Exploring the therapeutic process in therapies with young people using the Adolescent Psychotherapy Q-set
Background: The Adolescent Psychotherapy Q-set (APQ) is a pan-theoretical empirically validated process measure with potential for statistical analysis describing the unique characteristics of therapy sessions conducted with adolescents.
Aims: This literature review aims to examine the development of the APQ and its applications in research and practice to guide future applications.
Methods: A literature search on the PsycInfo and UCL Explore search engines and subsequent scanning of search results yielded nine published studies using the APQ. One unpublished study was made available to the author by the measure developer.
Process: Studies were critically reviewed one by one paying attention to their research methodology, innovative use of the APQ, and their contributions to the field of adolescent psychotherapy process research. The APQâs applications in this small body of literature was then evaluated. A review of relevant applications of the APQâs child and adult equivalents highlighted directions for the APQâs future potential.
Results: Following its empirical validation, the APQ has been applied theoretically, in research of clinical practice, and in supervision. The APQâs application to create therapy prototypes of therapy modalities has shown its good discriminatory quality and suitability to study treatment adherence. Using the APQ across therapies has shown it can detect important trends in therapy processes across large data sets. The APQâs applications in single-case studies has shown the measuresâ suitability to this research methodology, having already produced important insights in adolescent therapy process research. The review found the APQ to be sensitive to subtle differences in therapeutic techniques and interactions whilst possibly missing non-verbal factors and within-session variability. The APQâs use of direct observation in real-life clinical settings, suitability for various statistical analyses and triangulation as well as its potential to find process-outcome links in adolescent therapy are clear advantages.
Conclusions: Although empirical research conducted with the APQ is in its infancy, its applications so far have shown the versatility of this measure and its potential to add knowledge to the field of adolescent psychotherapy process research. The review highlights important methodological directions for future research, including a move beyond research based on modality prototypes.
Part 2: Empirical Research Project
What therapy processes precede dissatisfied dropout in STPP for adolescent depression: a single-case qualitative exploration guided by the Adolescent Psychotherapy Q-set
Abstract
Background: A significant percentage of young people in treatment for depression drop out of psychotherapy, some due to dissatisfaction with the therapy offered. The therapeutic processes preceding dissatisfied dropouts in the adolescent population are insufficiently understood.
Aims: The current single-case study aimed to explore the therapeutic process of a 12-session, prematurely-ended therapy with a young person dissatisfied with short-term psychoanalytic psychotherapy (STPP) received for depression.
Methods: The Adolescent Psychotherapy Q-set (APQ), an empirically validated process measure, was used in tandem with clinical case analysis to explore the therapy process over time.
Results: The analysis of twelve APQ ratings found a productive patient-therapy couple working collaboratively to understand the young personâs experiences and emotions. Following an initial phase of the young person presenting as emotional and vulnerable, she became increasingly ambivalent about partaking in psychotherapy. A lively and argumentative period exploring the young personâs ambivalence and increased sense of well-being culminated in eventual dropout.
