    Peptic ulcer

    The probable explanation of the development of a chronic peptic ulcer is that there is a lowering of local vitality. Important amongst numerous predisposing causes for this are faulty diet and nervous strain. Described types of organisms either attack the stomach and duodenum before such a devitilisation or more likely after it. The acid of the stomach tends to keep the ulcer open and to counteract a tendency for spontaneous cure. As Judd has truly remarked, "The final word concerning (the pathogenesis of) gastric and duodenal ulcer has not been spoken". Diagnosis rests particularly on two considerations. The first consideration is that following a careful clinical examination where especial note is made of the history. The second is a consideration of an X-ray examination. When operation has been advised or a post-mortem has been made, the excellence of modern radiology is proved, I have no regrets for basing my diagnosis so essentially on this ancillary science. Claims that a proper etiological basis had been found, led to the administration of histidine. Its use has been very doubtful, if not useless. A recent successful treatment of haematemesls has been described. The mortality rate is not high. The future health and working capacity of the individual is frequently impaired. Gastric acidity must be neutralised or buffered. Thus diet, alkalies and buffering agents are necessary. A reasonable proportion of cases does well. Relapses are likely in the present state of our knowledge. Routine treatments are useful. Physicians and Surgeons are co-operating to obtain a higher proportion of cures. There must, however, be a decided change from general treatment of the disease, to the individual study of the patient, and his circumstances

    Angular kyphosis as an indicator of the prevalence of Pott's disease in Transkei

    To understand better the prevalence, distribution and major causes of sagittal spinal deformity in a rural homeland, the authors conducted a study of angular kyphosis in the spines of 2 329 Transkei patients. Thirty one (1,33%) had angular kyphosis. Lateral chest radiographs were obtained from 22 of these patients. Radiographic kyphotic angles ranged from 280 to 1300 (mean: 70,3 ± 7,6). The vast majority (81 %) demonstrated classical clinical and/or radiographic findings of tuberculous aetiology. Less frequent aetiologies included fractures (2), osteoporosis (1), congenital malformation (1) and kyphosis of unknown origin (2). Eleven of the kyphotic patients were seeking care for unrelated problems and were asymptomatic in respect of their kyphoses. As a subset, the asymptomatic individuals demonstrated a similar aetiological distribution, with 73% strongly suggestive of tuberculous aetiology. The prevalence of asymptomatic angular kyphosis in this unselected Transkei patient population was 0,47% ± 0,14%. In this hospital-based study, angular kyphosis proved a valuable marker for spinal tuberculosis. Because tuberculous spondylitis is more successfully treated when detected early, spinal palpation should be included in the routine physical examination of patients or populations at risk for tuberculosis

    Aromatic Features in AGN: Star-Forming Infrared Luminosity Function of AGN Host Galaxies

    We describe observations of aromatic features at 7.7 and 11.3 um in AGN of three types including PG, 2MASS and 3CR objects. The feature has been demonstrated to originate predominantly from star formation. Based on the aromatic-derived star forming luminosity, we find that the far-IR emission of AGN can be dominated by either star formation or nuclear emission; the average contribution from star formation is around 25% at 70 and 160 um. The star-forming infrared luminosity functions of the three types of AGN are flatter than that of field galaxies, implying nuclear activity and star formation tend to be enhanced together. The star-forming luminosity function is also a function of the strength of nuclear activity from normal galaxies to the bright quasars, with luminosity functions becoming flatter for more intense nuclear activity. Different types of AGN show different distributions in the level of star formation activity, with 2MASS> PG> 3CR star formation rates.Comment: Accepted for publication in ApJ, 24 pages, 13 figure

    Recognition of aminoacyl-tRNA: a common molecular mechanism revealed by cryo-EM

    The accuracy of ribosomal translation is achieved by an initial selection and a proofreading step, mediated by EF-Tu, which forms a ternary complex with aminoacyl(aa)-tRNA. To study the binding modes of different aa-tRNAs, we compared cryo-EM maps of the kirromycin-stalled ribosome bound with ternary complexes containing Phe-tRNAPhe, Trp-tRNATrp, or Leu-tRNALeuI. The three maps suggest a common binding manner of cognate aa-tRNAs in their specific binding with both the ribosome and EF-Tu. All three aa-tRNAs have the same ‘loaded spring' conformation with a kink and twist between the D-stem and anticodon stem. The three complexes are similarly integrated in an interaction network, extending from the anticodon loop through h44 and protein S12 to the EF-Tu-binding CCA end of aa-tRNA, proposed to signal cognate codon–anticodon interaction to the GTPase centre and tune the accuracy of aa-tRNA selection

    Mortality among Norwegian doctors 1960-2000

    <p>Abstract</p> <p>Background</p> <p>To study the mortality pattern of Norwegian doctors, people in human service occupations, other graduates and the general population during the period 1960-2000 by decade, gender and age. The total number of deaths in the study population was 1 583 559.</p> <p>Methods</p> <p>Census data from 1960, 1970, 1980 and 1990 relating to education were linked to data on 14 main causes of death from Statistics Norway, followed up for two five-year periods after census, and analyzed as stratified incidence-rate data. Mortality rate ratios were computed as combined Mantel-Haenzel estimates for each sex, adjusting for both age and period when appropriate.</p> <p>Results</p> <p>The doctors had a lower mortality rate than the general population for all causes of death except suicide. The mortality rate ratios for other graduates and human service occupations were 0.7-0.8 compared with the general population. However, doctors have a higher mortality than other graduates. The lowest estimates of mortality for doctors were for endocrine, nutritional and metabolic diseases, diseases in the urogenital tract or genitalia, digestive diseases and sudden death, for which the numbers were nearly half of those for the general population. The differences in mortality between doctors and the general population increased during the periods.</p> <p>Conclusions</p> <p>Between 1960 and 2000 mortality for doctors converged towards the mortality for other university graduates and for people in human service occupations. However, there was a parallel increase in the gap between these groups and the rest of the population. The slightly higher mortality for doctors compared with mortality for other university graduates may be explained by the higher suicide rate for doctors.</p

    A flexible loop in yeast ribosomal protein L11 coordinates P-site tRNA binding

    High-resolution structures reveal that yeast ribosomal protein L11 and its bacterial/archael homologs called L5 contain a highly conserved, basically charged internal loop that interacts with the peptidyl-transfer RNA (tRNA) T-loop. We call this the L11 ‘P-site loop’. Chemical protection of wild-type ribosome shows that that the P-site loop is inherently flexible, i.e. it is extended into the ribosomal P-site when this is unoccupied by tRNA, while it is retracted into the terminal loop of 25S rRNA Helix 84 when the P-site is occupied. To further analyze the function of this structure, a series of mutants within the P-site loop were created and analyzed. A mutant that favors interaction of the P-site loop with the terminal loop of Helix 84 promoted increased affinity for peptidyl-tRNA, while another that favors its extension into the ribosomal P-site had the opposite effect. The two mutants also had opposing effects on binding of aa-tRNA to the ribosomal A-site, and downstream functional effects were observed on translational fidelity, drug resistance/hypersensitivity, virus maintenance and overall cell growth. These analyses suggest that the L11 P-site loop normally helps to optimize ribosome function by monitoring the occupancy status of the ribosomal P-site
