    Approach to The Occluded Permanent Hemodialysis Catheter

    Objective: Central venous catheterization (CVC) is a type of surgery that hemodialysis (HD) patients frequently undergo. Presently, permanent CVCs (pCVCs) are the alternative to vascular access for patients requiring long-term catheterization. Additionally, identification of the type of catheter has a great importance for the right intervention.Our study aims: The aim was to discuss the identification procedure of an HD catheter, whether it is permanent or temporary, by presenting an HD catheter case done by CVC.Case presentation: A 75-year-old female patient having a routine of 3 days/week HD treatment was admitted to the critical care unit. A nonfunctional HD catheter was present in the left subclavian vein. A new HD catheter was placed in the right subclavian vein and continuous renal replacement therapy with heparin was initiated. Radiography revealed an opacity with a size of around 3 cm detected at the tip of the nonfunctional catheter. The catheter was removed with an incision after an unsuccessful attempt of removal by pulling it out. Further investigation of the catheter revealed that it was occluded.Conclusion:It is crucial to determine whether the catheter is temporary or permanent in order to do the right intervention and not to have unwanted consequences while removing a nonfunctional HD catheter

    Relationship of percutaneous tracheostomy timing with APACHE II and SOFA scores on the first day of ICU for critically ill patients

    Objective: Our study aimed to assess the relation between APACHE II and SOFA scores of critically ill patients on their first day of admission and the timing of percutaneous dilatational tracheotomy (PDT).Methods: Following approval of the Ethics Board of Health Sciences University Istanbul Bağcılar Educational Research Hospital (25.08.2016-2016/495), data of all 91 patients who had been treated with PDT in the ICU between June 1, 2014 and June 1, 2016 have been retrospectively evaluated. We recorded the following information: demographical data (such as age, sex, body mass index) that could be obtained from patient records, APACHE II and SOFA scores on their first day in ICU, and PDT timing.Results: There was no statistically significant difference observed between the timing of the PDT and APACHE II and SOFA scores (p>0.05).Conclusion: Our results showed that most of the patients with PDT had an APACHE II score of 15-24. We noticed that the number of patients with an APACHE II score of 24 and higher was notably lower than the number of patients with scores between 15-24. The fact that the life expectancy for the patient group with a high APACHE II score is low may call the tracheostomy decision into question. On the other hand, no relation was found between differences in APACHE II and SOFA scores and the starting time of PDT

    Microfluidic-based virus detection methods for respiratory diseases

    With the recent SARS-CoV-2 outbreak, the importance of rapid and direct detection of respiratory disease viruses has been well recognized. The detection of these viruses with novel technologies is vital in timely prevention and treatment strategies for epidemics and pandemics. Respiratory viruses can be detected from saliva, swab samples, nasal fluid, and blood, and collected samples can be analyzed by various techniques. Conventional methods for virus detection are based on techniques relying on cell culture, antigen-antibody interactions, and nucleic acids. However, these methods require trained personnel as well as expensive equipment. Microfluidic technologies, on the other hand, are one of the most accurate and specific methods to directly detect respiratory tract viruses. During viral infections, the production of detectable amounts of relevant antibodies takes a few days to weeks, hampering the aim of prevention. Alternatively, nucleic acid–based methods can directly detect the virus-specific RNA or DNA region, even before the immune response. There are numerous methods to detect respiratory viruses, but direct detection techniques have higher specificity and sensitivity than other techniques. This review aims to summarize the methods and technologies developed for microfluidic-based direct detection of viruses that cause respiratory infection using different detection techniques. Microfluidics enables the use of minimal sample volumes and thereby leading to a time, cost, and labor effective operation. Microfluidic-based detection technologies provide affordable, portable, rapid, and sensitive analysis of intact virus or virus genetic material, which is very important in pandemic and epidemic events to control outbreaks with an effective diagnosis.Qatar National Research Fun

