    Application of a Self-Similar Pressure Profile to Sunyaev-Zel'dovich Effect Data from Galaxy Clusters

    We investigate the utility of a new, self-similar pressure profile for fitting Sunyaev-Zel'dovich (SZ) effect observations of galaxy clusters. Current SZ imaging instruments - such as the Sunyaev-Zel'dovich Array (SZA) - are capable of probing clusters over a large range in physical scale. A model is therefore required that can accurately describe a cluster's pressure profile over a broad range of radii, from the core of the cluster out to a significant fraction of the virial radius. In the analysis presented here, we fit a radial pressure profile derived from simulations and detailed X-ray analysis of relaxed clusters to SZA observations of three clusters with exceptionally high quality X-ray data: A1835, A1914, and CL J1226.9+3332. From the joint analysis of the SZ and X-ray data, we derive physical properties such as gas mass, total mass, gas fraction and the intrinsic, integrated Compton y-parameter. We find that parameters derived from the joint fit to the SZ and X-ray data agree well with a detailed, independent X-ray-only analysis of the same clusters. In particular, we find that, when combined with X-ray imaging data, this new pressure profile yields an independent electron radial temperature profile that is in good agreement with spectroscopic X-ray measurements.Comment: 28 pages, 6 figures, accepted by ApJ for publication (probably April 2009

    Toxicity associated with combination oxaliplatin plus fluoropyrimidine with or without cetuximab in the MRC COIN trial experience

    We present the preliminary toxicity data from the MRC COIN trial, a phase III randomised controlled trial of first-line therapy in advanced colorectal cancer, with particular reference to the addition of cetuximab to an oxaliplatin–fluoropyrimidine combination. A total of 804 patients were randomised between March 2005 and July 2006 from 78 centres throughout the United Kingdom. Patients were allocated to oxaliplatin plus fluoropyrimidine chemotherapy with or without the addition of weekly cetuximab. The choice of fluoropyrimidine (either 5-fluorouracil (5FU) or capecitabine) was decided by the treating physician and patient before randomisation. Toxicity data were collected from all patients. Two hundred and three patients received 5FU plus oxaliplatin (OxMdG, 25%), 333 oxaliplatin+capecitabine (Xelox, 41%), 102 received OxMdG+cetuximab (OxMdG+C, 13%) and 166 Xelox+cetuximab (21%). Percent grade 3/4 toxicities included diarrhoea 6, 15, 13 and 25%, nausea/vomiting 3, 7, 7 and 14% for OxMdG, Xelox, OxMdG+C and Xelox+C, respectively. Sixty-day all-cause mortality was 6, 5, 5 and 7%. Statistically significant differences were evident for patients receiving Xelox+cetuximab vs Xelox alone: diarrhoea relative risk (RR) 1.69 (1.17, 2.43, P=0.005) and nausea/vomiting RR 2.01 (1.16, 3.47, P=0.012). The excess toxicity observed in the oxaliplatin-, capecitabine-, cetuximab-treated patients led the trial management group to conclude that a capecitabine dose adjustment was required to maintain safety levels when using this regimen

    5-fluorouracil and folinic acid-induced mucositis: no effect of oral glutamine supplementation.

    In some clinical situations the endogenous production of glutamine may be insufficient to maintain optimal tissue structure and function such that glutamine becomes a conditionally essential amino acid. Studies in laboratory animals have demonstrated that glutamine supplementation can reduce the incidence and severity of cytotoxic-induced mucositis. This study examined the role of oral glutamine supplementation in the management of mucositis caused by 5-fluorouracil (5-FU) and folinic acid. Twenty-eight patients with gastrointestinal cancers were randomised to receive 16 g of glutamine per day for 8 days, or placebo, in a randomised double-blind trial before crossing over to the alternative supplement during the second treatment cycle. The supplement was well tolerated with no apparent adverse effects, but failed to have any significant effect on oral mucositis assessed by the patients or investigator. The possible reasons for this apparent lack of benefit are discussed

    A comparison of the strong lensing properties of the Sersic and the NFW profiles

    We investigate the strong lensing properties of the Sersic profile as an alternative to the NFW profile, focusing on applications to lens modelling of clusters. Given an underlying Sersic dark matter profile, we study whether an NFW profile can provide an acceptable fit to strong lensing constraints in the form of single or multiple measured Einstein radii. We conclude that although an NFW profile that fits the lensing constraints can be found in many cases, the derived parameters may be biased. In particular, we find that for n~2, which corresponds to massive clusters, the mass at r_200 of the best fit NFW is overestimated (by a factor of ~2) and the concentration is very low (c~2). The differences are important enough to warrant the inclusion of Sersic profile for future analysis of strong lensing clusters.Comment: 19 pages (single column format), 11 figures.     Costs and consequences of different chemotherapy regimens in metastatic colorectal cancer

