Perceptions of Heat-Susceptibility in Older Persons: Barriers to Adaptation
The increase in the frequency of very hot weather that is a predicted consequence of climate change poses an emerging threat to public health. Extreme heat can be harmful to the health of older persons who are known to be amongst the most vulnerable in the community. This study aimed to investigate factors influencing the ability of older persons to adapt to hot conditions, and barriers to adaptation. A qualitative study was conducted in Adelaide, Australia, involving focus groups and interviews with stakeholders including key personnel involved in aged care, community services, government sectors, emergency services and policy making. Findings revealed a broad range of factors that underpin the heat-susceptibility of the aged. These were categorized into four broad themes relating to: physiology and an age-related decline in health; socioeconomic factors, particularly those influencing air conditioning use; psychological issues including fears and anxieties about extreme heat; and adaptive strategies that could be identified as both enablers and barriers. As a consequence, the ability and willingness to undertake behavior change during heatwaves can therefore be affected in older persons. Additionally, understanding the control panels on modern air conditioners can present challenges for the aged. Improving heat-health knowledge and addressing the social and economic concerns of the older population will assist in minimizing heat-related morbidity and mortality in a warming climate
The Effect of Heat Waves on Mental Health in a Temperate Australian City
Objective: The goal of this study was to identify mental, behavioral, and cognitive disorders that may be triggered or exacerbated during heat waves, predisposing individuals to heat-related morbidity and mortality. Design: Using health outcome data from Adelaide, South Australia, for 1993â2006, we estimated the effect of heat waves on hospital admissions and mortalities attributed to mental, behavioral, and cognitive disorders. We analyzed data using Poisson regression accounting for overdispersion and controlling for season and long-term trend, and we performed threshold analysis using hockey stick regression. Results: Above a threshold of 26.7°C, we observed a positive association between ambient temperature and hospital admissions for mental and behavioral disorders. Compared with nonâheat-wave periods, hospital admissions increased by 7.3% during heat waves. Specific illnesses for which admissions increased included organic illnesses, including symptomatic mental disorders ; dementia ; mood (affective) disorders ; neurotic, stress related, and somatoform disorders ; disorders of psychological development ; and senility. Mortalities attributed to mental and behavioral disorders increased during heat waves in the 65- to 74-year age group and in persons with schizophrenia, schizotypal, and delusional disorders. Dementia deaths increased in those up to 65 years of age. Conclusion: Our results suggest that episodes of extreme heat pose a salient risk to the health and well-being of the mentally ill. Kultivierung transplantierbarer ZellverbÀnde aus cornealem Endothel
Die In-vitro-Kultivierung von cornealem Endothel, einer funktionalen, beim Menschen nicht regenerierbaren Schicht in der Hornhaut des Auges, eröffnet weitreichende Möglichkeiten zum Zell- und Gewebeersatz. Dieser Artikel beschreibt aktuelle und kĂŒnftige Optionen fĂŒr zellbasierte TherapieansĂ€tze sowie die Bedeutung unbegrenzt proliferationsfĂ€higer (immortalisierter) Zellpopulationen als Modellsystem fĂŒr die Entwicklung neuartiger Methoden. In diesem Zusammenhang werden schaltbare ZellkulturtrĂ€ger als Möglichkeit zur schonenden Gewinnung transplantierbarer âcell sheetsâ vorgestellt. DarĂŒber hinaus wird die serumfreie Kultivierung als wichtige Voraussetzung fĂŒr eine Anwendung am Menschen diskutiert.The in vitro cultivation of corneal endothelium â a functional, non-regenerable layer of the human cornea â is a promising approach for cell and tissue replacement. This paper introduces options for cell-based therapies and points out the importance of immortalised cell populations as a model system to develop tissue engineering strategies. In particular, the use of stimuli-responsive cell culture carriers for the gentle harvesting of âcell sheetsâ is described. Thermo-responsive cell culture carriers based on poly(vinyl methyl ether) - The effect of biomolecular ligands to balance cell adhesion and stimulated detachment
