    Enteric duplication cysts are rare congenital malformations with a low incidence and there are only a few reports in the literature. Their clinical presentation varies according to the location and the type of duplication. T heir overall prognosis is good if early surgical intervention is provided. We report a 2-month-old boy who presented with a case of a giant gastroduodenal duplication cyst with a juxta-pancreatic communication and was successfully treated surgically. It is imperative to be aware of this rare congenital malformation that can present clinically with a wide range of non-specific symptoms that can cause significant morbidity and mortality if the treatment is delayed

    Bilateral Internuclear Ophthalmoplegia in a Patient with Devic's Neuromyelitis Optica

    An unusual presentation of Devic's neuromyelitis optica (NMO) disease associated with bilateral internuclear ophthalmoplegia (INO) is described. A 32-year-old pregnant patient was diagnosed with NMO. First symptoms were headache and sudden visual loss in her right eye (RE). Eighteen months ago, she reported other neurologic symptoms such as paresthesia. Based on her visual field, fundoscopy and Ishihara test, she was diagnosed with retrobulbar neuritis of the RE. After delivery, new neurologic symptoms resembling transverse myelitis appeared. She was treated with methylprednisolone and plasmapheresis, which improved her visual acuity; however, a sudden bilateral INO appeared, with adduction defect and nystagmus with abduction in both eyes. No improvement was obtained after treatment with azathioprine and rituximab. Paresis of the legs and the right arm persisted, but double vision and OIN gradually disappeared. At the end, the patient had a residual exophoria in the RE and nystagmus with abduction in the left eye. Prevalence of NMO is lower than one case per one million inhabitants, and it is not likely to affect the encephalic trunk; furthermore, bilateral INO in NMO is rare. Two major criteria and at least two of the three minor ones are required to confirm a NMO diagnosis, and our patient fulfilled these diagnosis criteria

    Based on three case studies (Colombia, South East Asia and Mexico), the article analyzes land grabbing as a phenomenon linked with core crimes; The authors stipulate common characteristics of the phenomenon and propose the stipulation of land grabbing as an Economic Crime Against Humanity. In this task, the authors analyze the statute of the International Criminal Court, highlighting its inability to deal with the phenomenon, especially when it results in serious crimes. Seen from this perspective, the land grabbing is presented as a strategy rather than a tactic carried out by transnational companies that use, among others, crimes against humanity to carry out the land grab.A partir de tres casos de estudio (Colombia, Sud Este asiático y México) el artículo analiza el fenómeno del despojo de tierras o “land grabbing” identificando en cada uno de los casos las características comunes o “generales” del fenómeno con el objetivo de proponer la estipulación del despojo de tierras como un Crimen de Económico de Lesa Humanidad. En este propósito, los autores analizan el estatuto de la Corte Penal Internacional poniendo de presente su incapacidad para lidiar con el fenómeno, sobre todo cuando deriva en crímenes graves. Visto desde esta perspectiva el despojo de tierra se presenta como una estrategia o plan general de acción, no como una táctica, de empresas trasnacionales que se valen, entre otros, de crímenes de lesa humanidad para concretar el acaparamiento.Com base em três estudos de caso (Colômbia, Sudeste Asiático e México), o artigo analisa o “land grabbing” como fenômeno vinculado a crimes fundamentais; Os autores estipulam características comuns do fenômeno e propõem a tipificação da “land grabbing” como Crime Econômico contra a Humanidade.     Efficient and accurate algorithms for computing matrix trigonometric functions

    [EN] Trigonometric matrix functions play a fundamental role in second order differential equations. This work presents an algorithm based on Taylor series for computing the matrix cosine. It uses a backward error analysis with improved bounds. Numerical experiments show that MATLAB implementations of this algorithm has higher accuracy than other MATLAB implementations of the state of the art in the majority of tests. Furthermore, we have implemented the designed algorithm in language C for general purpose processors, and in CUDA for one and two NVIDIA GPUs. We obtained a very good performance from these implementations thanks to the high computational power of these hardware accelerators and our effort driven to avoid as much communications as possible.     Dynamic accessibility by car to tertiary care emergency services in Cali, Colombia, in 2020 : cross-sectional equity analyses using travel time big data from a Google API

    Objectives: To test a new approach to characterise accessibility to tertiary care emergency health services in urban Cali and assess the links between accessibility and sociodemographic factors relevant to health equity. Design: The impact of traffic congestion on accessibility to tertiary care emergency departments was studied with an equity perspective, using a web-based digital platform that integrated publicly available digital data, including sociodemographic characteristics of the population and places of residence with travel times. Setting and participants: Cali, Colombia (population 2.258million in 2020) using geographic and sociodemographic data. The study used predicted travel times downloaded for a week in July 2020 and a week in November 2020. Primary and secondary outcomes: The share of the population within a 15min journey by car from the place of residence to the tertiary care emergency department with the shortest journey (ie, 15min accessibility rate (15mAR)) at peak-traffic congestion hours. Sociodemographic characteristics were disaggregated for equity analyses. A time-series bivariate analysis explored accessibility rates versus housing stratification. Results: Traffic congestion sharply reduces accessibility to tertiary emergency care (eg, 15mAR was 36.8% during peak-traffic hours vs 84.4% during free-flow hours for the week of 6-12 July 2020). Traffic congestion sharply reduces accessibility to tertiary emergency care. The greatest impact fell on specific ethnic groups, people with less educational attainment and those living in low-income households or on the periphery of Cali (15mAR: 8.1% peak traffic vs 51% free-flow traffic). These populations face longer average travel times to health services than the average population. Conclusions: These findings suggest that health services and land use planning should prioritise travel times over travel distance and integrate them into urban planning. Existing technology and data can reveal inequities by integrating sociodemographic data with accurate travel times to health services estimates, providing the basis for valuable indicators

    Maternal imprinting on cognition markers of wild type and transgenic Alzheimer's disease model mice

    The risk of suffering from Alzheimer’s disease (AD) is higher in individuals from AD-affected mothers. The purpose of this investigation was to study whether maternal transmission might produce AD-related alterations in progenies of mice that do not have any genotypic alteration. We used cognitively-intact mothers harbouring in heterozygosity the transgene for overexpressing the Swedish double mutant version of the human amyloid precursor protein (hAβPPswe). The phenotype of the offspring with or without the transgene resulting from crossing young Tg2576 females with wild-type males were compared with those of the offspring resulting from crossing wild-type females with Tg2576 males. The hAβPPswe-bearing offspring from Tg2576 mothers showed an aggravated AD-like phenotype. Remarkably, cognitive, immunohistochemical and some biochemical features displayed by Tg2576 heterozygous mice were also found in wild-type animals generated from Tg2576 females. This suggests the existence of a maternal imprinting in the wild-type offspring that confers a greater facility to launch an AD-like neurodegenerative cascade. Such progeny, lacking any mutant amyloid precursor protein, constitutes a novel model to study maternal transmission of AD and, even more important, to discover early risk markers that predispose to the development of AD