    Lean and Sustainable Supplier Selection in the Furniture Industry

    The furniture manufacturing sector faces intricate challenges in pioneering sustainable supply chains, particularly with lean and sustainable supplier selection. This study focused on integrating key performance indicators (KPIs) associated with lean philosophy and sustainability into multi-criteria decision-making (MCDM) methodologies. The study methodically evaluated 18 criteria spanning economic, environmental, and social dimensions to discern supplier suitability in both leanness and sustainability realms. Through the ENTROPY method, weights were systematically assigned to these criteria. Subsequently, Fuzzy ARAS and Fuzzy TOPSIS methods were adeptly employed to comparatively assess supplier options. Noteworthy findings included the paramount importance of the distance to the customer and labor practices in supplier selection. The quality level, however, carried the least weight, mainly due to comparable performance scores among alternatives. Consistently, Fuzzy ARAS and Fuzzy TOPSIS results converged to pinpoint Supplier 2 as the optimal choice, reflecting its superior Ki and CCi metrics. Central to this research was the introduction of a structured and holistic framework for lean and sustainable supplier selection, a significant leap forward that promises to be an invaluable asset for practitioners and scholars in the furniture industry, supply chain management, multi-criteria decision-making, and policymaking

    Effects of Algan Hemostatic Agent on bleeding time in a rat tail hemorrhage model

In this study, we aimed to investigate the effects of AHA on bleeding time in a rat tail hemorrhage model. METHODS: Forty-eight Sprague Dawley rats (5-7 weeks old, 180-210 g) were randomly and equally allocated to six groups as follows: heparin plus saline (heparinized control), heparin plus AHA-soaked sponge, heparin plus liquid form of AHA, saline (non-heparinized control), AHA-soaked sponge and liquid form of AHA. Heparin (640 IU/kg) was administered intraperitoneally three times a day for three days in heparinized groups. For the bleeding model, the tail of rats was transected. According to the study group, either saline- or AHA-soaked sponge or liquid form of AHA was applied over the hemorrhage area. In AHA- or saline-soaked sponge groups, once the bleeding time had started, it was checked every 10 seconds. If the bleeding did not stop after 40 seconds, it was accepted as a failure. In liquid AHA group, the duration of bleeding was measured using a chronometer and defined as the time (seconds) from wounding until the bleeding stopped. RESULTS: Bleeding time in the heparinized and non-heparinized control groups was over 40 seconds. After applying the sponge form of AHA on the wound area, bleeding time was significantly shortened to less than 20 seconds in both heparinized and non-heparinized rats (p<0.001 for both). The liquid form of AHA stopped bleeding in 5.0 +/- 1.2 seconds and 8.0 +/- 1.3 seconds in heparinized and non-heparinized groups, respectively. CONCLUSION: AHA is a highly effective topical hemostatic agent in a rat tail hemorrhage model, thus may provide for a unique clinically effective option for control of bleeding during surgical operations or other emergencies

    Evaluation of Cytotoxicity and Mutagenicity of Wastewater from Istanbul: Data from Hospitals and Advanced Wastewater Treatment Plant

    Wastewater (WW) carry considerable amount of chemicals that could have mutagenic or cytotoxic effect from hospital discharges to aquatic environment. Our objective was to determinate the possible mutagenic and toxic effects of hospital originated WWs and effectiveness of the wastewater treatment plants (WTP) functions. In the study the mutagenic and cytotoxic potential of three hospitals and influent/effluent of a treatment plant WW collected in Istanbul and was examined using AMES, XTT, and lactate dehydrogenase (LDH) assays. Mutagenic effects were detected at both hospital discharges and advanced biological wastewater plant. We observed no cytotoxic effect in fibroblasts for LDH and XTT assays whereas high cytotoxicity for all samples was found in hepatocytes by XTT assay. According to the results even if advanced technology is used for treatment of WW, mutagenic and cytotoxic effects still remain, and the present technologies need to be further improved

    The safety and efficacy of first-line atezolizumab plus bevacizumab in patients with unresectable hepatocellular carcinoma: A multicenter real-world study from Turkey