    An economic sub-study was run alongside a large multi-centre randomised trial (MRC-CR06) comparing three chemotherapy regimens; de Gramont, Lokich and raltitrexed in patients with metastatic colorectal cancer. Patients in six of 45 centres in the main trial were approached to take part in the sub-study. Chemotherapy delivery costs were assessed in each sub-study centre with external validity verified by questionnaire to all other centres. Patient representativeness was assessed. Stochastic resource use data, including patient borne costs and non-hospital health service resource use were monitored prospectively. Mean total societal costs were de Gramont=£5051 (s.d. £1910), raltitrexed=£2616 (s.d. £991) and Lokich=£2576 (s.d. £1711). In pairwise comparisons, statistically significant mean total cost differences were shown for de Gramont vs Lokich (mean difference=£2475, 95%CI £914–£4037, P<0.01) and for de Gramont vs raltitrexed (mean difference=£2435, 95%CI £922–£2948, P<0.01). Sensitivity analyses showed little effect on overall costs. The main trial showed de Gramont and Lokich to be equally effective in terms of survival, quality of life and response rates but Lokich had higher toxicity and hand-foot syndrome. Raltitrexed showed similar response rates and overall survival but increased toxicity and inferior quality of life making it a clinically inferior regimen despite its ease of administration and costs. For a comparable clinical outcome, Lokich can be administered for approximately half the cost of de Gramont

    Hot Gas in Galaxy Groups: Recent Observations

    Galaxy groups are the least massive systems where the bulk of baryons begin to be accounted for. Not simply the scaled-down versions of rich clusters following self-similar relations, galaxy groups are ideal systems to study baryon physics, which is important for both cluster cosmology and galaxy formation. We review the recent observational results on the hot gas in galaxy groups. The first part of the paper is on the scaling relations, including X-ray luminosity, entropy, gas fraction, baryon fraction and metal abundance. Compared to clusters, groups have a lower fraction of hot gas around the center (e.g., r < r_2500), but may have a comparable gas fraction at large radii (e.g., r_2500 < r < r_500). Better constraints on the group gas and baryon fractions require sample studies with different selection functions and deep observations at r > r_500 regions. The hot gas in groups is also iron poor at large radii (0.3 r_500 - 0.7 r_500). The iron content of the hot gas within the central regions (r < 0.3 r_500) correlates with the group mass, in contrast to the trend of the stellar mass fraction. It remains to be seen where the missing iron in low-mass groups is. In the second part, we discuss several aspects of X-ray cool cores in galaxy groups, including their difference from cluster cool cores, radio AGN heating in groups and the cold gas in group cool cores. Because of the vulnerability of the group cool cores to radio AGN heating and the weak heat conduction in groups, group cool cores are important systems to test the AGN feedback models and the multiphase cool core models.     Contemporary sports nutrition guidelines recommend that each athlete develop a personalised, periodised and practical approach to eating that allows him or her to train hard, recover and adapt optimally, stay free of illness and injury and compete at their best at peak races. Competitive triathletes undertake a heavy training programme to prepare for three different sports while undertaking races varying in duration from 20 min to 10 h. The everyday diet should be adequate in energy availability, provide CHO in varying amounts and timing around workouts according to the benefits of training with low or high CHO availability and spread high-quality protein over the day to maximise the adaptive response to each session. Race nutrition requires a targeted and well-practised plan that maintains fuel and hydration goals over the duration of the specific event, according to the opportunities provided by the race and other challenges, such as a hot environment. Supplements and sports foods can make a small contribution to a sports nutrition plan, when medical supplements are used under supervision to prevent/treat nutrient deficiencies (e.g. iron or vitamin D) or when sports foods provide a convenient source of nutrients when it is impractical to eat whole foods. Finally, a few evidence-based performance supplements may contribute to optimal race performance when used according to best practice protocols to suit the triathlete’s goals and individual responsiveness

    The XXL Survey: XLVIII; X-ray follow-up of distant XXL clusters: Masses, scaling relations and AGN contamination

    We use deep follow-up XMM-Newton observations of 6 clusters discovered in the XXL Survey at z>1z>1 to gain robust measurements of their X-ray properties and to investigate the extent to which scaling relations at low redshift are valid at z>1z>1. This sample is unique as it has been investigated for AGN contamination, which ensures measurements are not undermined by systematic uncertainties, and pushes to lower mass at higher redshift than is usually possible, for example with Sunyaev-Zel'dovich (SZ) selected clusters. We determine the flux contribution of point sources to the XXL cluster flux in order to test for the presence of AGN in other high-redshift cluster candidates, and find 3XLSS J231626.8-533822 to be a point source misclassified as a cluster and 3XLSS J232737.3-541618 to be a genuine cluster. We present the first attempt to measure the hydrostatic masses in a bright subsample of z>1z>1 X-ray selected galaxy clusters with a known selection function. Periods of high particle background significantly reduced the effective exposure times of observations (losing >50% in some cases) limiting the power of this study. When combined with complementary SZ selected cluster samples at higher masses, the data appear broadly consistent with the self-similar evolution of the low redshift scaling relations between ICM properties and cluster mass, suggesting that properties such as the X-ray temperature, gas mass and SZ signal remain reliable mass proxies even at high redshift.Comment: 13 pages, 7 figures, published in MNRA
