Podoconiosis and soil-transmitted helminths (STHs): double burden of neglected tropical diseases in Wolaita zone, rural southern Ethiopia
Background
Both podoconiosis and soil-transmitted helminth (STH) infections occur among barefoot people in areas of extreme poverty; however, their co-morbidity has not previously been investigated. We explored the overlap of STH infection and podoconiosis in Southern Ethiopia and quantified their separate and combined effects on prevalent anemia and hemoglobin levels in podoconiosis patients and health controls from the same area.
Methods and Principal Findings
A two-part comparative cross-sectional study was conducted in Wolaita zone, southern Ethiopia. Data were collected from adult patients presenting with clinically confirmed podoconiosis, and unmatched adult neighborhood controls living in the same administrative area. Information on demographic and selected lifestyle factors was collected using interviewer-administered questionnaires. Stool samples were collected and examined qualitatively using the modified formalin-ether sedimentation method. Hemoglobin level was determined using two different methods: hemoglobinometer and automated hematology analyzer. A total of 913 study subjects (677 podoconiosis patients and 236 controls) participated. The prevalence of any STH infection was 47.6% among patients and 33.1% among controls (p<0.001). The prevalence of both hookworm and Trichuris trichiura infections was significantly higher in podoconiosis patients than in controls (AOR 1.74, 95% CI 1.25 to2.42, AOR 6.53, 95% CI 2.34 to 18.22, respectively). Not wearing shoes and being a farmer remained significant independent predictors of infection with any STH. There was a significant interaction between STH infection and podoconiosis on reduction of hemoglobin level (interaction p value = 0.002).
Conclusions
Prevalence of any STH and hookworm infection was higher among podoconiosis patients than among controls. A significant reduction in hemoglobin level was observed among podoconiosis patients co-infected with hookworm and ‘non-hookworm STH’. Promotion of consistent shoe-wearing practices may have double advantages in controlling both podoconiosis and hookworm infection in the study area
Experimental study of the atmospheric neutrino backgrounds for proton decay to positron and neutral pion searches in water Cherenkov detectors
The atmospheric neutrino background for proton decay to positron and neutral
pion in ring imaging water Cherenkov detectors is studied with an artificial
accelerator neutrino beam for the first time. In total, about 314,000 neutrino
events corresponding to about 10 megaton-years of atmospheric neutrino
interactions were collected by a 1,000 ton water Cherenkov detector (KT). The
KT charged-current single neutral pion production data are well reproduced by
simulation programs of neutrino and secondary hadronic interactions used in the
Super-Kamiokande (SK) proton decay search. The obtained proton to positron and
neutral pion background rate by the KT data for SK from the atmospheric
neutrinos whose energies are below 3 GeV is about two per megaton-year. This
result is also relevant to possible future, megaton-scale water Cherenkov

Search for coherent charged pion production in neutrino-carbon interactions
We report the result from a search for charged-current coherent pion
production induced by muon neutrinos with a mean energy of 1.3 GeV. The data
are collected with a fully active scintillator detector in the K2K
long-baseline neutrino oscillation experiment. No evidence for coherent pion
production is observed and an upper limit of is set on
the cross section ratio of coherent pion production to the total
charged-current interaction at 90% confidence level. This is the first
experimental limit for coherent charged pion production in the energy region of

Measurement of single charged pion production in the charged-current interactions of neutrinos in a 1.3 GeV wide band beam
Single charged pion production in charged-current muon neutrino interactions
with carbon is studied using data collected in the K2K long-baseline neutrino
experiment. The mean energy of the incident muon neutrinos is 1.3 GeV. The data
used in this analysis are mainly from a fully active scintillator detector,
SciBar. The cross section for single production in the resonance
region ( GeV/) relative to the charged-current quasi-elastic cross
section is found to be 0.734 . The energy-dependent cross
section ratio is also measured. The results are consistent with a previous

