    Expectations of Field Supervisors in Kenya: Implications for Community-based Human Service Practicums

    Community-based learning (CBL), which provides opportunities for undergraduate students to develop disciplinary and work-related knowledge and skills, is increasingly becoming an integral component of higher education. Similar to other countries, there is a widespread belief among employers in Kenya that there is a mismatch between university programs and labour market demands. In order to enhance the employability of graduates, many departments at a Kenyan university have incorporated work-integrated experiential learning opportunities such as practicums in the educational experience for undergraduate students. The aim of this article is to describe the expectations of field supervisors in host organisations participitating in a community-based human services program at a Kenyan University. Fifteen purposively sampled field supervisors participated in individual face-to-face interviews that included questions about their understanding of the department’s expectations of student learning activities during practicums, knowledge of the academic preparation of students in the program and challenges associated with the supervisory role. Six field supervisors exhibited some level of understanding of the expectations of their role in working with practicum students, while nine field supervisors indicated unclear expectations of the students’ practicum experience. Thematic analysis revealed key themes related to: (1) student abilities, learning goals and their contributions to the host organisations, and (2) the academic program of study and academic support available from the university faculty or staff to field supervisors. The results of our study revealed a lack of clarity around practicum expectations for most of the field supervisors interviewed and insufficient preparation of the community-based organisations to host a practicum student. Several recommendations are identified to clarify the expectations of community partner organisations and the staff providing student supervision to ensure benefits for both students and the host organisation. Results from this study can be used to inform the development or improvement of practicum opportunities focused on producing a skilled workforce

    Identification of ascorbic acid  and phytochemical profiles of Scotch Bonnet pepper (Capsicum chinense) using High-Performance Liquid Chromatography-Ultraviolet  Detector (HPLC-UV)

    Researchers have discovered that red and green scotch peppers are extremely rich in phytochemicals and that these secondary metabolites are connected to several biological processes in the human body. This study aimed to determine the numerous phytochemicals present in scotch pepper seeds, flesh, and red and green varieties. The samples were pulverized after being air-dried. The phytochemicals and ascorbic acid contents were examined using a High-performance Liquid Chromatography-Ultraviolet  detector (HPLC-UV). Twelve phytochemicals and ascorbic acid were identified in the flesh and seed of red scotch pepper, with lycopene having the highest content (83.10 mg/kg), the ascorbic acid content of 8.80 mg/kg; moreover, the seed of red scotch pepper revealed twelve phytochemicals and lycopene being the highest content (75.60 mg/kg) and ascorbic acid content of 6.80 mg/kg, furthermore, fourteen phytochemicals were identified in the flesh of red scotch bonnet pepper with lycopene the highest quantity (86.00 mg/kg) and ascorbic content of 10.60 mg/kg. Twelve phytochemicals were identified in the flesh and seed of green scotch bonnet pepper, lycopene (68.70 mg/kg) being the highest content, ascorbic acid content (6.50 mg/kg), twelve phytochemicals were revealed in the flesh of green pepper and lycopene had highest quantity (76.30 mg/kg), in the seed of green pepper, the lycopene content was 68.70 mg/kg. However, it has been shown that capsaicin, dihydrocapsaicin, and lutein concentrations varied, with red scotch pepper flesh having the highest concentration of capsaicin (7.50 mg/kg). The lycopene and ascorbic acid contents of flesh were the highest (86.00 mg/kg) and (10.60 mg/kg). The study's findings showed that red pepper flesh was better than other components that were looked at.

    Supervision Challenges Encountered During Kenyan University Students’ Practicum Attachment

    There is little published research that examines the supervision experience of field attachment supervisors in Kenya. In this study, we identify the challenges encountered by field supervisors during student field attachments with community organizations. Fifteen organizations that had hosted third year students from the Department of Community Resource Management and Extension (CRM&E) at Kenyatta University during their practicum attachment were sampled using purposive and convenience sampling. Data was collected using face-to-face key informant interviews with field supervisors (n=15). Data was analyzed using NVivo10 software for qualitative analysis. Thematic analysis resulted in the identification of key themes related to clarity of learning objectives for students and the supervision challenges encountered by field supervisors during practicum. Key findings indicate that the supervisors largely lacked a clear understanding of practicum expectations and the academic preparation of students in the Community Resource Management and Extension program. The main supervision challenge is largely on orienting students to community based field work so as to enable them link theory to practice. These results can be helpful for any academic program to improve the field attachment experience of university students and the community organizations hosting the students. A better understanding of the challenges that field supervisors’ encounter is fundamental to the enhancement of the practicum experience of university students since the requisite and appropriate support can be provided to the host organizations. Subsequently, the students will be more likely to optimize the intended benefits of the practicum as a result of the host organizations enhancing the quality of field-based learning.     Nephropathy in patients with recently diagnosed type 2 diabetes mellitus black Africans

