    A mobil kommunikáció, ezen belül a mobil Internet az agrárgazdaság számá-ra a használat hely és idÅ‘ függetlensége miatt fontos lehetÅ‘séget, gazdasági elÅ‘-nyöket, a vállalkozások, szervezetek számára hatékonyabb működést jelenthet. Széleskörű elterjedése, innovációs hatása és elÅ‘nyei gazdaságosan csak a technológia és a szolgáltatások hatásrendszerének figyelembevételével érhetÅ‘ el. A technológiai, társadalmi és gazdasági komplex hatásrendszer befolyásolja a gazdasági alkalmazások terjedését. Az alkalmazható eszközök köre folyamatosan bÅ‘vül, fejlÅ‘dik. Már ma is sok készüléktípus áll rendelkezésünkre: mobiltelefon, smartphone, PDA, laptop, TabletPC. A kommunikációs technológiák és szabványok körébÅ‘l kiemelhetÅ‘ a 3G és a WiFi szabvány. Társadalmi szempontból négy fÅ‘ csoport különíthetÅ‘ el: a mobilkészülék gyártók, a vállalkozások, a fogyasztók és a dolgozók. Az internettechnológia és a hálózat ma már sok területen az üzleti folyamatok alapvetÅ‘ kommunikációs eszközévé vált. A mobil eszközökkel való Internet szolgáltatások elérése pedig tovább növeli a lehetÅ‘ségeket. Az üzleti folyamat oldaláról vizsgálva a költségek, elÅ‘nyök, hátrányok markánsan megjelennek a különbözÅ‘ alkalmazási területeken. Napjainkban már a mezÅ‘gazdaságban és élelmiszeriparban, illetve az ágazat különbözÅ‘ szegmenseiben és feladataiban, mint például a szaktanácsadásban, precíziós gazdálkodásban, logisztikában, jól használható külföldi és hazai alkalmazások találhatók. Megállapítható, hogy a mobil internet nemzetközi és hazai fejlÅ‘dési trendjei, az Európai Unió kutatási és fejlesztési keretprogramjai a mobil szolgáltatások és alkalmazások széleskörű elterjedését segítik. A hazai mobilpiac elmúlt évekbeli gyors fejlÅ‘dése alapot adhat az új, nagysebességű mobil internet alkalmazások hasonló gyorsütemű elterjedésének. E rendszerek és szolgáltatások hozzájárulhatnak az agrárgazdaság, a vállalkozások, a vidék fejlÅ‘déséhez, jelentÅ‘sen támogathatják a termelési, kereskedelmi, szolgáltatási, termékkövetési feladatokat. A 2007-2014 idÅ‘szakra vonatkozó Nemzeti Fejlesztési Terv és Operatív Programok tervezésénél célszerű elÅ‘nyben részesíteni az EU által is preferált mobil technológiák és alkalmazások fejlesztését. However, their wide spread economic use, their innovative effects and advantages can be fully achieved only if the background to the technology and service is appreciated. The complex technological, social and economic consequences affect the spread of economical applications. The choice of applicable equipment is continually increasing. Numerous types of instruments and equipment are available in these days such as mobile phones, smart phones, PDA, laptop, TabletPC and recently developed communication technologies and standards include 3G and WiFi. From the social point of view the following four groups of people play a role in this business: mobile phone manufacturers, enterprises, consumers and employees. Internet technology and network has become an essential communication tools in business life. The use of mobile equipment for linking to internet further widens its application. From the point of view of business life costs, advantages and disadvantages are apparent in various fields of applications. Numerous highly useful home and foreign applications are available nowadays for the agriculture and food industry, for their various segments and problem areas, such as consultancies, precision farming and logistics. Domestic and international innovations, research and development programs sponsored and coordinated by the EU advance a great deal the wider use of mobile internet services and applications. Rapid development of the domestic mobile market may provide the foundations for the rapid spread of high speed broadband internet services. These technologies and services may contribute to the development of farms, enterprises and the countryside and significantly support tasks in production, commerce, services and product tracing.     Walley School Community Arts Center Feasibility Study

    The Walley School community arts center initiative began in the fall of 2011 as a joint project between the Town of Bristol and Roger Williams University’s School of Architecture. Shortly thereafter, the Gabelli School of Business was asked to development a business case for the project. Four students in the course “Management 439: Business Planning” took on the Walley School as their team project for the semester. The business case study was developed in conjunction with the Town of Bristol, the initiative’s steering committee, Roger Williams University’s Community Partnerships Center, the School of Architecture and the School of Construction Management. After a year of student work and public workshops, the conclusion has been made that it would be programmatically, architecturally and financially feasible to reopen the Walley School as a community arts and education facility for the Town of Bristol

    Vaginal Repair of Cystocele with Anterior Wall Mesh via Transobturator Route: Efficacy and Complications with Up to 3-Year Followup

