a la publicada a Catalunya. Predomini femení, amb diagnòstic més tardà que en homes. Presentació clínica predominant: síndrome Löfgren. Notable retard diagnòstic i baixa mortalitat
Eficacia y seguridad del tratamiento antibiótico domiciliario endovenoso en patologia infecciosa procedente del servicio de urgencias
Este estudio de cohortes analiza la eficacia y seguridad del tratamiento antibiótico domiciliario endovenoso (TADE) en pacientes con infecciones procedentes del servicio de urgencias e ingresados en la unidad de Hospitalización a domicilio (HaD) del Hospital de Sabadell. Los pacientes procedentes de Urgencias presentaron mayor edad, mayor deterioro funcional, estancia media más corta, mayor proporción de infecciones urinarias y menor porcentaje de infecciones por P.aeruginosa. Estos pacientes no presentaron mayor riesgo de reingreso hospitalario, ni peor evolución de la infección, ni un incremento de las infecciones asociadas a cuidados sanitarios por lo que el TADE autoadministrado es seguro y eficaz.Aquest estudi de cohorts analitza l'eficàcia i seguretat del tractament antibiòtic domiciliari endovenós (TADE) en pacients procedents del servei de Urgències i ingressats a la unitat d'Hospitalització a domicili (HaD) de l'Hospital de Sabadell. Effectiveness of Rituximab in Severe Wegener’s Granulomatosis: Report of Two Cases and Review of the Literature
We hereby describe the satisfactory evolution of rituximab treatment in two patients with Wegener’s granulomatosis (WG). Rituximab was indicated for refractoriness to standard treatment in one case and life-threatening myelotoxicity due to alkylating agents in the other. Circulating soluble adhesion molecules in patients with giant cell arteritis. Correlation between soluble intercellular adhesion molecule-1 (sICAM-1) concentrations and disease activity
Objective—To evaluate whether changes
in concentrations of circulating adhesion
molecules are related to disease activity in
patients with giant cell arteritis (GCA).
Methods—A sandwich ELISA was used to
measure soluble intercellular adhesion
molecule-1 (sICAM-1), sICAM-3, vascular
cell adhesion molecule-1 (sVCAM-1),
E-selectin (sE-selectin), and L-selectin
(sL-selectin) in serum and plasma samples
from patients with GCA. A cross sectional
study was performed on 64 GCA patients
at diVerent activity stages and on 35 age
and sex matched healthy donors. Thirteen
of these patients were evaluated at the time
of diagnosis and serially during follow up.
Results—At the time of diagnosis,
sICAM-1 concentrations were significantly
higher in active GCA patients than in controls
(mean (SD) 360.55 (129.78) ng/ml versus
243.25 (47.43) ng/ml, p<0.001). In
contrast, sICAM-3, sVCAM-1, sE-selectin,
and sL-selectin values did not diVer from
those obtained in normal donors. With
corticosteroid administration, a decrease
in sICAM-1 concentrations was observed,
reaching normal values when clinical remission
was achieved (263.18 (92.7) ng/ml
globally, 293.59 (108.39) ng/ml in the group
of patients in recent remission, and 236.83
(70.02) ng/ml in those in long term remission).
In the 13 patients followed up
longitudinally, sICAM-1 values also normalised
with clinical remission (225.87
(64.25) ng/ml in patients in recent remission,
and 256.29 (75.15) ng/ml in those in
long term remission).