    Background: Albuminuria is long recognised as a sign of renal disease in diabetes. In type 1 diabetes, renal disease occurs after a longer duration of diabetic state. In type 2 diabetes, it is more variable. Objective: To determine the prevalence and any risk factors of albuminuria in short­ term (≤2 yrs) type 2 diabetes. Design: Cross sectional, descriptive study. Microalbuminuria was assessed using micro II strips. Setting: Outpatient diabetic clinic at Kenyatta National Hospital, Nairobi. Subjects: Patients who were newly diagnosed or had had type 2 diabetes for two years or less. Main outcome measures: Microalbuminuria, lipids, glycated haemoglobin, fasting blood glucose and blood pressure. Results: One hundred and thirty nine patients who had type 2 diabetes mellitus for ≤2 yrs were seen, but only 100 patients were included in the study over a six month period. Their mean (SD) age was 53.7 (9.3) years. Mean (SD) duration of diabetes was 10.3 (7.5) months. Fifty per cent of the study patients were hypertensive. Only 48% had HbAic \u3c8% while 36% had HbAic \u3e9%. The lipid profile of total, LDL - HDL­ cholesterol and triglycerides were predominantly within normal limits. Twenty six per cent were established to have albuminuria of which one patient had macroalbuminuria. Blood pressure, glycated haemoglobin and lipid parameters were not significantly different from patients without albuminuria Conclusion: Albuminuria occurred in a significant proportion of patients with short term type 2 diabetes. Comparable to studies done elsewhere on short-term type 2 diabetes, albuminuria is both a sign of nephropathy and a cardiovascular risk factor. It should be looked for in all patients with type 2 diabetes attending this clinic, even at diagnosis

    Assessing the long-term welfare effects of the biological control of cereal stemborer pests in East and Southern Africa: Evidence from Kenya, Mozambique and Zambia

    The International Centre of Insect Physiology and Ecology (icipe), undertook a biological control (BC) programme for control of stemborers from 1993 to 2008, to reduce cereal yield losses due to stemborer attack in East and Southern Africa. The programme released four biological control agents—the larval parasitoids Cotesia flavipes and Cotesia sesamiae, the egg parasitoid Telenomus isis and the pupal parasitoid Xanthopimpla stemmator—to control the economically important stemborer pests Busseola fusca, Chilo partellus and Sesamia calamistis. Two of the natural enemies that were released got established and spread to many localities in the region. This study adopted the economic surplus model based on production, market and GIS data to evaluate the economic benefits and cost-effectiveness of the programme in three countries—Kenya, Mozambique and Zambia. Findings show that the biological control intervention has contributed to an aggregate monetary surplus of US1.4billiontotheeconomiesofthethreecountrieswith84 1.4 billion to the economies of the three countries with 84% from maize production and the remaining 16% from sorghum production. The net present value over the twenty years period was estimated at US 272 million for both crops and ranged from US142millionforKenyatoUS 142 million for Kenya to US 39 million for Zambia. The attractive internal rate of return (IRR) of 67% compared to the considered discount rate of 10%, as well as the estimated benefit–cost ratio (BCR) of 33:1, illustrate the efficiency of investment in the BC research and intervention. The estimated number of people lifted out of poverty through the BC-programme was on average 57,400 persons (consumers and producers) per year in Kenya, 44,120 persons in Mozambique, and 36,170 persons in Zambia, representing an annual average reduction of poor populations, respectively of 0.35, 0.25 and 0.20% in each of the three countries. These findings underscore the need for increased investment in BC research to sustain cereal production and improve poor living conditions

    Effects of lime, phosphorus and rhizobia on Sesbania sesban performance in a Western Kenyan acid soil

    Aluminium (Al) toxicity, phosphorus (P) deficiency and low rhizobia populations limit Sesbania (Sesbania sesban) performance in tropical acid soils. The study determined the i) indigenous rhizobia populations that nodulate sesbania and ii) effects of lime (0 and 4 t/ha), P-fertilizer (0 and 60 kg/ha) and acid tolerant rhizobia (0 and inoculation) on soil and selected sesbania accessions performance in Western Kenya acid soil. Study site had acid soil, low available P, nitrogen (N) and rhizobia populations that nodulate Sesbania (146 cells/g soil). Lime increased soil pH, while both lime and P-fertilizer increased available P. Aluminium toxicity tolerant and P-efficient accessions (SSBSA004, SSUG3, SSUG4 and SSUG5) had faster growth, higher nodulation, shoot P, and shoot N and response to treatments than the sensitive one (SSBSA203). After 7 months of growth, SSUG3 had highest shoot length (306 cm) and dry matter (5.64 tons/ha), hence, most suitable for building poles and fuel wood. SSUG5 accumulated the highest shoot N (222 kg N/ha) and was therefore, most suitable soil N replenishment. Thus, in acid P deficient and low rhizobial population soils of Western Kenya, the use of lime, P-fertilizer, rhizobia inoculation and Al toxicity tolerant Sesbania are important for Sesbania establishment and growth.     Remote Ischaemic Conditioning in STEMI Patients in Sub-Saharan AFRICA: Rationale and Study Design for the RIC-AFRICA Trial

