The management of patients positive to hepatitis C virus antibody in Malta
Hepatitis C virus (HCV) infection is one of the main causes of chronic liver disease and hepatocellular carcinoma worldwide and is an important public health concern. A retrospective analysis of the demographics and management of patients who had a positive anti-HCV detected by enzyme immunoassay test done at Mater Dei Hospital was carried out to analyse the epidemiology of HCV infection in Malta and assess our management when compared to the European Association for the Study of the Liver (EASL) guidelines. 72% of patients were male. The majority of patients were aged 21-50 years. The main mode of infection was via intravenous drugs use, accounting for 68% of cases. Only 56% of patients found to be HCV Ab positive had a scheduled appointment with an infectious diseases specialist or gastroenterologist documented on the MDH online appointment system. 58% of patients had HCV RNA testing done and 45% had genotype testing. 7.3% with HCV infection were given treatment, of which 43% had a Sustained Virological Response (SVR).peer-reviewe

The process, outcomes, and challenges of feasibility studies conducted in partnership with stakeholders: A health intervention for women survivors of intimate partner violence
Feasibility studies play a crucial role in determining whether complex, community-based interventions should be subject to efficacy testing. Reports of such studies often focus on efficacy potential but less often examine other elements of feasibility, such as acceptance by clients and professionals, practicality, and system integration, which are critical to decisions for proceeding with controlled efficacy testing. Although stakeholder partnership in feasibility studies is widely suggested to facilitate the research process, strengthen relevance, and increase knowledge transfer, little is written about how this occurs or its consequences and outcomes. We began to address these gaps in knowledge in a feasibility study of a health intervention for women survivors of intimate partner violence (IPV) conducted in partnership with policy, community and practitioner stakeholders. We employed a mixed-method design, combining a single-group, pre-post intervention study with 52 survivors of IPV, of whom 42 completed data collection, with chart review data and interviews of 18 purposefully sampled participants and all 9 interventionists. We assessed intervention feasibility in terms of acceptability, demand, practicality, implementation, adaptation, integration, and efficacy potential. Our findings demonstrate the scope of knowledge attainable when diverse elements of feasibility are considered, as well as the benefits and challenges of partnership. The implications of diverse perspectives on knowledge transfer are discussed. Our findings show the importance of examining elements of feasibility for complex community-based health interventions as a basis for determining whether controlled intervention efficacy testing is justified and for refining both the intervention and the research design
estructuradas, observación no participante y revisión bibliográfica.
This research has a qualitative approach and focuses on the analysis and
explanation of a specific case. The NGO Soluciones Prácticas is an organization that
finds itself in a changing and convulsed environment. Thus, the economic, political and
social changes -national and international- that arose at the end of the 80s until the
first decade of the 2000s, have forced this organization to restructure itself. In this
framework, it would be expected that the NGO managers would respond in an agile
and feasible way, a situation that did not happen, presenting limitations to face the new
scenario. The main objective of this research is to explain the scope of the informal
system in the institutional performance of the NGO Soluciones Prácticas considering
three dimensions: i) power relations, ii) coordination and iii) organizational culture. This
within an unstable environment and in a process of transition towards a modern
organizational model, which occurred in the first fifteen years of the 21st century. For
this, the Theory of Human Relations is used, taking as its main premise the concept
developed by Johansen (1982), which indicates that the organization is composed of
a formal and informal system, and contemplating the approach of the concrete action
system, developed by Crozier and Friedberg (1990).
In the collection of information, structured and semi-structured interviews, non-
participant observation and bibliographic review were used. The main conclusion
points to the fact that economic, political and social changes have caused a "transition
stage" in the NGO Soluciones Prácticas. Stage that could not be effectively faced by
senior management and the consulting and innovation areas of the headquarters.
Faced with such a situation of uncertainty, the managers and collaborators of the
programs have developed their own strategies using the informal system of the
organization. With this, the NGO has been able to survive and maintain itself in a
sustained manner in an environment of instability

Linking Dynamical and Population Genetic Models of Persistent Viral Infection
This article develops a theoretical framework to link dynamical and population genetic models of persistent viral infection. This linkage is useful because, while the dynamical and population genetic theories have developed independently, the biological processes they describe are completely interrelated. Parameters of the dynamical models are important determinants of evolutionary processes such as natural selection and genetic drift. We develop analytical methods, based on coupled differential equations and Markov chain theory, to predict the accumulation of genetic diversity within the viral population as a function of dynamical parameters. These methods are first applied to the standard model of viral dynamics and then generalized to consider the infection of multiple host cell types by the viral population. Each cell type is characterized by specific parameter values. Inclusion of multiple cell types increases the likelihood of persistent infection and can increase the amount of genetic diversity within the viral population. However, the overall rate of gene sequence evolution may actually be reduced
