    Severe Invasive Streptococcal Disease - Emerging Disease?

    Introdução: A incidência da doença estreptocócica invasiva (DSI) tem vindo a aumentar na Europa e América do Norte desde o final dos anos 1980, provavelmente relacionada com a emergência de estirpes mais virulentas. Em oito meses foram internados no nosso hospital seis casos desta entidade rara. Objectivos: Descrever as características da doença estreptocócica invasiva grave. Métodos: Estudo descritivo, de Dezembro de 2007 a Julho de 2008. Analisaram-se parâmetros demográficos, factores de risco, clínica, terapêutica, complicações e evolução. Resultados: Identificaram-se seis casos com mediana de idade de 2,5 anos: síndrome de choque tóxico estreptocócico (STSS) (2), fasceíte necrotizante (2), bacteriémia (1) e infecção estreptocócica grave (1). Cinco casos ocorreram entre Dezembro e Fevereiro. Em quatro doentes registaram-se eventuais factores de risco (infecções virais e anti-inflamatórios não esteroides). Duas crianças necessitaram de cirurgia e três de tratamento em cuidados intensivos. Todas as estirpes eram susceptíveis à penicilina e clindamicina. Ocorreram complicações em 5/6 doentes: choque séptico (3), coagulação intravascular disseminada (2), insuficiência renal (2), abcesso de tecidos moles (2), sobreinfecção bacteriana (2), síndrome de dificuldade respiratória do adulto (1), osteomielite /artrite séptica (1) e pneumonia/derrame pleural (1). Não se registaram óbitos. Comentários: Seis casos de DSI num curto espaço de tempo podem indiciar a emergência de estirpes de Streptococcus grupo A (GAS) de maior virulência no nosso país, pelo que a realização de estudos moleculares será fundamental na identificação de clones invasivos. Apesar da susceptibilidade à penicilina, a DSI cursa com morbilidade elevada, pelo que, o importante parece ser procurar novas formas de tratar o doente e não o agente

    O diagnóstico diferencial de hepatite colestática aguda na criança em idade escolar previamente saudável inclui múltiplas causas, sendo as infeciosas, tóxicas e autoimunes as mais comuns. Descreve-se o caso clínico de um rapaz de 10 anos com icterícia prolongada, eosinofilia e evidência bioquímica de lesão hepática de tipo misto (transaminases, fosfatase alcalina,bilirrubina total e direta elevadas), com gama-glutamiltransferase e função hepática normais. Após investigação detalhada, incluindo biópsia hepática, admitiu-se etiologia tóxica, tendo-se identificado consumo recente de chás de Cymbopogon citratus (erva-príncipe) e Equisetum arvense (erva-cavalinha) contaminados com pesticidas. A hipótese de hepatotoxicidade induzida por chás contaminados com pesticidas foi admitida como a causa mais provável, após exclusão de outras causas de doença hepática e considerando a recuperação total do doente após quatro meses de terapêutica com ácido ursodesoxicólico e evicção dos referidos consumos. Na última década, assistiu-se a um aumento substancial de publicações referentes a lesão hepática induzida por tóxicos (fármacos, suplementos alimentares, produtos de ervanária, metais e mesmo pesticidas).     LOCUS (LOng Covid–Understanding Symptoms, events and use of services in Portugal): A three-component study protocol

    Approximately 10% of patients experience symptoms of Post COVID-19 Condition (PCC) after a SARS-CoV-2 infection. Akin acute COVID-19, PCC may impact a multitude of organs and systems, such as the cardiovascular, respiratory, musculoskeletal, and neurological systems. The frequency and associated risk factors of PCC are still unclear among both community and hospital settings in individuals with a history of COVID-19. The LOCUS study was designed to clarify the PCC's burden and associated risk factors. LOCUS is a multi-component study that encompasses three complementary building blocks. The "Cardiovascular and respiratory events following COVID-19" component is set to estimate the incidence of cardiovascular and respiratory events after COVID-19 in eight Portuguese hospitals via electronic health records consultation. The "Physical and mental symptoms following COVID-19" component aims to address the community prevalence of self-reported PCC symptoms through a questionnaire-based approach. Finally, the "Treating and living with Post COVID-19 Condition" component will employ semi-structured interviews and focus groups to characterise reported experiences of using or working in healthcare and community services for the treatment of PCC symptoms. This multi-component study represents an innovative approach to exploring the health consequences of PCC.     Polimiosite : investigação clínica em duas irmãs

    We present an investigation of a case of polymyositis affecting two sisters of one same parenthood. Their cases have been documented for almost two decades, being investigated by means of a diagnostic protocol which combined clinical findings as well as laboratorial, histopathological and image tests. In both cases, clinical manifestations started in childhood, without signs of involvement of the central and peripheral nervous system. Both patients proved to respond to a therapeutics based on corticosteroids. The degree of relatedness between their parents corroborate the notion that genetic factors may contribute to the development of the disease. ___________________________________________________________________________________________________ RESUMOApresentamos a investigação de dois casos de polimiosite, ocorridos entre irmãs de uma mesma filiação. Seus casos foram documentados ao longo de quase duas décadas, tendo sido diagnosticados utilizando- se de protocolo diagnóstico que combinou achados clínicos, exames laboratoriais, histopatológicos e por imagem. Em ambos os casos, as manifestações clínicas se iniciaram ainda na infância, sendo constatada ausência de acometimento do sistema nervoso central ou periférico. Ambas as pacientes responderam satisfatoriamente a terapia baseada em corticosteróide. O grau de parentesco entre os genitores das pacientes sugere que fatores genéticos podem predispor ao desenvolvimento da doença

