    Three-Dimensional Bioprinting Materials with Potential Application in Preprosthetic Surgery

    Current methods in handling maxillofacial defects are not robust and are highly dependent on the surgeon’s skills and the inherent potential in the patients’ bodies for regenerating lost tissues. Employing custom-designed 3D printed scaffolds that securely and effectively reconstruct the defects by using tissue engineering and regenerative medicine techniques can revolutionize preprosthetic surgeries. Various polymers, ceramics, natural and synthetic bioplastics, proteins, biomolecules, living cells, and growth factors as well as their hybrid structures can be used in 3D printing of scaffolds, which are still under development by scientists. These scaffolds not only are beneficial due to their patient-specific design, but also may be able to prevent micromobility, make tension free soft tissue closure, and improve vascularity. In this manuscript, a review of materials employed in 3D bioprinting including bioceramics, biopolymers, composites, and metals is conducted. A discussion of the relevance of 3D bioprinting using these materials for craniofacial interventions is included as well as their potential to create analogs to craniofacial tissues, their benefits, limitations, and their application

    Agreement between Lenstar and Pentacam in Keratometry and Anterior Chamber Depth Measurements

    Purpose:  To compare keratometry and anterior chamber depth (ACD) measurements performed using Lenstar LS 900 (Haag-Streit AG, Switzerland) and the Pentacam (Oculus, Weltzar, Germany) devices in healthy eyes.Method: Sixty eyes of 30 healthy volunteers were included in this prospective study. Keratometry and ACD measurements were obtained using Lenstar, followed by Pentacam on the same day.  The readings of the two instruments were compared to evaluate their agreement.Results: The mean age of participants was 40.01 ± 12.29 years (Range 10 to 65years).  The mean ACD was 2.762 ± 0.281 millimeters when measured using Lenstar and 2.801 ± 0.273 millimeters when measured with Pentacam (P = 0.03). The average mean keratometry was 44.45 ± 1.65 diopter when measured with Lenstar and 44.16 ± 1.55 diopter when measured using  Pentacam (P < 0.001). The Bland-Altman plots demonstrated a wide range of inter-device differences in mean keratectomy and also ACD measurements between the two devices.Conclusion: Our findings suggest that the ACD and keratometry measurements obtained using the Lenstar and Pentacam devices might not be interchangeable.Key words: Anterior Chamber; Cornea; Pentacam; Lenstar

    A Comparative Analysis of Clinical Characteristics and Laboratory Findings of COVID-19 between Intensive Care Unit and Non-Intensive Care Unit Pediatric Patients: A Multicenter, Retrospective, Observational Study from Iranian Network for Research in Viral

    Introduction: To date, little is known about the clinical features of pediatric COVID-19 patients admitted to intensive care units (ICUs). Objective: Herein, we aimed to describe the differences in demographic characteristics, laboratory findings, clinical presentations, and outcomes of Iranian pediatric COVID-19 patients admitted to ICU versus those in non-ICU settings. Methods: This multicenter investigation involved 15 general and pediatrics hospitals and included cases with confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection based on positive real-time reverse transcription polymerase chain reaction (RT-PCR) admitted to these centers between March and May 2020, during the initial peak of the COVID-19 pandemic in Iran. Results: Overall, 166 patients were included, 61 (36.7%) of whom required ICU admission. The highest number of admitted cases to ICU were in the age group of 1–5 years old. Malignancy and heart diseases were the most frequent underlying conditions. Dyspnea was the major symptom for ICU-admitted patients. There were significant decreases in PH, HCO3 and base excess, as well as increases in creatinine, creatine phosphokinase (CPK), lactate dehydrogenase (LDH), and potassium levels between ICU-admitted and non-ICU patients. Acute respiratory distress syndrome (ARDS), shock, and acute cardiac injury were the most common features among ICU-admitted patients. The mortality rate in the ICU-admitted patients was substantially higher than non-ICU cases (45.9% vs. 1.9%, respectively; p<0.001). Conclusions: Underlying diseases were the major risk factors for the increased ICU admissions and mortality rates in pediatric COVID-19 patients. There were few paraclinical parameters that could differentiate between pediatrics in terms of prognosis and serious outcomes of COVID-19. Healthcare providers should consider children as a high-risk group, especially those with underlying medical conditions