    Study Objective. The objective of this study was to report on the safety and efficacy of cystocele repair with anterior wall mesh placed via a transobturator route (Perigee system, AMS, Minnetonka, MN). Design. Single center retrospective study. Setting. Single center hospital setting and Urogynecology practice in the United States. Patients. 77 women presenting with symptomatic anterior wall prolapse. Intervention. Repair of cystocele with an anterior wall Type I soft-polypropylene mesh placed via a transobturator approach. Concomitant procedures in other compartment were also completed as indicated. Measurements and Main Results. 77 women underwent the Perigee procedure at our institution over a 2-year period. The mesh was attached to the pelvic sidewalls at the level of the bladder neck and near the ischial spine apically with needles passed through the groins and obturator space. Mean follow-up was 18.2 months (range 3–36 months). Objective cure rate was 93%. Subjectively only two patients have had recurrent symptoms of prolapse, and only 1 of these has required repeat surgery for cystocele. Mesh exposure vaginally occurred in 5 patients (6.5%); however all were treated with estrogen and/or local excision of exposed mesh and had no further sequelae. There were no incidences of chronic pain, infection, or abscess, and no patient required complete mesh removal for infection, pain, or extrusion. Conclusion. In select patients with anterior wall prolapse, repair with mesh augmentation via the transobturator route is a safe and effective procedure with up to 3 years of follow-up

    LQG for Constrained Linear Systems: Indirect Feedback Stochastic MPC with Kalman Filtering

    We present an output feedback stochastic model predictive control (SMPC) approach for linear systems subject to Gaussian disturbances and measurement noise and probabilistic constraints on system states and inputs. The presented approach combines a linear Kalman filter for state estimation with an indirect feedback SMPC, which is initialized with a predicted nominal state, while feedback of the current state estimate enters through the objective of the SMPC problem. For this combination, we establish recursive feasibility of the SMPC problem due to the chosen initialization, and closed-loop chance constraint satisfaction thanks to an appropriate tightening of the constraints in the SMPC problem also considering the state estimation uncertainty. Additionally, we show that for specific design choices in the SMPC problem, the unconstrained linear-quadratic-Gaussian (LQG) solution is recovered if it is feasible for a given initial condition and the considered constraints. We demonstrate this fact for a numerical example, and show that the resulting output feedback controller can provide non-conservative constraint satisfaction.Comment: 7 pages, 1 figur

    Shrunken Pore Syndrome Is Frequently Occurring in Severe COVID-19

Publisher Copyright: © 2022 by the authors.A selective decrease in the renal filtration of larger molecules is attributed to the shrinkage of glomerular pores, a condition termed Shrunken Pore Syndrome (SPS). SPS is associated with poor long-term prognosis. We studied SPS as a risk marker in a cohort of patients with COVID-19 treated in an intensive care unit. SPS was defined as a ratio < 0.7 when the estimated glomerular filtration rate (eGFR), determined by cystatin C, calculated by the Cystatin C Caucasian-Asian-Pediatric-Adult equation (CAPA), was divided by the eGFR determined by creatinine, calculated by the revised Lund–Malmö creatinine equation (LMR). Clinical data were prospectively collected. In total, SPS was present in 86 (24%) of 352 patients with COVID-19 on ICU admission. Patients with SPS had a higher BMI, Simplified Physiology Score (SAPS3), and had diabetes and/or hypertension more frequently than patients without SPS. Ninety-nine patients in the total cohort were women, 50 of whom had SPS. In dexamethasone-naïve patients, C-reactive protein (CRP), TNF-alpha, and interleukin-6 did not differ between SPS and non-SPS patients. Demographic factors (gender, BMI) and illness severity (SAPS3) were independent predictors of SPS. Age and dexamethasone treatment did not affect the frequency of SPS after adjustments for age, sex, BMI, and acute severity. SPS is frequent in severely ill COVID-19 patients. Female gender was associated with a higher proportion of SPS. Demographic factors and illness severity were independent predictors of SPS.publishersversionpublishe

Coronavirus disease 2019 (COVID-19) patients in the intensive care unit (ICU) often have acute kidney injury (AKI) and are at increased risk of drug-induced kidney injury. The aim of this study was to compare creatinine-based eGFR equations to cystatin C-based eGFR in ICU patients with COVID-19. Methods: After informed consent, we included 370 adult ICU patients with COVID-19. Creatinine and cystatin C were analyzed at admission to the ICU as part of the routine care. Creatinine-based eGFR (ml/min) was calculated using the following equations, developed in chronological order; the Cockcroft–Gault (C-G), Modified Diet in Renal Disease (MDRD)1999, MDRD 2006, Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI), and Lund–Malmö revised (LMR) equations, which were compared with eGFR calculated using the cystatin C-based Caucasian Asian Pediatric Adult (CAPA) equation. Results: The median eGFR when determined by C-G was 99 ml/min and interquartile range (IQR: 67 ml/min). Corresponding estimations for MDRD1999 were 90 ml/min (IQR: 54); MDRD2006: 85 ml/min (IQR: 51); CKD-EPI: 91 ml/min (IQR: 47); and for LMR 83 ml/min (IQR: 41). eGFR was calculated using cystatin C and the CAPA equation value was 70 ml/min (IQR: 38). All differences between creatinine-based eGFR versus cystatin C-based eGFR were significant (p <.00001). Conclusions: Estimation of GFR based on various analyses of creatinine are higher when compared with a cystatin C-based equation. The C-G equation had the worst performance and should not be used in combination with modern creatinine analysis methods for determination of drug dosage in COVID-19 patients.publishersversionepub_ahead_of_prin
