    Responsiveness and value chain in sugar-ethanol production

    Brazil is the world’s major sugarcane producer. In 2018/19, the country will have produced about 47.34 million tons of sugar and 58.8 billion liters of ethanol. Sugar and ethanol are produced in the same production process and the definition of both quantities is pre-established to sugarcane agro-industry. The purpose of this paper is to identify how managers define the production mix of sugar-ethanol in an agro-industry and how this decision adds value to its operations. The results showed that the searched mill adds value to its production through responsiveness and flexibility while orienting the production to sugar and/or ethanol according to the most profitable market during the moment of the decision making

    O trabalho tem o objetivo de apresentar uma comparação dos parâmetros físicos, químicos e bacteriológicos da água da chuva com a Portaria de consolidação n° 05/2017 do Ministério da Saúde que trata da potabilidade da água para consumo humano, e a viabilidade de uso para o beneficiamento de açaí. O método utilizado foi a avaliação das características físico-químicos e microbiológicos da água da chuva coletada no sistema de captação instalado nas dependências do NUMA/UFPA. O parâmetro pH apresentou valores dentro do padrão da portaria. Em relação aos coliformes totais, este apresentou um número incontáveis de colônias. Assim, faz-se necessário a implantação de métodos corretivos como a cloração para diminuir ou eliminar a presença dessas bactérias no sistema.The objective of this study is to compare the physical, chemical and bacteriological parameters of rainwater with the consolidation of the Ordinance n °05/2017 of the Ministry of Health, which deals with the potability of water for human consumption and the feasibility of use for the treatment of açaí. The method used was the evaluation of the physical-chemical and microbiological characteristics of the rainwater collected in the catchment system installed in the NUMA/UFPA facilities. The pH parameter presented values within the ordicance standard. In relation to the total coliforms, this presented a countless number of colonies. Thus, it is necessary to implement corrective methods such as chlorination to reduce or eliminate the presence of these bacteria in the system

    Malignant transformation of oral leukoplakia:a multicentric retrospective study in Brazilian population

    Among the oral potentially malignant disorders, leukoplakia stands out as the most prevalent. The purpose of this study was to analyse the clinical-pathological features of oral leukoplakia in groups of patients from three major pathology centers in two different regions of Brazil, in order to determine which factors would be associated to the clinical risk of malignant transformation. A total of 148 patients was analyzed, and data regarding gender, age, site, classification of the clinical subtype, harmful habits such as use of tobacco and alcohol, time of evolution and presence of dysplasia were collected. The association between risk factors and malignant transformation was investigated using the chi-square test and Fischer's exact test for correlation of variables. A significance level of 5% (p?0.05) was used. The mean age of the patients was 60 years, and 56% were female. Most of the lesions (34,5%) were located in the lateral and ventral regions of the tongue. Of the 148 patients, ninety had clinical follow-up. Malignant transformation occurred in 13 patients (8.8%), with an average of 44 months of follow up. Non-smoker, nonhomogeneous clinical presentation, location at the tongue, and the presence of high degree of dysplasia were statistically relevant factors associated with a higher risk of transformation transformation

    Pre-exposure prophylaxis among Brazilian men who have sex with men: a comparative study between migrants and non-migrants

    IntroductionInvestigating the use and adherence to pre-exposure prophylaxis (PrEP) in MSM is a global health priority in the fight against HIV. Strategies must be capable of increasing usage and reaching not only the population living in the country but also those who immigrate, who face additional vulnerabilities. Based on this, in this observational, cross-sectional, and analytical study, our aim is to analyze the use of PrEP among Brazilian men who have sex with men, whether they are migrants or not. We aim to highlight preventive opportunities and strategies for the global health scenario.MethodsWe utilized a sample of Brazilians living in the country and Brazilian immigrants residing in Portugal, one of the main destinations for Brazilians in recent years. To estimate the prevalence ratio (PR) of PrEP use, we employed the Poisson regression model with robust variance estimation using a covariance matrix.ResultsA total of 1,117 Brazilian MSM PrEP users participated in this study, with 788 residing in Brazil and 328 in Portugal. Multivariate analysis was conducted in three stages: overall, and for subgroups of residents in Brazil and immigrants in Portugal. We identified four convergent factors that increased the prevalence of PrEP use in Brazilians regardless of migration status: having two or more casual sexual partners per month, engaging in challenging sexual practices as the receptive partner, disclosing serological status on apps, and being single. Among native Brazilians, four unique factors stood out: being in a polyamorous relationship, having sexual relations with unknown casual partners, and having higher levels of education.DiscussionThis study highlights the need to implement strategies to strengthen PrEP adherence in Brazil and create international programs that facilitate its usage among populations migrating between these two countries

    Identification of clusters of asthma control: A preliminary analysis of the inspirers studies