    Tracking development assistance for health and for COVID-19: a review of development assistance, government, out-of-pocket, and other private spending on health for 204 countries and territories, 1990-2050

    Background The rapid spread of COVID-19 renewed the focus on how health systems across the globe are financed, especially during public health emergencies. Development assistance is an important source of health financing in many low-income countries, yet little is known about how much of this funding was disbursed for COVID-19. We aimed to put development assistance for health for COVID-19 in the context of broader trends in global health financing, and to estimate total health spending from 1995 to 2050 and development assistance for COVID-19 in 2020. Methods We estimated domestic health spending and development assistance for health to generate total health-sector spending estimates for 204 countries and territories. We leveraged data from the WHO Global Health Expenditure Database to produce estimates of domestic health spending. To generate estimates for development assistance for health, we relied on project-level disbursement data from the major international development agencies' online databases and annual financial statements and reports for information on income sources. To adjust our estimates for 2020 to include disbursements related to COVID-19, we extracted project data on commitments and disbursements from a broader set of databases (because not all of the data sources used to estimate the historical series extend to 2020), including the UN Office of Humanitarian Assistance Financial Tracking Service and the International Aid Transparency Initiative. We reported all the historic and future spending estimates in inflation-adjusted 2020 US,2020US, 2020 US per capita, purchasing-power parity-adjusted USpercapita,andasaproportionofgrossdomesticproduct.Weusedvariousmodelstogeneratefuturehealthspendingto2050.FindingsIn2019,healthspendinggloballyreached per capita, and as a proportion of gross domestic product. We used various models to generate future health spending to 2050. Findings In 2019, health spending globally reached 8. 8 trillion (95% uncertainty interval UI] 8.7-8.8) or 1132(11191143)perperson.Spendingonhealthvariedwithinandacrossincomegroupsandgeographicalregions.Ofthistotal,1132 (1119-1143) per person. Spending on health varied within and across income groups and geographical regions. Of this total, 40.4 billion (0.5%, 95% UI 0.5-0.5) was development assistance for health provided to low-income and middle-income countries, which made up 24.6% (UI 24.0-25.1) of total spending in low-income countries. We estimate that 54.8billionindevelopmentassistanceforhealthwasdisbursedin2020.Ofthis,54.8 billion in development assistance for health was disbursed in 2020. Of this, 13.7 billion was targeted toward the COVID-19 health response. 12.3billionwasnewlycommittedand12.3 billion was newly committed and 1.4 billion was repurposed from existing health projects. 3.1billion(22.43.1 billion (22.4%) of the funds focused on country-level coordination and 2.4 billion (17.9%) was for supply chain and logistics. Only 714.4million(7.7714.4 million (7.7%) of COVID-19 development assistance for health went to Latin America, despite this region reporting 34.3% of total recorded COVID-19 deaths in low-income or middle-income countries in 2020. Spending on health is expected to rise to 1519 (1448-1591) per person in 2050, although spending across countries is expected to remain varied. Interpretation Global health spending is expected to continue to grow, but remain unequally distributed between countries. We estimate that development organisations substantially increased the amount of development assistance for health provided in 2020. Continued efforts are needed to raise sufficient resources to mitigate the pandemic for the most vulnerable, and to help curtail the pandemic for all. Copyright (C) 2021 The Author(s).     Les correlats neuronaux des états socio-émotionnels chez le macaque

