    CIS-based registration of quality of life in a single source approach

    Background: Documenting quality of life (QoL) in routine medical care and using it both for treatment and for clinical research is not common, although such information is absolutely valuable for physicians and patients alike. We therefore aimed at developing an efficient method to integrate quality of life information into the clinical information system (CIS) and thus make it available for clinical care and secondary use. Methods: We piloted our method in three different medical departments, using five different QoL questionnaires. In this setting we used structured interviews and onsite observations to perform workflow and form analyses. The forms and pertinent data reports were implemented using the integrated tools of the local CIS. A web-based application for mobile devices was developed based on XML schemata to facilitate data import into the CIS. Data exports of the CIS were analysed with statistical software to perform an analysis of data quality. Results: The quality of life questionnaires are now regularly documented by patients and physicians. The resulting data is available in the Electronic Health Record (EHR) and can be used for treatment purposes and communication as well as research functionalities. The completion of questionnaires by the patients themselves using a mobile device (iPad) and the import of the respective data into the CIS forms were successfully tested in a pilot installation. The quality of data is rendered high by the use of automatic score calculations as well as the automatic creation of forms for follow-up documentation. The QoL data was exported to research databases for use in scientific analysis. Conclusion: The CIS-based QoL is technically feasible, clinically accepted and provides an excellent quality of data for medical treatment and clinical research. Our approach with a commercial CIS and the web-based application is transferable to other sites

    Interaction between Amyloid Beta Peptide and an Aggregation Blocker Peptide Mimicking Islet Amyloid Polypeptide

    Assembly of amyloid-beta peptide (Aβ) into cytotoxic oligomeric and fibrillar aggregates is believed to be a major pathologic event in Alzheimer's disease (AD) and interfering with Aβ aggregation is an important strategy in the development of novel therapeutic approaches. Prior studies have shown that the double N-methylated analogue of islet amyloid polypeptide (IAPP) IAPP-GI, which is a conformationally constrained IAPP analogue mimicking a non-amyloidogenic IAPP conformation, is capable of blocking cytotoxic self-assembly of Aβ. Here we investigate the interaction of IAPP-GI with Aβ40 and Aβ42 using NMR spectroscopy. The most pronounced NMR chemical shift changes were observed for residues 13–20, while residues 7–9, 15–16 as well as the C-terminal half of Aβ - that is both regions of the Aβ sequence that are converted into β-strands in amyloid fibrils - were less accessible to solvent in the presence of IAPP-GI. At the same time, interaction of IAPP-GI with Aβ resulted in a concentration-dependent co-aggregation of Aβ and IAPP-GI that was enhanced for the more aggregation prone Aβ42 peptide. On the basis of the reduced toxicity of the Aβ peptide in the presence of IAPP-GI, our data are consistent with the suggestion that IAPP-GI redirects Aβ into nontoxic “off-pathway” aggregates

    The Cortisol Response to Anticipated Intergroup Interactions Predicts Self-Reported Prejudice

    Objectives: While prejudice has often been shown to be rooted in experiences of threat, the biological underpinnings of this threat–prejudice association have received less research attention. The present experiment aims to test whether activations of the hypothalamus-pituitary-adrenal (HPA) axis, due to anticipated interactions with out-group members, predict self-reported prejudice. Moreover, we explore potential moderators of this relationship (i.e., interpersonal similarity; subtle vs. blatant prejudice). Methodology/Principal findings: Participants anticipated an interaction with an out-group member who was similar or dissimilar to the self. To index HPA activation, cortisol responses to this event were measured. Then, subtle and blatant prejudices were measured via questionnaires. Findings indicated that only when people anticipated an interaction with an out-group member who was dissimilar to the self, their cortisol response to this event significantly predicted subtle (r =.50) and blatant (r =.53) prejudice. Conclusions: These findings indicate that prejudicial attitudes are linked to HPA-axis activity. Furthermore, when intergroup interactions are interpreted to be about individuals (and not so much about groups), experienced threat (or its biological substrate) is less likely to relate to prejudice. This conclusion is discussed in terms of recent insights from social neuroscience

    Rationally Designed Turn Promoting Mutation in the Amyloid-β Peptide Sequence Stabilizes Oligomers in Solution

