    Documenting occupational sorting by gender in the UK across three cohorts: does a grand convergence rely on societal movements?

    We consider the extent to which temporal shifts have been responsible for an increased tendency for females to sort into traditionally male roles over time, versus childhood factors. Drawing on three cohort studies, which follow individuals born in the UK in 1958, 1970 and 2000, we compare the shift in the tendency of females in these cohorts to sort into traditionally male roles compared to males, to the combined effect of a large set of childhood variables. For all three cohorts, we find strong evidence of sorting along gendered lines, which has decreased over time, yet there is no erosion of the gender gap in the tendency to sort into occupations with the highest share of males. Within the cohort, we find little evidence that childhood variables change the tendency for females of either the average or highest ability to sort substantively differently. Our work is highly suggestive that temporal shifts are what matter in determining the differential gendered sorting patterns we have seen over the last number of decades, and also those that remain today. These temporal changes include attitudinal changes, technology advances, policy changes and economic shifts

    Utilization of health care services by patients with chronic obstructive pulmonary disease

    AbstractIn order to identify healthcare resource use patterns associated with chronic obstructive pulmonary disease (COPD), resource utilization (RU) data collection was integrated into a randomized, double-blind placebo-controlled study of Viozanâ„¢ (sibenadet HCI). This study enrolled patients with symptomatic, smoking-related COPD, randomized to receive sibenadet or placebo for a 52-week treatment period. A questionnaire establishing typical pre-trial, COPD, randomized to RU was completed by each patient. Subsequent data were collected by means of an Interactive Voice Response System (IVRS) at 30-day intervals (14 time points) during the study and in the follow-up period. The IVRS system facilitated data collection and minimized inconvient to the patients. Subsequent data were collected by means of an Interactive Voice Response System health services during the year-long study was high.No overall trend for lower RU was associated with sibenadet therapy, which correlates with the lack of sustained clinical effect seen in studies conducted concurrently. These data do, however, provide valuable information on RU associated with COPD and insights into adjustments associated with changes in disease course. Physicians were seen to be the most common source of care for patients with COPD and more of the patients with severe COPD (stage III) than mild (stage I) were seen to utilize the most expensive resources (e.g. inpatient hospital care). For those patients who experienced an exacerbation during the trial (irrespective of treatment group), resource use was increased during the periods when an exacerbation was reported when compared with the periods before or after an exacerbation. The proportion of cases attending the physician doubled and with a trip to the Emergency Room (ER) increased approximately ninefold during the reporting period in which the exacerbation occurred compared with the previous month.This study has shown that use of an IVRS, even in elderly patients, is an effective means of gathering RU data over long periods. The study findings suggest that the advent of effective therapeutic interventions, particularly any with the ability to minimize exacerbations and limit disease progression, could impact on the health care services used and potentially reduce associated costs

    Natural fruitlet abscission as related to apple tree carbon balance estimated with the MaluSim model

    Apple trees produce many more flower clusters than needed for a full crop, but natural early season flower and fruitlet abscission drastically reduce the final fruit number. Natural fruit abscission varies significantly year to year. There have been attempts to try to model apple fruit abscission in the past. However, due to the great complexity of a perennial crop system in a dynamic environment with significant plant manipulations, regulatory processes and controlling environmental variables have been difficult to elucidate. In 1995, a field trial was planted at the New York State Agricultural Experiment Station in Geneva, New York with 3 apple cultivars (‘Delicious’, ‘Gala’, and ‘McIntosh’). Beginning in 2000 and for 18 years thereafter, we recorded the natural whole-season fruit abscission of untreated trees that received no chemical or hand thinning. We also estimated early season patterns of carbohydrate supply-to-demand each year with a carbon balance model. These data were used to correlate tree carbon balance status and other environmental variables with natural fruit abscission responses. In general terms, natural set, defined as final fruit/flower cluster, of ‘Gala’ averaged ˜1 fruit for each flower cluster (fruit set = 0.9), whereas fewer fruits were set on ‘Delicious’ and ‘McIntosh’ (fruit set = 0.7 and 0.6, respectively). Fruit set of ‘Gala’ was less variable than of ‘Delicious’ or ‘McIntosh’, and there was a clear pattern for decreasing fruit set when the number of initial flower clusters per tree increased. Fruit weight was less dependent on fruit number for ‘Delicious’ and ‘McIntosh’ than for ‘Gala’. Multiple regression models indicated that number of flower clusters per tree and average carbohydrate balance between 0–60 degree days (DD) after bloom and 300–360 DD after bloom were the main significant variables that explained 60–80% of the variability in natural fruit set or final fruit number. For ‘Delicious’, temperatures of the previous fall also explained a significant amount of variation in final fruit set and final fruit number. For ‘Gala’, carbon balance from bloom to shortly after petal fall and when fruits were about 18 mm were the two main periods, which were more sensible to carbohydrate deficiency triggering fruit abscission.     Modelling physiological and environmental factors regulating relative fruit set and final fruit numbers in apple trees

