    [18F]FDG PET/CT in Short-Term Complications of COVID-19: Metabolic Markers of Persistent Inflammation and Impaired Respiratory Function

    SARS-CoV-2 virus infects organs other than the lung, such as mediastinal lymph nodes, spleen, and liver, but, to date, metabolic imaging studies obtained in short-term follow-ups of patients hospitalized with severe COVID-19 infection are rare. Our objective was to evaluate the usefulness of [18F]FDG-PET/CT in the short-term follow-up of patients admitted for COVID-19 pneumonia and to explore the association of the findings with clinical prognostic markers. The prospective study included 20 patients with COVID-19 pneumonia (November 2020–March 2021). Clinical and laboratory test findings were gathered at admission, 48–72 h post-admission, and 2–3 months post-discharge, when [18F]FDG-PET/CT and respiratory function tests were performed. Lung volumes, spirometry, lung diffusion capacity for carbon monoxide (DLCO), and respiratory muscle strength were measured. Volumetric [18F]FDG-PET/CT results were correlated with laboratory and respiratory parameters. Eleven [18F]FDG-PET/CT (55%) were positive, with hypermetabolic mediastinal lymphadenopathy in 90.9%. Mediastinal lesion’s SUVpeak was correlated with white cells’ count. Eleven (55%) patients had impaired respiratory function, including reduced DLCO (35%). SUVpeak was correlated with %predicted-DLCO. TLG was negatively correlated with %predicted- DLCO and TLC. In the short-term follow-up of patients hospitalized for COVID-19 pneumonia, [18F]FDG-PET/CT findings revealed significant detectable inflammation in lungs and mediastinal lymph nodes that correlated with pulmonary function impairment in more than half of the patients

    Synthesis, crystal and molecular structure of methyl[(4-acetamidophenyl)sulfonyl]carbamate, precursor of herbicide Asulam

    The methyl[(4-aminophenyl)sulfonyl]carbamate is commercially known as Asulam. It is the most common trade name for a systemic and synergistic herbicide, which is incorporated by the plants through their foliage and roots, and is extensively used against weeds and brackens in the cultivation of gramineous, citric, banana and sugarcane, among others. The industrial procedure for obtaining this carbamic compound includes the base-catalyzed hydrolysis of the methyl[(4-acetamidophenyl)sulfonyl]carbamate. In this work a simple and conventional method for the synthesis of the Asulam intermediate and the characterization of its crystal and molecular structure utilizing X-Ray Diffraction and IR, NMR 1H and 13C spectroscopic techniques is presented. The synthetic precursor will be used as substrate of the hydrolytic enzyme in the development of a new biocatalytic preparation of the herbicide Asulam. Therefore, the knowledge of its structure will help not only to define the physical characteristics of its more stable conformation, but also will allow predicting the compatibility between the possible enzymes to be used and the substrate, according to the properties of their catalytic centers. The title compound was synthesized from 4-acetamidophenylsulfonamide and methylchloroformate in the presence of anhydrous K2CO3 and acetone, extracted with ethyl ether and recrystallized from methanol. The studied compound was obtained as crystalline solid in 62% yield. The crystals of intermediate were grown by slow evaporation from methanol solution. They are monoclinic, space group P21/n; a = 8.681(4), b = 8.174(3), c = 17.583(3) Å, ß= 92.40(1) º, Z = 4. The molecular packing shows a bidimensional hydrogen bond network

    Comparación de la dosis estimada en recto y vejiga con planificación de Braquiterapia 3D vs 2D: Estimated dose comparison in the rectum and bladder with 3D vs. 2D brachytherapy planning

