    Supraspinatus tendon micromorphology in individuals with subacromial pain syndrome

    Study Design: Cross-sectional cohort. Introduction: Tendon collagen organization can be estimated by peak spatial frequency radius (PSFR) on ultrasound images. Characterizing PSFR can define the contribution of collagen disruption to shoulder symptoms. Purpose of the Study: The purpose of this was to characterize the (1) supraspinatus tendon PSFR in participants with sub acromial pain syndrome (SPS) and healthy controls; (2) PSFR between participants grouped on a tendon visual quality score; and (3) relationship between PSFR with patient-reported pain, function, and shoulder strength. Methods: Participants with SPS (n ¼ 20) and age, sex, and arm-dominance matched healthy controls (n ¼ 20) completed strength testing in scaption and external rotation, and patient-reported pain, and functional outcomes. Supraspinatus tendon ultrasound images were acquired, and PSFR was calculated for a region of interest 15 mm medial to the supraspinatus footprint. PSFR was compared between groups using an independent t-test and an analysis of variance to compare between 3 groups for visually qualitatively rated tendon abnormalities. Relationships between PSFR with pain, function, and strength were assessed using Pearson correlation coefficient. Results: Supraspinatus tendon PSFR was not different between groups (P ¼ .190) or tendon qualitative ratings (P ¼ .556). No relationship was found between PSFR and pain, functional loss, and strength (P \u3e .05). Conclusions: Collagen disruption (PSFR) measured via ultrasound images of the supraspinatus tendon was not different between participants with SPS or in those with visually rated tendon defects. PSFR is not related to shoulder pain, function, and strength, suggesting that supraspinatus tendon collagen disorganization may not be a contributing factor to shoulder SPS. However, collagen disruption may not be isolated to a single region of interest. Level of Evidence: 3b: case-control study

    Sensory imagery in craving: From cognitive psychology to new treatments for addiction

    Sensory imagery is a powerful tool for inducing craving because it is a key component of the cognitive system that underpins human motivation. The role of sensory imagery in motivation is explained by Elaborated Intrusion (EI) theory. Imagery plays an important role in motivation because it conveys the emotional qualities of the desired event, mimicking anticipated pleasure or relief, and continual elaboration of the imagery ensures that the target stays in mind. We argue that craving is a conscious state, intervening between unconscious triggers and consumption, and summarise evidence that interfering with sensory imagery can weaken cravings. We argue that treatments for addiction can be enhanced by the application of EI theory to maintain motivation, and assist in the management of craving in high-risk situations

    National Athletic Trainers\u27 Association Position Statement: Evaluation, Management, and Outcomes of and Return-to-Play Criteria for Overhead Athletes With Superior Labral Anterior-Posterior Injuries

    Objective: To present recommendations for the diagnosis, management, outcomes, and return to play of athletes with superior labral anterior-posterior (SLAP) injuries. Background: In overhead athletes, SLAP tears are common as either acute or chronic injuries. The clinical guidelines presented here were developed based on a systematic review of the current evidence and the consensus of the writing panel. Clinicians can use these guidelines to inform decision making regarding the diagnosis, acute and long-term conservative and surgical treatment, and expected outcomes of and return-to-play guidelines for athletes with SLAP injuries. Recommendations: Physical examination tests may aid diagnosis; 6 tests are recommended for confirming and 1 test is recommended for ruling out a SLAP lesion. Combinations of tests may be helpful to diagnose SLAP lesions. Clinical trials directly comparing outcomes between surgical and nonoperative management are absent; however, in cohort trials, the reports of function and return-to-sport outcomes are similar for each management approach. Nonoperative management that includes rehabilitation, nonsteroidal anti-inflammatory drugs, and corticosteroid injections is recommended as the first line of treatment. Rehabilitation should address deficits in shoulder internal rotation, total arc of motion, and horizontal-adduction motion, as well as periscapular and glenohumeral muscle strength, endurance, and neuromuscular control. Most researchers have examined the outcomes of surgical management and found high levels of satisfaction and return of shoulder function, but the ability to return to sport varied widely, with 20% to 94% of patients returning to their sport after surgical or nonoperative management. On average, 55% of athletes returned to full participation in prior sports, but overhead athletes had a lower average return of 45%. Additional work is needed to define the criteria for diagnosing and guiding clinical decision making to optimize outcomes and return to play

    Population genetic evidence for sex-specific dispersal in an inbred social spider

It has been suggested that such inbred sociality is advantageous in the short term, but likely to lead to extinction or reduced speciation rates in the long run. In this situation, very low levels of dispersal and gene flow among colonies may have unusually important impacts on fitness and persistence of social spiders. We investigated sex-specific differences in dispersal and gene flow among colonies, as reflected in the genetic structure within colonies and populations of the African social spider Stegodyphus dumicola Pocock, 1898 (Eresidae). We used DNA fingerprinting and mtDNA sequence data along with spatial mapping of colonies to compare male and female patterns of relatedness within and among colonies at three study sites. Samples were collected during and shortly after the mating season to detect sex-specific dispersal. Distribution of mtDNA haplotypes was consistent with proliferation of social nests by budding and medium- to long-distance dispersal by ballooning females. Analysis of molecular variance and spatial autocorrelation analyses of AFLPs showed high levels of genetic similarity within colonies, and STRUCTURE analyses revealed that the number of source populations contributing to colonies ranged from one to three. We also showed significant evidence of male dispersal among colonies at one site. These results support the hypothesis that in social spiders, genetic cohesion among populations is maintained by long-distance dispersal of female colony founders. Genetic diversity within colonies is maintained by colony initiation by multiple dispersing females, and adult male dispersal over short distances. Male dispersal may be particularly important in maintaining gene flow among colonies in local populations

