    Public Spending, Quality of Bureaucracy and Economic Growth: A Theoretical Analysis

    This paper develops a theoretical framework to investigate the relationship between public spending and economic growth, where public spending provides both productive capital and unproductive services. We take into account the quality of bureaucracy with the possibility of rent-seeking motives. A key feature of the model is that it distinguishes between utility enhancing and productivity enhancing public spending. In the absence of rent-seeking motives, the paper demonstrates that public spending will promote economic growth only if marginal productivity of spending is high enough to offset the potential output loss due to increased taxation. In the presence of rent-seeking, however, the impact of public spending on economic growth depends on the quality of bureaucracy and how the latter impinges upon the rentseeking behaviour. The analysis shows that while improvement in bureaucratic quality would unambiguously raise the share of utility enhancing public spending, its impact on economic growth would depend on how bureaucratic quality influences the relative magnitudes of the two types of public spending as well as on how far bureaucratic extraction will be controlled as a result of improvement in bureaucratic quality. Bureaucratic extraction is likely to be minimised with strong institutions and effective monitoring and accountability mechanisms thereby improving the prospects of economic growth.     The need for entrepreneurs for economic development has always been crucial in history because they are the leaders who invent innovative ideas that give spark to economic activities. They are responsible for the combination of factors of production by capital formation, creating employment opportunities, wealth distribution that facilitates development and growth. A well explained definition of entrepreneurship in the words of Wennekers and Thurik (1999) that successfully makes the functional roles of entrepreneurs is: “…the manifest ability and willingness of individuals, on their own, in teams within and outside existing organisations, to perceive and create new economic opportunities (new products, new production methods, new organisational schemes and new product-market combinations) and to introduce their ideas in the market, in the face of uncertainty and other obstacles, by making decisions on location, form and the use of resources and institutions.” (46–47) High and sustained economic growth is the fundamental objective of every developed or developing country’s governmental policy.     Training Effective Altruism

    Randomizing different schools of thought –via a month-long training– finds that training deputy ministers in effective altruism renders 0.4-0.6 standard deviations increase in altruism. Treated ministers increased mentalizing of others: blood donations doubled, but only when blood banks requested their exact blood type. Perspective-taking in strategic dilemmas improved. Orphanage visits and volunteering in impoverished schools also increased. We then trace the impact of the training on their policymaking: one year after training, amid official duties, ministers were 50-100% more likely to choose social policies and recommend over 4-fold additional funding for them. Overall, our results underscore that effective altruism may be a parsimonious foundation for formation of prosociality, even impacting the behavior of adults in the field and their high-stakes policymakin

    Renal Artery Pseudo-aneurysms: Do All of Them Require Endovascular Management?

    Purpose: Our study examines the etiological profile, clinical and imaging features of renal artery pseudo aneurysms (RAPs), as well as the efficacy and need for the angioembolization of RAPs in a resource-constrained setting. Materials and Methods: A total of 36 patients with RAPs were included in our study. Initial diagnosis was made by Doppler Ultrasonography (USG) followed by CT renal angiography in all cases. DSA was performed in 28 patients, as eight patients showed spontaneous resolution by thrombosis on immediate pre-procedure Doppler study. Angioembolization with a microcoil was performed for 30 aneurysms in 28 patients. Technical success was confirmed at the end of the procedure by a renal angiogram. To assess clinical success, we followed up with patients (with clinical and Doppler USG) for a period of six months. Results: The most common cause of RAPs in our study was percutaneous nephrolithotomy (PCNL), seen in 21 patients (58.3%), followed by trauma (25%), and partial nephrectomy (11%). All patients presented to us were within 21 days of the etiological event of hematuria or flank pain. USG was able to detect the RAP in 22 cases (61%). CT renal angiography was diagnostic in all patients but failed to demonstrate two additional aneurysms in one patient. RAP size ≤ 4 mm and absence of brisk filling on CT renal angiography was associated with spontaneous resolution in eight patients, probably an indication of the beginning of spontaneous thrombosis. Angioembolization was done using microcoils and showed 100% technical and clinical success. Conclusion: PCNL is the most common etiological factor for RAPs in our setting. Such patients should have a Doppler USG done prior to discharge from the hospital. CT angiographic flow dynamics (delayed peak enhancement) may be helpful in the identification of RAPs with a high probability of subsequent spontaneous resolution. Angiography followed by embolization using microcoils is the most effective and safe treatment for RAPs with no significant loss of renal parenchyma, although cost remains a limiting factor in our setting

