    AbstractNanocomposites in the form of continuous and aligned polyvinyl alcohol (PVA) nanofibers have been obtained by the electrospinning process. These fibers were used as matrix for hydroxyapatite (HA). PVA and PVA/HA nanofibers were fabricated and crosslinked with glutaraldehyde (GA). The hybrid of polymer with ceramics (HA) shows good properties of large surface area and good porosity that were demonstrated by different characterization techniques such as FTIR, SEM, XRD and EDX which illustrate the ratio of HA. The prepared nanofibrous membranes are used for removal of Zn2+ from simulated water. The potential of these synthesized sorbents to remove Zn2+ ions from aqueous solutions was investigated in batch experiments, where several parameters such as the sorbate/sorbent’s contact time, initial Zn2+ ions concentration and sorbent dosage were investigated. The equilibrium concentration of Zn2+ ions was identified by the atomic absorption spectrophotometry and the adsorption process was studied by the isotherm models such as Langmuir, Freundlich and Temkin. The Zn2+ adsorption process for PVA and PVA/HA nanofibrous membranes was fitted to Langmuir and Freundlich isotherm models respectively

    Abstract: The properties of barely straw play a crucial role in some industrial applications. Despite that, there is a lack of its scientific data concerning thermal, mechanical and chemical properties of these fibers. To maximize their potential use it is necessary to understand their characteristic. In this work, barely straw was characterized by analytical techniques such as FTIR, X-ray, TGA and SEM. The straw treated with distilled water and compare the new analytical properties with the properties of the raw, which revealed that the adsorption properties improved with steam treatment due to increase of functional groups which can share in adsorption reaction

    Prophylactic use of temporary uterine packing combined with topical tranexamic acid reduces blood loss and transfusion requirements in patients undergoing cesarean section: A double-blind, randomized controlled trial

    Background: since antiquity post-partum haemorrhage (PPH) has been a terrible event for obstetricians. It accounts for 34% of maternal deaths in Africa and developed countries. It complicates 6% of cesarean delivery (CD) and still raising and correlates with increased frequency of caesarean sections, many uterotonic agents have successfully used alone or in combinations for prevention of such catastrophic, but the most effective medication is still up for controversy. Objective: To compare the efficacy and safety of temporary uterine packing combined with topical tranexamic acid (TA) as adjunct for reducing blood loss following an elective cesarean delivery with intravenous tranexamic acid and placebo in women who have at least one risk factor for postpartum hemorrhage. Patients and methods: A double-blind, randomized clinical trial (: NCT03706339) conducted on 450 pregnant women at term (38–40 weeks) gestation scheduled for elective cesarean delivery, who were assigned to either intravenous TA, topical TA, or placebo(saline). The main outcome measures were blood loss at and 6 hours after cesarean delivery, the need for any additional oxytoxic drugs, and TA-related side effects. Results: There was a significant decrease in the intraoperative blood loss and total blood loss in both topical TA and IV TA groups compared to placebo group (p=0.0001,0.0001,0.0001,0.0001). Also, the need to extra uterotonics was significant decrease in IV TA group, 9 (6%) patients compared to 33 (22%) patients in placebo group, and 24 (16%) patients in topical TA group, (p=0.0001 and 0.006) respectively. Finally, operative time, hospital stay, postoperative hemoglobin, and post-operative complication showed no significant difference between the three groups (P= 0.276, 0.126, 0.853, 0.955, 1.00, 1.00) respectively. Conclusion: IV TA and temporary uterine packing combined with topical tranexamic acid is more effective than placebo in reducing total blood loss during and after cesarean delivery in women who have at least one risk factor for postpartum hemorrhage, but IV TA more effective

    Stop Turning a Blind Eye: Tobacco Smoking Among Egyptian Patients With Schizophrenia

    Background: Patients with schizophrenia have considerably higher rates of mortality than general population. Multiple factors may play a role in this. Despite being a major preventable cause of death, smoking is usually overlooked when dealing with patients with schizophrenia. Understanding the pattern of smoking, its severity, and the reasons to quit might be helpful in managing patients with schizophrenia and decreasing the mortality gap.Subjects and Methods: The study included smokers divided into two groups; the first included 346 patients with schizophrenia while the second group had 150 smokers with no mental illness. Both groups were assessed and compared regarding sociodemographic variables, pattern of smoking, severity of nicotine dependence, and motivation to quit smoking.Results: Earlier age of starting to smoke, higher number of cigarettes per day, and lower dependency scores were noted in patients with Schizophrenia. Positive correlation was found between positive symptoms and severity of dependence. Specific positive symptoms were correlated to number of cigarettes per day and time before first cigarette. Patients with Schizophrenia showed a significant difference in intrinsic reasons to quit (health concerns and self-control), which were also positively correlated to their positive symptoms score. Linear regression analysis for predictors of FTND score revealed that only age, sex, and schizophrenia were significant predictors of FTND score.Conclusion: Patients with schizophrenia smoke at earlier ages and smoke more cigarettes per day, yet, have less severe dependence than non-schizophrenic counterparts. Positive symptoms play a role in their smoking pattern and severity. Health concerns and self-control are their main motives to quit smoking

