Exertional rhabdomyolysis in a 21-year-old, healthy female after performing three sets of the biceps curl exercise to failure with 30% 1RM: A case report
Background: The optimal resistance training program to elicit muscle hypertrophy has been consistently debated and researched. Although 3 sets of 10 repetitions at 70-80% of the 1-repetition maximum (1RM) is widely recommended, recent studies have shown that low-load (~30% 1RM), high-repetition (3 sets of 30-40 repetitions) resistance training can elicit similar muscular hypertrophy. Therefore, this type of resistance training has gained popularity, perhaps because less weight is lifted for a longer duration. In the process of testing this hypothesis in a research study in our laboratory, a subject diagnosed with exertional rhabdomyolysis after completing a single resistance training session that involved 3 sets to failure at 30% 1RM. Exertional rhabdomyolysis is a condition characterized by the excessive breakdown of striated skeletal muscle that releases proteins from the muscle cell, particularly myoglobin, into the blood that can be toxic to the kidneys and is a significant health concern. Case Report: Reviewed were the events leading up to and throughout the diagnosis of exertional rhabdomyolysis in a healthy, recreationally-trained, 21-year-old female that was enrolled in a study that compared the acute effects of the traditional high-load, low-repetition versus low-load, high-repetition resistance training. The subject completed a total of 143 repetitions of the bilateral dumbbell bicep curl exercise. Three days post-exercise she reported excessive muscle soreness and swelling and sought medical attention. She was briefly hospitalized and then discharged with instructions to take acetaminophen for soreness, drink plenty of water, rest, and monitor her creatine kinase (CK) concentrations. Changes in the subject’s CK concentrations, ultrasound-determined muscle thickness and echo intensity were monitored over a 14-day period are reported. Discussion: This case illustrates the potential risk of developing exertional rhabdomyolysis after a low-load, high-repetition resistance training session in healthy, young, recreationally-trained women. The fact that exertional rhabdomyolysis is a possible outcome is enough to warrant caution when prescribing this type of resistance exercise
Phase-slip induced dissipation in an atomic Bose-Hubbard system
Phase slips play a primary role in dissipation across a wide spectrum of
bosonic systems, from determining the critical velocity of superfluid helium to
generating resistance in thin superconducting wires. This subject has also
inspired much technological interest, largely motivated by applications
involving nanoscale superconducting circuit elements, e.g., standards based on
quantum phase-slip junctions. While phase slips caused by thermal fluctuations
at high temperatures are well understood, controversy remains over the role of
phase slips in small-scale superconductors. In solids, problems such as
uncontrolled noise sources and disorder complicate the study and application of
phase slips. Here we show that phase slips can lead to dissipation for a clean
and well-characterized Bose-Hubbard (BH) system by experimentally studying
transport using ultra-cold atoms trapped in an optical lattice. In contrast to
previous work, we explore a low velocity regime described by the 3D BH model
which is not affected by instabilities, and we measure the effect of
temperature on the dissipation strength. We show that the damping rate of
atomic motion-the analogue of electrical resistance in a solid-in the confining
parabolic potential fits well to a model that includes finite damping at zero
temperature. The low-temperature behaviour is consistent with the theory of
quantum tunnelling of phase slips, while at higher temperatures a cross-over
consistent with the transition to thermal activation of phase slips is evident.
Motion-induced features reminiscent of vortices and vortex rings associated

A genetic programming based fuzzy regression approach to modelling manufacturing processes
Fuzzy regression has demonstrated its ability to model manufacturing processes in which the processes have fuzziness and the number of experimental data sets for modelling them is limited. However, previous studies only yield fuzzy linear regression based process models in which variables or higher order terms are not addressed. In fact, it is widely recognised that behaviours of manufacturing processes do often carry interactions among variables or higher order terms. In this paper, a genetic programming based fuzzy regression GP-FR, is proposed for modelling manufacturing processes. The proposed method uses the general outcome of GP to construct models the structure of which is based on a tree representation, which could carry interaction and higher order terms. Then, a fuzzy linear regression algorithm is used to estimate the contributions and the fuzziness of each branch of the tree, so as to determine the fuzzy parameters of the genetic programming based fuzzy regression model.To evaluate the effectiveness of the proposed method for process modelling, it was applied to the modelling of a solder paste dispensing process. Results were compared with those based on statistical regression and fuzzy linear regression. It was found that the proposed method can achieve better goodness-of-fitness than the other two methods. Also the prediction accuracy of the model developed based on GP-FR is better than those based on the other two methods
c-erbB-2 is not a major factor in the development of colorectal cancer
We have investigated c-erbB-2 protein expression in a large cohort of well-characterized colorectal tumours, and in a subset of lymph node metastases. We have also evaluated a Val655Ile single nucleotide polymorphism, which is associated with an increased risk of breast cancer, in a subset of the colorectal cancer patients and in healthy control subjects. Immunohistochemical studies revealed that while 81.8% of tumours expressed c-erbB-2, in the majority of cases equivalent levels of c-erb-B2 were seen in adjacent normal mucosa. Colon tumours were significantly more likely to express c-erbB-2 than rectal tumours (P=0.015). Only 52.4% of the metastases displayed staining patterns concordant with their primary tumour, indicating that determination of c-erbB-2 protein in colorectal tumours cannot predict the status of lymph node metastases. PCR–RFLP analysis of the Val655Ile single nucleotide polymorphism demonstrated that allele frequencies were identical between colorectal cancer patients and a control group of Caucasian subjects (Ile=0.80 and Val=0.20 in each case), indicating that it is not related to the risk of developing colorectal cancer in this population. Furthermore, there was no relationship between c-erbB-2 protein expression and gene polymorphism (P=0.58). In terms of prognosis, no association was seen between either c-erbB-2 protein expression or the presence of the Val allele and patient survival (P>0.05 in each case), suggesting that c-erbB-2 is not a prognostic marker in colorectal cancer
Rhinosinusitis derived Staphylococcal enterotoxin B possibly associates with pathogenesis of ulcerative colitis
BACKGROUND: During clinical practice, we noticed that some patients with both ulcerative colitis (UC) and chronic rhinosinusitis (CRS) showed amelioration of UC after treatment of CRS. This study was designed to identify a possible association between CRS and UC. METHODS: Thirty-two patients with both CRS and UC received treatment with functional endoscopic sinus surgery (FESS) for CRS. Clinical symptom scores for CRS and UC, as well as serum levels of anti-Staphylococcal enterotoxin B (SEB) were evaluated at week 0 and week 12. Sinus wash fluid SEB content was measured with enzyme-linked immunosorbent assay (ELISA). The surgically removed tissues were cultured to identify growth of Staphylococcus. aureus (S. aureus). Immunohistochemistry was employed to identify anti-SEB positive cells in the colonic mucosa. Colonic biopsies were obtained and incubated with SEB. Mast cell activation in the colonic mucosa in response to incubation with SEB was observed with electron microscopy and immunoassay. RESULTS: The clinical symptom scores of CRS and UC severe scores (UCSS) were significantly reduced in the UC-CRS patients after FESS. The number of cultured S. aureus colonies from the surgically removed sinus mucosa significantly correlated with the decrease in UCSS. High levels of SEB were detected in the sinus wash fluids of the patients with UC-CRS. Histamine and tryptase release was significantly higher in the culture supernate in the patients with UC-CRS than the patients with UC-only and normal controls. Anti-SEB positive cells were located in the colonic mucosa. CONCLUSION: The pathogenesis of UC in some patients may be associated with their pre-existing CRS by a mechanism of swallowing sinusitis-derived SEB. We speculate that SEB initiates inappropriate immune reactions and inflammation in the colonic mucosa that further progresses to UC
The impact of albendazole treatment on the incidence of viral- and bacterial-induced diarrhea in school children in southern Vietnam: study protocol for a randomized controlled trial
Anthelmintics are one of the more commonly available classes of drugs to treat infections by parasitic helminths (especially nematodes) in the human intestinal tract. As a result of their cost-effectiveness, mass school-based deworming programs are becoming routine practice in developing countries. However, experimental and clinical evidence suggests that anthelmintic treatments may increase susceptibility to other gastrointestinal infections caused by bacteria, viruses, or protozoa. Hypothesizing that anthelmintics may increase diarrheal infections in treated children, we aim to evaluate the impact of anthelmintics on the incidence of diarrheal disease caused by viral and bacterial pathogens in school children in southern Vietnam.This is a randomized, double-blinded, placebo-controlled trial to investigate the effects of albendazole treatment versus placebo on the incidence of viral- and bacterial-induced diarrhea in 350 helminth-infected and 350 helminth-uninfected Vietnamese school children aged 6-15 years. Four hundred milligrams of albendazole, or placebo treatment will be administered once every 3 months for 12 months. At the end of 12 months, all participants will receive albendazole treatment. The primary endpoint of this study is the incidence of diarrheal disease assessed by 12 months of weekly active and passive case surveillance. Secondary endpoints include the prevalence and intensities of helminth, viral, and bacterial infections, alterations in host immunity and the gut microbiota with helminth and pathogen clearance, changes in mean z scores of body weight indices over time, and the number and severity of adverse events.In order to reduce helminth burdens, anthelmintics are being routinely administered to children in developing countries. However, the effects of anthelmintic treatment on susceptibility to other diseases, including diarrheal pathogens, remain unknown. It is important to monitor for unintended consequences of drug treatments in co-infected populations. In this trial, we will examine how anthelmintic treatment impacts host susceptibility to diarrheal infections, with the aim of informing deworming programs of any indirect effects of mass anthelmintic administrations on co-infecting enteric pathogens.ClinicalTrials.gov: NCT02597556 . Coupling models of cattle and farms with models of badgers for predicting the dynamics of bovine tuberculosis (TB)
Bovine TB is a major problem for the agricultural industry in several
countries. TB can be contracted and spread by species other than cattle and
this can cause a problem for disease control. In the UK and Ireland, badgers
are a recognised reservoir of infection and there has been substantial
discussion about potential control strategies. We present a coupling of
individual based models of bovine TB in badgers and cattle, which aims to
capture the key details of the natural history of the disease and of both
species at approximately county scale. The model is spatially explicit it
follows a very large number of cattle and badgers on a different grid size for
each species and includes also winter housing. We show that the model can
replicate the reported dynamics of both cattle and badger populations as well
as the increasing prevalence of the disease in cattle. Parameter space used as
input in simulations was swept out using Latin hypercube sampling and
sensitivity analysis to model outputs was conducted using mixed effect models.
By exploring a large and computationally intensive parameter space we show that
of the available control strategies it is the frequency of TB testing and
whether or not winter housing is practised that have the most significant
effects on the number of infected cattle, with the effect of winter housing
becoming stronger as farm size increases. Whether badgers were culled or not
explained about 5%, while the accuracy of the test employed to detect infected
cattle explained less than 3% of the variance in the number of infected cattle
Radiation-hypersensitive cancer patients do not manifest protein expression abnormalities in components of the nonhomologous end-joining (NHEJ) pathway
Radiation therapy (RT) is utilised for the treatment of around half of all oncology patients during the course of their illness. Despite great clinical progress in the rational deployment of RT, the underlying molecular basis for its efficacy and toxicity are currently imperfectly understood. In this study, we took a biochemical approach to evaluate the potential role of key ionising radiation repair proteins in the treatment outcomes of patients with severe acute or late RT side effects. Lymphoblastoid cell lines were established from blood samples from 36 radiosensitive cases and a number of controls (the latter had had RT but did not develop significant toxicity). The expression level and migration of key proteins from the nonhomologous end-joining (NHEJ) pathway was evaluated by Western blot analysis on cases and controls. We did not observe any abnormalities in expression level or migration pattern of the following NHEJ proteins in radiosensitive cancer cases: Ku70, Ku80, XRCC4, DNA Ligase IV. These important negative results provide evidence that mutations that affect protein expression of these NHEJ components are unlikely to underlie clinical radiation sensitivity
Crystal Structures of Two Aminoglycoside Kinases Bound with a Eukaryotic Protein Kinase Inhibitor
Antibiotic resistance is recognized as a growing healthcare problem. To address this issue, one strategy is to thwart the causal mechanism using an adjuvant in partner with the antibiotic. Aminoglycosides are a class of clinically important antibiotics used for the treatment of serious infections. Their usefulness has been compromised predominantly due to drug inactivation by aminoglycoside-modifying enzymes, such as aminoglycoside phosphotransferases or kinases. These kinases are structurally homologous to eukaryotic Ser/Thr and Tyr protein kinases and it has been shown that some can be inhibited by select protein kinase inhibitors. The aminoglycoside kinase, APH(3′)-IIIa, can be inhibited by CKI-7, an ATP-competitive inhibitor for the casein kinase 1. We have determined that CKI-7 is also a moderate inhibitor for the atypical APH(9)-Ia. Here we present the crystal structures of CKI-7-bound APH(3′)-IIIa and APH(9)-Ia, the first structures of a eukaryotic protein kinase inhibitor in complex with bacterial kinases. CKI-7 binds to the nucleotide-binding pocket of the enzymes and its binding alters the conformation of the nucleotide-binding loop, the segment homologous to the glycine-rich loop in eurkaryotic protein kinases. Comparison of these structures with the CKI-7-bound casein kinase 1 reveals features in the binding pockets that are distinct in the bacterial kinases and could be exploited for the design of a bacterial kinase specific inhibitor. Our results provide evidence that an inhibitor for a subset of APHs can be developed in order to curtail resistance to aminoglycosides

Vulnerability assessment and feasibility analysis of seismic strengthening of school buildings
The majority of structures in seismic-prone areas worldwide are structures that have been designed either without seismic design considerations, or using codes of practice that are seriously inadequate in the light of current seismic design principles. In Cyprus, after a series of earthquakes that occurred between 1995 and 1999, it was decided to carry out an unprecedented internationally seismic retrofitting of all school buildings, taking into account the sensitivity of the society towards these structures. In this paper representative school buildings are analysed in both their pristine condition and after applying retrofitting schemes typical of those implemented in the aforementioned large-scale strengthening programme. Non-linear analysis is conducted on calibrated analytical models of the selected buildings and fragility curves are derived for typical reinforced concrete and unreinforced masonry structures. These curves are then used to carry out a feasibility study, including both benefit-cost and life-cycle analysis, and evaluate the effectiveness of the strengthening programme