    Evaluation of various patient-, lesion-, and procedure-related factors on the occurrence of pneumothorax as a complication of CT-guided percutaneous transthoracic needle biopsy

    Purpose: To assess the influence of various patient-, lesion-, and procedure-related variables on the occurrence of pneumothorax as a complication of CT-guided percutaneous transthoracic needle biopsy. Material and methods: In a total of 208 patients, 215 lung/mediastinal lesions (seven patients were biopsied twice) were sampled under CT guidance using coaxial biopsy set via percutaneous transthoracic approach. Incidence of post procedure pneumothorax was seen and the influence of various patient-, lesion-, and procedure-related variables on the frequency of pneumothorax with special emphasis on procedural factors like dwell time and needle-pleural angle was analysed. Results: Pneumothorax occurred in 25.12% (54/215) of patients. Increased incidence of pneumothorax had a statistically significant correlation with age of the patient (p = 0.0020), size (p = 0.0044) and depth (p = 0.0001) of the lesion, and needle-pleural angle (p = 0.0200). Gender of the patient (p = 0.7761), emphysema (p = 0.2724), site of the lesion (p = 0.9320), needle gauge (p = 0.7250), patient position (p = 0.9839), and dwell time (p = 0.9330) had no significant impact on the pneumothorax rate. Conclusions: This study demonstrated a significant effect of the age of the patient, size and depth of the lesion, and needle-pleural angle on the incidence of post-procedural pneumothorax. Emphysema as such had no effect on pneumothorax rate, but once pneumothorax occurred, emphysematous patients were more likely to be symptomatic, necessitating chest tube placement. Gender of the patient, site of the lesion, patient position during the procedure, and dwell time had no statistically significant relation with the frequency of post-procedural pneumothorax. Surprisingly, needle gauge had no significant effect on pneumothorax frequency, but due to the small sample size, non-randomisation, and bias in needle size selection as per lesion size, further studies are required to fully elucidate the causal relationship between needle size and post-procedural pneumothorax rate. The needle should be as perpendicular as possible to the pleura (needle-pleural angle close to 90°), to minimise the possibility of pneumothorax after percutaneous transthoracic needle biopsy

    The spectrum of imaging findings in pulmonary hydatid disease and the additive value of T2-weighted magnetic resonance imaging in its diagnosis

    Purpose: To describe the spectrum of imaging findings in pulmonary echinococcosis and to study the additive value of T2-weighted magnetic resonance imaging (MRI) in the characterisation of pulmonary hydatid disease. Material and methods: This was a descriptive, prospective study conducted for a period of 3 years from December 2016 to November 2019. Patients suspected of having pulmonary echinococcosis (n = 110) on preliminary chest radiography were examined with chest computed tomography (CT). Among them 41 cases were additionally examined with T2-weighted MRI of thorax. Final diagnosis was based on surgery or histopathology. Results: Of the 110 patients enrolled for the study 15 were lost to attrition, and among the final cohort of 95 patients CT correctly diagnosed 68/84 (80.9%) as hydatid cyst, whereas 16/84 (19.1%) received an erroneous alternate diagnosis on CT. Based on the classical findings of hyperintense pulmonary cystic lesion with T2-weighted hypointense rim or detached internal T2-weighted hypointense membrane, a correct diagnosis of hydatid cyst was possible in 30 patients whereas a correct alternate diagnosis was made in 8 cases. T2-weighted MRI was found to have sensitivity of 96.7%, specificity of 80%, positive predictive value (PPV) of 93.7% and negative predictive value (NPV) of 88.9% with an overall diagnostic accuracy of 92.6%. Using the McNemar test, MRI was found to be diagnostically superior to CT (p = 0.019). Conclusions: Most of the pulmonary hydatid cysts can be diagnosed on CT; however, sometimes the findings may be indeterminate or atypical, leading to a diagnostic dilemma. MRI, owing to its ability to demonstrate hypointense endocyst, can act as a useful adjunct to correctly diagnose hydatid cyst or suggest an alternative diagnosis