Conclusion: Even in a strong, collaborative working relationship with an engaged young person, ambivalence around dependency and vulnerability can threaten treatment completion. The APQ is a suitable measure to explore general treatment processes preceding dropout, but its usefulness could be enhanced through systematic methodological pluralism
Mapping Invasive Giant Goldenrod (Solidago gigantea) with Multispectral Images Acquired by Unmanned Aerial Vehicle
Invasive alien species are one of the main threats to worldwide biodiversity loss. Unmanned aerial vehicles with multispectral sensors offer a cost-effective alternative to monitor invasive plant species at a centimetre scale. Giant Goldenrod (Solidago gigantea) is one of the most problematic invasive alien plant species in Switzerland and controlling this species â especially in nature protection areas â is a priority. In this study, a methodology is developed to detect the Giant Goldenrod coverage via unmanned aerial vehicle (UAV) equipped with multispectral sensors. Very high resolution maps (6.5 cm) are produced and high accuracy is achieved for the classification of the Giant Goldenrod coverage with a kappa coefficient of 0.902 and an overall accuracy of 92.12%. These results indicate that UAV equipped with multispectral sensors is a valuable tool in monitoring and combatting invasive alien species
Foundational nonuniform (co)datatypes for higher-order logic
Nonuniform (or ânestedâ or âheterogeneousâ) datatypes are recursively defined types in which the type arguments vary recursively. They arise in the implementation of finger trees and other efficient functional data structures. We show how to reduce a large class of nonuniform datatypes and codatatypes to uniform types in higher-order logic. We programmed this reduction in the Isabelle/HOL proof assistant, thereby enriching its specification language. Moreover, we derive (co)recusion and (co)induction principles based on a weak variant of parametricity
Bleiben Sie zuhause! : Einfluss der Covid-19-Pandemie auf die repetitiven und restriktiven Verhaltensweisen von Kindern und Jugendlichen mit einer Autismus-Spektrum-Störung
Darstellung des Themas: Viele Kinder mit einer Autismus-Spektrum-Störung (ASS) zeigen in herausfordernden Situationen repetitive und restriktive Verhaltensweisen (RRV) (Caldwell-Harris, 2021). Solche herausfordernden Situationen traten vermehrt wÀhrend der Covid-19-Pandemie auf (Ameis et al., 2020).
Ziel: Ziel dieser Arbeit ist herauszufinden, welchen Einfluss die Coronapandemie auf die RRV von Kindern und Jugendlichen mit ASS zwischen zwei und achtzehn Jahren wÀhrend den Schulschliessungen hatte.
Methode: Durch eine systematische Literaturrecherche konnten vier Hauptstudien ausgewĂ€hlt werden. Diese wurden nach Law et al. (1998) kritisch gewĂŒrdigt. Die Resultate wurden mit dem F-Words Konzept nach Rosenbaum & Gorter (2012) analysiert und diskutiert.
Relevante Ergebnisse: Die Mehrheit der RRV und der Stress haben bei Kindern mit ASS wĂ€hrend den Schulschliessungen zugenommen. Durch die Analyse mit den FWords konnte festgestellt werden, dass der vermehrte Stress wĂ€hrend der Pandemie der wesentliche Auslöser fĂŒr das Auftreten der RRV war.
Schlussfolgerung: In ausserordentlichen Situationen wie der Coronapandemie ist es wichtig, die Therapien und FreizeitaktivitĂ€ten weiter aufrechtzuerhalten. Zudem sollten in der Ergotherapie Strategien fĂŒr den Umgang mit Stress erlernt und die Eltern der Kinder mit ASS miteinbezogen werden, um die RRV zu reduzieren
Real-time OCT feedback-controlled RPE photodisruption in ex vivo porcine eyes using 8 microsecond laser pulses
Selective retinal pigment epithelium (RPE) photodisruption requires reliable real-time feedback dosimetry (RFD) to prevent unwanted overexposure. In this study, optical coherence tomography (OCT) based RFD was investigated in ex vivo porcine eyes exposed to laser pulses of 8 ”s duration (wavelength: 532â
nm, exposure area: 90âĂâ90â
”m2, radiant exposure: 247 to 1975 mJ/”m2). For RFD, fringe washouts in time-resolved OCT M-scans (central wavelength: 870â
nm, scan rate: 85 kHz) were compared to an RPE cell viability assay. Statistical analysis revealed a moderate correlation between RPE lesion size and applied treatment energy, suggesting RFD adaptation to inter- and intraindividual RPE pigmentation and ocular transmission
Unraveling, contributing factors to the severity of postprandial hypoglycemia after gastric bypass surgery.
BACKGROUND
Despite the increasing prevalence of postbariatric hypoglycemia (PBH), a late metabolic complication of bariatric surgery, our understanding of its diverse manifestations remains incomplete.
OBJECTIVES
To contrast parameters of glucose-insulin homeostasis in 2 distinct phenotypes of PBH (mild versus moderate hypoglycemia) based on nadir plasma glucose.
SETTING
University Hospital (Bern, Switzerland).