Two established material systems for thermally stimulated detachment of adherent cells were combined in a cross-linked polymer blend to merge favorable properties. Through this approach poly(N-isopropylacrylamide) (PNiPAAm) with its superior switching characteristic was paired with a poly(vinyl methyl ether)-based composition that allows adjusting physico-chemical and biomolecular properties in a wide range. Beyond pure PNiPAAm, the proposed thermo-responsive coating provides thickness, stiffness and swelling behavior, as well as an apposite density of reactive sites for biomolecular functionalization, as effective tuning parameters to meet specific requirements of a particular cell type regarding initial adhesion and ease of detachment. To illustrate the strength of this approach, the novel cell culture carrier was applied to generate transplantable sheets of human corneal endothelial cells (HCEC). Sheets were grown, detached, and transferred onto planar targets. Cell morphology, viability and functionality were analyzed by immunocytochemistry and determination of transepithelial electrical resistance (TEER) before and after sheet detachment and transfer. HCEC layers showed regular morphology with appropriate TEER. Cells were positive for function-associated marker proteins ZO-1, Na+/K+-ATPase, and paxillin, and extracellular matrix proteins fibronectin, laminin and collagen type IV before and after transfer. Sheet detachment and transfer did not impair cell viability. Subsequently, a potential application in ophthalmology was demonstrated by transplantation onto de-endothelialized porcine corneas in vitro. The novel thermo-responsive cell culture carrier facilitates the generation and transfer of functional HCEC sheets. This paves the way to generate tissue engineered human corneal endothelium as an alternative transplant source for endothelial keratoplasty
Risk factors, health effects and behaviour in older people during extreme heat: A survey in South Australia
Older people had a high incidence of hospitalisation during the 2009 heat wave in South Australia. We sought to explore resilience, behaviours, health risk factors and health outcomes during recent heat waves for a representative sample of independently living residents. A telephone survey of 499 people aged 65 years and over was conducted, and included both metropolitan and rural residences. A variety of adaptive strategies were reported, with 75% maintaining regular appointments and activities during the heat. However, 74% took medication for chronic disease and 25% assessed their health status to be fair to poor. In a multivariate model, factors associated with heat health outcomes included medication for mental health, heart failure, diabetes or respiratory health, reporting a reduced health status, use of mobility aids and being female. Compared with younger participants, those over 75 had more check-up calls and visits by family, friends and neighbours. However, confidence to call on support was associated with indicators of social isolation. Impact of two recent extreme heat episodes on morbidity and mortality in Adelaide, South Australia: a case-series analysis
Extent: 9p.BACKGROUND: Extreme heatwaves occurred in Adelaide, South Australia, in the summers of 2008 and 2009. Both heatwaves were unique in terms of their duration (15 days and 13 days respectively), and the 2009 heatwave was also remarkable in its intensity with a maximum temperature reaching 45.7°C. It is of interest to compare the health impacts of these two unprecedented heatwaves with those of previous heatwaves in Adelaide. METHODS: Using case-series analysis, daily morbidity and mortality rates during heatwaves (â„35°C for three or more days) occurring in 2008 and 2009 and previous heatwaves occurring between 1993 and 2008 were compared with rates during all non-heatwave days (1 October to 31 March). Incidence rate ratios (IRRs) were established for ambulance call-outs, hospital admissions, emergency department presentations and mortality. Dose response effects of heatwave duration and intensity were examined. RESULTS: Ambulance call-outs during the extreme 2008 and 2009 events were increased by 10% and 16% respectively compared to 4.4% during previous heatwaves. Overall increases in hospital and emergency settings were marginal, except for emergency department presentations in 2008, but increases in specific health categories were observed. Renal morbidity in the elderly was increased during both heatwaves. During the 2009 heatwave, direct heat-related admissions increased up to 14-fold compared to a three-fold increase seen during the 2008 event and during previous heatwaves. In 2009, marked increases in ischaemic heart disease were seen in the 15-64 year age group. Only the 2009 heatwave was associated with considerable increases in total mortality that particularly affected the 15-64 year age group (1.37; 95% CI, 1.09, 1.71), while older age groups were unaffected. Significant dose-response relationships were observed for heatwave duration (ambulance, hospital and emergency setting) and intensity (ambulance and mortality). CONCLUSIONS: While only incremental increases in morbidity and mortality above previous findings occurred in 2008, health impacts of the 2009 heatwave stand out. These findings send a signal that the intense and long 2009 heatwave may have exceeded the capacity of the population to cope. It is important that risk factors contributing to the adverse health outcomes are investigated to further improve preventive strategies.Monika Nitschke, Graeme R. Tucker, Alana L. Prevalence and incidence of iron deficiency in European community-dwelling older adults : An observational analysis of the DO-HEALTH trial
Background and aim
Iron deficiency is associated with increased morbidity and mortality in older adults. However, data on its prevalence and incidence among older adults is limited. The aim of this study was to investigate the prevalence and incidence of iron deficiency in European community-dwelling older adults agedââ„â70 years.