    Structural organization and pattern of innervations of human Meissner’s corpuscle: a light microscopic study

    Background: Human glabrous skin has very rich nerve supply in the form of specialized nerve endings like Meissner’s corpuscles, Pacinian corpuscles, Krause end bulbs etc for carrying sensory information to brain. Aim of study: To study the structure, pattern of innervations and nerve terminations of human Meissner’s corpuscle. Methods: Skin samples from sixty human beings (age range 2 to 72 years) were taken, sections prepared and stained with a cytological (Haematoxylin – Eosin and Vongieson’s stains) and a neural stain (Silver Impregnation). Results: With cytological stain, Meissner’s corpuscles were seen in dermal papillae of glabrous skin, each consisting of a cellular structure having a peripheral capsule and central core of transversely arranged cells. With neural stain, each corpuscle was seen to be oval, globular or cylindrical structure, having a capsule surrounding the core of spirally arranged nerve fibers, sandwiched by Schwann cells. In between nerve fibers of the core were seen small bundles of collagen fibers. 2-6 nerve fibers innervated each Mc from the sub corial plexus of nerves and formed various patterns of nerve endings like networks, end bulbs and varicosities inside the Meissner’s corpuscle. Conclusion: Meissner’s corpuscle is a complex structure composed of capsule - consisting of spindle shaped capsular cells interspersed in collagen fibers, surrounding a core of helically arranged nerve fibers, Schwann cells and collagen fibers

    Antibacterial activity of phyto-mediated silver nanoparticles developed from Melia azedarach

    BACKGROUNDː Nanoparticles formed by plant extracts present a good alternative of existing antibiotics to compete with the resistant strains of bacteria. Antioxidants present in plants synthesize the nanoparticles from metal salt and also cap them.METHODSː In the present study, Melia azedarach fresh leaves were extracted with water. These extracts were reduced by adding silver nitrate (AgNO3) solution separately. Plant extract in different concentration was used to develop nanoparticles with constant strength of salt solution. Color change of extracts represented the development of silver nanoparticles due to reduction of silver ions to form silver nanoparticles. Absorbance of reaction mixtures were determined by UV Vis spectrophotometry. Further antimicrobial activity of these nanoparticles was tested against Borditella pertussis and Xanthomonas axonopodis by agar well diffusion method.RESULTSː Maximum absorbance was noticed between 400-500 nm. EDX analysis proved the presence of silver ions and SEM analysis showed size and shape of nanoparticles (105 nm). Silver nanoparticles developed from water extract of M. azedarach exhibited maximum inhibition zones (25.4±0.36) and (47.2±0.25) against Borditella pertussis and Xanthomonas axonopodis respectively.CONCLUSIONSː The conclusion was established that silver nanoparticles from M. azedarach revealed enhanced antibacterial activity with comparison to pure plant extract and silver nitrate solution and can be used in different antibacterial products.</p

    The Policy Choices of Bureaucrats: an institutional analysis

    textabstractThis dissertation analyzes the over-arching question of how the institutional and social context of public bureaucracy impact the choices of bureaucrats. These choices are introduced in the form of inertial-preferences and identity-driven preferences in the decision-making of bureaucrats. The study proposes theoretical frameworks to explain policy drift. These frameworks determine how “context-oriented preferences” moderate the trade-off between budget maximizing motivations and the policy choice of bureaucrats. In order to understand the context of bureaucratic choices directly stemming from organizational, institutional and social factors, the study encompasses two generic research routes that are covered in the three content chapters. In the first research route, the impact of history on present organizational structures is investigated with the theory of organizational imprinting. This refers to the process through which economic, social and institutional factors that prevailed at the time of founding shape present organizational forms and attributes. The second research route builds on the notion that a bureaucratic organization is a formalized social system. The bureaucrats consider it as essential to comply with organizational goals in order to be considered as a member of the organization. In order to analytically seize the social context of the bureaucratic organization, the notion of identity is conceptually integrated into the decision calculus of the bureaucrat. Apart from the introductory chapter, which sets the stage for the dissertation, there are three content chapters and a concluding chapter