    A cornerstone of a successful social life is the ability to correctly predict others’ actions and empathically perceive their emotional states. Studies on primates’ social interaction have shown that thanks to their keen cognitive abilities monkeys are able to deduce what others can hear or see, and to predict others’ emotions and intentions. It has been shown that primates are able to display different degrees of prosocial behavior, from cooperation to even altruism and empathically driven behavior. Studies using fMRI techniques inhumans have identified the anterior insula (AI) as a key brain region in the processing of empathy. More precisely, this region emerged as the overlapping area activated for both experienced and observed pain,leading to the idea that empathy for pain may involve a mirror-matching model of the affective and sensory features of others' pain. However, the neuronal basis of this process has yet to be uncovered. In an attempt toextend and to investigate the role of the AI in the process of empathy we have recorded single cell activity inthe AI of two monkeys while they were engaged in a social task where based on the performed trials positiveor negative reinforcements could be delivered to self, another monkey, or nobody. Behavioral results showed that monkeys take into account the welfare of their partners even when this has no impact on their ownwelfare. Our neuronal findings report that distinct population of neurons respond differentially to outcomesfor self and other, and to appetitive and aversive outcomes. Interestingly the neuronal population responding to the aversive outcome showed mainly three profiles of activity: neuronal representation of conspecifics’unpleasant experience, neuronal representation of own unpleasant experience and a minority of neurons showing mirroring properties between self and other. Thus, our results suggest a neuronal model of empathy that accounts for the distinctive features between feeling and empathizingUn pilier d'une vie sociale fructueuse est la capacité de prédire correctement les actions des autres et de percevoir leurs états émotionnels. Des études d'interaction sociale chez les primates ont montré qu'ils sont capables de déduire ce que les autres peuvent entendre ou voir, et de prédire leurs émotions et intentions. Il a été montré qu'ils peuvent manifester différents degrés de comportements prosociaux, allant de la coopération jusqu'à des comportements altruistes et empathiques. Des études d'imageries fonctionnelles chez l'homme ont identifié l'insula antérieur (AI) comme une région cérébrale clé dans le traitement de l'empathie.Spécifiquement, cette région apparait comme l'aire intégratrice des activités liées à la douleur ressentie et observée, suggérant que l'empathie pourrait impliquer un modèle « miroir » des propriétés affectives et sensorielles de la douleur d'autrui. Cependant, les bases neuronales de ce processus n'ont pas encore été découvertes. Dans le but d'examiner le rôle de l'AI dans le traitement de l'empathie, nous avons enregistré l'activité des neurones dans l'AI de deux singes pendant qu'ils sont engagés dans une tâche sociale leur permettant de délivrer un stimulus aversif ou appétitif à leur partenaire, à lui-même ou à personne. Les résultats comportementaux ont montré que les singes prennent en compte le bien-être de leur partenaire. Les données neuronales rapportent différentes populations neuronales répondant aux stimuli aversif ou appétitif et ceux délivrés à soi ou à autrui. Notamment, la population neuronale répondant au stimulus aversif a montré trois profils d'activité : une représentation neuronale de l'expérience désagréable du partenaire, une représentation neuronale de sa propre sensation désagréable et une minorité de neurones montrant des propriétés miroirs entre soi et autrui. Nos résultats suggèrent un modèle neuronal de l'empathie représentant des propriétés distinctes entre l'expérience vécue et observé

    Dental Applications of Natural-Origin Polymers in Hard and Soft Tissue Engineering

    Clinical advances in the treatment of dentoalveolar defects continue to evolve with the introduction of new innovations in regenerative medicine and tissue bioengineering. Recent developments in tissue engineering are aimed at safely and effectively regenerating a damaged or necrotic area by replenishing its cells and increasing surrounding gene expression. Various techniques have successfully given rise to porous scaffolds being used by clinicians to treat the defect and initiate the repair process. Tissue reconstruction using bioengineered scaffolds is advantageous over traditional autografting, since it prevents the instigation of pain and donor site morbidity while ultimately creating both the environment and machinery needed to induce cell proliferation, migration, and reattachment within the affected area. This review article aims to describe and review the available literature regarding the regenerative capacity of natural polymers used for the treatment of dentoalveolar defects. The repair mechanisms, advantages of protein and polysaccharide derivatives, and the potential of stem cell therapy are discussed

