Chagas disease in Latin American migrants: a Spanish challenge
AbstractChagas’ disease affects millions in Latin America and is the leading cause of cardiomyopathy and death due to cardiovascular disease in patients aged 30–50 years. As a consequence of immigration it has settled in several European countries, where besides imported cases, autochthonous infections arise through vertical transmission and blood/organ donation. All Latin American immigrants who attended our Unit were screened for T. cruzi infection (ELISA and IFAT ± PCR). An ECG and echocardiogram were requested for all positive patients, and oesophageal manometry, barium swallow and barium enema were requested according to patient symptoms. All patients under 50 years without severe cardiac involvement and who had not received correct treatment previously were treated with benznidazole 5 mg/kg/day for 60 days. Patients were followed-up with serology and PCR 1 month after treatment ended and every 6 months thereafter. A total of 1146 Latin Americans were screened for T. cruzi (357 positive serology results). The typical patient profile was a Bolivian female, of rural origin, in her fourth decade of life, without evidence of visceral involvement. Treatment tolerance was poor, with 29.7% discontinuing treatment due to adverse reactions. Among those with adverse reactions (52%), the most frequent were cutaneous hypersensitivity (68.7%), gastrointestinal upset (20%) and nervous system disturbances (16.2%). T. cruzi infection is no longer limited to Latin America. Poor treatment tolerance can limit current treatment options. More epidemiological data are necessary to estimate the magnitude of a problem of great relevance for public health and health resource planning
Evolution of Chemistry in the envelope of Hot Corinos (ECHOS). I. Extremely young sulphur chemistry in the isolated Class 0 object B335
Within the project Evolution of Chemistry in the envelope of HOt corinoS
(ECHOS), we present a study of sulphur chemistry in the envelope of the Class 0
source B335 through observations in the spectral range 7, 3, and 2 mm. We have
modelled observations assuming LTE and LVG approximation. We have also used the
code Nautilus to study the time evolution of sulphur species. We have detected
20 sulphur species with a total gas-phase S abundance similar to that found in
the envelopes of other Class 0 objects, but with significant differences in the
abundances between sulphur carbon chains and sulphur molecules containing
oxygen and nitrogen. Our results highlight the nature of B335 as a source
especially rich in sulphur carbon chains unlike other Class 0 sources. The low
presence or absence of some molecules, such as SO and SO+, suggests a chemistry
not particularly influenced by shocks. We, however, detect a large presence of
HCS+ that, together with the low rotational temperatures obtained for all the S
species (<15 K), reveals the moderate or low density of the envelope of B335.
We also find that observations are better reproduced by models with a sulphur
depletion factor of 10 with respect to the sulphur cosmic elemental abundance.
The comparison between our model and observational results for B335 reveals an
age of 10t10 yr, which highlights the particularly early
evolutionary stage of this source. B335 presents a different chemistry compared
to other young protostars that have formed in dense molecular clouds, which
could be the result of accretion of surrounding material from the diffuse cloud
onto the protostellar envelope of B335. In addition, the analysis of the
SO2/C2S, SO/CS, and HCS+/CS ratios within a sample of prestellar cores and
Class 0 objects show that they could be used as good chemical evolutionary
indicators of the prestellar to protostellar transition

Medical record linkage in health information systems by approximate string matching and clustering
BACKGROUND: Multiplication of data sources within heterogeneous healthcare information systems always results in redundant information, split among multiple databases. Our objective is to detect exact and approximate duplicates within identity records, in order to attain a better quality of information and to permit cross-linkage among stand-alone and clustered databases. Furthermore, we need to assist human decision making, by computing a value reflecting identity proximity. METHODS: The proposed method is in three steps. The first step is to standardise and to index elementary identity fields, using blocking variables, in order to speed up information analysis. The second is to match similar pair records, relying on a global similarity value taken from the Porter-Jaro-Winkler algorithm. And the third is to create clusters of coherent related records, using graph drawing, agglomerative clustering methods and partitioning methods. RESULTS: The batch analysis of 300,000 "supposedly" distinct identities isolates 240,000 true unique records, 24,000 duplicates (clusters composed of 2 records) and 3,000 clusters whose size is greater than or equal to 3 records. CONCLUSION: Duplicate-free databases, used in conjunction with relevant indexes and similarity values, allow immediate (i.e.: real-time) proximity detection when inserting a new identity