METHODS
Twenty-five subjects with PBH following gastric bypass surgery (age, 41 ± 12 years; body mass index, 28.1 ± 6.1kg/m2) received 75g of glucose with frequent blood sampling for glucose, insulin, C-peptide, and glucagon-like peptide 1 (GLP)-1. Based on nadir plasma glucose (</â„50mg/dL), subjects were grouped into level 1 (L1) and level 2 (L2) PBH groups. Beta-cell function (BCF), GLP-1 exposure (λ), beta-cell sensitivity to GLP-1 (Ï), potentiation of insulin secretion by GLP-1 (PI), first-pass hepatic insulin extraction (HE), insulin sensitivity (SI), and rate of glucose appearance (Ra) were calculated using an oral model of GLP-1 action coupled with the oral minimal model.
RESULTS
Nadir glucose was 43.3 ± 6.0mg/dL (mean ± standard deviation) and 60.1 ± 9.1mg/dL in L2- and L1-PBH, respectively. Insulin exposure was significantly higher in L2 versus L1 (P = .004). Mathematical modeling revealed higher BCF in L2 versus L1 (34.3 versus 18.8 10-9âmin-1; P = .003). Despite an increased GLP-1 exposure in L2 compared to L1 PBH (50.7 versus 31.9pmolâL-1âminâ102; P = .021), no significant difference in PI was observed (P = .204). No significant differences were observed for HE, Ra, and SI.
CONCLUSIONS
Our results suggest that higher insulin exposure in PBH patients with lower postprandial nadir glucose values mainly relate to a higher responsiveness to glucose, rather than GLP-1
Genome-wide RNAi screen identifies novel players in human 60S subunit biogenesis including key enzymes of polyamine metabolism
Ribosome assembly is an essential process that is linked to human congenital diseases and tumorigenesis. While great progress has been made in deciphering mechanisms governing ribosome biogenesis in eukaryotes, an inventory of factors that support ribosome synthesis in human cells is still missing, in particular regarding the maturation of the large 60S subunit. Here, we performed a genome-wide RNAi screen using an imaging-based, single cell assay to unravel the cellular machinery promoting 60S subunit assembly in human cells. Our screen identified a group of 310 high confidence factors. These highlight the conservation of the process across eukaryotes and reveal the intricate connectivity of 60S subunit maturation with other key cellular processes, including splicing, translation, protein degradation, chromatin organization and transcription. Intriguingly, we also identified a cluster of hits comprising metabolic enzymes of the polyamine synthesis pathway. We demonstrate that polyamines, which have long been used as buffer additives to support ribosome assembly in vitro, are required for 60S maturation in living cells. Perturbation of polyamine metabolism results in early defects in 60S but not 40S subunit maturation. Collectively, our data reveal a novel function for polyamines in living cells and provide a rich source for future studies on ribosome synthesis.Peer reviewe
MYCN mediates cysteine addiction and sensitizes neuroblastoma to ferroptosis
Aberrant expression of MYC transcription factor family members predicts poor clinical outcome in many human cancers. Oncogenic MYC profoundly alters metabolism and mediates an antioxidant response to maintain redox balance. Here we show that MYCN induces massive lipid peroxidation on depletion of cysteine, the rate-limiting amino acid for glutathione (GSH) biosynthesis, and sensitizes cells to ferroptosis, an oxidative, non-apoptotic and iron-dependent type of cell death. The high cysteine demand of MYCN-amplified childhood neuroblastoma is met by uptake and transsulfuration. When uptake is limited, cysteine usage for protein synthesis is maintained at the expense of GSH triggering ferroptosis and potentially contributing to spontaneous tumor regression in low-risk neuroblastomas. Pharmacological inhibition of both cystine uptake and transsulfuration combined with GPX4 inactivation resulted in tumor remission in an orthotopic MYCN-amplified neuroblastoma model. These findings provide a proof of concept of combining multiple ferroptosis targets as a promising therapeutic strategy for aggressive MYCN-amplified tumors
