    Seven steps to successful change: How a large academic medical center prepared patients for organizational change

    Vanderbilt University Medical Center (VUMC) launched a new electronic health record (EHR) in a “big bang” implementation that saw the new software go live across multiple hospitals, clinics and geographic locations in a single morning. The organization rightly focused most of its energy on preparing its nearly 25,000 employees for the impacts of the transition, but it also considered the effects that would be felt by its patients and families. Survey data indicate that patient satisfaction scores demonstrably dip before, during and after an EHR implementation, and take approximately a year to recover. A team at DMC employed a seven-step approach to preparing patients for the impacts of the transition, which led to a return to pre-implementation patient satisfaction scores in about half the time of its peer institutions. The article explores these seven steps in detail and offers recommendations for how healthcare organizations facing large-scale change can use a similar structured approach to mitigate negative impacts to patients.     Common carp (Cyprinus carpio L.) alters its feeding niche in response to changing food resources: direct observations in simulated ponds

    We used customized fish tanks as model fish ponds to observe grazing, swimming, and conspecific social behavior of common carp (Cyprinus carpio) under variable food-resource conditions to assess alterations in feeding niche. Different food and feeding situations were created by using only pond water or pond water plus pond bottom sediment or pond water plus pond bottom sediment and artificial feeding. All tanks were fertilized twice, prior to stocking and 2 weeks later after starting the experiment to stimulate natural food production. Common carp preferred artificial feed over benthic macroinvertebrates, followed by zooplankton. Common carp did not prefer any group of phytoplankton in any treatment. Common carp was mainly benthic in habitat choice, feeding on benthic macroinvertebrates when only plankton and benthic macroinvertebrates were available in the system. In the absence of benthic macroinvertebrates, their feeding niche shifted from near the bottom of the tanks to the water column where they spent 85% of the total time and fed principally on zooplankton. Common carp readily switched to artificial feed when available, which led to better growth. Common carp preferred to graze individually. Behavioral observations of common carp in tanks yielded new information that assists our understanding of their ecological niche. This knowledge could be potentially used to further the development of common carp aquaculture

    Reversible protein biotinylation is readily affected via conjugation with a bromomaleimide-based reagent followed by reductive cleavage. The intermediate biotinylated protein constructs are stable at physiological temperature and pH 8.0. Quantitative reversibility is elegantly delivered under mild conditions of using a stoichiometric amount of a bis-thiol, thus providing an approach that will be of general interest in chemical biology and proteomics

    Hybrid and pureline hard winter wheat yield and stability

    Pureline wheat (Triticum aestivum L.) cultivars continue to dominate production fields in the southern Great Plains despite numerous attempts to introduce hybrids during the past 28 yr. The objective of this study was to analyze yield trends and yield stability in both hybrid and pureline entries in the Oklahoma Variety-Hybrid Performance Nursery (VHPN). Grain yield data from 1975 to 1995 from four locations were selected and analyzed by relative yield indices. Regression equations across time were calculated for both hybrids and pure- lines relative to the mean performance of long-term check cultivars. Both hybrids and purelines evidenced yield improvement, with the yield of hybrids, in general, increasing at a greater rate than that of purelines. Predicted values in the last year tested indicated a 10.9% advantage of hybrids over purelines. Stability parameters were com- pared by regressing hybrid and pureline yields on an environmental index based on location mean yields for checks. Regression coefficients for hybrids and purelines were not significantly different from one, nor from each other. Confidence intervals for hybrid and pureline performance generally overlapped throughout the observed yield ranges, indicating no divergence in predicted grain yield as environ- mental yield potential increased. No significant differences in stability between hybrids and purelines were found by comparing variances represented by the pooled deviations for each cultivar type. Hybrid wheat offers an opportunity for increased grain yield in the southern Great Plains of the USA, but without a stability advantage over pure- line cultivars.Peer reviewedPlant and Soil Science

    High prevalence of bronchiectasis is linked to HTLV-1-associated inflammatory disease.