    Enhanced production of a 42-residue beta amyloid peptide (Aβ42) in affected parts of the brain has been suggested to be the main causative factor for the development of Alzheimer's Disease (AD). The severity of the disease depends not only on the amount of the peptide but also its conformational transition leading to the formation of oligomeric amyloid-derived diffusible ligands (ADDLs) in the brain of AD patients. Despite being significant to the understanding of AD mechanism, no atomic-resolution structures are available for these species due to the evanescent nature of ADDLs that hinders most structural biophysical investigations. Based on our molecular modeling and computational studies, we have designed Met35Nle and G37p mutations in the Aβ42 peptide (Aβ42Nle35p37) that appear to organize Aβ42 into stable oligomers. 2D NMR on the Aβ42Nle35p37 peptide revealed the occurrence of two β-turns in the V24-N27 and V36-V39 stretches that could be the possible cause for the oligomer stability. We did not observe corresponding NOEs for the V24-N27 turn in the Aβ21–43Nle35p37 fragment suggesting the need for the longer length amyloid peptide to form the stable oligomer promoting conformation. Because of the presence of two turns in the mutant peptide which were absent in solid state NMR structures for the fibrils, we propose, fibril formation might be hindered. The biophysical information obtained in this work could aid in the development of structural models for toxic oligomer formation that could facilitate the development of therapeutic approaches to AD

    Functionally Relevant Domains of the Prion Protein Identified In Vivo

    The prion consists essentially of PrPSc, a misfolded and aggregated conformer of the cellular protein PrPC. Whereas PrPC deficient mice are clinically healthy, expression of PrPC variants lacking its central domain (PrPΔCD), or of the PrP-related protein Dpl, induces lethal neurodegenerative syndromes which are repressed by full-length PrP. Here we tested the structural basis of these syndromes by grafting the amino terminus of PrPC (residues 1–134), or its central domain (residues 90–134), onto Dpl. Further, we constructed a soluble variant of the neurotoxic PrPΔCD mutant that lacks its glycosyl phosphatidyl inositol (GPI) membrane anchor. Each of these modifications abrogated the pathogenicity of Dpl and PrPΔCD in transgenic mice. The PrP-Dpl chimeric molecules, but not anchorless PrPΔCD, ameliorated the disease of mice expressing truncated PrP variants. We conclude that the amino proximal domain of PrP exerts a neurotrophic effect even when grafted onto a distantly related protein, and that GPI-linked membrane anchoring is necessary for both beneficial and deleterious effects of PrP and its variants

    Towards a robust quantification of the societal impacts of consumer-facing cybercrime

    Da sich profitorientierte Internetkriminalität weltweit verbreitet und zudem zuverlässige Kostenschätzungen nur unzureichend zur Verfügung stehen, ist das Ziel dieser Dissertation die robuste Quantifizierung der gesellschaftlichen Kosten von Internetkriminalität. Die Arbeit dokumentiert dazu drei empirische Studien, um die Messung von verbraucherorientierter Internetkriminalität zu verbessern. Die erste Studie diskutiert robuste Methoden zur Schätzung von direkten Kosten mit Bevölkerungsbefragungen und vergleicht Schätzwerte über mehrere Dimensionen. Die zweite Studie entwickelt ein Model des Konsumentenverhaltens als Reaktion auf Internetkriminalität. Wir liefern empirische Evidenz für indirekte gesellschaftliche Kosten durch die Vermeidung von Online-Diensten auf Basis mehrerer Strukturgleichungsmodelle. Die dritte Studie ergänzt die gesamtgesellschaftliche Perspektive um die Mikroebene. Wir messen und erklären Vermeidungsverhalten von Opfern von Kreditkartenmissbrauch nach einem Missbrauchsvorfall.Recognizing the global scale of profit-oriented cybercrime and the wide spread of unreliable cost estimates, the aim of this dissertation is the robust quantification of the societal impact of cybercrime. We conduct three empirical studies to advance the measurement of consumer-facing cybercrime in different directions. The first part discusses methods for robust estimation of direct costs with population surveys and compares estimates on the societal level along multiple dimensions. The second part develops a model of consumer behavior in reaction to cybercrime. We use structural equation modeling to provide empirical evidence of the indirect impact of cybercrime in the form of online service avoidance. The third study supplements the societal perspective with a micro level measurement of actual behavior. We measure credit card avoidance by the victims of credit card fraud and explain it with different perceptions and attitudes.<br

    Analyzing Persistent Impact of Cybercrime on the Societal Level: Evidence for Individual Security Behavior

    Cybercrime has evolved into a serious global problem with considerable social and economic impact. Avoidance, one form of individual security behavior, can lead to long-lasting negative outcomes on the societal level, but is rarely studied. While avoidance effects are difficult to study for recent innovations, theoretical models and data exist for established online services. Building on a parsimonious research model, we study the persistence of aggregate avoidance effects towards the use of online services along with protection behavior using a longitudinal approach. We use structural equation modelling in a secondary analysis of three representative pan-European surveys, conducted in 2012-2014. We find that cybercrime experience increases perceived risk and ultimately leads to avoidance of online banking, online shopping, and unknown websites. It also has a direct impact on two forms of protection behavior, namely: changing security settings and using different passwords. Trend analyses show that these effects are persistent over time