    Chemical thinning of apple (Malus domestica Borkh.) has been practised for 50 years but it remains an unpredictable part of apple production with large variations from year to year and within years. Carbohydrate availability to support young fruitlet growth may play a significant role in apple tree response to chemical thinners, especially when the carbohydrate supply is the limiting factor for fruit growth. To address the carbohydrate component, we have tested the MaluSim model that integrates many environmental and tree physiological factors as a tool to predict chemical thinner response. The model suggests that carbon supply-to-demand variations may explain some of the great variations in thinning spray response. Relative fruit set and final fruit number per tree were affected by the carbohydrate balance within 2 days before the spray and up to 5 days after. There was a period, 15–29 days after bloom that thinners showed higher action. The greater the carbohydrate supply relative to demand, the greater the relative set and the final fruit number. This suggested that carbohydrate supply-demand balance may be a baseline for thinner responses, and that integrative modelling of these balances can be useful in understanding variation in thinning responses.     Towards energy aware cloud computing application construction

    The energy consumption of cloud computing continues to be an area of significant concern as data center growth continues to increase. This paper reports on an energy efficient interoperable cloud architecture realised as a cloud toolbox that focuses on reducing the energy consumption of cloud applications holistically across all deployment models. The architecture supports energy efficiency at service construction, deployment and operation. We discuss our practical experience during implementation of an architectural component, the Virtual Machine Image Constructor (VMIC), required to facilitate construction of energy aware cloud applications. We carry out a performance evaluation of the component on a cloud testbed. The results show the performance of Virtual Machine construction, primarily limited by available I/O, to be adequate for agile, energy aware software development. We conclude that the implementation of the VMIC is feasible, incurs minimal performance overhead comparatively to the time taken by other aspects of the cloud application construction life-cycle, and make recommendations on enhancing its performance

    An integrated approach to diagnosis and management of severe haemoptysis in patients admitted to the intensive care unit: a case series from a referral centre

    BACKGROUND: Limited data are available concerning patients admitted to the intensive care unit (ICU) for severe haemoptysis. We reviewed a large series of patients managed in a uniform way to describe the clinical spectrum and outcome of haemoptysis in this setting, and better define the indications for bronchial artery embolisation (BAE). METHODS: A retrospective chart review of 196 patients referred for severe haemoptysis to a respiratory intermediate care ward and ICU between January 1999 and December 2001. A follow-up by telephone interview or a visit. RESULTS: Patients (148 males) were aged 51 (± sd, 16) years, with a median cumulated amount of bleeding averaging 200 ml on admission. Bronchiectasis, lung cancer, tuberculosis and mycetoma were the main underlying causes. In 21 patients (11%), no cause was identified. A first-line bronchial arteriography was attempted in 147 patients (75%), whereas 46 (23%) received conservative treatment. Patients who underwent BAE had a higher respiratory rate, greater amount of bleeding, persistent bloody sputum and/or evidence of active bleeding on fiberoptic bronchoscopy. When completed (n = 131/147), BAE controlled haemoptysis in 80% of patients, both in the short and long (> 30 days) terms. Surgery was mostly performed when bronchial arteriography had failed and/or bleeding recurred early after completed BAE. Bleeding was controlled by conservative measures alone in 44 patients. The ICU mortality rate was low (4%). CONCLUSION: Patients with evidence of more severe or persistent haemoptysis were more likely to receive BAE rather than conservative management. The procedure was effective and safe in most patients with severe haemoptysis, and surgery was mostly reserved to failure of arteriography and/or early recurrences after BAE

    Guidelines for the definition of operational management units

    The objective of fisheries management is the sustainable exploitation of the fish resources over the extent of their spatial distribution. Along with the Common Fisheries Policy (CFP) objectives, the socio-economic viability of the fisheries exploiting the resource is also to be achieved. To reach these aims, managers need to define the management units they are going to work with. For the purpose of GEPETO project, we define a management unit (MU) as the set of fishing fleets exploiting a common pool of fish resources with strong spatial overlapping and sharing of habitats, which make them being typically fished together. In other words, a MU is the set of fishing fleets exploiting a common fish community over their spatial distribution. MUs have to be defined by the fish community, by the spatial range of distribution of the fish community, and by the set of fishing fleets sharing the exploitation of the fish communityL'objectif de gestion de la pêche est l'exploitation durable des ressources halieutiques sur l'étendue de leur répartition spatiale.     <p>Abstract</p> <p>Background</p> <p>Retroperitoneal enterogenous cysts are uncommon and adenocarcinoma within such cysts is a rare complication.</p> <p>Case presentation</p> <p>We present the third described case of a retroperitoneal enterogenous cyst with adenocarcinomatous changes and only the second reported case whereby the cyst was not arising from any anatomical structure.</p> <p>Conclusion</p> <p>This case demonstrates the difficulties in making a diagnosis as well as the importance of a multi-disciplinary approach, and raises further questions regarding post-operative treatment with chemotherapy.</p
