    Tratamiento del Síndrome del Túnel Carpiano con la Técnica del Retinaculotomo de PAINE

    Los autores han realizado un estudio con 36 casos intervenidos quirúrgicamente en el Servicio de Neurocirugía del Hospital General de Especialidades "Virgen de las Nieves" de Granada, tanto a corto como a largo plazo, empleando la técnica quirúrgica de PAINE, basada en el instrumento por él diseñado, el retinaculotomo. Realizan un estudio de los resultados obtenidos, siendo totalmente satisfactorios en el 74% de los casos. Finalmente exponen las ventajas de esta técnica quirúrgica, así como, la práctica ausencia de complicaciones.36 Patients with Carpal Tunnel Syndrome are colected.     Dietary Inflammatory Index, Dietary Non-Enzymatic Antioxidant Capacity, and Colorectal and Breast Cancer Risk (MCC-Spain Study)

    Inflammation and antioxidant capacity have been associated with colorectal and breast cancer. We computed the dietary inflammatory index (DII®), and the total dietary non-enzymatic antioxidant capacity (NEAC) and associated them with colorectal and breast cancer risk in the population-based multi case-control study in Spain (MCC-Spain). We included 1852 colorectal cancer and 1567 breast cancer cases, and 3447 and 1486 population controls, respectively. DII score and NEAC were derived using data from a semi-quantitative validated food frequency questionnaire. Unconditional logistic regression models were used to estimate odds ratios (OR) and 95% confidence intervals (95%CI) for energy-adjusted DII (E-DII), and a score combining E-DII and NEAC. E-DII was associated with colorectal cancer risk (OR = 1.93, highest quartile versus lowest, 95%CI:1.60-2.32; p-trend: 0.10). The combined score of high E-DII scores and low antioxidant values were associated with colorectal cancer risk (OR = 1.48, highest quartile versus lowest, 95%CI: 1.26-1.74; p-trend: <0.001), but not breast cancer.     Macrodactilia : aportacion de un caso y consideraciones anatomopatologicas, clinicas y terapeuticas

    Dentro del grupo de procesos que cursan con alteraciones en la forma y tamaño de los dedos, la macrodactilia es una malformación congénita de causa desconocida de la que se calcula hay unos 150 casos puhlicados en la literatura. Este artículo presenta un caso clínico asistido en nuestro Departamento. La paciente presentó ya desde el nacimiento un gigantismo localizado en los dedos 2." y 3." de la mano izquierda. En la historia clínica no hay datos familiares ni personales que expliquen la anomalía. En el desarrollo posterior la hipertrofia digital se fue haciendo cada vez más ostensible. El estudio radiológico mostraba la afectación de falanges y metacarpianos. Tanto la angiografía como la exploración macroscópica eran normales, salvo el aumento del tamaño de todas las estructuras digitales. Con estos datos llegamos al diagnóstico de Macrodactilia verdadera de tipo progresivo

    The associations between physical fitness and cardiometabolic risk and body-size phenotypes in perimenopausal women

    Objective: To study the association between physical fitness and body-size phenotypes, and to test which aspects of physical fitness show the greatest independent association with cardiometabolic risk in perimenopausal women. Study design: This cross-sectional study involved 228 women aged 53 +/- 5 years from southern Spain. Main outcome measurements: Physical fitness was assessed by means of the Senior Fitness Test Battery (additionally including handgrip strength and timed up-and-go tests). Anthropometry, resting heart rate, blood pressure and plasma markers of lipid, glycaemic and inflammatory status were measured by standard procedures. The harmonized definition of the 'metabolically healthy but obese' (MHO) phenotype was employed to classify individuals. Results: The overall prevalence of the MHO phenotype was 13% but was 43% among the obese women. Apart from traditional markers, metabolically healthy non-obese women had lower levels of C-reactive protein than women with the other phenotypes (p <0.001), and levels of glycosylated haemoglobin were lower in MHO women than in metabolically abnormal non-obese women (overall p = 0.004). Most of the components of physical fitness differed with body-size phenotypes. The 6-min walk and the back-scratch tests presented the most robust differences (both p <0.001). Moreover, the women's performance on the back-scratch (beta = 0.32; p <0.001) and the 6-min walk (beta = 0.22; p = 0.003) tests was independently associated with the clustered cardiometabolic risk. The back-scratch test explained 10% of the variability (step 1, p <0.001), and the final model, which also included the 6-min walk test (step 2, p = 0.003), explained 14% of the variability. Conclusion: Low upper-body flexibility was the most important fitness indicator of cardiometabolic risk in perimenopausal women, but cardiorespiratory fitness also played an important role.     Sondeo FAM-1: primeros resultados del sondeo de investigación de la Falla de Alhama de Murcia, Cordillera Bética, España

    We present the preliminary results of the borehole FAM-1 a 175 m depth scientific drilling of the Alhama de Murcia fault. The borehole was drilled close to La Torrecilla rambla three km SW of Lorca where the fault zone shearing is more concentrated and it is dominated by well-developed clay rich fault gouge. To select the drilling point and to perform a prognosis of it, three trenches were excavated crossing the fault zone that allowed us to determine the detailed 3D structure of the fault zone. We have collected more than 100 m of unaltered high quality fault rock to be studied using mineralogical and microtectonic analysis, and geomechanical testing that will improve the knowledge of the influence of tectonic microfabric and mineralogy in the seismogenic behavior of the AMF. The borehole FAM-1 and the seismic monitoring borehole FamSis-1 constitute the first stage of a future geological-geophysical observatory for monitoring the activity of the AMF.Se presentan los resultados preliminares del sondeo de investigación FAM-1 de 175 m de profundidad en el que se ha perforado el tramo de fault gouge de la Falla de Alhama de Murcia (AMF) con mayor grado de exhumación. El sondeo se ha realizado en el sector de la rambla de La Torrecilla (3 km al SO de Lorca) donde la mayor parte del desplazamiento de la AMF se concentra en una única zona de cizalla de unos 130-150 m de espesor dominada por una fault gouge rica en arcillas. La cartografía y el registro de las trincheras realizadas para definir la prognosis del sondeo, y la información de éste han permitido determinar la estructura 3d de la zona de falla. Se ha podido extraer más de 100 m de roca de falla inalterada de muy alta calidad con el fin de ser utilizadas en análisis mineralógicos, microtectónicos y geomecánicos que permitirán mejorar el conocimiento de la influencia de la microfábrica tectónica y mineralógica en el comportamiento sismogénico de la AMF.
