    Application of Non-destructive Testing for Measurement of Partial Discharges in Oil Insulation Systems

    The subject area regards to metrology and measurement methods applied for non-destructive investigation of electrical discharges occurring in oil insulation systems of high-voltage devices. The main aim of performed research studies is a detailed and multivariate analysis of physical phenomena associated with generation of electrical partial discharges (PD), which occur in oil insulation of electrical equipment. An important cognitive component was the verification whether the form of PD has an effect on the energy contribution of the physical phenomena associated with their generation. For investigating the physical processes associated with generation of PD, a system for modelling, the study and analysis of physical phenomena associated with their generation in insulating oil were designed and implemented. In particular, the PD were simulated in three setups: (1) a surface system, (2) needle-needle system in insulating oil and (3) needle-needle system in insulating oil with gas bubbles. In these experimental setups, optical signals (IR, UV and visible), ultra–high frequency electromagnetic and high-energy X-ray radiation, acoustic emission and thermal images were registered.     Evaluation of the life quality of patients with lumbosacral spine pain performing sedentary work after using Kaltenborn-Evejth manual therapy techniques

    Introduction: Pain in the lumbosacral spine is a substantial problem of modern civilization. It leads to lowering the quality of life and to the reduced functioning in social and professional context. This problem mainly affects people with sedentary work such as drivers and office workers. Long-term work in one position and a frequent lack of physical activity are two of the factors conducive to the occurrence of pain in people doing work in a sitting position. Aim of the study: To assess the influence of Kaltenborn-Evejth manual therapy techniques on the quality of life of people doing sedentary work. Material and Method: The research was carried out in the Medi-Sport Rehabilitation and Sports Center from March to October 2018. The study included 60 people doing sedentary work of which 37 were women and 23 men. The Oswestry Questionnaire was the research tool. In the study group there were 21 people aged 30-40, 23 aged 41-50 and 16 aged 51-60. Results: The use of Kaltenborn-Evejth manual therapy techniques in people doing sedentary work reduced pain in the lumbosacral spine, improved its functions, which improved the quality of life of patients. After the therapy, the severity of pain in the lumbosacral spine decreased. Conclusions: Techniques of Kaltenborn-Evejth manual therapy mobilization significantly reduce pain in the lumbosacral spine, which improves the quality of life of persons performing sedentary work. The severity of pain in the lumbosacral spine decreased after the therapy

    The role of calcium score in evaluation of the advancing of ischemic heart disease

    Introduction:  Thanks to development of technology, new noninvasive techniques of coronary arteries evaluation, such as Multislice Computed Tomography (MSCT) were introduced. Nowadays tomographs are characterized by spatial and time resolution sufficient for reliable assessment of coronary arteries. Moreover, the dose of radiation taken  by patient was substantially decreased. Two main methods of evaluation were introduced. Examination performed without contrast agent enables quantitive assessment of coronary arteries calcification by so called Agatston score. Another method performed after contrast injection is Computed Tomography Angiography (CTA)Goal: This studies purpose was to determine the role of Calcium Score in assessment of Coronary Arteries Disease (CAD) advancementMaterials and methods: In this studythe results of 100 consecutive patients (45% men, mean age 55 ± 8.9 years) were analyzed. All patients had a positive ECG stress test score. With a 64-row CT scan, coronary artery imaging of all patients was performed. The descriptive analysis of the group was based on information obtained from referrals issued by cardiologists to the MSCT. Analyzed parameters were: age, gender, indication for referral, Calcium Score and final diagnosis.Results: In most cases (84%) of patients with positive ECG stress test score, MSCT excluded significant changes in coronary arteries. Also, the correlation between Calcium Score and sex and diagnosis was proven.Conclusions: MSCT and Calcium Score enables to find patients without significant changes in coronary arteries and positive ECG stress test and to reduce the number of referrals to coronarography

    The use of cannabinoids in the treatment of attention deficit hyperactivity disorder - a review of the latest literature

    Introduction: Attention Deficit Hyperactivity Disorder (ADHD) is a common problem in adolescents and adults around the world. ADHD is particularly characterized by inattention, impulsiveness and hyperactivity, and in order to deal with these problems, patients often resort to other methods of treatment in addition to classic pharmacotherapy. Increasing numbers of ADHD patients are self-medicating with cannabis, despite the lack of specific evidence on its efficacy and safety. Aim of the study: The purpose of this study was to review the most recent available literature on the use of cannabidiol in the treatment of ADHD. For this purpose, PubMed and scopus databases were reviewed. The phrase "cannabidiol and ADHD" was used in the database search. After reviewing the titles and abstracts, 15 articles and articles cited in them were obtained and analyzed in detail. Results: Cannabinoids are reported to have pharmacological anxiolytic, neuroprotective, antidepressant, antipsychotic and hypnotic effects. Single-case studies of patients treated with them show that cannabinoids reduced hyperactivity, improved focus and control of impulsive behavior. Patients also reported side effects of cannabis use, the most common being mild short-term memory problems, dry mouth, sleepiness, and occasional experiences of constant thirst and greater forgetfulness. Most of the acute side effects were related to neuropsychiatric symptoms and were associated with cardiac sympathomimetic effects  Conclusion:  There is evidence for the effects of cannabinoids on ADHD symptoms as well as the influence of the cannabinoid system on involvement in neurological and psychiatric disorders. However, more controlled clinical trials are needed to determine the effect of cannabis use on ADHD symptoms and the norms of its use in this indication

