    Preparation and Characterization of Isosorbide Mononitrate Hydrogels Obtained by Free-Radical Polymerization for Site-Specific Delivery

    Purpose: To prepare and characterize acrylic acid and ethyl cellulose hydrogels of isosorbide mononitrate for site-specific delivery.Methods: Free radical polymerization method was employed using benzoyl peroxide as initiator and N, N’-Methylenebisacrylamide (MBA) crosslinked copolymer of ethyl cellulose and acrylic acid. Benzyl peroxide and N, N'-Methylenebisacrylamide in varying amounts were dissolved in acrylic acid. The two solutions were mixed together to a final weight of 100 g. Hydrogels were evaluated for sol-gel characteristics, diffusion coefficient, and porosity. Hydrogel formation was examined by FTIR while drug loading efficiency study was carried out using 1 % (w/v) drug solution.Results: Swelling and drug release decreased with increasing acrylic acid and MBA concentrations due to high degree of crosslinking. Increasing acrylic acid content of hydrogel produced a decrease in drug release from 29.89 to 25.79 %, 75.37 to 67.87 % and 84.91 to 75.85 % at pH 1.2, 6.5 and 7.5, respectively. Remarkably, high swelling was observed at higher pH. Gel fraction and porosity results showed that acrylic acid and crosslinker raised gel fraction but reduced porosity, while ethyl cellulose exhibited a reverse effect. FTIR confirmed graft copolymer formation.Conclusion: Isosorbide mononitrate hydrogels prepared with crosslinked copolymer of ethyl cellulose and acrylic acid can be suitably formulated for targeted delivery of the drug to the small intestine.Keywords: N, N'-Methylenebisacrylamide, Ethyl cellulose, Acrylic acid, Isosorbide mononitrate, Free radical polymerization, Graft copolymer, Site-specific delivery, Hydrogel, pH-sensitiv

    Obesity and cardiovascular disease risk factors among adult Sudanese

    Subjects and methods: In this study, the effect of obesity on the development of dyslipidemia, hypertension and glucose intolerance among Sudanese adults attending weight reduction programs was investigated. According to the BMI (Body mass index), 98 overweight/obese and 60 normal weight adults were included. Anthropometric measures were taken, lipid profile and C – reactive protein (CRP) were determined using commercial kits.Results: Obesity related dyslipidemia seems to affect overweight/obese males more than females. On the other hand, overweight /obesity among females, not like males, was found to be associated with high blood pressure probably due inflammation, as determined by CRP level.Conclusion and recommendation: Obesity related dyslipidemia is more prominent among males while obesity related hypertension is a phenomenon among females probably due to release of CRP. We recommend a more detailed study of inflammatory cytokines, in relation to obesity, that mayreflect the mass and/or activity of the adipose tissue.Key words: overweight, dyslipedemia, CVD

    SITTING VERSUS LATERAL POSITION FOR INDUCTION OF SPINAL ANESTHESIA IN ELDERLY PATIENTS – A RANDOMIZED CONTROLLED TRIAL

    Objectives: To compare the effect of induction position (sitting versus lateral) for spinal anaesthesia in the elderly patient on hemodynamic, sensory block and motor block characteristics and patient satisfaction.Material and methods: Randomized controlled trial of patients undergoing spinal anaesthesia for lower abdominal, pelvic, lower limb and urological surgeries aged more than 60 years. Hyperbaric Bupivacain (0.05%) was injected into the spinal space while the patients were either in sitting or lateral position. Effects on hemodynamic parameters, sensory block and motor block characteristics and patient satisfaction were analysed.Results: Induction position for spinal anaesthesia does not affect the hemodynamic parameters and incidence of adverse effects when adequate preloading is done. There was no statistically significant difference in the sensory level and motor level achieved. However lateral position appears to be more comfortable for elderly patients (P= 0.03).Conclusions: Induction position for administration of spinal anaesthesia has no effect on hemodynamic parameters or block characteristics except that patients feel more comfortable in lateral position.Keywords: Spinal anesthesia, Induction position, Hyperbaric bupivacaine

    Antibacterial and antifungal activities of the extract and fractions of aerial parts of Heliotropium bacciferum

    Background: H. bacciferum belonging to the family Boraginaceae is an important medicinal plant. The current research was carried out to investigate the medicinal properties of this plant.Methodology: The crude (methanol fraction) and n-hexane, ethyl acetate, butanol and aqueous fractions were subjected to antibacterial and antifungal activities by using standard methodology available in literature. Bacterial strains of Salmonella typhi, Escherichia coli, Pseudomonas Aeroginosa, Staphylococcus aureus, Erwinia carotovora, Klebsiella Pneumoniae, Bacillus subtilis and Bacillus atrophaeus were used for antibacterial activity.Results: All the fractions were active against different bacterial strains but n-hexane and ethyl acetate showed (Zone of inhibition ranged from 18-30 mm) highest activity. The fungal strains, Trichoderma longibrachiantum, Aspergillus flavus, Aspergillus niger, Fusarium solani and Candida albican were used for antifungal activity. Excellent inhibitory effect was observed against all fungal strains. The minimum inhibitory concentration (MIC) against various fungal strains was determined. The minimum inhibitory concentrations (MICs) of the investigated plant fractions ranged from 0.5- 2.00 mg/ml.Conclusion: The plant showed significant antibacterial and antifungal activities. All tested plant extracts exhibit activities against different fungal strains.     Organization Commitment of Public Primary School Senior Head Teachers

