    Quality of life associated with treatment adherence in patients with type 2 diabetes: a cross-sectional study

    <p>Abstract</p> <p>Background</p> <p>Despite certain contradictions, an association has been identified between adherence to drug treatment and the quality of life in patients with type 2 diabetes. The contradictions observed emphasize the importance of using different methods to measure treatment adherence, or the association of psychological precursors of adherence with quality of life. For this reason, we have used an indirect method to measure adherence (pill count), as well as two adherence behaviour precursors (attitude and knowledge), to assess the association between adherence and the quality of life in type 2 diabetes patients.</p> <p>Methods</p> <p>A cross-sectional comparative study on a random sample of 238 type 2 diabetic patients was carried out over one year in four family medicine units of the Mexican Institute of Social Security (IMSS) in Aguascalientes, Mexico. Treatment adherence was measured using the indirect method of pill count to assess adherence behaviour, obtaining information at two home visits. In the first we recorded the medicine prescribed and in the second, we counted the medicine remaining to determine the proportion of the medicine taken. We also assessed two adherence behaviour precursors: the patients' knowledge regarding their medical prescription measured through a structured questionnaire; and attitudes to treatment adherence using a Likert scale. Quality of life was measured through the WHOQOL-100 (the WHO Quality of Life questionnaire). Information concerning both knowledge and attitude was obtained through interviews with the patients. A multiple linear regression model was constructed to establish the relationship between each quality of life domain and the variables related to adherence, controlling for covariates.</p> <p>Results</p> <p>There was no association between quality of life and treatment adherence behaviour. However, the combination of strong knowledge and a positive attitude was associated with five of the six quality of life domains.</p> <p>Conclusion</p> <p>The results suggest that it is important to explore psychological precursors of treatment adherence behaviour in type 2 diabetic patients. Indeed, we consider that it will be useful to carry out interventions that change negative attitudes towards treatment adherence and that promote medical prescription knowledge, which may help to improve the quality of life of such patients.</p

    Plant-wide modelling in wastewater treatment: showcasing experiences using the Biological Nutrient Removal Model

    [EN] Plant-wide modelling can be considered an appropriate approach to represent the current complexity in water resource recovery facilities, reproducing all known phenomena in the different process units. Nonetheless, novel processes and new treatment schemes are still being developed and need to be fully incorporated in these models.     Intelligent alarms detection for the analysis of system fault impact on business

    The tools for fault impact analysis are important for the deployment of critical mission systems. These tools can be also used as a development phase aid. We introduce several concepts related to "business alarms". Business alarms are an approximation to the company's business conceptual scheme driven by the business rules from systems conceptual schemes. In order to specify them we propose the utilization of Knowledge Engineering typical techniques. The object of alarm detection for impact analysis of fault business systems is to reduce the breach between business controls and typical control system. For it, furthermore specifying an alarm visualization system for the direction responsible of the systems, we are going to specificity the alarm transmission to a center with following purpose: Development support Help desk with an end user problems and resolution expert database Contingency maneuvers coordination The rush in the projects makes that most of the time the errors are not taken into account into the development phase, and a transactional structure for handling them is added later. Sometimes this structure is not completely implemented and the error handling is left to the database engines and operating systems mechanisms. The fact of the existence of a development support center can facilitate and allow the adding of code chunks with the purpose of centralized debugging. The support center support will have a rule based main kernel that will do a nexus between development and help desk to final users. This kernel will allow: give a greater quality help to final users relate user faults with systems faults in the development manager format. Moreover, the own nature of the objectives, this eases the acceptation of the knowledge engineer in the organization (one of the primary steps to make possible knowledge acquisition). It’s going to be a company’s direction responsibility to present him as the developer of this center and help desk for development and final users. The role of the knowledge engineer covers a wide spectrum in a critical mission environment, from the support of the original design to the startup and tuning of the end user’s help desk. Due to the strong relationship between time and alarms, we verified the importance of having and unique time for all the company’s computer systems, or provide the necessary mechanism in order to adjust the time registry of each separate system. The formalization of rules and knowledge in the area of critical mission systems allows the company to make predictions of the dimensioning and correct forecasting of the systems taking into account their critical nature. As results of the processes of systematization and formalization of knowledge we have: Rules for the platform specification in order to forecast a scheme of high availability and contingency. We documented the need of the creation of a software alarms and a development support center based in this alarms. The cost issues related to the startup of an end user help desk were verified.     Overview of the Evolution of Silica-Based Chromo-Fluorogenic Nanosensors