    Risk factors for reoperation after surgical treatment for degenerative spinal disease in Poland: a nationwide retrospective study of 38,953 hospitalisations

    Introduction. Degenerative spinal disease (DSD) is one of the most common musculoskeletal conditions and a leading cause of sickness absence. It also contributes significantly to the global burden of disease. The aim of this study was to assess the frequency of reoperation after surgical treatment of DSDs in Poland, and to identify risk factors for reoperation.Material and methods. A retrospective analysis of hospitalisations for DSD in 2018 that were reported to Poland’s National Health Fund (NHF) was performed. Reoperations reported within 365 days of hospital discharge were identified. Demographic factors and multimorbidities were included in the analysis. A logistic regression model was then performed to assess risk factors for reoperations.Results. In 2018, 38,953 surgical hospitalszations for DSD were reported. A total of 3,942 hospitalised patients (10.12%) required reoperation within 365 days. Patients requiring reoperation were predominantly female (female-to-male ratio 1.34:1) and elderly (mean age of reoperated patients 56.66 years, mean age of other patients 53.24). The percentage reoperated upon correlated with multiple diseases (from 8.81% in the group of patients without comorbidities to 15.31% in the group of patients with three or more comorbidities). The risk of reoperation was most increased by comorbid depression, neurological diseases, obesity, and older age. The risk of reoperation was reduced by instrumented spinal surgery, surgery in a neurosurgical unit, and hospitalisations other than same-day surgery.Conclusions. Reoperations within a year after DSD surgical treatment are common. Identifying risk factors for reoperation, including those related to the presence of comorbidities and the phenomenon of multimorbidity, can be an important tool in reducing reoperation rates

    A three-headed piriformis muscle: an anatomical case study and narrative review of literature

    Introduction: The piriformis muscle (PM) is found in the gluteal region, exiting the pelvis through the greater sciatic foramen and dividing it into the suprapiriform and infrapiriform foramina. The piriformis works as part of the hip external rotator muscle group, and is responsible for rotation of the femur upon hip extension and abduction of the femur during flexion of the hip joint. The aim of the present report is to describe a very rare case of the primary three-headed piriformis muscle. To the best knowledge of the authors, the said variant has not yet been described in the existing literature. Case report: The 71-year-old male formalin-fixed cadaver was subjected to routine dissection. After careful removal of the connecting tissue, three separate, primary heads of the PM were identified. The lower head of the PM arose from the middle part of the sacral bone; 87.56 mm long and 9.73 mm wide. The medial head was attached to the internal part of the posterior inferior iliac spine; 121.6 mm long and 20.97 mm wide. The upper head was attached to the external part of the posterior inferior iliac spine; 78.89 mm long and 23.94 mm wide. All heads converged into a common tendon which inserted onto the greater trochanter. Conclusions: The clinical importance of this work comes down to the fact that the aberrant PM may be the reason behind the piriformis syndrome and its associated symptoms. Moreover, knowledge regarding the variant anatomy of the PM is of immense importance to, e.g., anesthesiologists performing CT- or ultrasound-guided SN injection for local anesthesia, radiologists interpreting imaging studies, and surgeons, especially during posterior approaches to the hip and pelvis

    Psychosocial and environmental risk factors of obesity and hypertension in children and adolescents—a literature overview

    Childhood obesity has become a worldwide epidemic in the 21st century. Its treatment is challenging and often ineffective, among others due to complex, often not obvious causes. Awareness of the existence and meaning of psychosocial and environmental risk factors seems to be an essential element in the prevention and treatment of obesity and its complications, especially arterial hypertension. In this review, we will discuss the role of that risk factors linking obesity and increased cardiovascular disorders including the role of nutritional factors (including the role of unhealthy diet, inadequate hydration), unhealthy behaviors (e.g. smoking, alcohol and drugs, sedentary behavior, low physical activity, disrupted circadian rhythms, sleep disorders, screen exposure), unfavorable social factors (such as dysfunctional family, bullying, chronic stress, mood disorders, depression, urbanization, noise, and environmental pollution), and finally differences in cardiovascular risk in girls and boys