Parental Smoking Modifies the Relation between Genetic Variation in Tumor Necrosis Factor-α (TNF) and Childhood Asthma
BACKGROUND: Polymorphisms in the proinflammatory cytokine genes tumor necrosis factor-α (TNF) and lymphotoxin-α (LTA, also called TNF-β) have been associated with asthma and atopy in some studies. Parental smoking is a consistent risk factor for childhood asthma. Secondhand smoke and ozone both stimulate TNF production. OBJECTIVES: Our goal was to investigate whether genetic variation in TNF and LTA is associated with asthma and atopy and whether the association is modified by parental smoking in a Mexican population with high ozone exposure. METHODS: We genotyped six tagging single nucleotide polymorphisms (SNPs) in TNF and LTA, including functional variants, in 596 nuclear families consisting of asthmatics 4–17 years of age and their parents in Mexico City. Atopy was determined by skin prick tests. RESULTS: The A allele of the TNF-308 SNP was associated with increased risk of asthma [relative risk (RR) = 1.54; 95% confidence interval (CI), 1.04–2.28], especially among children of non-smoking parents (RR = 2.06; 95% CI, 1.19–3.55; p for interaction = 0.09). Similarly, the A allele of the TNF-238 SNP was associated with increased asthma risk among children of nonsmoking parents (RR = 2.21; 95% CI, 1.14–4.30; p for interaction = 0.01). LTA SNPs were not associated with asthma. Haplotype analyses reflected the single SNP findings in magnitude and direction. TNF and LTA SNPs were not associated with the degree of atopy. CONCLUSIONS: Our results suggest that genetic variation in TNF may contribute to childhood asthma and that associations may be modified by parental smoking
Determination of the b quark mass at the M_Z scale with the DELPHI detector at LEP
An experimental study of the normalized three-jet rate of b quark events with
respect to light quarks events (light= \ell \equiv u,d,s) has been performed
using the CAMBRIDGE and DURHAM jet algorithms. The data used were collected by
the DELPHI experiment at LEP on the Z peak from 1994 to 2000. The results are
found to agree with theoretical predictions treating mass corrections at
next-to-leading order. Measurements of the b quark mass have also been
performed for both the b pole mass: M_b and the b running mass: m_b(M_Z). Data
are found to be better described when using the running mass. The measurement
yields: m_b(M_Z) = 2.85 +/- 0.18 (stat) +/- 0.13 (exp) +/- 0.19 (had) +/- 0.12
(theo) GeV/c^2 for the CAMBRIDGE algorithm. This result is the most precise
measurement of the b mass derived from a high energy process. When compared to
other b mass determinations by experiments at lower energy scales, this value
agrees with the prediction of Quantum Chromodynamics for the energy evolution
of the running mass. The mass measurement is equivalent to a test of the
flavour independence of the strong coupling constant with an accuracy of 7

Study of Leading Hadrons in Gluon and Quark Fragmentation
The study of quark jets in e+e- reactions at LEP has demonstrated that the
hadronisation process is reproduced well by the Lund string model. However, our
understanding of gluon fragmentation is less complete. In this study enriched
quark and gluon jet samples of different purities are selected in three-jet
events from hadronic decays of the Z collected by the DELPHI experiment in the
LEP runs during 1994 and 1995. The leading systems of the two kinds of jets are
defined by requiring a rapidity gap and their sum of charges is studied. An
excess of leading systems with total charge zero is found for gluon jets in all
cases, when compared to Monte Carlo Simulations with JETSET (with and without
Bose-Einstein correlations included) and ARIADNE. The corresponding leading
systems of quark jets do not exhibit such an excess. The influence of the gap
size and of the gluon purity on the effect is studied and a concentration of
the excess of neutral leading systems at low invariant masses (<~ 2 GeV/c^2) is
observed, indicating that gluon jets might have an additional hitherto
undetected fragmentation mode via a two-gluon system. This could be an
indication of a possible production of gluonic states as predicted by QCD.Comment: 19 pages, 6 figures, Accepted by Phys. Lett.
Measurement and Interpretation of Fermion-Pair Production at LEP energies above the Z Resonance
This paper presents DELPHI measurements and interpretations of
cross-sections, forward-backward asymmetries, and angular distributions, for
the e+e- -> ffbar process for centre-of-mass energies above the Z resonance,
from sqrt(s) ~ 130 - 207 GeV at the LEP collider. The measurements are
consistent with the predictions of the Standard Model and are used to study a
variety of models including the S-Matrix ansatz for e+e- -> ffbar scattering
and several models which include physics beyond the Standard Model: the
exchange of Z' bosons, contact interactions between fermions, the exchange of
gravitons in large extra dimensions and the exchange of sneutrino in R-parity

A Determination of the Centre-of-Mass Energy at LEP2 using Radiative 2-fermion Events
Using e+e- -> mu+mu-(gamma) and e+e- -> qqbar(gamma) events radiative to the
Z pole, DELPHI has determined the centre-of-mass energy, sqrt{s}, using energy
and momentum constraint methods. The results are expressed as deviations from
the nominal LEP centre-of-mass energy, measured using other techniques. The
results are found to be compatible with the LEP Energy Working Group estimates