    Cervical cancer is one of the main female neoplasia the treatment requires multidisciplinary management.Intracavity brachytherapy occupies a fundamental role. Brachytherapy represented an important improvement in the last years with the growth of the three-dimensional planning. But in many countries, two dimensional planning is still the only option of treatment. We seek to establish the benefit that is obtained with brachytherapy in three dimensions, in the protection to organs of risk. This is an observational, longitudinal cohort study; we extrapolated treatment schemes in three dimensions, to planning in two dimensions. Then turned to move this planning, to the tridimensional system again and we got doses- olume histograms of rectum and bladder. Finally, we determined the average of doses received in 2cc of rectum and bladder in three-dimensional planning and compared with those obtained in the two dimensional planning, using Student’s T test. The dose average in 2cc bladder in two-dimensional plan was 5.5 Gy. And in three-dimensional plan was 5.1 Gy (p=0.01). The dose average in 2cc rectum in two-dimensional plan was 2.8 Gy, and in three-dimensional plan was 3.0 Gy (p=0.06). This study gives an approximation to the dosimetry improvement obtained through the using of the three- dimensional planning, for the reduction of doses in the bladder.El cáncer de cuello uterino se ubica entre las principales neoplasia femeninas, su tratamiento requiere del manejo multidisciplinario, donde la braquiterapia ocupa un sitial fundamental. Está ha presentado una importante evolución en los últimos años con el surgimiento de la planificación en tres dimensiones. En muchas partes del mundo, la modalidad en dos dimensiones sigue siendo la única opción de tratamiento. Como objetivo buscamos establecer el beneficio que se obtiene con braquiterapia en tres dimensiones sobre la técnica convencional de braquiterapia, en la protección a los órganos de riesgo. Planteamos un estudio observacional de cohorte longitudinal, extrapolando planificaciones de tratamiento en tres dimensiones, a planificación en dos dimensiones. Posteriormente se volvió a trasladar esta planificación, al sistema en tres dimensiones y se obtuvo histogramas dosis volumen de recto y vejiga. Determinamos el promedio de dosis recibidas en 2 cc de recto y vejiga en la planificación tridimensional y comparamos con las obtenidos en la planificación en dos dimensiones, mediante la T de Student. El promedio de la planificación de las dosis en vejiga con dos dimensiones fue 5.5 Gy, y con tres dimensiones fue 5.1 Gy, p= 0.01. El promedio de la planificación de las dosis en recto con dos dimensiones fue 2.8 Gy; y con tres dimensiones fue 3.0 Gy p= 0.06. Este estudio nos da una aproximación al beneficio dosimétrico que se obtiene mediante el uso de la planificación tridimensional, para la reducción de dosis que llega a vejiga

    Towards the identification of a new taphonomic agent: An analysis of bone accumulations obtained from modern Egyptian vulture (Neophron percnopterus) nests

    This paper presents the results of a study of bones recovered in various current Egyptian vulture (Neophron percnopterus) nests in a Mediterranean region of the Iberian Peninsula. The Egyptian vulture, a diurnal, scavenging, rupicolous bird of prey, is one of four vulture species that currently inhabit the Iberian Peninsula. An analysis of the remains found in the nests confirms that it has a heterogeneous diet that includes remains from human activities (butchery and food production) and the carcasses of dead animals, although it is possible that they also prey on small-sized taxa. The taphonomic study determines these birds" capability of transporting, accumulating and altering bone remains. Some of the elements show marks caused by beak and/or claw impacts brought about primarily during feeding, which have characteristic typologies. Despite the fact that this is not a bone-eating vulture, it can also be seen that some bones are swallowed. The characteristics of the bone set studied here are important for establishing the origin of bone accumulations on archaeological sites

    Patient preferences and treatment safety for uncomplicated vulvovaginal candidiasis in primary health care

    <p>Abstract</p> <p>Background</p> <p>Vaginitis is a common complaint in primary care. In uncomplicated candidal vaginitis, there are no differences in effectiveness between oral or vaginal treatment. Some studies describe that the preferred treatment is the oral one, but a Cochrane's review points out inconsistencies associated with the report of the preferred way that limit the use of such data. Risk factors associated with recurrent vulvovaginal candidiasis still remain controversial.</p> <p>Methods/Design</p> <p>This work describes a protocol of a multicentric prospective observational study with one year follow up, to describe the women's reasons and preferences to choose the way of administration (oral vs topical) in the treatment of not complicated candidal vaginitis. The number of women required is 765, they are chosen by consecutive sampling. All of whom are aged 16 and over with vaginal discharge and/or vaginal pruritus, diagnosed with not complicated vulvovaginitis in Primary Care in Madrid.</p> <p>The main outcome variable is the preferences of the patients in treatment choice; secondary outcome variables are time to symptoms relief and adverse reactions and the frequency of recurrent vulvovaginitis and the risk factors. In the statistical analysis, for the main objective will be descriptive for each of the variables, bivariant analysis and multivariate analysis (logistic regression).. The dependent variable being the type of treatment chosen (oral or topical) and the independent, the variables that after bivariant analysis, have been associated to the treatment preference.</p> <p>Discussion</p> <p>Clinical decisions, recommendations, and practice guidelines must not only attend to the best available evidence, but also to the values and preferences of the informed patient.</p
