    Presentation and management of keloid scarring following median sternotomy: a case study

    <p>Abstract</p> <p>Introduction</p> <p>Keloid scars following median sternotomy are rare and occur more frequently in pigmented skin. Different management strategies have been described with variable success. We present a case of keloid scar formation following cardiac surgery including our management and the final aesthetic result.</p> <p>Case description</p> <p>A 64 year old female of fair complexion underwent mitral valve replacement. The procedure and postoperative recovery were uncomplicated, however, during the following year, thick keloid scars formed over the incision sites. Initial non surgical measures failed to relieve pain and did not offer any tangible aesthetic benefit. Eventually surgical excision was attempted. She presented to our clinic for nine months follow up with significant improvement in pain and aesthetic result.</p> <p>Discussion and Evaluation</p> <p>Several theories have attempted to explore the pathophysiology of keloid scar formation. A number of predisposing factors have been documented however none existed in this case. A variety of invasive and non invasive approaches have been described but significant differences in success rates and methodology of investigations still precludes a standardized management protocol.</p> <p>Conclusions</p> <p>In this case study a rare presentation of keloid scar has been presented. The variety of methods used to improve pain and aesthetic result demonstrates the propensity of keloid scars to recur and the therapeutic challenges that surgeons have to face in their quest for a satisfactory patient outcome.</p

    Ultrafine grained plates of Al-Mg-Si alloy obtained by Incremental Equal Channel Angular Pressing : microstructure and mechanical properties

    In this study, an Al-Mg-Si alloy was processed using via Incremental Equal Channel Angular Pressing (I-ECAP) in order to obtain homogenous, ultrafine grained plates with low anisotropy of the mechanical properties. This was the first attempt to process an Al-Mg-Si alloy using this technique. Samples in the form of 3 mm-thick square plates were subjected to I-ECAP with the 90˚ rotation around the axis normal to the surface of the plate between passes. Samples were investigated first in their initial state, then after a single pass of I-ECAP and finally after four such passes. Analyses of the microstructure and mechanical properties demonstrated that the I-ECAP method can be successfully applied in Al-Mg-Si alloys. The average grain size decreased from 15 - 19 µm in the initial state to below 1 µm after four I-ECAP passes. The fraction of high angle grain boundaries in the sample subjected to four I-ECAP passes lay within 53-57 % depending on the examined plane. The mechanism of grain refinement in Al-Mg-Si alloy was found to be distinctly different from that in pure aluminium with the grain rotation being more prominent than the grain subdivision, which was attributed to lower stacking fault energy and the reduced mobility of dislocations in the alloy. The ultimate tensile strength increased more than twice, whereas the yield strength - more than threefold. Additionally, the plates processed by I-ECAP exhibited low anisotropy of mechanical properties (in plane and across the thickness) in comparison to other SPD processing methods, which makes them attractive for further processing and applications

    Desde la Sede de Occidente y, específicamente, en el seno de la Coordinación de Investigación y con el apoyo de la Dirección de la Sede y las Coordinaciones de Docencia, Administración y Acción Social, así como de la Vicerrectoría de Investigación, se han celebrado en el 2011 y en el 2012 dos coloquios internacionales sobre diversidad cultural y estudios regionales. El propósito de ambos consistió principalmente en motivar a investigadores e investigadoras de las distintas unidades académicas de la Universidad de Costa Rica y de otras instituciones de Educación Superior, así como a representantes de Centros e Institutos de Investigación nacionales e internacionales, a presentar resultados de investigaciones que contribuyeran a un mayor conocimiento de los procesos culturales y que dieran a conocer resultados que permitieran la comprensión de las realidades de distintos sectores y regiones.     Gluteal V-Y advancement fasciocutaneous flap for treatment of chronic pilonidal sinus disease

The procedure should cure the patient, and allow speedy resurription of normal activities by reducing pain and disability. This retrospective Study was conducted to evaluate our experience with the V-Y fasciocutaneous advancement flap and to review current publications about flap surgery for the treatment of sacrococcygeal pilonidal sinus. We describe the application of the fasciocutaneous V-Y advancement flap for reconstruction or defects after radical excision of recurrent pilonidal sinus in 11 cases. Primary and uneventful wound healing was achieved in all patients but two who developed minor wound breakdown. Large defects after excision can easily be closed using the V-Y advancement flap. This type of flap closure in selected cases offers tension-free, recurrence-free, and reliable skin coverage while flattening the natal cleft that predisposes to recurrences. Reliable flap closure reduces hospital stay, costs, as well as disability and time spent off work

    Texture evolution of IF-steel sheets processed by equal-channel angular sheet extrusion (ECASE)

    Interstitial-Free steel (IF-steel) sheets were severe plastically deformed using a continuous equal-channel angular extrusion/pressing technique called "Equal-Channel Angular Sheet Extrusion (ECASE). After processing, texture development as well as microstructural alteration and tensile properties were investigated. The microstructural investigations revealed that the processed sheets exhibited a dislocation cell and/or subgrain structures with mostly low angle grain boundaries. It was also observed that the strength of the processed sheets increased substantially after ECASE processing in the expense of ductility. It was shown that the ECASE has moderate influence on the texture of IF-steel sheets through route A. Intially there was θ partial fiber which changes to {110} θ with straining. © (2012) Trans Tech Publications, Switzerland
