    Skeletal and cardiovascular consequences of a positive calcium balance during hemodialysis

    Patients on hemodialysis are exposed to calcium via the dialysate at least three times a week. Changes in serum calcium vary according to calcium mass transfer during dialysis, which is dependent on the gradient between serum and dialysate calcium concentration (d[Ca]) and the skeleton turnover status that alters the ability of bone to incorporate calcium. Although underappreciated, the d[Ca] can potentially cause positive calcium balance that leads to systemic organ damage, including associations with mortality, myocardial dysfunction, hemodynamic tolerability, vascular calcification, and arrhythmias. The pathophysiology of these adverse effects includes serum calcium changes, parathyroid hormone suppression, and vascular calcification through indirect and direct effects. Some organs are more susceptible to alterations in calcium homeostasis.     Angular momentum of focused beams: beyond the paraxial approximation

    We investigate in detail the focusing of a circularly polarized Laguerre-Gaussian laser beam (\hbar \ell orbital angular momentum per photon; σ=1/1\sigma=1/-1 for left/right-handed polarization) by a high numerical aperture objective. The diffraction-limited focused beam has unexpected properties, resulting from a strong interplay between the angular spatial structure and the local polarization in the non-paraxial regime. In the region near the beam axis, and provided that 2|\ell|\ge 2 and \ell and σ\sigma have opposite signs, the energy locally counter-propagates and the projection of the electric field onto the focal plane counter-rotates with respect to the circular polarization of the incident beam. We explicitly show that the total angular momentum flux per unit power is conserved after focusing, as expected by rotational symmetry, but the spin and orbital separate contributions change.Comment: 10 pages, 9 figure

    A geographical population analysis of dental trauma in school-children aged 12 and 15 in the city of Curitiba-Brazil

    <p>Abstract</p> <p>Background</p> <p>The study presents a geographical analysis of dental trauma in a population of 12 and 15 year-old school-children, in the city of Curitiba, Brazil (n = 1581), using a database obtained in the period 2005-2006. The main focus is to analyze dental trauma using a geographic information system as a tool for integrating social, environmental and epidemiological data.</p> <p>Methods</p> <p>Geostatistical analysis of the database and thematic maps were generated showing the distribution of dental trauma cases according to Curitiba's Health Districts and other variables of interest. Dental trauma spatial variation was assessed using a generalized additive model in order to identify and control the individual risk-factors and thus determine whether spatial variation is constant or not throughout the Health Districts and the place of residence of individuals. In addition, an analysis was made of the coverage of dental trauma cases taking the spatial distribution of Curitiba's primary healthcare centres.</p> <p>Results</p> <p>The overall prevalence of dental trauma was 37.1%, with 53.1% in males and 46.7% in females. The spatial analysis confirms the hypothesis that there is significant variation in the occurrence of dental trauma, considering the place of residence in the population studied (Monte Carlo test, p = 0,006). Furthermore, 28.7% of cases had no coverage by the primary healthcare centres.</p> <p>Conclusions</p> <p>The effect of the place of residence was highly significant in relation to the response variable. The delimitation of areas, as a basis for case density, enables the qualification of geographical territories where actions can be planned based on priority criteria. Promotion, control and rehabilitation actions, applied in regions of higher prevalence of dental trauma, can be more effective and efficient, thus providing healthcare refinement.</p

    Double shunt technique for hybrid palliation of hypoplastic left heart syndrome: a case report

    We report a technique to palliate hypoplastic left heart syndrome, with no PDA stenting, but with double polytetrafluoroethylene shunt from pulmonary artery to ascending and descending aorta by combined thoracotomies. A 30-day-old female was operated with this technique. Five months after first operation, the child was submitted to Norwood/Glenn operation. Good hemodinamic recovery and initial clinical evolution was observed. The child was extubated in 8th post operatory day and reentubated in the next day due to pulmonary infection. Despite antibiotic treatment, the child died after systemic infectious complications

    Agricultura urbana agroecológica na perspectiva da promoção da saúde

    A agricultura urbana tem sido tema de diversas pesquisas no contexto brasileiro e no mundo, principalmente no que diz respeito à geração de trabalho e renda. O presente artigo tem por objetivo sistematizar a experiência do projeto Colhendo Sustentabilidade: práticas comunitárias de segurança alimentar e agricultura urbana (PCS), realizado em Embu das Artes, região metropolitana de São Paulo, durante o período compreendido entre 2008 e 2011, analisando-o sob a ótica da Promoção da Saúde (PS). Foram sujeitos da pesquisa os participantes, os técnicos dos serviços públicos envolvidos e integrantes da equipe técnica do projeto. Os dados foram obtidos através da realização de três oficinas de sistematização, seguidas de outra em que os resultados foram apresentados aos participantes, e também por meio de entrevistas com informantes-chave. Os mesmos foram analisados à luz dos princípios e campos de ação da PS. As conclusões revelaram a agricultura urbana agroecológica (AUA) como ferramenta promotora de saúde por contribuir com o fortalecimento do protagonismo individual e comunitário, empoderamento, criação de ambientes favoráveis à saúde, bem como seu potencial para a multiplicação de "saberes", pelos envolvidos, sobre temas relacionados à geração de renda, saúde e meio ambiente

    Micromechanical Properties of Injection-Molded Starch–Wood Particle Composites

    The micromechanical properties of injection molded starch–wood particle composites were investigated as a function of particle content and humidity conditions. The composite materials were characterized by scanning electron microscopy and X-ray diffraction methods. The microhardness of the composites was shown to increase notably with the concentration of the wood particles. In addition,creep behavior under the indenter and temperature dependence were evaluated in terms of the independent contribution of the starch matrix and the wood microparticles to the hardness value. The influence of drying time on the density and weight uptake of the injection-molded composites was highlighted. The results revealed the role of the mechanism of water evaporation, showing that the dependence of water uptake and temperature was greater for the starch–wood composites than for the pure starch sample. Experiments performed during the drying process at 70°C indicated that the wood in the starch composites did not prevent water loss from the samples.Peer reviewe
