    Studies on Trypanosoma (nannomonas) congolense: III. Antigenic variation in three cyclically transmitted stocks

    Cyclical transmission of different variable antigen types of Trypanosoma congolense STIB 228 resulted in the development of metacyclic trypanosome populations which were similar in their variable antigen composition as judged by immunofluorescence and neutralization assays. The variable antigen types present in the ingested bloodstream populations were not found in the metacyclic populations. The bloodstream populations which were obtained from cyclically infected, irradiated (900 rad.) mice contained variable antigen types which were not present in the corresponding metacyclic populations. When derivatives of 2 other stocks of T. congolense, isolated in a different area of Tanzania, underwent cyclical development in the tsetse fly, the metacyclic populations of each stock also had a characteristic variable antigen composition. The metacyclic populations of the 3 stocks were, however, completely dissimilar in variable antigen composition. Simultaneous infection of tsetse flies with a mixture of different stocks resulted in the concurrent production of metacyclic trypanosomes which contained the characteristic variable antigen types of each stock. The effect of cyclical transmission on the process of antigenic variation in T. congolense infections is therefore similar to that in T. brucei infection

    Groups of mice were exposed to multiple bites by tsetse flies (Glossina morsitans morsitans) infected with a clone of Trypanosoma congolense spread over a period of 8 days. The mice were subsequently treated with Berenil 10 days after the first fly bite as were uninfected control mice. The group of mice which received 12-15 infectious fly bites on two occasions, 21 days apart, were subsequently resistant to infection when re-challenged by flies infected with the same clone of T. congolense. These mice were also immune to challenge by flies infected with a different bloodstream variable antigen type derived from this same stock.     Social Justice Approach to Road Safety in Kenya: Addressing the Uneven Distribution of Road Traffic Injuries and Deaths across Population Groups

    Road traffic injury and deaths (RTID) are an important public health problem in Kenya, primarily affecting uneducated and disenfranchised people from lower socioeconomic groups. Studies conducted by Kenyan experts from police reports and surveys have shown that pedestrian and driver behaviors are the most important proximal causes of crashes, signifying that the occurrence of crashes results directly from human action. However, behaviors and risk factors do not fully explain the magnitude of RTID neither does it account for socioeconomic gradient in RTID. Instead, a social justice approach to RTID highlights the need for emphasizing distal causal factors. They allow us to understand how social inequities determine risk for RTID. Hence, designing policies that focus on behaviors will simply mask the underlying systemic causes of this growing phenomenon. To eradicate the RTID and address the gradient, a broader policy framework that includes the social dimension of injury, a strong political will to address the underlying causes of RTID and an effective partnership with stakeholders needs to be developed

    Severe road traffic injuries in Kenya, quality of care and access

    Background: Road traffic injuries (RTI) are on increase in developing countries. Health care facilities are poorly equipped to provide the needed services. Objective: Determine access and quality of care for RTI casualties in Kenya. Design: Cross-sectional survey Setting: 53 large and medium size private, faith-based and public hospitals. Participants: In-patient road traffic crash casualties and health personnel in the selected hospitals were interviewed on availability of emergency care and resources. Onsite verification of status was undertaken. Results: Out of 310 RTI casualties interviewed, 72.3%, 15.6% and 12.2% were in public, faith-based and private hospitals, respectively. Peak age of the injured was 15-49 years. First aid was availed to 16.0% of casualties. Unknown persons transported 76.5% of the injured. Police and ambulance vehicles transported 6.1% and 1.4%, respectively. 51.9% reached health facilities within 30 minutes of crash and medical care provided to 66.2% within one hour. 40.8% of recipient facilities were adequately prepared for RTI emergencies. Conclusions: Most RTI casualties were young and from poor backgrounds. Training of motorists and general public in first aid should be considered in RTI control initiatives. Availability of basic trauma care medical supplies in public health facilities was highly deficient