    [EN] This review includes examples of silica-based, chromo-fluorogenic nanosensors with the aim of illustrating the evolution of the discipline in recent decades through relevant research developed in our group. Examples have been grouped according to the sensing strategies. A clear evolution from simply functionalized materials to new protocols involving molecular gates and the use of highly selective biomolecules such as antibodies and oligonucleotides is reported. Some final examples related to the evolution of chromogenic arrays and the possible use of nanoparticles to communicate with other nanoparticles or cells are also included.     Towards the optimisation of ceramic-based microbial fuel cells: A three-factor three-level response surface analysis design

    © 2019 The Authors Microbial fuel cells (MFCs) are an environment-friendly technology, which addresses two of the most important environmental issues worldwide: fossil fuel depletion and water scarcity. Modelling is a useful tool that allows us to understand the behaviour of MFCs and predict their performance, yet the number of MFC models that could accurately inform a scale-up process, is low. In this work, a three-factor three-level Box–Behnken design is used to evaluate the influence of different operating parameters on the performance of air-breathing ceramic-based MFCs fed with human urine. The statistical analysis of the 45 tests run shows that both anode area and external resistance have more influence on the power output than membrane thickness, in the range studied. The theoretical optimal conditions were found at a membrane thickness of 1.55 mm, an external resistance of 895.59 Ω and an anode area of 165.72 cm2, corresponding to a maximum absolute power generation of 467.63 μW. The accuracy of the second order model obtained is 88.6%. Thus, the three-factor three-level Box–Behnken-based model designed is an effective tool which provides key information for the optimisation of the energy harvesting from MFC technology and saves time in terms of experimental work

    Muons in air showers at the Pierre Auger Observatory: Mean number in highly inclined events

    We present the first hybrid measurement of the average muon number in air showers at ultrahigh energies, initiated by cosmic rays with zenith angles between 62° and 80°. The measurement is based on 174 hybrid events recorded simultaneously with the surface detector array and the fluorescence detector of the Pierre Auger Observatory. The muon number for each shower is derived by scaling a simulated reference profile of the lateral muon density distribution at the ground until it fits the data. A 10^19  eV shower with a zenith angle of 67°, which arrives at the surface detector array at an altitude of 1450 m above sea level, contains on average (2.68±0.04±0.48(sys))×10^7 muons with energies larger than 0.3 GeV. The logarithmic gain d ln N_μ/dlnE of muons with increasing energy between 4×1018  eV and 5×1019  eV is measured to be (1.029±0.024±0.030(sys)

    Diferenciación y desarrollo postnatal de los tipos de fibras musculares en el cerdo chato murciano

    Mediante la técnica que detecta la actividad de la enzima ATPasa miosínica se analiza el tamaño y porcentajes de los distintos tipos de fibras que integran el músculo longísimo lumbar del cerdo “Chato Murciano”, desde el nacimiento hasta los 120 días de edad. Al nacimiento, el músculo longísimo está conformado por un 5,5% de fibras tipo I, de gran tamaño y un 94,5% de fibras tipo II, de menor tamaño. Durante el primer mes de vida postnatal las fibras tipo II se irán diferenciando en nuevas fibras tipo I y subtipos IIA y IIX. A partir de los dos meses tiene lugar un aumento del diámetro fibrilar (hipertrofia) y una distribución fibrilar que recuerda a la de animales adultos. Aunque la hipertrofia observada afecta a los tres tipos de fibras evaluados, parece claro que el crecimiento y conformación muscular en el cerdo chato murciano está muy asociado al notable incremento en diámetro que experimentan las fibras IIX, ya que éstas crecen más rápidamente que las tipo I y IIA