    Commitment to organization is important and plays key role in the formation of an integrated human effort in an organization. The importance of organizational commitment has attracted consideration over recent years and has been reflected in many management studies. Specifically, the objective of this study was to identify the significant difference between the selected demographic variables and organizational commitment. A quantitative cross sectional research design with purposive sampling was employed in collecting data. The cross sectional survey design was used to determine the demographic variables of gender and ethnicity. The questionnaires for Organizational Commitment was originated by Mowday et al., (2003). The respondents for this study were senior public primary school head teachers who attended the three years intensive program of Bachelor Degree in Education Management. Two cohorts comprised of 107 students were chosen from 600 students who enrolled for the Head Teacher Degree Program. They were purposively selected because they represented almost equal numbers of respondents based on gender and ethnicity of the ratio of people in Malaysia. The results of the analysis revealed that organizational commitment showed no significant different between male and female. There was significant difference for organizational commitment related to ethnic of Malay, Chinese and Indian. In conclusion, the degree of commitment of the organization among senior teacher has shown the differences between the ethnics but no different between the gender. Therefore, organization need to build up the necessary efforts to encourage and enable the different ethnics to strive their strong commitment to the organization. This will enable the organization to meet future challenges and at the same time maintain employee’s attachment to the organization

    Subaponeurotic fluid collection (SFC) in an infant

    Subaponeurotic fluid collection is an infrequent cause of scalp swelling in infants. We report a 10-week-old male who had a scalp swelling for five days without any recent history of head injury. There was a soft, mobile, non-tender, transilluminating, fluctuant swelling over posterior parieto-occipital area and it was not limited by suture lines. Cranial ultrasonography showed a subaponeurotic collection at the posterior parieto-occipital area without definite communication with the intracranial space or the brain parenchyma. This infant was treatedconservatively. The swelling resolved spontaneously after 4 weeks. The lack of awareness of some doctors in the Emergency Department about this condition may lead to unnecessary investigations, ward admission and intervention. It is hoped that this case report will provide emergency health care professionals the confidence to make accurate diagnosis and treat patient with subaponeurotic fluid collection optimally

    Expanding the set of rhodococcal Baeyer–Villiger monooxygenases by high-throughput cloning, expression and substrate screening

    To expand the available set of Baeyer–Villiger monooxygenases (BVMOs), we have created expression constructs for producing 22 Type I BVMOs that are present in the genome of Rhodococcus jostii RHA1. Each BVMO has been probed with a large panel of potential substrates. Except for testing their substrate acceptance, also the enantioselectivity of some selected BVMOs was studied. The results provide insight into the biocatalytic potential of this collection of BVMOs and expand the biocatalytic repertoire known for BVMOs. This study also sheds light on the catalytic capacity of this large set of BVMOs that is present in this specific actinomycete. Furthermore, a comparative sequence analysis revealed a new BVMO-typifying sequence motif. This motif represents a useful tool for effective future genome mining efforts.

    Enhancing river health monitoring: Developing a reliable predictive model and mitigation plan

    The escalating environmental harm inflicted upon rivers is an unavoidable outcome resulting from climatefluctuations and anthropogenic activities, leading to a catastrophic impact on water quality and thousands ofindividuals succumb to waterborne diseases. Consequently, the water quality monitoring stations have beenestablished worldwide. Regrettably, the real-time evaluation of Water Quality Index (WQI) is hindered by theintricate nature of off-site water quality parameters. Thus, there is a pressing need to create a precise and robustwater quality prediction model. The dynamic and non-linear characteristics of water quality parameters posesignificant challenges for conventional machine learning algorithms like multi-linear regression, as they struggleto capture these complexities. In this particular investigation, machine learning model called FeedforwardArtificial Neural Networks (FANNs) was employed to develop WQI prediction model of Batu Pahat River,Malaysia exclusively utilizing on-site parameters. The proposed method involves a consideration of whether toinclude or exclude parameters such as BOD and COD, which are not measured in real time and can be costly tomonitor as model inputs. Validation accuracy values of 99.53%, 97.99%, and 91.03% were achieved in threedifferent scenarios: the first scenario utilized the full input, the second scenario excluded BOD, and the thirdscenario excluded both BOD and COD. It was suggested that the model has better predictive power between inputvariables and output variables. Factor contributed to river pollution has been identified and mitigation plan forBatu Pahat river pollution has been proposed. This could provide an effective alternative to compute thepollution, better manage water resources and mitigate negative impacts of climate change of river ecosystems
