Control of Vancomycin-Resistant Enterococcus in Health Care Facilities in a Region
Background
In late 1996, vancomycin-resistant enterococci were first detected in the Siouxland region of Iowa, Nebraska, and South Dakota. A task force was created, and in 1997 the assistance of the Centers for Disease Control and Prevention was sought in assessing the prevalence of vancomycin-resistant enterococci in the region’s facilities and implementing recommendations for screening, infection control, and education at all 32 health care facilities in the region.
Methods
The infection-control intervention was evaluated in October 1998 and October 1999. We performed point-prevalence surveys, conducted a case– control study of gastrointestinal colonization with vancomycin-resistant enterococci, and compared infection-control practices and screening policies for vancomycin-resistant enterococci at the acute care and long-term care facilities in the Siouxland region.
Results
Perianal-swab samples were obtained from 1954 of 2196 eligible patients (89 percent) in 1998 and 1820 of 2049 eligible patients (89 percent) in 1999. The overall prevalence of vancomycin-resistant enterococci at 30 facilities that participated in all three years of the study decreased from 2.2 percent in 1997 to 1.4 percent in 1998 and to 0.5 percent in 1999 (P
Conclusions
An active infection-control intervention, which includes the obtaining of surveillance cultures and the isolation of infected patients, can reduce or eliminate the transmission of vancomycinresistant enterococci in the health care facilities of a region. (N Engl J Med 2001;344:1427-33.

next-generation ground-based cosmic microwave background experiment, CMB-S4,
envisioned to consist of dedicated telescopes at the South Pole, the high
Chilean Atacama plateau and possibly a northern hemisphere site, all equipped
with new superconducting cameras. CMB-S4 will dramatically advance cosmological
studies by crossing critical thresholds in the search for the B-mode
polarization signature of primordial gravitational waves, in the determination
of the number and masses of the neutrinos, in the search for evidence of new
light relics, in constraining the nature of dark energy, and in testing general
relativity on large scales
Isokinetic eccentric exercise substantially improves mobility, muscle strength and size, but not postural sway metrics in older adults, with limited regression observed following a detraining period
© 2020, The Author(s). Introduction: Eccentric exercise can reverse age-related decreases in muscle strength and mass; however, no data exist describing its effects on postural sway. As the ankle may be more important for postural sway than hip and knee joints, and with older adults prone to periods of inactivity, the effects of two 6-week seated isokinetic eccentric exercise programmes, and an 8-week detraining period, were examined in 27 older adults (67.1 ± 6.0 years). Methods: Neuromuscular parameters were measured before and after training and detraining periods with subjects assigned to ECC (twice-weekly eccentric-only hip and knee extensor contractions) or ECCPF (identical training with additional eccentric-only plantarflexor contractions) training programmes. Results: Significant (P \u3c 0.05) increases in mobility (decreased timed-up-and-go time [− 7.7 to − 12.0%]), eccentric strength (39.4–58.8%) and vastus lateralis thickness (9.8–9.9%) occurred after both training programmes, with low-to-moderate weekly rate of perceived exertion (3.3–4.5/10) reported. No significant change in any postural sway metric occurred after either training programme. After 8 weeks of detraining, mobility (− 8.2 to − 11.3%), eccentric strength (30.5–50.4%) and vastus lateralis thickness (6.1–7.1%) remained significantly greater than baseline in both groups. Conclusion: Despite improvements in functional mobility, muscle strength and size, lower-limb eccentric training targeting hip, knee and ankle extensor muscle groups was not sufficient to influence static balance. Nonetheless, as the beneficial functional and structural adaptations were largely maintained through an 8-week detraining period, these findings have important implications for clinical exercise prescription as the exercise modality, low perceived training intensity, and adaptive profile are well suited to the needs of older adults

The Atacama Cosmology Telescope: Detection of Patchy Screening of the Cosmic Microwave Background
Spatial variations in the cosmic electron density after reionization generate
cosmic microwave background anisotropies via Thomson scattering, a process
known as the ``patchy screening" effect. In this paper, we propose a new
estimator for the patchy screening effect that is designed to mitigate biases
from the dominant foreground signals. We use it to measure the
cross-correlation between \textit{unWISE} galaxies and patchy screening, the
latter measured by the Atacama Cosmology Telescope and \textit{Planck}
satellite. We report the first detection of the patchy screening effect, with
the statistical significance of the cross-correlation exceeding . This
measurement directly probes the distribution of electrons around these galaxies
and provides strong evidence that gas is more extended than the underlying dark
matter. By comparing our measurements to electron profiles extracted from
simulations, we demonstrate the power of these observations to constrain galaxy
evolution models. Requiring only the 2D positions of objects and no individual
redshifts or velocity estimates, this approach is complementary to existing gas

Measurement of the cosmic ray spectrum above eV using inclined events detected with the Pierre Auger Observatory
A measurement of the cosmic-ray spectrum for energies exceeding
eV is presented, which is based on the analysis of showers
with zenith angles greater than detected with the Pierre Auger
Observatory between 1 January 2004 and 31 December 2013. The measured spectrum
confirms a flux suppression at the highest energies. Above
eV, the "ankle", the flux can be described by a power law with
index followed by
a smooth suppression region. For the energy () at which the
spectral flux has fallen to one-half of its extrapolated value in the absence
of suppression, we find

The Atacama Cosmology Telescope: Mitigating the impact of extragalactic foregrounds for the DR6 CMB lensing analysis
We investigate the impact and mitigation of extragalactic foregrounds for the
CMB lensing power spectrum analysis of Atacama Cosmology Telescope (ACT) data
release 6 (DR6) data. Two independent microwave sky simulations are used to
test a range of mitigation strategies. We demonstrate that finding and then
subtracting point sources, finding and then subtracting models of clusters, and
using a profile bias-hardened lensing estimator, together reduce the fractional
biases to well below statistical uncertainties, with the inferred lensing
amplitude, , biased by less than . We also show
that another method where a model for the cosmic infrared background (CIB)
contribution is deprojected and high frequency data from Planck is included has
similar performance. Other frequency-cleaned options do not perform as well,
incurring either a large noise cost, or resulting in biased recovery of the
lensing spectrum. In addition to these simulation-based tests, we also present
null tests performed on the ACT DR6 data which test for sensitivity of our
lensing spectrum estimation to differences in foreground levels between the two
ACT frequencies used, while nulling the CMB lensing signal. These tests pass
whether the nulling is performed at the map or bandpower level. The
CIB-deprojected measurement performed on the DR6 data is consistent with our
baseline measurement, implying contamination from the CIB is unlikely to
significantly bias the DR6 lensing spectrum. This collection of tests gives
confidence that the ACT DR6 lensing measurements and cosmological constraints
presented in companion papers to this work are robust to extragalactic