    The aim of the study was to evaluate the real-world clinical outcomes of atezolizumab and bevacizumab (Atez/Bev) as the initial therapy for advanced hepatocellular carcinoma (HCC). We retrospectively analyzed 65 patients treated with Atez/Bev for advanced HCC from 22 institutions in Turkey between September 2020 and March 2023. Responses were evaluated by RECIST v1.1 criteria. The median progression-free survival (PFS) and overall survival (OS) were calculated using the Kaplan-Meier method. Cox regression model was employed to conduct multivariate analyses. The median age was 65 (range, 22-89) years, and 83.1% of the patients were male. A total of 1.5% achieved a complete response, 35.4% had a partial response, 36.9% had stable disease, and 26.2% had progressive disease. The disease control rate was 73.8% and associated with alpha-fetoprotein levels at diagnosis and concomitant antibiotic use. The incidence rates of any grade and grade ≥ 3 adverse events were 29.2% and 10.7%, respectively. At a median follow-up of 11.3 (3.4-33.3) months, the median PFS and OS were 5.1 (95% CI: 3-7.3) and 18.1 (95% CI: 6.2-29.9) months, respectively. In univariate analyses, ECOG-PS ≥ 1 (relative to 0), Child-Pugh class B (relative to A), neutrophil-to-lymphocyte ratio (NLR) > 2.9 (relative to ≤ 2.9), and concomitant antibiotic use significantly increased the overall risk of mortality. Multivariate analysis revealed that ECOG-PS ≥ 1 (HR: 2.69, P = .02), NLR > 2.9 (HR: 2.94, P = .017), and concomitant antibiotic use (HR: 4.18, P = .003) were independent predictors of OS. Atez/Bev is an effective and safe first-line therapy for advanced-stage HCC in a real-world setting. The survival benefit was especially promising in patients with a ECOG-PS score of 0, Child-Pugh class A, lower NLR, and patients who were not exposed to antibiotics during the treatment

    Varicella can cause complications that are potentially serious and require hospitalization. Our current understanding of the causes and incidence of varicella-related hospitalization in Turkey is limited and sufficiently accurate epidemiological and economical information is lacking. The aim of this study was to estimate the annual incidence of varicella-related hospitalizations, describe the complications, and estimate the annual mortality and cost of varicella in children. VARICOMP is a multi-center study that was performed to provide epidemiological and economic data on hospitalization for varicella in children between 0 and 15 years of age from October 2008 to September 2010 in Turkey. According to medical records from 27 health care centers in 14 cities (representing 49.3% of the childhood population in Turkey), 824 children (73% previously healthy) were hospitalized for varicella over the 2-year period. Most cases occurred in the spring and early summer months. Most cases were in children under 5 years of age, and 29.5% were in children under 1 year of age. The estimated incidence of varicella-related hospitalization was 5.29-6.89 per 100,000 in all children between 0-15 years of age in Turkey, 21.7 to 28 per 100,000 children under 1 year of age, 9.8-13.8 per 100,000 children under 5 years of age, 3.96-6.52 per 100,000 children between 5 and 10 years of age and 0.42 to 0.71 per 100,000 children between 10 and 15 years of age. Among the 824 children, 212 (25.7%) were hospitalized because of primary varicella infection. The most common complications in children were secondary bacterial infection (23%), neurological (19.1%), and respiratory (17.5%) complications. Secondary bacterial infections (p < 0.001) and neurological complications (p < 0.001) were significantly more common in previously healthy children, whereas hematological complications (p < 0.001) were more commonly observed in children with underlying conditions. The median length of the hospital stay was 6 days, and it was longer in children with underlying conditions (< 0.001). The median cost of hospitalization per patient was 338andwassignificantlyhigherinchildrenwithunderlyingconditions(p<0.001).Theestimateddirectannualcost(notincludingthelossofparentalworktimeandschoolabsence)ofvaricellarelatedhospitalizationinchildrenundertheageof15yearsinTurkeywas338 and was significantly higher in children with underlying conditions (p < 0.001). The incidence of this disease was higher in children < 1 year of age, and there are no prevention strategies for these children other than population-wide vaccination. Universal vaccination is therefore the only realistic option for the prevention of severe complications and deaths. The surveillance of varicella-associated complications is essential for monitoring of the impact of varicella immunization

    Conclusion: This is the largest and most comprehensive epidemiological study to determine the prevalence of psychopathologies in children and adolescents in Turkey. Our results partly higher than, and partly comparable to previous national and international studies. It also contributes to the literature by determining the independent predictors of psychopathologies in this age group
