    Testicular fusocellular rhabdomyosarcoma as a metastasis of elbow sclerosing rhabdomyosarcoma: A clinicopathologic, immunohistochemical and molecular study of one case

    Sclerosing rhabdomyosarcoma (SRMS) is an infrequent variant of rhabdomyosarcoma characterized by extensive intercellular hyaline fibrosis. We report the case of a 37 year-old male with a 9 × 6 cm SRMS on the right elbow. Histologically, the tumor showed an abundant extracellular hyaline matrix with extratumoral vascular emboli and microscopic foci of fusocellular embryonal rhabdomyosarcoma (FRMS) separated by a fibrotic band from the sclerosing areas. One year later the patient presented with a right intratesticular tumor of 1.2 × 0.8 cm, which was reported as pure FRMS. Immunohistochemically, SRMS was positive only for MyoD1 and Vimentin and negative for Myogenin and Desmin. Both the elbow emboli with the extratumoral foci of FRMS and the intratesticular tumor were positive for Myogenin, MyoD1, Vimentin and Desmin. Using fluorescent in situ hybridization (FISH), the SRMS and the FRMS tumor cells of the elbow and the FRMS tumor cells of the testis were found to be negative for FOXO1A translocation in chromosome 13. PCR chimeric transcriptional products PAX3-FKHR and PAX7-FKHR were not found. Six months following testicular resection, the patient died of multiple metastases in the mediastinum, lung and right thigh

    Identification of neuronal network properties from the spectral analysis of calcium imaging signals in neuronal cultures

    Neuronal networks in vitro are prominent systems to study the development of connections in living neuronal networks and the interplay between connectivity, activity and function. These cultured networks show a rich spontaneous activity that evolves concurrently with the connectivity of the underlying network. In this work we monitor the development of neuronal cultures, and record their activity using calcium fluorescence imaging. We use spectral analysis to characterize global dynamical and structural traits of the neuronal cultures. We first observe that the power spectrum can be used as a signature of the state of the network, for instance when inhibition is active or silent, as well as a measure of the network's connectivity strength. Second, the power spectrum identifies prominent developmental changes in the network such as GABAA switch. And third, the analysis of the spatial distribution of the spectral density, in experiments with a controlled disintegration of the network through CNQX, an AMPA-glutamate receptor antagonist in excitatory neurons, reveals the existence of communities of strongly connected, highly active neurons that display synchronous oscillations. Our work illustrates the interest of spectral analysis for the study of in vitro networks, and its potential use as a network-state indicator, for instance to compare healthy and diseased neuronal networks

    Actualización en el tratamiento del dolor orofacial

    En los últimos años se han desarrollado nuevos tratamientos en el dolor orofacial. En la cefalea en racimos se ha incorporado la administración de nuevos fármacos por vía intranasal. En el dolor masticatorio la aplicación tópica de capsaicina y el uso de férulas orales en combinación con ejercicios de movilización mandibular, son los tratamientos más utilizados. En el dolor neurógeno se han desarrollando nuevos fármacos anticonvulsivos con menos efectos secundarios que la carbamacepina. Los últimos avances farmacológicos se dirigen hacia el uso de AINES y opioides de forma tópica y hacia la combinación de distintos analgésicos.     Correcting loss of a papilla following orthodontic space opening (Atherton's patch) through implant supported rehabilitation. A case report

    The objective of this case report is to describe a surgical and prosthetic technique to create a lost papilla following orthodontic space opening (Atherton`Ls patch) through implant supported rehabilitation. A switching platform implant was used to replace a left maxillary canine in a unitary interdental edentulous ridge with Atherton'Ls patch in the distal area of the upper lateral left incisor. The radiographic study revealed correct level of the interproximal bone of the adjacent teeth. A mucoperiosteal flap with crest incision and sulcular extension to the adjacent teeth was made. Special attention was paid to correct position of the implant and the distance (. 1.5 mm) between the platform and the roots of the adjacent teeth. A submerged technique was used. Tissue modeling through provisional crown was performed in order to create an ideal emergence profile with total papilla fill recorded at the Atherton'Ls patch area. Final screw retained CAD-CAM zirconia structure was place. Final follow up was performed 2 years after provisional crown placement, and total fill of both papilla, including at Atherton'Ls patch area, was recorded

    Periapical surgery of maxillary posterior teeth : A review of the literature

    In recent years, periapical surgery has evolved thanks to new diagnostic and technical advances. A review is made of the literature on periapical surgery of the antral teeth, based on a Medline search and on the revision of Spanish dental journals in the period between 1974 and 2003. The anatomy of the maxillary sinus is discussed, along with the diagnosis of periapical lesions and the relation of the maxillary sinus to the antral teeth. The surgical technique, special considerations and prognosis of periapical surgery in these teeth are also addressed. Recent studies postulate that the proximity of the antral teeth to the maxillary sinus should not be viewed as a contraindication to periapical surgery, and recommend such surgery in teeth with chronic periapical disease that are refractory to conventional endodontic management, despite the proximity of the maxillary sinus

    Action semantics at the bottom of the brain: Insights from dysplastic cerebellar gangliocytoma

    Recent embodied cognition research shows that access to action verbs in shallow-processing tasks becomes selectively compromised upon atrophy of the cerebellum, a critical motor region. Here we assessed whether cerebellar damage also disturbs explicit semantic processing of action pictures and its integration with ongoing motor responses. We evaluated a cognitively preserved 33-year-old man with severe dysplastic cerebellar gangliocytoma (Lhermitte-Duclos disease), encompassing most of the right cerebellum and the posterior part of the left cerebellum. The patient and eight healthy controls completed two semantic association tasks (involving pictures of objects and actions, respectively) that required motor responses. Accuracy results via Crawford's modified t-tests revealed that the patient was selectively impaired in action association. Moreover, reaction-time analysis through Crawford's Revised Standardized Difference Test showed that, while processing of action concepts involved slower manual responses in controls, no such effect was observed in the patient, suggesting that motor-semantic integration dynamics may be compromised following cerebellar damage. Notably, a Bayesian Test for a Deficit allowing for Covariates revealed that these patterns remained after covarying for executive performance, indicating that they were not secondary to extra-linguistic impairments. Taken together, our results extend incipient findings on the embodied functions of the cerebellum, offering unprecedented evidence of its crucial role in processing non-verbal action meanings and integrating them with concomitant movements. These findings illuminate the relatively unexplored semantic functions of this region while calling for extensions of motor cognition models.Fil: Cervetto Manciameli, Sabrina Fabiana.