    Call for emergency action to restore dietary diversity and protect global food systems in times of COVID-19 and beyond: Results from a cross-sectional study in 38 countries

    Background: The COVID-19 pandemic has revealed the fragility of the global food system, sending shockwaves across countries\u27 societies and economy. This has presented formidable challenges to sustaining a healthy and resilient lifestyle. The objective of this study is to examine the food consumption patterns and assess diet diversity indicators, primarily focusing on the food consumption score (FCS), among households in 38 countries both before and during the first wave of the COVID-19 pandemic. Methods: A cross-sectional study with 37 207 participants (mean age: 36.70 ± 14.79, with 77 % women) was conducted in 38 countries through an online survey administered between April and June 2020. The study utilized a pre-tested food frequency questionnaire to explore food consumption patterns both before and during the COVID-19 periods. Additionally, the study computed Food Consumption Score (FCS) as a proxy indicator for assessing the dietary diversity of households. Findings: This quantification of global, regional and national dietary diversity across 38 countries showed an increment in the consumption of all food groups but a drop in the intake of vegetables and in the dietary diversity. The household\u27s food consumption scores indicating dietary diversity varied across regions. It decreased in the Middle East and North Africa (MENA) countries, including Lebanon (p \u3c 0.001) and increased in the Gulf Cooperation Council countries including Bahrain (p = 0.003), Egypt (p \u3c 0.001) and United Arab Emirates (p = 0.013). A decline in the household\u27s dietary diversity was observed in Australia (p \u3c 0.001), in South Africa including Uganda (p \u3c 0.001), in Europe including Belgium (p \u3c 0.001), Denmark (p = 0.002), Finland (p \u3c 0.001) and Netherland (p = 0.027) and in South America including Ecuador (p \u3c 0.001), Brazil (p \u3c 0.001), Mexico (p \u3c 0.0001) and Peru (p \u3c 0.001). Middle and older ages [OR = 1.2; 95 % CI = [1.125–1.426] [OR = 2.5; 95 % CI = [1.951–3.064], being a woman [OR = 1.2; 95 % CI = [1.117–1.367], having a high education (p \u3c 0.001), and showing amelioration in food-related behaviors [OR = 1.4; 95 % CI = [1.292–1.709] were all linked to having a higher dietary diversity. Conclusion: The minor to moderate changes in food consumption patterns observed across the 38 countries within relatively short time frames could become lasting, leading to a significant and prolonged reduction in dietary diversity, as demonstrated by our findings

    Impact of COVID-19 lockdown on smoking (waterpipe and cigarette) and participants' BMI across various sociodemographic groups in Arab countries in the Mediterranean Region

    INTRODUCTION: Tobacco smokers are at high risk of developing severe COVID-19. Lockdown was a chosen strategy to deal with the spread of infectious diseases; nonetheless, it influenced people's eating and smoking behaviors. The main objective of this study is to determine the impact of the COVID-19 lockdown on smoking (waterpipe and cigarette) behavior and its associations with sociodemographic characteristics and body mass index. METHODS: The data were derived from a large-scale retrospective cross-sectional study using a validated online international survey from 38 countries (n=37207) conducted between 17 April and 25 June 2020. The Eastern Mediterranean Region (WHO-EMR countries) data related to 10 Arabic countries that participated in this survey have been selected for analysis in this study. A total of 12433 participants were included in the analysis of this study, reporting their smoking behavior and their BMI before and during the COVID-19 lockdown. Descriptive and regression analyses were conducted to examine the association between smoking practices and the participant's country of origin, sociodemographic characteristics, and BMI (kg/m2). RESULTS: Overall, the prevalence rate of smoking decreased significantly during the lockdown from 29.8% to 23.5% (p<0.05). The percentage of females who smoke was higher than males among the studied population. The highest smoking prevalence was found in Lebanon (33.2%), and the lowest was in Oman (7.9%). In Egypt, Kuwait, Lebanon, and Saudi Arabia, the data showed a significant difference in the education level of smokers before and during the lockdown (p<0.05). Smokers in Lebanon had lower education levels than those in other countries, where the majority of smokers had a Bachelor's degree. The findings show that the BMI rates in Jordan, Lebanon, Oman, and Saudi Arabia significantly increased during the lockdown (p<0.05). The highest percentages of obesity among smokers before the lockdown were in Oman (33.3%), followed by Bahrain (28.4%) and Qatar (26.4%), whereas, during the lockdown, the percentage of obese smokers was highest in Bahrain (32.1%) followed by Qatar (31.3%) and Oman (25%). According to the logistic regression model, the odds ratio of smoking increased during the pandemic, whereas the odds ratio of TV watching decreased. This finding was statistically significant by age, gender, education level, country of residence, and work status. CONCLUSIONS: Although the overall rates of smoking among the studied countries decreased during the lockdown period, we cannot attribute this change in smoking behavior to the lockdown. Smoking cessation services need to anticipate that unexpected disruptions, such as pandemic lockdowns, may be associated with changes in daily tobacco consumption. Public health authorities should promote the adoption of healthy lifestyles to reduce the long-term negative effects of the lockdown

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat
