    Comparative analysis of the secondary electron yield from carbon nanoparticles and pure water medium

    The production of secondary electrons generated by carbon nanoparticles and pure water medium irradiated by fast protons is studied by means of model approaches and Monte Carlo simulations. It is demonstrated that due to a prominent collective response to an external field, the nanoparticles embedded in the medium enhance the yield of low-energy electrons. The maximal enhancement is observed for electrons in the energy range where plasmons, which are excited in the nanoparticles, play the dominant role. Electron yield from a solid carbon nanoparticle composed of fullerite, a crystalline form of C60 fullerene, is demonstrated to be several times higher than that from liquid water. Decay of plasmon excitations in carbon-based nanosystems thus represents a mechanism of increase of the low-energy electron yield, similar to the case of sensitizing metal nanoparticles.     Problems and possibilities on the margins: LGBT experiences in the 2011 Queensland floods

    Vulnerability to disasters is not inherent to particular social groups but results from existing marginality. Marginalisation from social, political and economic resources and recognition underpins vulnerability and impedes recovery. Yet concurrently, disasters can reveal the resilient capacities of some marginal groups, who often develop specific means of coping with marginality. This article applies these perspectives to the experiences of LGBT (lesbian, gay, bisexual, trans) people during the 2011 disaster in Queensland, Australia, which resulted from catastrophic flooding of Brisbane and South-East Queensland. The findings come from a survey conducted by the Queensland Association for Healthy Communities (QAHC) a year after the floods, which sought to understand LGBT experiences, resources and needs. An agreement was established between QAHC and university researchers to facilitate data analysis. This article analyses some key findings using the concept of marginality to understand both vulnerability and resilience. This framework helps grasp the particular issues facing LGBT people. The data reveal vulnerability due to social and political marginality, including discrimination and inhibited access to assistance, but simultaneously examples of resilience borne by self-reliance and coping strategies developed in a context of marginality. Understanding LGBT marginality, vulnerability and resilience helps contribute to inclusive and effective disaster preparation, response and recovery.Australian Research Council-DP13010265

    Global Analysis of Transcription Start Sites and Enhancers in Endometrial Stromal Cells and Differences Associated with Endometriosis.

    Identifying tissue-specific molecular signatures of active regulatory elements is critical to understanding gene regulatory mechanisms. In this study, transcription start sites (TSS) and enhancers were identified using Cap analysis of gene expression (CAGE) across endometrial stromal cell (ESC) samples obtained from women with (n = 4) and without endometriosis (n = 4). ESC TSSs and enhancers were compared to those reported in other tissue and cell types in FANTOM5 and were integrated with RNA-seq and ATAC-seq data from the same samples for regulatory activity and network analyses. CAGE tag count differences between women with and without endometriosis were statistically tested and tags within close proximity to genetic variants associated with endometriosis risk were identified. Over 90% of tag clusters mapping to promoters were observed in cells and tissues in FANTOM5. However, some potential cell-type-specific promoters and enhancers were also observed. Regions of open chromatin identified using ATAC-seq provided further evidence of the active transcriptional regions identified by CAGE. Despite the small sample number, there was evidence of differences associated with endometriosis at 210 consensus clusters, including IGFBP5, CALD1 and OXTR. ESC TSSs were also located within loci associated with endometriosis risk from genome-wide association studies. This study provides novel evidence of transcriptional differences in endometrial stromal cells associated with endometriosis and provides a valuable cell-type specific resource of active TSSs and enhancers in endometrial stromal cells

    Genotype-free demultiplexing of pooled single-cell RNA-seq.

    A variety of methods have been developed to demultiplex pooled samples in a single cell RNA sequencing (scRNA-seq) experiment which either require hashtag barcodes or sample genotypes prior to pooling. We introduce scSplit which utilizes genetic differences inferred from scRNA-seq data alone to demultiplex pooled samples. scSplit also enables mapping clusters to original samples. Using simulated, merged, and pooled multi-individual datasets, we show that scSplit prediction is highly concordant with demuxlet predictions and is highly consistent with the known truth in cell-hashing dataset. scSplit is ideally suited to samples without external genotype information and is available at: https://github.com/jon-xu/scSplit

    A realist evaluation of an enhanced court‐based liaison and diversion service for defendants with neurodevelopmental disorders

    Background: In England, court‐based mental health liaison and diversion (L&D) services work across courts and police stations to support those with severe mental illness and other vulnerabilities. However, the evidence around how such services support those with neurodevelopmental disorders (NDs) is limited. Aims: This study aimed to evaluate, through the lens of court and clinical staff, the introduction of a L&D service for defendants with NDs, designed to complement the existing L&D service. Methods: A realist evaluation was undertaken involving multiple agencies based within an inner‐city Magistrates' Court in London, England. We developed a logic model based on the initial programme theory focusing on component parts of the new enhanced service, specifically training, screening, signposting and interventions. We conducted semi‐structured interviews with the court staff, judiciary and clinicians from the L&D service. Results: The L&D service for defendants with NDs was successful in identifying and supporting the needs of those defendants. Benefits of this service included knowledge sharing, awareness raising and promoting good practice such as making reasonable adjustments. However, there were challenges for the court practitioners and clinicians in finding and accessing local specialist community services. Conclusion: A L&D service developed for defendants with NDs is feasible and beneficial to staff and clinicians who worked in the court setting leading to good practice being in place for the defendants. Going forward, a local care pathway would need to be agreed between commissioners and stakeholders including the judiciary to ensure timely and equitable access to local services by both defendants and practitioners working across diversion services for individuals with NDs

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P < 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Investigation of the use of novel ceramic oxide nanoparticles to improve the clinical outcome of radiation therapies

    The use of nanoparticles (NPs) to enhance the efficacy of different radiotherapy techniques is investigated by means of Monte Carlo simulation in this PhD project. The dose enhancement of radiation therapy in order to increase the ratio of tumour cell killing to normal tissue damage is an area of much interest in the medical physics research community. With increasing spatial and energy modulation of radiotherapy beams, the dose to tumour tissue can be intensified whilst sparing neighbouring healthy tissue. The advancement in capabilities of beam modulation does however have limitations and this is why methods of dose escalation preferential to cancerous tumour cells are an interesting area of current research. Nanoparticle dose enhancement shows great potential for the optimisation of radiation therapy modalities which is demonstrated in simulations and by experimental evidence. In order for NP dose enhancement to be applied in a clinical radiotherapy setting, a thorough fundamental study of the mechanism of dose enhancement is required. The mechanism of physical dose enhancement of radiation therapy modalities, including conventional X-ray and proton therapy, is studied by means of Geant4 Monte Carlo simulations presented in this thesis. This PhD thesis presents simulation studies on both micro- and nano-scales calculating dose enhancement from novel ceramic oxide NPs, compared to widely studied, dose enhancing gold NPs. Customised Geant4 simulations have been developed in this project to model dose enhancement in water volumes in proximity to various NPs, approximating the conditions of radiation dose enhancement in cells or tissue. The effect of size, shape, concentration and distribution of the NPs within a cell volume is investigated by means of Geant4 simulations

    This paper examines a group of Aboriginal bark-paintings featuring Macassan prau s that were collected in 1948 during the American-Australian Scientific Expedition to Arnhem Land. These paintings were studied in an effort to understand issues related to Aboriginal and Macassan cross-cultural interaction, stylistic adaptation and change, and maritime technologies. Furthermore it is one example in which the collaborative efforts of maritime and Indigenous archaeologies can achieve a more holistic technological and stylistic cross-analysis of watercraft imagery
