    Innovations in Hemodialysis Access

    The established types of vascular access for hemodialysis are central venous catheters (CVCs), arteriovenous fistulas (AVFs), and arteriovenous grafts (AVGs). Innovations in CVC tip and coating design may improve patency and blood flow rates. AVFs are preferred over CVCs as they are less prone to clotting and infection, while providing reliable and adequate blood flow rates. However, AVF creation requires a surgical procedure with associated risks. Because of a paucity of surgeons available to create high-quality dialysis access, newer access creation techniques have been developed, including a percutaneous endovascular method that has the potential to revolutionize dialysis access. Innovations in AVGs include drug-eluting devices that may reduce neointimal hyperplasia and bioengineered blood vessels. To bypass vessel stenoses, a hybrid AVG/CVC device has been developed. Although many of these innovations have yet to become mainstream, they promise to improve dialysis access in the future

    Sequence determinants for DNA packaging specificity in the S. aureus pathogenicity island SaPI1

    The SaPIs and their relatives are a family of genomic islands that exploit helper phages for high frequency horizontal transfer. One of the mechanisms used by SaPIs to accomplish this molecular piracy is the redirection of the helper phage DNA packaging machinery. SaPIs encode a small terminase subunit that can be substituted for that of the phage. In this study we have determined the initial packaging cleavage sites for helper phage 80α, which uses the phage-encoded small terminase subunit, and for SaPI1, which uses the SaPI-encoded small terminase subunit. We have identified a 19 nt SaPI1 sequence that is necessary and sufficient to allow high frequency 80α transduction of a plasmid by a terminase carrying the SaPI1-encoded small subunit. We also show that the hybrid enzyme with the SaPI1 small terminase subunit is capable of generalized transduction.This work was performed in part under FDA's Medical Countermeasures Initiative, Contract #HHSF2232010000521 and NIH 1R56 AI081837 (to GEC).     On the principal bifurcation branch of a third order nonlinear long-wave equation

    We study the principal bifurcation curve of a third order equation which describes the nonlinear evolution of several systems with a long--wavelength instability. We show that the main bifurcation branch can be derived from a variational principle. This allows to obtain a close estimate of the complete branch. In particular, when the bifurcation is subcritical, the large amplitude stable branch can be found in a simple manner.Comment: 11 pages, 3 figure

    Applying a User-centred Approach to Interactive Visualization Design

    Analysing users in their context of work and finding out how and why they use different information resources is essential to provide interactive visualisation systems that match their goals and needs. Designers should actively involve the intended users throughout the whole process. This chapter presents a user-centered approach for the design of interactive visualisation systems. We describe three phases of the iterative visualisation design process: the early envisioning phase, the global specification hase, and the detailed specification phase. The whole design cycle is repeated until some criterion of success is reached. We discuss different techniques for the analysis of users, their tasks and domain. Subsequently, the design of prototypes and evaluation methods in visualisation practice are presented. Finally, we discuss the practical challenges in design and evaluation of collaborative visualisation environments. Our own case studies and those of others are used throughout the whole chapter to illustrate various approaches

    Transcription and Translation Products of the Cytolysin Gene psm-mec on the Mobile Genetic Element SCCmec Regulate Staphylococcus aureus Virulence

    The F region downstream of the mecI gene in the SCCmec element in hospital-associated methicillin-resistant Staphylococcus aureus (HA-MRSA) contains two bidirectionally overlapping open reading frames (ORFs), the fudoh ORF and the psm-mec ORF. The psm-mec ORF encodes a cytolysin, phenol-soluble modulin (PSM)-mec. Transformation of the F region into the Newman strain, which is a methicillin-sensitive S. aureus (MSSA) strain, or into the MW2 (USA400) and FRP3757 (USA300) strains, which are community-acquired MRSA (CA-MRSA) strains that lack the F region, attenuated their virulence in a mouse systemic infection model. Introducing the F region to these strains suppressed colony-spreading activity and PSMα production, and promoted biofilm formation. By producing mutations into the psm-mec ORF, we revealed that (i) both the transcription and translation products of the psm-mec ORF suppressed colony-spreading activity and promoted biofilm formation; and (ii) the transcription product of the psm-mec ORF, but not its translation product, decreased PSMα production. These findings suggest that both the psm-mec transcript, acting as a regulatory RNA, and the PSM-mec protein encoded by the gene on the mobile genetic element SCCmec regulate the virulence of Staphylococcus aureus

    Attrition among families of divorce: Patterns in an outpatient psychiatric population

    The accelerated divorce rate has greatly increased the number of children of divorce. In addition, the children and families of divorce are proportionately overrepresented in populations seeking child guidance and psychiatric services. However, the patterns in the use of such services by these families has been unexamined. In our study we find no differences in the use of outpatient psychiatric services between families where both biological parents are present and families of divorce. A closer look at the divorced families reveals that recently divorced mother-headed families more frequently interrupt both the evaluation and treatment phases of clinic contact. Several clinical patterns are described which help explain the finding and it is suggested that the traditional child guidance model may not be the most suitable intervention modality in working with these families.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/44293/1/10591_2004_Article_BF00927093.pd