    Purpose: Despite evidence of myocardial infarct size reduction in animal studies, remote ischaemic conditioning (RIC) failed to improve clinical outcomes in the large CONDI-2/ERIC-PPCI trial. Potential reasons include that the predominantly low-risk study participants all received timely optimal reperfusion therapy by primary percutaneous coronary intervention (PPCI). Whether RIC can improve clinical outcomes in higher-risk STEMI patients in environments with poor access to early reperfusion or PPCI will be investigated in the RIC-AFRICA trial. // Methods: The RIC-AFRICA study is a sub-Saharan African multi-centre, randomized, double-blind, sham-controlled clinical trial designed to test the impact of RIC on the composite endpoint of 30-day mortality and heart failure in 1200 adult STEMI patients without access to PPCI. Randomized participants will be stratified by whether or not they receive thrombolytic therapy within 12 h or arrive outside the thrombolytic window (12–24 h). Participants will receive either RIC (four 5-min cycles of inflation [20 mmHg above systolic blood pressure] and deflation of an automated blood pressure cuff placed on the upper arm) or sham control (similar protocol but with low-pressure inflation of 20 mmHg and deflation) within 1 h of thrombolysis and applied daily for the next 2 days. STEMI patients arriving greater than 24 h after chest pain but within 72 h will be recruited to participate in a concurrently running independent observational arm. // Conclusion: The RIC-AFRICA trial will determine whether RIC can reduce rates of death and heart failure in higher-risk sub-optimally reperfused STEMI patients, thereby providing a low-cost, non-invasive therapy for improving health outcomes

    Animal-related factors associated with moderate-to-severe diarrhea in children younger than five years in western Kenya: A matched case-control study

    Background Diarrheal disease remains among the leading causes of global mortality in children younger than 5 years. Exposure to domestic animals may be a risk factor for diarrheal disease. The objectives of this study were to identify animal-related exposures associated with cases of moderate-to-severe diarrhea (MSD) in children in rural western Kenya, and to identify the major zoonotic enteric pathogens present in domestic animals residing in the homesteads of case and control children. Methodology/Principal findings We characterized animal-related exposures in a subset of case and control children (n = 73 pairs matched on age, sex and location) with reported animal presence at home enrolled in the Global Enteric Multicenter Study in western Kenya, and analysed these for an association with MSD. We identified potentially zoonotic enteric pathogens in pooled fecal specimens collected from domestic animals resident at children’s homesteads. Variables that were associated with decreased risk of MSD were washing hands after animal contact (matched odds ratio [MOR] = 0.2; 95% CI 0.08–0.7), and presence of adult sheep that were not confined in a pen overnight (MOR = 0.1; 0.02–0.5). Variables that were associated with increased risk of MSD were increasing number of sheep owned (MOR = 1.2; 1.0–1.5), frequent observation of fresh rodent excreta (feces/urine) outside the house (MOR = 7.5; 1.5–37.2), and participation of the child in providing water to chickens (MOR = 3.8; 1.2–12.2). Of 691 pooled specimens collected from 2,174 domestic animals, 159 pools (23%) tested positive for one or more potentially zoonotic enteric pathogens (Campylobacter jejuni, C. coli, non-typhoidal Salmonella, diarrheagenic E. coli, Giardia, Cryptosporidium, or rotavirus). We did not find any association between the presence of particular pathogens in household animals, and MSD in children. Conclusions and significance Public health agencies should continue to promote frequent hand washing, including after animal contact, to reduce the risk of MSD. Future studies should address specific causal relations of MSD with sheep and chicken husbandry practices, and with the presence of rodents

    The future of zoonotic risk prediction

    In the light of the urgency raised by the COVID-19 pandemic, global investment in wildlife virology is likely to increase, and new surveillance programmes will identify hundreds of novel viruses that might someday pose a threat to humans. To support the extensive task of laboratory characterization, scientists may increasingly rely on data-driven rubrics or machine learning models that learn from known zoonoses to identify which animal pathogens could someday pose a threat to global health. We synthesize the findings of an interdisciplinary workshop on zoonotic risk technologies to answer the following questions. What are the prerequisites, in terms of open data, equity and interdisciplinary collaboration, to the development and application of those tools? What effect could the technology have on global health? Who would control that technology, who would have access to it and who would benefit from it? Would it improve pandemic prevention?