    Importance of thermophilous habitats for protection of wild bees (Apiformes)

    Research on wild bees (Apiformes) was conducted in the Lower Oder Valley (NW Poland) at Natura 2000 sites near the border between Poland and Germany. The analysis involved 3 landscape types with xerothermic and sandy grasslands, differing in the proportion of woody vegetation. In total, we collected there 4158 specimens of Apiformes, representing 180 species. We have proved that mid-forest grasslands with a high proportion of thermophilous broad-leaved forests and xerothermic shrub communities are equally attractive to wild bees as open habitats (sandy grasslands, xerothermic grasslands/heaths). We observed varied responses of wild bee species with specific functional characteristics to increasing proportion of woody vegetation. The grasslands surrounded by forests were characterized by the highest number of cleptoparasitic species. In contrast, solitary and social bee species preferred forest-steppe habitats. However, in open habitats, solitary bees were the most abundant. Moreover, open habitats were distinguished by the highest number and abundance of rare species. Active protection of thermophilous grasslands is crucial for biodiversity conservation, also with respect to the natural resources of Apiformes. Preservation of biodiversity in threatened xerothermic and sandy grasslands should be one of the key objectives of nature conservation in European countries. Currently, more and more actions are undertaken to improve their condition and to restore those particularly valuable and threatened habitat types

    Taping patients with clinical signs of subacromial impingement syndrome: the design of a randomized controlled trial

    <p>Abstract</p> <p>Background</p> <p>Shoulder problems are a common complaint of the musculoskeletal system. Physical therapists treat these patients with different modalities such as exercise, massage, and shoulder taping. Although different techniques have been described, the effectiveness of taping has not yet been established. The aim of this study is to assess the effectiveness and cost-effectiveness of usual physical therapy care in combination with a particular tape technique for subacromial impingement syndrome of the shoulder compared to usual physical therapy care without this tape technique in a primary healthcare setting.</p> <p>Methods and design</p> <p>An economic evaluation alongside a randomized controlled trial will be conducted. A sample of 140 patients between 18 and 65 years of age with a diagnosis of subacromial impingement syndrome (SAIS) as assessed by physical therapists will be recruited. Eligible patients will be randomized to either the intervention group (usual care in combination with the particular tape technique) or the control group (usual care without this tape technique). In both groups, usual care will consist of individualized physical therapy care. The primary outcomes will be shoulder-specific function (the Simple Shoulder Test) and pain severity (11-point numerical rating scale). The economic evaluation will be performed using a societal perspective. All relevant costs will be registered using cost diaries. Utilities (Quality Adjusted Life Years) will be measured using the EuroQol. The data will be collected at baseline, and 4, 12, and 26 weeks follow-up.</p> <p>Discussion</p> <p>This pragmatic study will provide information about the effectiveness and cost-effectiveness of taping in patients presenting with clinical signs of SAIS.</p> <p>Trial registration</p> <p>Trial registration number: <a href="http://www.trialregister.nl/trialreg/admin/rctview.asp?TC=2575">NTR2575</a></p

    Direct determination of cellulosic glucan content in starch-containing samples

    A simple and highly selective analytical procedure is presented for the determination of cellulosic glucan content in samples that contain both cellulose and starch. This method eliminates the unacceptably large compounding errors of current two-measurement methods. If both starch and cellulose are present before analytical hydrolysis, both will be hydrolyzed to glucose causing bias and inaccuracy in the method. To prevent this interference, the removal of starch prior to cellulosic quantification is crucial. The method presented here is a concise in-series procedure with minimal measurements, eliminating large compounding errors. Sample preparation consists of a starch extraction employing enzymatic hydrolysis followed by a simple filtration and wash. The samples are then subjected to a two-stage acid hydrolysis. The concentration of glucose is determined by ion exchange high-performance liquid chromatography with a Pb2+ column and a refractive index detector. The cellulosic glucan content is calculated based on the initial dry weight of the starting material. Data for the native biomass materials studied show excellent reproducibility, with coefficients of variance of 3.0% or less associated with the method. This selectivity for cellulosic glucan by the procedure was validated with several analytical techniques such as liquid chromatography coupled with mass spectrometry (LC–MS), Raman spectroscopy, and nuclear magnetic resonance

    Analysis of the Diversity of Megachilidae Bees on the Northern Subplateau of the Iberian Peninsula

    In the western Mediterranean, 772 species of bees in the family Megachilidae have been reported. Special emphasis has been placed on the Iberian Peninsula, where to date 218 species are known. However, few intensive studies providing information about communities of Megachilidae have been carried out. Two earlier works cite 70 species; almost one third of those known on the Peninsula. With an aim of gaining insight into the structure of the communities of Megachilidae and the factors influencing them, an analysis was made of the alpha and beta diversity of different localities in the northern subplateau. Malaise traps (black and white) were used, and 559 specimens belonging to 55 species were identified of which most exhibited a nest-holder-type nesting habit. Abundance and richness were higher for white traps, although a considerable degree of complementarity was observed with the black traps. In the study zone, diversity can be considered medium-high with a phylogenetic diversity corresponding to stable populations. Regarding the composition of the Megachilidae communities, the influence of the landscape structure, of the microhabitat, and of the colour of the trap used to collect the specimens was detected. The following are recommended: (1) the use of both black and white traps, since they show high complementarity and offer different information about community structure, (2) homogenization of the samples in comparisons among communities, owing to the influence of the color of the trap, which masks the importance of ecological factors in community structuring, and (3) the collection of samples from at least two years previous, in view of the elevated “replacement” of species observed with species richness estimators