    Zoonotic pathogens of dromedary camels in Kenya:a systematised review

    Kenya is home to Africa’s third largest population of dromedary camels, and production at commercial and local levels are increasingly important. In pastoral and nomadic communities in the arid and semi-arid lands (ASALs), camels play a vital role in food security, while commercial milk production and formalized export markets are rapidly emerging as camel populations expand into non-traditional areas. Until recently, little focus was placed on camels as hosts of zoonotic disease, but the emergence of Middle Eastern respiratory coronavirus (MERS-CoV) in 2012, and the discovery of exposure to the virus in Kenyan camels, highlighted the need for further understanding of this area. This systematised review utilised a robust search strategy to assess the occurrence of camel-associated zoonoses in Kenya and to evaluate the quality of the published literature. Seventy-four studies were identified, covering sixteen pathogens, with an increasing number of good quality studies in recent years. Despite this, the area remains under-researched and there is a lack of robust, high-quality research. Trypanosome spp., Echinococcus granulosus and Brucella spp. appeared most frequently in the literature. Pathogens with the highest reported prevalence were MERS-CoV (0–100%), Echinococcus granulosa (7–60%) and Rift Valley fever virus (7–57%). Exposure to Brucella spp., Coxiella burnetii and Crimean-Congo haemorrhagic fever virus showed higher levels in camel or camel-associated vectors than other livestock species, although brucellosis was the only disease for which there was robust evidence linking camel and human exposure. Zoonotic agents with less severe human health outcomes, such as Dermatophilosus congolensis and contagious ecthyma, were also represented in the literature. This review provides an important summary of the scope and quality of current knowledge. It demonstrates that further research, and improved adherence to robust study design and reporting are essential if the zoonotic risk from camels in Kenya, and elsewhere, is to be better understood

    Usefulness of hospital emergency department records to explore access to injury care in Nepal

Despite increasing morbidity and mortality from injuries in Nepal, it is not recognised in the government’s policy and programmes and few population-based studies have been published. This study describes the usefulness of hospital emergency department records to explore access to injury care in Nepal. Methods: A retrospective ED-based study was conducted at a governmental hospital in Nepal to review the routinely collected data for 1year (1 January 2010 to 31 December 2010). The study was designed to provide cross-sectional data to describe the distribution of injuries by age, gender, ethnic group and injury mechanism. Results: Results showed that twice as many males as females attended the emergency department (14.6 vs. 7.0 per 1000), attendance varied by age with most (39.8%) attendances in young adults of working age and over half of attendances were from just two ethnic groups (Brahmin (26%) and Tamang (25.5%). Road traffic injuries were the most common cause of injury (37.6%). Conclusions: This study therefore showed the feasibility of using routinely collected hospital emergency department data to monitor injury inequalities in Nepal

Through enhanced technological capabilities, robust relationships with foreign militaries, increased access to overseas military bases and dual-use facilities, and the implementation of major structural reforms, the People’s Liberation Army has built a more integrated joint force capable of conducting a wider and more complex array of missions. This volume advances the understanding of the People’s Liberation Army’s capability to conduct overseas missions by examining China’s military relations with Europe, Africa, and Latin America; the country’s military activities in the Indian Ocean, polar regions, and Pacific Island countries; and the emerging roles of the People’s Liberation Army Rocket Force and the Joint Logistic Support Force.     A new initiative for the development of new diagnostic tests for human African trypanosomiasis

    Human African trypanosomiasis is a threat to millions of people living in sub-Saharan countries and is fatal unless treated. At present, the serological and parasitological tests used in the field for diagnosis of sleeping sickness have low specificity and sensitivity. There is clearly an urgent need for accurate tools for both diagnosis and staging of the disease. The Foundation for Innovative New Diagnostics and the World Health Organization have announced that they will collaborate to develop and evaluate new diagnostic tests for human African trypanosomiasis

    Africanizing scientific knowledge: the Multilateral Initiative on Malaria as a model?

    In November 2009, the fifth Pan African Malaria conference was held in Nairobi. Thirteen years after the founding initiative in Dakar, the first African Secretariat based in Africa (TANZANIA) organized this major event for the malaria community. Looking back, it has been a long way: changes in the research landscape, new funding opportunities came out and establishment of new partnerships between Europe, America and Africa. Goals identified in 1997 have not all been achieved because the critical mass of scientists has not been reached yet. However a new generation of African scientists have emerged through MIM/TDR funding and advocacy for more support remains on the agenda. Could it be rightly stated today that the MIM concept reflects the africanization of malaria research
