A review on solid dispersion: a modern formulation approach in drug delivery system
Drugs those are given as solid dosage form and having low solubility often have a lack of flexibility in drug formulation and administration. The dissolution rate could be the rate-limiting process in the absorption of a drug from a solid dosage form of relatively insoluble drugs. Solid dispersion technologies are promising techniques for improving the water solubility, and hence dissolution and bioavailability of hydrophobic drugs. It is done for Biopharmaceutical Classification System (BCS) II Class drugs. Solid dispersion is the dispersion of one or more active ingredients in hydrophilic inert carrier matrix at molecular level. Solid dispersions of poorly water-soluble drugs with water-soluble carriers have been reduced the incidence of these problems and enhanced dissolution. The focus of this review article is on advantages, disadvantages and the method of preparation, and characterization of the solid dispersion. This review also discusses the recent advances in the field of solid dispersion technology
Umbilical hernia repair and recurrence: need for a clinical trial?
Abstract Introduction Umbilical hernia repair, despite its perceived simplicity, is associated with recurrence between 2.7 and 27%, in mesh repair and non mesh repair respectively. Many factors are recognized contributors to recurrence however multiple defects in the linea alba, known to occur in up to 30% of patients, appear to have been overlooked by surgeons. Aims This systematic review assessed reporting of second or multiple linea alba defects in patients undergoing umbilical hernia repair to establish if these anatomical variations could contribute to recurrence along with other potential factors. Methods A systematic review of all published English language articles was undertaken using databases PubMed, Embase, Web of Science and Cochrane Library from January 2014 to 2019. The search terms ‘Umbilical hernia’ AND ‘repair’ AND ‘recurrence’ were used across all databases. Analysis was specified in advance to avoid selection bias, was registered with PROSPERO (154173) and adhered to PRISMA statement. Results Six hundred and forty-six initial papers were refined to 10 following article review and grading. The presence of multiple linea alba defects as a contributor to recurrence was not reported in the literature. One paper mentioned the exclusion of six participants from their study due multiple defects. In all 11 factors were significantly associated with umbilical hernia recurrence. These included: large defect, primary closure without mesh, high BMI in 5/10 publications; smoking, diabetes mellitus, surgical site Infection (SSI) and concurrent hernia in 3/10. In addition, the type of mesh, advanced age, liver disease and non-closure of the defect were identified in individual papers. Conclusion This study identified many factors already known to contribute to umbilical hernia recurrence in adults, but the existence of multiple defects in the linea, despite it prevalence, has evaded investigators. Surgeons need to be consider documentation of this potential confounder which may contribute to recurrence
Expression profiling of G2/M phase regulatory proteins in normal, premalignant and malignant uterine cervix and their correlation with survival of patients
Background : Cell regulatory G2/M phase proteins are the key regulators
of mitosis and have been reported with abnormal expressions in various
malignancies. Aim : To determine the expressions of these proteins in
neoplastic uterine cervix tissue. Materials and Methods : This study
evaluates the G2/M phase regulatory protein expression of Cyclin B1,
Aurora-B, Pololike kinase 1 (PLK1) and LIM kinase1 (LIMK1) in tissues
of 25 normal (control), 16 dysplastic (dysplasia) and 34 neoplastic
(cancer) patients of uterine cervix. The expressions of different
proteins were obtained by using Western Blot technique. Statistical
Analysis : One way analysis of variance (ANOVA), Pearson correlation,
Kaplan-Meier and other tests are used for analysis. Results and
Conclusion : The level of expression of LIMK1 in cervical cancer
patients was found to be significantly higher (P < 0.01) than both
the controls and dysplasia. The expression of Aurora B and PLK1 in
cervical cancer patients was also found to be significantly higher ( P
< 0.05) than controls but it did not differ with dysplasia. However,
the expression of Cyclin B1 was similar among cervical cancer patients,
dysplasia and controls ( P> 0.05). The expression of all the above
proteins showed significant ( P < 0.01) and inverse relation with
the survival of cancer patients. Among the selected candidate proteins,
it was LIMK1 that showed the most positive correlation with the
aggressiveness of the disease and negative correlation (r= -0.64; P
< 0.01) with the survival of patients