All rights reserved. Aims: To identify distinct asthma control clusters based on Control of Allergic Rhinitis and Asthma Test (CARAT) and to compare patients’ characteristics among these clusters. Methods: Adults and adolescents (≥13 years) with persistent asthma were recruited at 29 Portuguese hospital outpatient clinics, in the context of two observational studies of the INSPIRERS project. Demographic and clinical characteristics, adherence to inhaled medication, beliefs about inhaled medication, anxiety and depression, quality of life, and asthma control (CARAT, >24 good control) were collected. Hierarchical cluster analysis was performed using CARAT total score (CARAT-T). Results: 410 patients (68% adults), with a median (percentile 25–percentile 75) age of 28 (16-46) years, were analysed. Three clusters were identified [mean CARAT-T (min-max)]: cluster 1 [27(24-30)], cluster 2 [19(14-23)] and cluster 3 [10(2-13)]. Patients in cluster 1 (34%) were characterised by better asthma control, better quality of life, higher inhaler adherence and use of a single inhaler. Patients in clusters 2 (50%) and 3 (16%) had uncontrolled asthma, lower inhaler adherence, more symptoms of anxiety and depression and more than half had at least one exacerbation in the previous year. Further-more, patients in cluster 3 were predominantly female, had more unscheduled medical visits and more anxiety symp-toms, perceived a higher necessity of their prescribed inhalers but also higher levels of concern about taking these inhalers. There were no differences in age, body mass index, lung function, smoking status, hospital admissions or specialist physician follow-up time among the three clusters. Conclusion: An unsupervised method based on CARAT--T, identified 3 clusters of patients with distinct, clinically meaningful characteristics. The cluster with better asthma control had a cut-off similar to the established in the validation study of CARAT and an additional cut-off seems to distinguish more severe disease.     Hexapoda Yearbook (Arthropoda: Mandibulata: Pancrustacea) Brazil 2020: the first annual production survey of new Brazilian species

    This paper provided a list of all new Brazilian Hexapoda species described in 2020. Furthermore, based on the information extracted by this list, we tackled additional questions regarding the taxa, the specialists involved in the species descriptions as well as the journals in which those papers have been published. We recorded a total of 680 new Brazilian species of Hexapoda described in 2020, classified in 245 genera, 112 families and 18 orders. These 680 species were published in a total of 219 articles comprising 423 different authors residing in 27 countries. Only 30% of these authors are women, which demonstrates an inequality regarding sexes. In relation to the number of authors by species, the majority of the new species had two authors and the maximum of authors by species was five. We also found inequalities in the production of described species regarding the regions of Brazil, with Southeast and South leading. The top 10 institutions regarding productions of new species have four in the Southeast, two at South and with one ate North Region being the outlier of this pattern. Out of the total 219 published articles, Zootaxa dominated with 322 described species in 95 articles. The average impact factor was of 1.4 with only seven articles being published in Impact Factors above 3, indicating a hardship on publishing taxonomic articles in high-impact journals.The highlight of this paper is that it is unprecedent, as no annual record of Hexapoda species described was ever made in previous years to Brazil.Fil: Silva Neto, Alberto Moreira.     Manejo da hanseníase na atenção primária: diagnóstico e tratamento / Leprosy management in primary care: diagnosis and treatment

    A Hanseníase é uma patologia de evolução alongada, e seu agente causador é o Mycobacterium leprae. Seus sinais e sintomas são no nível dermatoneurológico. Nesse contexto, esse estudo tem por objetivo geral analisar por meio da pesquisa baseada em evidências - PBE- achados da literatura, sobre estigmas que envolvem pacientes com hanseníase e praticas da educação em saude durante o tratamento na atenção básica. Pesquisa baseada em evidências, utilizando os indexadores Pubmed, Scielo e Cohcrane com os seguintes filtros: resumo abstract, textos open access, idioma português e inglês, período 2016 a 2021. Também foram utilizados manuais e boletins epidemiológicos disponíveis na página do Ministério da Saúde. O material foi selecionado de modo a buscar responder os objetivos dessa pesquisa e portanto utilizou os descritores em saúde: hanseníase, atenção básica, diagnóstico, tratamento. Foram abordadas práticas de educação em saude que envolvem orientações e estigmas em saude. Entende-se que o manejo da hanseníase envolve sensibilidade, consciência e sobretudo conhecimento embasado para consubstanciar a prática clínica

    Das considerações acerca do paciente politraumatizado com ênfase em trauma cranio encefalico: Considerations about the polytrauma patient with emphasis on head trauma

    A morte decorrente do trauma é um grande problema de saúde no mundo inteiro, resultando em quase 14 mil mortes diariamente, o trauma se evidencia entre as cinco principais causas de morte. Nos últimos 10 anos, mais de 1 milhão de pessoas ficaram inválidas devido a traumas mecânicos no Brasil, sendo os acidentes de trânsito os principais responsáveis por estas taxas. Além do aumento do número de veículos em circulação, a desorganização, a deficiência geral da fiscalização, as péssimas condições de muitos veículos, o comportamento dos usuários e a impunidade dos infratores fizeram com que nas últimas décadas o Brasil se colocasse entre os campeões mundiais de acidentes de trânsito. As lesões cerebrais constituem uma causa importante de morte, especialmente em adultos jovens, e uma causa importante de incapacidade. Atualmente estão cada vez mais disseminados os agentes causais de lesões nos seres humanos, tais como: os meios de transportes, o maquinário agrícola e industrial, o aumento da agressividade das armas de fogo, entre outros. Se por um lado o aumento tecnológico tem contribuído para o aumento da qualidade de vida, por outro ameaça a sobrevivência dos cidadãos. Esta pesquisa se enquadra como revisão de literatura, do tipo sistemática de caráter exploratório e abordagem qualitativa.