    BACKGROUND: Human T-lymphotropic virus type 1 (HTLV-1), a retrovirus, is the causative agent of HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukaemia/lymphoma (ATLL). The reported association with pulmonary disease such as bronchiectasis is less certain. METHODS: A retrospective case review of a HTLV-1 seropositive cohort attending a national referral centre. The cohort was categorised into HTLV-1 symptomatic patients (SPs) (ATLL, HAM/TSP, Strongyloidiasis and HTLV associated inflammatory disease (HAID)) and HTLV-1 asymptomatic carriers (ACs). The cohort was reviewed for diagnosis of bronchiectasis. RESULT: 34/246 ACs and 30/167 SPs had been investigated for respiratory symptoms by computer tomography (CT) with productive cough +/- recurrent chest infections the predominant indications. Bronchiectasis was diagnosed in one AC (1/246) and 13 SPs (2 HAID, 1 ATLL, 10 HAM/TSP) (13/167, RR 19.2 95 % CI 2.5-14.5, p = 0.004) with high resolution CT. In the multivariate analysis ethnicity (p = 0.02) and disease state (p < 0.001) were independent predictors for bronchiectasis. The relative risk of bronchiectasis in SPs was 19.2 (95 % CI 2.5-14.5, p = 0.004) and in HAM/TSP patients compared with all other categories 8.4 (95 % CI 2.7-26.1, p = 0.0002). Subjects not of African/Afro-Caribbean ethnicity had an increased prevalence of bronchiectasis (RR 3.45 95 % 1.2-9.7, p = 0.02). CONCLUSIONS: Bronchiectasis was common in the cohort (3.4 %). Risk factors were a prior diagnosis of HAM/TSP and ethnicity but not HTLV-1 viral load, age and gender. The spectrum of HTLV-associated disease should now include bronchiectasis and HTLV serology should be considered in patients with unexplained bronchiectasis

    Quantum Acoustics with Surface Acoustic Waves

    It has recently been demonstrated that surface acoustic waves (SAWs) can interact with superconducting qubits at the quantum level. SAW resonators in the GHz frequency range have also been found to have low loss at temperatures compatible with superconducting quantum circuits. These advances open up new possibilities to use the phonon degree of freedom to carry quantum information. In this paper, we give a description of the basic SAW components needed to develop quantum circuits, where propagating or localized SAW-phonons are used both to study basic physics and to manipulate quantum information. Using phonons instead of photons offers new possibilities which make these quantum acoustic circuits very interesting. We discuss general considerations for SAW experiments at the quantum level and describe experiments both with SAW resonators and with interaction between SAWs and a qubit.     Beyond a Zero-Sum Game: How Does the Impact of COVID-19 Vary by Gender?

    Epidemics and pandemics, like COVID-19, are not gender neutral. Much of the current work on gender, sex, and COVID-19, however, has seemed implicitly or explicitly to be attempting to demonstrate that either men or women have been hardest hit, treating differences between women and men as though it is not important to understand how each group is affected by the virus. This approach often leaves out the effect on gender and sexual minorities entirely. Believing that a more nuanced approach is needed now and for the future, we brought together a group of gender experts to answer the question: how are people of different genders impacted by COVID-19 and why? Individuals working in women's, men's, and LGBTQ health and wellbeing wrote sections to lay out the different ways that women, men, and gender and sexual minorities are affected by COVID-19. We demonstrate that there is not one group "most affected," but that many groups are affected, and we need to move beyond a zero-sum game and engage in ways to mutually identify and support marginalized groups

    Prediction of preterm birth with and without preeclampsia using mid-pregnancy immune and growth-related molecular factors and maternal characteristics.

    OBJECTIVE:To evaluate if mid-pregnancy immune and growth-related molecular factors predict preterm birth (PTB) with and without (±) preeclampsia. STUDY DESIGN:Included were 400 women with singleton deliveries in California in 2009-2010 (200 PTB and 200 term) divided into training and testing samples at a 2:1 ratio. Sixty-three markers were tested in 15-20 serum samples using multiplex technology. Linear discriminate analysis was used to create a discriminate function. Model performance was assessed using area under the receiver operating characteristic curve (AUC). RESULTS:Twenty-five serum biomarkers along with maternal age &lt;34 years and poverty status identified &gt;80% of women with PTB ± preeclampsia with best performance in women with preterm preeclampsia (AUC = 0.889, 95% confidence interval (0.822-0.959) training; 0.883 (0.804-0.963) testing). CONCLUSION:Together with maternal age and poverty status, mid-pregnancy immune and growth factors reliably identified most women who went on to have a PTB ± preeclampsia
