    Patient Specific Seizure Prediction System Using Hilbert Spectrum and Bayesian Networks Classifiers

    The aim of this paper is to develop an automated system for epileptic seizure prediction from intracranial EEG signals based on Hilbert-Huang transform (HHT) and Bayesian classifiers. Proposed system includes decomposition of the signals into intrinsic mode functions for obtaining features and use of Bayesian networks with correlation based feature selection for binary classification of preictal and interictal recordings. The system was trained and tested on Freiburg EEG database. 58 hours of preictal data, 40-minute data blocks prior to each of 87 seizures collected from 21 patients, and 503.1 hours of interictal data were examined resulting in 96.55% sensitivity with 0.21 false alarms per hour, 13.896% average proportion of time spent in warning, and 33.21 minutes of average detection latency using 30-second EEG segments with 50% overlap and a simple postprocessing technique resulting in a decision (a seizure is expected/not expected) every 5 minutes. High sensitivity and low false positive rate with reasonable detection latency show that HHT based features are acceptable for patient specific seizure prediction from intracranial EEG data. Time spent for testing an EEG segment was 4.1451 seconds on average, which makes the system viable for use in real-time seizure control systems

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

The essential oil of Mentha longifolia subsp. longifolia growing in the Bahesaray area (Van Province, Turkey) was studied. This study designed for determine of essential oil constituent Mentha longifolia subsp. longifolia that collected from wild area. Mint leaves sample essential oils obtained by hydro distillation and essential oil components were determined using GC-MS. The main component of wild grown Mentha longifolia subsp. longifolia was Menthone (19.31%). Second one and others were Pulegone (12.42%), Piperitone (11.05%), Dihydrocarvon (8.32%), Limonene (6.1%), 3-Terpinolenone (5.66%), 1,8-Cineole (4.37%), Germacrene D (3.38%) and Caryopyllene (3.19%), respectively

    Knowledge About Cervical Cancer Risk Factors and Practices of Pap Testing Among Turkish Immigrant Women in the United States

    WOS: 000444041100023PubMed ID: 28929252The objective of this study was to examine knowledge about cervical cancer risk factors and practices of Pap testing among female Turkish immigrants in the state of Florida in the United States of America (USA). This descriptive study was conducted between April and September 2012. The study sampling was consist of 156 Turkish women living in the state of Florida. A questionnaire-based survey was conducted among the population of Turkish immigrant women. On the survey form comprised of a total of 37 questions and three sections there are questions pertaining to the socio-demographic characteristics of the individuals, their knowledge on the cervical cancer risk factors and their approach to getting Pap smear tests. Statistical Package for Social Sciences (version 16.0) was used to compute frequency and descriptive statistics related to demographic data. The average age of the women is 35.67 +/- 10.0. More than half of women (66%) women reported cervical cancer screening at least one. Over two-thirds knew that having abnormal vaginal bleeding (85.8%) and vaginal infections (78.2%), having sexual activity with a man who has had multiple sexual partners (61.5%), and having multiple sexual partners (61.5%) increase the risk of cervical cancer. The result of the multivariate regression analysis have determined that the age of immigrant women (OR 11.3, 95 % CI 5.1-25.2, p:0.000) and the number of children ( OR 3.4, 95 % CI 1.7-6.9, p:0.000) are factors that impact pap smear testing behavior. Our study findings confirm low levels of Pap testing and show important knowledge deficits about cervical cancer risk factors and the importance of Pap testing among Turkish immigrant women. It is recommended that the immigrant women be educated about cervical cancer by means of organizing conferences in their own language and preparing informative materials

    Parents' and teachers' expectations of school nurse roles: A scale development study

The purpose of this study is to develop a valid and reliable assessment tool that can be used to determine parents' and teachers' expectations of school nurse's roles. Methods: For the methodological research "Parents' and Teachers' Expectations of School Nurse's Roles" scale was designed and administered at primary and high schools in Turkey. To assess the data, assessment of item variability, internal consistency, factor analysis, correlations between the scale and subscales, and test-retest reliability were utilized. Results: The content validity index for the scale was 0.94. As a result of core components analysis varimax rotation, 7 factors were obtained from 54 items. Cronbach's a coefficient for the factors ranged from 0.82 to 0.92. The intra-class correlation coefficient for the test-retest reliability was 0.859, P < 0.001. Conclusions: The scale was a rather valid and reliable assessment tool in determining the parents' and teachers' expectations of school nurses' roles. (C) 2017 Chinese Nursing Association.     CHEMICAL DIVERSITY OF ESSENTIAL OIL FROM LEAVES OF ZIZIPHORA CLINOPODIOIDES GROWING IN VAN, TURKEY

    WOS: 000436522600021Blue Mint Bush (Ziziphora clinopodioides) belongs to the family Lamiaceae and grows in Iran, Iraq, Central and Eastern part of Turkey. The leaves of Ziziphora clinopodioides L. samples were collected on the beginning of July (2010) from Kocko-pru area (Van Province, Turkey) at an altitude of 1924 m and described accordingly. Hydrodistillation was used to isolate the essential oil of the plant and then 33 constituents were identified by using GC-MS. The majority presented compounds of the essential oil were Pulegone (29.31%), Menthone (21.79%), 1,8-Cineole (15.31%), beta-Pinene (8.74%), Limonene (3.91%), alpha-Pinene (3.17%), 3-Terpinolenone (3.09%), beta-Phellandrene (2.88%), respectively. Menthofurane, Isopulegone and Germacrene D are represented by low concentrations. A cluster analysis was carried on the essential oil composition of Z. clinopodioides to identify homogenous groups

    A series of 1,4-benzoquinone derivatives from 2,5-dichloro-3,6-diethoxy-1,4-benzoquinone and 2,6-dichloro-3,5-diethoxy-1,4-benzoquinone were prepared by nucleophilic substitution reactions of sulfur and nitrogen nucleophiles. Spectral techniques (H-1 NMR, C-13 NMR, FT-IR, and LC-MS) were employed to structurally characterize the reaction products of alkoxy, chloro substituted-1,4-benzoquinones with thiols and amines in the presence of sodium carbonate in ethanol at room temperature. The orientations and the regioselectivity of the reactions of alkoxy, chloro substituted-1,4-benzoquinones with various thiol and amine nucleophiles are discussed. (C) 2013 Academie des sciences.