Methods
Secondary analysis of the DO-HEALTH trial, a 3-year clinical trial including 2157 community-dwelling adults agedââ„â70 years from Austria, France, Germany, Portugal and Switzerland. Iron deficiency was defined as soluble transferrin receptor (sTfR)â>â28.1 nmol/L. Prevalence and incidence rate (IR) of iron deficiency per 100 person-years were examined overall and stratified by sex, age group, and country. Sensitivity analysis for three commonly used definitions of iron deficiency (ferritinââ1.5) were also performed.
Results
Out of 2157 participants, 2141 had sTfR measured at baseline (mean age 74.9 years; 61.5% women). The prevalence of iron deficiency at baseline was 26.8%, and did not differ by sex, but by age (35.6% in age groupââ„â80, 29.3% in age group 75â79, 23.2% in age group 70â74); Pââ1.5. Occurrences of iron deficiency were observed with IR per 100 person-years of 9.2 (95% CI 8.3â10.1) and did not significantly differ by sex or age group. The highest IR per 100 person-years was observed in Austria (20.8, 95% CI 16.1â26.9), the lowest in Germany (6.1, 95% CI 4.7â8.0). Regarding the other definitions of iron deficiency, the IR per 100 person-years was 4.5 (95% CI 4.0â4.9) for ferritinââ1.5.
Conclusions
Iron deficiency is frequent among relatively healthy European older adults, with people agedââ„â80 years and residence in Austria and Portugal associated with the highest risk

Gegenstand der vorliegenden retrospektiven Untersuchung war es, sowohl die Eutergesundheit anhand von Milchleistung und Zellzahl, als auch das Verbleiben in der Herde von KĂŒhen nach erworbenen Milchabflussstörungen zu ermitteln. Die Ergebnisse wurden mit denen speziell angepaarter Kontrolltiere verglichen, um eine langfristige Prognose fĂŒr Patientinnen stellen zu können. Rund 98% der Milchabflussstörungen resultierten aus gedeckten Zitzenverletzungen, die mit Hilfe der Endoskopie (nach MEDL) festgestellt und behandelt worden waren.Im Melkjahr der Feststellung der Milchabflussstörung und im nachfolgenden Melkjahr erbrachten KĂŒhe mit Milchabflussstörung am monatlichen PrĂŒftag und in der gesamten Laktationsperiode die gleiche Milchleistung wie die Kontrolltiere. Das Vorhandensein einer Milchabflussstörung erhöhte in beiden untersuchten Melkjahren die monatliche Zellzahl/ml in der Sammelmilch der Patientinnen am PrĂŒftag. Dem Ă€hnlich war bei KĂŒhen mit Milchabflussstörungen in beiden Melkjahren das Risiko gestörter Eutergesundheit am PrĂŒftag (> 100.000 Zellen/ml) als auch das Risiko, die Norm der Rohmilchannahme am PrĂŒftag nicht zu erfĂŒllen (> 400.000 Zellen/ml) erhöht. FĂŒr Tiere mit Milchabflussstörung war das Risiko, die Herde zu verlassen, gleich groĂ wie fĂŒr Herdengenossinnen. AuĂerdem war der absolute Verbleib der Tiere in der Herde in beiden Gruppen gleich lang.The aim of the study was to evaluate milk quality by observations of milk yield and somatic cell count (SCC) and to follow the fate of dairy cows after milk flow disorders. The results were compared with those of matched herdmates to ensure a long-term prognosis for patients. About 98% of all milk flow disorders were seen as a consequence of covered lesions with most of them being ruptures of the teat canal skin ("deep stenoses"). Teat stenoses were diagnosed and treated using classical endoscopy (by MEDL). In the lactation the milk flow disorder was diagnosed and in the subsequent lactation, cows with milk flow disorders had the same test day milk yields and the same lactational milk yields as the control cows. Obstructed teats increased test day SCC of the patients in both lactations observed. Just so the risk of disordered udder health on test day (> 100,000 cells/ml) and the risk of violating the milk shipping regulations on test day (> 400,000 cells/ml) increased with teat stenoses in both lactations. Cows with milkflow disorders showed no higher risk of removal of the herd than herdmates, and absolute survival time was equal in both groups