Conclusions—Circulating sICAM-1 concentrations
clearly correlate with clinically
apparent disease activity in GCA
patients. DiVerences with results previously
found in patients with other vasculitides
may indicate that diVerent
pathogenic mechanisms contribute to
vascular inflammation in diVerent disorder

Von Willebrand factor in the outcome of temporal arteritis
OBJECTIVE: To determine fluctuation in circulating von Willebrand factor (vWF) in the outcome of patients with temporal arteritis. METHODS: Plasma vWF antigen concentrations were measured in 65 patients with biopsy proven temporal arteritis at different disease activity stages, in 12 with isolated polymyalgia rheumatica, and in 16 controls. Fourteen temporal arteritis patients underwent serial determinations during the course of their disease. RESULTS: vWF concentrations were significantly raised in temporal arteritis (mean 220 [arbitrary units], range 96 to 720) and in polymyalgia rheumatica (mean 196, range 103 to 266) compared with healthy controls (mean 98, range 75 to 137) (P < 0.05). Although vWF values tended to be higher in temporal arteritis, no significant differences were found between temporal arteritis and polymyalgia rheumatica patients nor between temporal arteritis patients with ischaemic complications (mean 269, range 130 to 720) and those who presented with polymyalgia rheumatica or constitutional symptoms only (mean 179, range 140 to 220). The highest levels were obtained in patients with associated, mainly infectious, diseases (mean 631, range 240 to 1680). Raised vWF values found in active temporal arteritis patients (mean 220, range 96 to 720) persisted within the first two years after the beginning of treatment (mean 244, range 102 to 510) but tended to normalise in patients in long term remission (mean 143, range 50 to 260). CONCLUSIONS: Persistent elevation of vWF during early remission of temporal arteritis might represent an endothelial activation status induced by a remaining inflammatory microenvironment rather than a marker of endothelial cell injury. In long term remission, decreasing vWF concentrations might reflect progression of inflammatory lesions to a healing stage
Udział osób starszych w badaniach klinicznych — prezentacja założeń projektu PREDICT
Background. Several research reports indicate that elderly persons are underrepresented in the clinical trials on
which clinical recommendations are based. This gave stimulus to perform the ‘Increasing the PaRticipation of the
ElDerly In Clinical Trials’ (PREDICT) project, financed by the EU within the FP7. The goal of the PREDICT was to
establish the reasons and extent to which old people are excluded from trials based on age-related criteria.
Material and methods. The project gathered investigators from 11 institutions in 9 countries: Czech Republic,
Israel, Italy, Lithuania, the Netherlands, Poland, Romania, Spain, and the UK. The systematic review of literature
has been applied to assess the scope of the problem in terms of participation, predisposing and obstructive
factors, reasons of low participation and possible interventions to enhance it.
Results. The hypothesis of low and unequal representation of older people in clinical research has been confirmed.
The systematic review results became the background for conducting in 9 countries a study of opinion
shared by the health care professionals and elderly patients on their participation in clinical research.
Conclusions. The analysis of gathered data enabled creation of PREDICT charter which contains recommendations
aiming to assure the equal rights to older people to participate in clinical trials.

wieloośrodkowego, międzynarodowego programu badawczego „Zwiększanie udziału osób starszych w
badaniach klinicznych” (PREDICT), finansowanego przez Komisję Europejską w ramach VII Projektu Ramowego.
Celem projektu było rozpoznanie przyczyn i skali zjawiska oraz stworzenie zaleceń odnośnie do włączania osób
w starszym wieku do badań.
Materiał i metody. W projekcie uczestniczyło 11 instytucji z 9 krajów: Czech, Hiszpanii, Holandii, Izraela, Litwy,
Polski, Rumunii, Wielkiej Brytanii i Włoch. Zastosowano systematyczny przegląd literatury w celu zbadania zagadnienia
udziału osób starszych w badaniach klinicznych, czynników sprzyjających ich uczestnictwu i utrudniających
to uczestnictwo, przyczyn niskiego uczestnictwa oraz sposobów jego zwiększania.
Wyniki. Potwierdzono hipotezę małej i nierównej reprezentacji osób starszych w badaniach klinicznych, a wyniki
przeglądu stały się podstawą do przeprowadzenia w 9 krajach badań opinii pracowników opieki zdrowotnej
oraz opinii pacjentów w starszym wieku na temat udziału osób starszych w badaniach klinicznych.
Wnioski. Ostatecznym rezultatem tych badań jest karta zawierająca rekomendacje zmierzające do zapewnienia
równoprawnego udziału osób starszych w badaniach klinicznych.
