    Data Warehouse and BI to catalize information use in health sector for decision making: a case study

    Health sector in most countries including Zanzibar lacks the single repository that integrates health management data produced by multitude of sources. The study proves cost effective but successful implementation of Data Warehouse (DW) plays useful role to influence informed decision making in the health sector. Successful DW needs to include necessary features such as Online Analytic Process (OLAP) which gives decision makers the freedom to analyses information into different format business intelligence (BI) tools, data mining and data presentation tools. In this study, we attempted to identify DW needs to consider culture of organization and what organization actual wants for decision making. The study proposed the cost effective DW and BI tools solution to catalyze informed decision making in health care industry. The study also aims to discuss challenges and sets out recommendations

    Teaching and learning considerations during the COVID 19 pandemic: Supporting multimodal student learning preferences

    Background. The advent of COVID‑19 and the subsequent national lockdown has catapulted higher education institutions into emergency remote teaching (ERT). A principal challenge in this shift is the ability to stimulate student interest towards engagement with, and retention of, course content. The creation of teaching and learning (T&L) resources and activities using a combination of the visual, aural, read/write and kinaesthetic (VARK) modes is fundamental in ensuring student engagement. Objectives. To determine the learning style profiles of undergraduate students and to explore how student learning profiles may be incorporated in T&L approaches during ERT. Methods. This descriptive study profiles the learning preferences of undergraduate students in a health science faculty using the VARK questionnaire. The study further outlines modifications in T&L implemented to support the varied learning preferences during the COVID‑19 ERT response. Results. Our findings demonstrate that the majority of our students have a multimodal learning preference, with the kinaesthetic modality being the most preferred. Voice-over PowerPoint presentations with transitioning images, and audio files, supported the visual and aural learners through asynchronous engagement. Additionally, online discussion forums and applied projects (such as theme park designs) enhanced asynchronous learning by stimulating the visual, read/write and kinaesthetic preferences, respectively. Microsoft Team sessions with PowerPoint presentations supported visual and aural learning preferences through synchronous engagement. Conclusions. Rethinking traditional T&L approaches towards supporting the diverse student learning preferences is critical in student-centred T&L amidst the many challenges that ERT has precipitated. Academics need to be dynamic in their T&L approaches and intuitive in their awareness of how subject content may be modified/enhanced in the ERT environment

    Exploring the Roles of Social Participation in Mobile Social Media Learning: A Social Network Analysis

    Social media is increasingly becoming an essential platform for social connectivity in our daily lives. The availability of mobile technology has further fueled its importance – making it a ubiquitous tool for social interaction. However, limited studies have been conducted to investigate roles of social participation in this field. Thus, the study investigates roles of social participation in mobile social media learning using the “ladder of participation and mastering.” Participants were students taking an educational technology course in a local university. The study was conducted in a four-month period. Data was collected from discussions while learning among the students a social media platform, Facebook groups, on mobile devices. The data was analyzed using a social network analysis tool, NodeXL. Data was analyzed based on egocentric networks, betweeness centrality, and closeness centrality. The findings revealed that there are four roles of social participation in mobile social media, which are: (i) lurkers; (ii) gradually mastering members/passive members; (iii) recognized members; and (iv) coaches. The findings also indicated that over the course of four months, learners can inter-change roles of social participation – becoming more central or less central in learning discussions. As a result, a roles of social participation continuum for mobile social media learning is proposed. Future research could be conducted in other fields to investigate whether the continuum produced could be used to understand the relationship between mobile social media learning and social participation roles

    Endothelial Nitric Oxide Synthase (eNOS) and the Cardiovascular System: In Physiology and in Disease States

    Endothelial nitric oxide synthase (eNOS) plays a critical role in regulating and maintaining a healthy cardiovascular system. The importance of eNOS can be emphasized from the genetic polymorphisms of the eNOS gene, uncoupling of eNOS dimerization, and its numerous signaling regulations. The activity of eNOS on the cardiac myocytes, vasculature, and the central nervous system are discussed. The effects of eNOS on the sympathetic autonomic nervous system (SANS) and the parasympathetic autonomic nervous system (PANS), both of which profoundly influence the cardiovascular system, will be elaborated. The relationship between the eNOS protein with cardiovascular autonomic reflexes such as the baroreflex and the Exercise Pressor Reflex will be discussed. For example, the effects of endogenous nitric oxide (NO) are shown to be mediated by the eNOS protein and that eNOS-derived endothelial NO is most effective in regulating blood pressure oscillations via modulating the baroreflex mechanisms. The protective action of eNOS on the CVS is emphasized here because dysfunction of the eNOS enzyme is intricately correlated with the pathogenesis of several cardiovascular diseases such as hypertension, arteriosclerosis, myocardial infarction, and stroke. Overall, our current understanding of the eNOS protein with a focus on its role in the modulation, regulation, and control of the cardiovascular system in a normal physiological state and in cardiovascular diseases are discussed

    Idiopathic perinatal hepatic infarct as a cause of liver mass

    We present the case of a 2-week-old male infant who presented with an asymptomatic liver mass and underwent surgical resection because of suspicion of malignancy after extensive radiological study with ultrasonography, computed tomography, and MRI. Pathological examination revealed a peripheral hepatic infarct with calcifications and a rim of peripheral organization suggestive of being at least 2 weeks old. This case reviews the scarce cases of hepatic infarct in newborns and highlights the fact that, although untreated perinatal hepatic infarction usually progresses to atrophy of the affected area with compensation by the unaffected liver, surgical resection remains an option in cases of complications or uncertain diagnosis