Evidence for muon neutrino oscillation in an accelerator-based experiment
We present results for muon neutrino oscillation in the KEK to Kamioka (K2K)
long-baseline neutrino oscillation experiment. K2K uses an accelerator-produced
muon neutrino beam with a mean energy of 1.3 GeV directed at the
Super-Kamiokande detector. We observed the energy dependent disappearance of
muon neutrino, which we presume have oscillated to tau neutrino. The
probability that we would observe these results if there is no neutrino
oscillation is 0.0050% (4.0 sigma).Comment: 5 pages, 4 figure
Cost of Community Integrated Prevention Campaign for Malaria, HIV, and Diarrhea in Rural Kenya
BACKGROUND: Delivery of community-based prevention services for HIV, malaria, and diarrhea is a major priority and challenge in rural Africa. Integrated delivery campaigns may offer a mechanism to achieve high coverage and efficiency. METHODS: We quantified the resources and costs to implement a large-scale integrated prevention campaign in Lurambi Division, Western Province, Kenya that reached 47,133 individuals (and 83% of eligible adults) in 7 days. The campaign provided HIV testing, condoms, and prevention education materials; a long-lasting insecticide-treated bed net; and a water filter. Data were obtained primarily from logistical and expenditure data maintained by implementing partners. We estimated the projected cost of a Scaled-Up Replication (SUR), assuming reliance on local managers, potential efficiencies of scale, and other adjustments. RESULTS: The cost per person served was 31.98 for the SUR. The SUR cost included 67% for commodities (mainly water filters and bed nets) and 20% for personnel. The SUR projected unit cost per person served, by disease, was 15.80 for diarrhea (filters and training), and $9.91 for HIV (test kits, counseling, condoms, and CD4 testing at each site). CONCLUSIONS: A large-scale, rapidly implemented, integrated health campaign provided services to 80% of a rural Kenyan population with relatively low cost. Scaling up this design may provide similar services to larger populations at lower cost per person

How much can we gain from improved efficiency? An examination of performance of national HIV/AIDS programs and its determinants in low- and middle-income countries
<p>Abstract</p> <p>Background</p> <p>The economic downturn exacerbates the inadequacy of resources for combating the worldwide HIV/AIDS pandemic and amplifies the need to improve the efficiency of HIV/AIDS programs.</p> <p>Methods</p> <p>We used data envelopment analysis (DEA) to evaluate efficiency of national HIV/AIDS programs in transforming funding into services and implemented a Tobit model to identify determinants of the efficiency in 68 low- and middle-income countries. We considered the change from the lowest quartile to the average value of a variable a "notable" increase.</p> <p>Results</p> <p>Overall, the average efficiency in implementing HIV/AIDS programs was moderate (49.8%). Program efficiency varied enormously among countries with means by quartile of efficiency of 13.0%, 36.4%, 54.4% and 96.5%. A country's governance, financing mechanisms, and economic and demographic characteristics influence the program efficiency. For example, if countries achieved a notable increase in "voice and accountability" (e.g., greater participation of civil society in policy making), the efficiency of their HIV/AIDS programs would increase by 40.8%. For countries in the lowest quartile of per capita gross national income (GNI), a notable increase in per capita GNI would increase the efficiency of AIDS programs by 45.0%.</p> <p>Conclusions</p> <p>There may be substantial opportunity for improving the efficiency of AIDS services, by providing more services with existing resources. Actions beyond the health sector could be important factors affecting HIV/AIDS service delivery.</p
Integrated HIV Testing, Malaria, and Diarrhea Prevention Campaign in Kenya: Modeled Health Impact and Cost-Effectiveness
Efficiently delivered interventions to reduce HIV, malaria, and diarrhea are essential to accelerating global health efforts. A 2008 community integrated prevention campaign in Western Province, Kenya, reached 47,000 individuals over 7 days, providing HIV testing and counseling, water filters, insecticide-treated bed nets, condoms, and for HIV-infected individuals cotrimoxazole prophylaxis and referral for ongoing care. We modeled the potential cost-effectiveness of a scaled-up integrated prevention campaign.We estimated averted deaths and disability-adjusted life years (DALYs) based on published data on baseline mortality and morbidity and on the protective effect of interventions, including antiretroviral therapy. We incorporate a previously estimated scaled-up campaign cost. We used published costs of medical care to estimate savings from averted illness (for all three diseases) and the added costs of initiating treatment earlier in the course of HIV disease.Per 1000 participants, projected reductions in cases of diarrhea, malaria, and HIV infection avert an estimated 16.3 deaths, 359 DALYs and 37,097 (reducing total averted costs to 32,000, the campaign saves an estimated 20.A mass, rapidly implemented campaign for HIV testing, safe water, and malaria control appears economically attractive