    Spermatogonial Stem Cell Niche and Spermatogonial Stem Cell Transplantation in Zebrafish

    Background Spermatogonial stem cells (SSCs) are the foundation of spermatogenesis, and reside within a specific microenvironment in the testes called “niche” which regulates stem cell properties, such as, self-renewal, pluripotency, quiescence and their ability to differentiate. Methodology/Principal Findings Here, we introduce zebrafish as a new model for the study of SSCs in vertebrates. Using 5′-bromo-2′-deoxyuridine (BrdU), we identified long term BrdU-retaining germ cells, type A undifferentiated spermatogonia as putative stem cells in zebrafish testes. Similar to rodents, these cells were preferentially located near the interstitium, suggesting that the SSC niche is related to interstitial elements and might be conserved across vertebrates. This localization was also confirmed by analyzing the topographical distribution of type A undifferentiated spermatogonia in normal, vasa::egfp and fli::egfp zebrafish testes. In the latter one, the topographical arrangement suggested that the vasculature is important for the SSC niche, perhaps as a supplier of nutrients, oxygen and/or signaling molecules. We also developed an SSC transplantation technique for both male and female recipients as an assay to evaluate the presence, biological activity, and plasticity of the SSC candidates in zebrafish. Conclusions/Significance We demonstrated donor-derived spermato- and oogenesis in male and female recipients, respectively, indicating the stemness of type A undifferentiated spermatogonia and their plasticity when placed into an environment different from their original niche. Similar to other vertebrates, the transplantation efficiency was low. This might be attributed to the testicular microenvironment created after busulfan depletion in the recipients, which may have caused an imbalance between factors regulating self-renewal or differentiation of the transplanted SSCs

    Worldwide Genetic Variability of the Duffy Binding Protein: Insights into Plasmodium vivax Vaccine Development

    The dependence of Plasmodium vivax on invasion mediated by Duffy binding protein (DBP) makes this protein a prime candidate for development of a vaccine. However, the development of a DBP-based vaccine might be hampered by the high variability of the protein ligand (DBPII), known to bias the immune response toward a specific DBP variant. Here, the hypothesis being investigated is that the analysis of the worldwide DBPII sequences will allow us to determine the minimum number of haplotypes (MNH) to be included in a DBP-based vaccine of broad coverage. For that, all DBPII sequences available were compiled and MNH was based on the most frequent nonsynonymous single nucleotide polymorphisms, the majority mapped on B and T cell epitopes. A preliminary analysis of DBPII genetic diversity from eight malaria-endemic countries estimated that a number between two to six DBP haplotypes (17 in total) would target at least 50% of parasite population circulating in each endemic region. Aiming to avoid region-specific haplotypes, we next analyzed the MNH that broadly cover worldwide parasite population. The results demonstrated that seven haplotypes would be required to cover around 60% of DBPII sequences available. Trying to validate these selected haplotypes per country, we found that five out of the eight countries will be covered by the MNH (67% of parasite populations, range 48–84%). In addition, to identify related subgroups of DBPII sequences we used a Bayesian clustering algorithm. The algorithm grouped all DBPII sequences in six populations that were independent of geographic origin, with ancestral populations present in different proportions in each country. In conclusion, in this first attempt to undertake a global analysis about DBPII variability, the results suggest that the development of DBP-based vaccine should consider multi-haplotype strategies; otherwise a putative P. vivax vaccine may not target some parasite populations

    Combined Treatment of Heterocyclic Analogues and Benznidazole upon Trypanosoma cruzi In Vivo

    Chagas disease caused by Trypanosoma cruzi is an important cause of mortality and morbidity in Latin America but no vaccines or safe chemotherapeutic agents are available. Combined therapy is envisioned as an ideal approach since it may enhance efficacy by acting upon different cellular targets, may reduce toxicity and minimize the risk of drug resistance. Therefore, we investigated the activity of benznidazole (Bz) in combination with the diamidine prodrug DB289 and in combination with the arylimidamide DB766 upon T. cruzi infection in vivo. The oral treatment of T.cruzi-infected mice with DB289 and Benznidazole (Bz) alone reduced the number of circulating parasites compared with untreated mice by about 70% and 90%, respectively. However, the combination of these two compounds decreased the parasitemia by 99% and protected against animal mortality by 100%, but without providing a parasitological cure. When Bz (p.o) was combined with DB766 (via ip route), at least a 99.5% decrease in parasitemia levels was observed. DB766+Bz also provided 100% protection against mice mortality while Bz alone provided about 87% protection. This combined therapy also reduced the tissular lesions induced by T. cruzi infection: Bz alone reduced GPT and CK plasma levels by about 12% and 78% compared to untreated mice group, the combination of Bz with DB766 resulted in a reduction of GPT and CK plasma levels of 56% and 91%. Cure assessment through hemocultive and PCR approaches showed that Bz did not provide a parasitological cure, however, DB766 alone or associated with Bz cured ≥13% of surviving animals
