    Diagnostic criteria to aid the differential diagnosis of patients presenting with transient loss of consciousness: A systematic review

    BACKGROUND: Transient loss of consciousness (TLOC) is a common presentation in primary care. Over 90% of these are due to epileptic seizures (ES), syncope, or psychogenic non-epileptic seizures (PNES). Misdiagnosis rates are as high as 30%. METHODS: Systematic review of inter-ictal clinical criteria to aid differential diagnosis of TLOC. We searched Medline, EMBASE, CINAHL and PsycInfo databases, as well as relevant grey literature depositories and citations of relevant reviews and guidelines for studies giving sensitivity and specificity of inter-ictal clinical characteristics used to differentiate between causes of TLOC. Two independent reviewers selected studies for inclusion and performed critical appraisal of included articles. We performed a narrative synthesis of included studies. RESULTS: Of 1023 results, 16 papers were included. Two compared syncope, ES, and PNES; all others compared ES and PNES. All were at significant risk of bias in at least one domain. 6 studied patient symptoms, 6 medical and social history, 3 witness reports and 1 examination findings. No individual criterion differentiated between diagnoses with high sensitivity and specificity. CONCLUSIONS: There is a lack of validated diagnostic criteria to help clinicians assessing patients in primary or emergency care settings to discriminate between common causes of TLOC. Performance may be improved by combining sets of criteria in a clinical decision rule, but no such rule has been validated prospectively against gold-standard diagnostic criteria

    Characteristics of long-duration inhibitory postsynaptic potentials in rat neocortical neurons in vitro

    1. The characteristics of long-duration inhibitory postsynaptic potentials (l-IPSPs) which are evoked in rat frontal neocortical neurons by local electrical stimulation were investigated with intracellular recordings from anin vitro slice preparation. 2. Stimulation with suprathreshold intensities evoked l-IPSPs with typical durations of 600–900 msec at resting membrane potential. Conductance increases of 15–60% were measured at the peak amplitude of l-IPSPs (150–250 msec poststimulus). 3. The duration of the conductance increases during l-IPSPs displayed a significant voltage dependence, decreasing as the membrance potential was depolarized and increasing with hyperpolarization. 4. The reversal potential of l-IPSPs is significantly altered by reductions in the extracellular potassium concentration. Therefore it is concluded that l-IPSPs in rat neocortical neurons are generated by the activation of a potassium conductance. 5. l-IPSPs exhibit stimulation fatigue. Stimulation with a frequency of 1 Hz produces a complete fatigue of the conductance increases during l-IPSPs after approximately 20 consecutive stimuli. Recovery from this fatigue requires minutes. 6. l-IPSPs are not blocked by bicuculline but are blocked by baclofen

    Quercetin improves and protects Calu-3 airway epithelial barrier function

    Introduction: In light of the impact of airway barrier leaks in COVID-19 and the significance of vitamin D in COVID-19 outcomes, including airway barrier protection, we investigated whether the very common dietary flavonoid quercetin could also be efficacious in supporting airway barrier function.Methods: To address this question, we utilized the widely used airway epithelial cell culture model, Calu-3.Results: We observed that treating Calu-3 cell layers with quercetin increased transepithelial electrical resistance while simultaneously reducing transepithelial leaks of 14C-D-mannitol (Jm) and 14C-inulin. The effects of quercetin were concentration-dependent and exhibited a biphasic time course. These effects of quercetin occurred with changes in tight junctional protein composition as well as a partial inhibition of cell replication that resulted in decreased linear junctional density. Both of these effects potentially contribute to improved barrier function. Quercetin was equally effective in reducing the barrier compromise caused by the pro-inflammatory cytokine TNF-α, an action that seemed to derive, in part, from reducing the elevation of ERK 1/2 caused by TNF-α.Discussion: Quercetin improved Calu-3 barrier function and reduced TNF-α-induced barrier compromise, mediated in part by changes in the tight junctional complex

    Cluster Abell 520: a perspective based on member galaxies. A cluster forming at the crossing of three filaments?

    The connection of cluster mergers with the presence of extended, diffuse radio sources in galaxy clusters is still debated. An interesting case is the rich, merging cluster Abell 520, containing a radio halo. A recent gravitational analysis has shown in this cluster the presence of a massive dark core suggested to be a possible problem for the current cold dark matter paradigm. We aim to obtain new insights into the internal dynamics of Abell 520 analyzing velocities and positions of member galaxies. Our analysis is based on redshift data for 293 galaxies in the cluster field obtained combining new redshift data for 86 galaxies acquired at the TNG with data obtained by CNOC team and other few data from the literature. We also use new photometric data obtained at the INT telescope. We combine galaxy velocities and positions to select 167 cluster members around z~0.201. We analyze the cluster structure using the weighted gap analysis, the KMM method, the Dressler-Shectman statistics and the analysis of the velocity dispersion profiles. We compare our results with those from X-ray, radio and gravitational lensing analyses. We find that Abell 520 is definitely a very complex system. Our results suggest that we are looking at a cluster forming at the crossing of three filaments of the large scale structure. In particular, we detect a filament aligned with the LOS and projected onto the center of the forming cluster.     Encouraging eyewitnesses to falsely corroborate allegations: effects of rapport-building and incriminating evidence

    Building rapport involves developing a harmonious relationship with another person and conveying understanding and acceptance towards that person. Law enforcement officers use rapport-building to help gather information from witnesses. But could rapport-building, in some situations, work to contaminate eyewitness testimony? Research shows that compelling incriminating evidence can lead people to corroborate false accusations made against another person. We investigated whether rapport-building – when combined with either Verbal or Verbal+Visual false evidence – might boost these corroboration rates. Subjects took part in a pseudo-gambling task, in which their counterpart was falsely accused of cheating. Using a 2 (Rapport: Rapport vs. No-rapport) × 2 (Incriminating Evidence: Verbal vs. Verbal+Visual) between-subjects design, we persuaded subjects to corroborate the accusation. We found that both rapport and verbal+visual incriminating evidence increased the compliance rate. Even when the incriminating evidence was only presented verbally, rapport-building subjects were almost three times as likely to corroborate a false accusation compared to subjects who did not undergo rapport-building. Our results suggest that although there is widespread and strong support for using rapport-building in interviews, doing so also has the potential to aggravate the contaminating power of suggestive interview techniques