    Design and Implementation of Distributed Identity and Access Management Framework for Internet of Things (IoT) Enabled Distribution Automation

    The smart grid and Internet of Things (IoT) technologies play vital roles in improving the quality of services offered in traditional electrical grid. They open a room for the introduction of new services like distribution automation (DA) that has a significant advantage to both utility companies and final consumers. DA integrates sensors, actuators, intelligent electrical devices (IED) and information and communication technologies to monitor and control electrical grid. However, the integration of these technologies poses security threats to the electrical grid like Denial of Service (DoS) attacks, false data injection attacks, and masquerading attacks like system node impersonation that can transmit wrong readings, resulting in false alarm reports and hence leading to incorrect node actuation. To overcome these challenges, researchers have proposed a centralized public key infrastructure (PKI) with bridged certificate authority (CA) which is prone to DoS attacks. Moreover, the proposed blockchain based distributed identity and access management (DIAM) in IoT domain at the global scale is adding communicational and computational overheads. Also. It is imposing new security threats to the DA system by integrating it with online services like IoTEX and IoTA. For those reasons, this study proposes a DIAM security scheme to secure IoT-enabled distribution automation. The scheme divides areas into clusters and each cluster has a device registry and a registry controller. The registry controller is a command line tool to access and manage a device registry. The results show that the scheme can prevent impersonated and non-legitimate system nodes and users from accessing the system by imposing role-based access control (RBAC) at the cluster level.     Ocular manifestations of HIV infection at Chris Hani Baragwanath Academic Hospital, Johannesburg, South Africa

    Background. The pattern of HIV-associated eye disease has changed with ongoing advancements in highly active antiretroviral therapy (HAART). HIV-infected individuals now live longer, enabling us to observe the long-term effects of HIV and HAART on the eye. There are few recent studies on HIV-related ocular disease in sub-Saharan Africa. Objectives. To describe the ocular manifestations of HIV in patients attending the Nthabiseng HIV clinic at Chris Hani Baragwanath Academic Hospital, Johannesburg, South Africa. Methods. A cross-sectional study was conducted in 2021 and 2022 using convenience sampling of patients at the HIV clinic. The participants’ clinical history was taken, their files were reviewed, and they underwent ocular examination. Correlation between eyes was managed by taking disease in one eye as the presence of disease in the participant. Descriptive statistics were used to summarise participant characteristics. Univariate and multivariate logistic regression models were used to assess the odds ratio (OR) of developing HIV-associated ocular diseases, and a p-value of <0.05 was used to define statistical significance. Results. There were 182 participants (139 females and 43 males), with a mean (standard deviation) age of 48.9 (10.6) years. The most common anterior segment diagnoses were conjunctival microangiopathy (34.6%), pinguecula (31.3%) and cataracts (30.2%), while the most common posterior segment finding was peripheral retinal scarring with features in keeping of previous cytomegalovirus retinitis (24.2%). Notably, only 1.1% of patients had HIV retinopathy. A CD4 count <200 cells/μL showed an increased OR for cataracts (OR 4.24; p=0.003) and any anterior segment diagnoses (OR 10.05; p=0.029), while a CD4 count ≥200 cells/μL showed an increased risk of conjunctival microangiopathy (OR 2.14; p=0.017). Conclusion. With the advent of HAART, ocular manifestations of HIV are changing and the incidence of severe ocular opportunistic infections and HIV retinopathy has decreased precipitously. Although this study has shown that patients with a CD4 count <200 cells/μL are at increased risk of developing anterior ocular manifestations of HIV, including cataracts, these diseases are relatively innocuous or easily treatable. Routine ocular screening of HIV patients seems to be substantially less important now than it was in the pre-HAART era

    Linking environmental variables with regional-scale variability in ecological structure and standing stock of carbon within UK kelp forests

    Kelp forests represent some of the most productive and diverse habitats on Earth. Understanding drivers of ecological patterns at large spatial scales is critical for effective management and conservation of marine habitats. We surveyed kelp forests dominated by Laminaria hyperborea (Gunnerus) Foslie 1884 across 9° latitude and \u3e1000 km of coastline and measured a number of physical parameters at multiple scales to link ecological structure and standing stock of carbon with environmental variables. Kelp density, biomass, morphology and age were generally greater in exposed sites within regions, highlighting the importance of wave exposure in structuring L. hyperborea populations. At the regional scale, wave-exposed kelp canopies in the cooler regions (the north and west of Scotland) were greater in biomass, height and age than in warmer regions (southwest Wales and England). The range and maximal values of estimated standing stock of carbon contained within kelp forests was greater than in historical studies, suggesting that this ecosystem property may have been previously undervalued. Kelp canopy density was positively correlated with large-scale wave fetch and fine-scale water motion, whereas kelp canopy biomass and the standing stock of carbon were positively correlated with large-scale wave fetch and light levels and negatively correlated with temperature. As light availability and summer temperature were important drivers of kelp forest biomass, effective management of human activities that may affect coastal water quality is necessary to maintain ecosystem functioning, while increased temperatures related to anthropogenic climate change may impact the structure of kelp forests and the ecosystem services they provide