    Efficacy of alum for treatment of recurrent aphthous stomatitis

    Background: Recurrent aphthous stomatitis (RAS) is the most common painful ulcers of oral mucosal which can cause many sufferings. Treatment of RAS often includes administration of corticosteroids, analgesics and regulators of the immune system. However, considering the side effects of these medications, even their topical application must be done with caution. Alum is used in traditional medicine for treatment of oral ulcers without significant side effect. This study sought to assess the effect of topical application of alum on aphthous ulcers. Methods: This clinical randomized double-blind placebo-controlled study was conducted on 50 females aged 21 to 27 years. Mucosal adhesive patches were prepared in two forms of basic and 7% alum-containing patches. Subjects in two groups of case and control randomly received the mucosal adhesive patches containing alum and the basic patches, respectively three times in five days. Duration of recovery, changes in size of lesion and severity of pain were recorded. Data were entered into SPSS Version 16 and analyzed using t-test. Results: The average period of full recovery was&nbsp; 7.52 days in the case and 12.2 days in the control groups; which was significantly different (p<0.001). Size of wound and severity of pain were significantly lower at one, three and five days posttreatment compared to baseline values before treatment in the case group (p<0.001) and the difference in this regard between the case and control groups was statistically significant. Conclusion: Alum can significantly decrease the size of aphthous lesions, severity of pain and expedite the recovery of patients with RAS

    Chemical composition, cholinesterase, and α-glucosidase inhibitory activity of the essential oils of some Iranian native Salvia species

    Abstract Background The plants from Salvia genus contain widely distributed species which have been used in folk medicine as well as pharmaceutical and food industries. Methods The chemical composition of 12 native Iranian Salvia species (14 plants) was identified using gas chromatography-mass spectrometry (GC–MS). Also, the inhibitory activity of all essential oils (EOs) was evaluated toward α-glucosidase and two types of cholinesterase (ChE) using spectrophotometric methods. The in vitro α-glucosidase inhibition assay was performed by the determination of p-nitrophenol (pNP) obtained from the enzymatic dissociation of p-nitrophenol-α-D-glucopyranoside (pNPG) as the substrate. In vitro ChE inhibitory assay was conducted based on the modified Ellman’s procedure using the measurement of 5-thio-2-nitrobenzoic acid produced from the hydrolysis of thiocholine derivatives as the substrate, in the presence of acetylcholinesterase (AChE) and butyrylcholinesterase (BChE). Results Totally, 139 compounds were detected and caryophyllene oxide and trans-β-caryophyllene were the most abundant compounds in all EOs. The yield of EOs extracted from the plants were also calculated in the range of 0.06 to 0.96% w/w. Herein, α-glucosidase inhibitory activity of 8 EOs was reported for the first time and among all, S. spinosa L. was found to be the most potent inhibitor (90.5 inhibition at 500 μg/mL). Also, the ChE inhibitory activity of 8 species was reported for the first time and our results showed that the BChE inhibitory effect of all EOs was more potent than that of AChE. The ChE inhibition assay indicated that S. mirzayanii Rech.f. & Esfand. collected from Shiraz was the most potent inhibitor (72.68% and 40.6% at the concentration of 500 μg/mL, toward AChE and BChE, respectively). Conclusions It seems that native Salvia species of Iran could be considered in the development of anti-diabetic and anti-Alzheimer's disease supplements

    Evaluation of Salivary Nitric Oxide and Epidermal Growth Factor in Diabetic Patients and Healthy Group

    Introduction: Nitric oxide (NO) and epidermal growth factor (EGF) play an important role in biologic systems. The aim of the present study was to evaluate salivary NO and EGF levels changes in type I and II diabetes mellitus comparing to the control group. Materials & Methods: In this cross-sectional study, five ml, saliva of 20 patients with type 1 diabetes mellitus and five ml saliva of 20 patients with type 2 diabetes mellitus attended to Hamadan diabetes research center as well as 20 healthy individuals matched according to age and sex, were collected. NO and EGF were assessed via Griess reaction and Immunoassay methods respectively. Data were analyzed by t-test and Mann-Whitney test.Results: Compared to the control group, the level of NO was increased in patients with type I diabetes (P=0.037), while it did not significantly increase in type II diabetes (P=0.058). The level of EGF in diabetic patients was significantly higher than the control group. There was no significant difference between the salivary level of EGF and NO of patient with type 1 and type 2 mellitus diabetes (P>0.05). The correlation coefficient between NO and EGF levels in type II diabetic patients was -0.278 (P=0.0235). The level of NO and EGF was significantly related to fasting blood sugar and HbA1c (P=0.001).Conclusion: The level of salivary NO in type I diabetes and EGF in type I and II diabetes was higher compared to those of healthy individuals and was related to the severity of the disease
